본 연구에서는 조직확장 응용을 위한 생체적합성 생분해성 하이드로젤을 제조하고, 그 기본특성을 분석하였다. 친수성 고분자인 poly(ethylene glycol)의 양 말단에 다양한 몰비의 D,L-lactide와 glycolide를 개환 중합시켜 PLGA-PEG-PLGA 삼중공중합체를 합성한 뒤 비닐기를 도입하여 하이드로젤 제조 시 swelling/degradation controllers(SDC)로 사용하였다. 합성한 SDC와 PEG diacrylate를 사용하여 라디칼 중합으로 제조한 하이드로젤은 건조된 상태에서도 유연하고 탄성을 보였으며 분해테스트에서는 SDC의 조성과 SDC/PEG의 몰비에 따라 다양한 팽윤지연시간과 분해기간을 갖는 것으로 나타났다. 그 밖에 기계적 물성과 팽윤압력을 측정하였고, 이식시험을 통해 이식용 하이드로젤을 사용목적에 맞게 이식하거나 삽입하였을 때의 생체 조직의 국소적인 병리적 양상을 육안관찰과 현미경적 관찰을 통하여 평가하였다.
만손주혈흠충 충란의 soluble eff antigen (SEA)을 mini-pump를 이용하여 마우스, 복강내에 지속적으로 주입시키면서 말초혈액내의 호산구수 변동, delayed hypersensitivity인 ear swelling test, 간조직병변, 효소 표식면역법을 이용한 혈청내 항체가를 경시적으로 관찰한 결과 다음 성적을 얻었다. 1. 말초혈액 호산구수는 SEA투여후 2~3주후에 최고에 달하였고 그후 서서히 감소하였다. 2. Ear swelling test 실시에서 SEA투여 2주후에 종창이 최고에 달하고 그후 서서히 감소하였다. 3. 간조직병변은 SEA투여후 4주에 육아종형성을 관찰하였고 그후 조직손상이 복귀하였다. 4. 현청에서의 항체가는 SEA투여후 4주후에 양성대조군보다 높았으며 시간이 경과함에 따라 항채가가 증가하였다.
Acute delayed neurotoxicity of KH-502 [O.O-Diethyl O-(1-phenyyl-3-trifluoromethyl-5-pyrazoyl) thiophosphoric acid ester], an insecticide synthesized newly in Korea, was studied in White Leghorn hens. The doses were determined on the basis of preliminary $LD_{50}$ study. High, middle and low doses were determined to be 1123 mg/kg, 762 mg/kg and 518 mg/kg, respectively. The animals were pretreated with atropine (30 mg/kg) prior to administration of KH-502. The chemical was administrated at the first and 21st day of the study. As positive controls, animals were admlnistrated with triorthocresylphosphate (TOCP 1000 mg/kg and 500 mg/kg). Animals administrated with TOCP or KH-502 were sacrificed by perfusion-fixation at 21st and 42nd day of the study, respectively. The central and peripheral nerve tissues were routinely treated for microscopic observation. As results, eight, three, one, and one chickens died within 2 day after adminiatration with signs of cholinergic acute toxicity in high, middle low and TOCP dose-group (500 mg/kg), respectively. No abnormal clinical signs were observed in the survived chickens administrated with KH-502 in the duration of the study. The chickens in positive control groups showed ataxia and incoordination at the 14th day after administration of TOCP. From necropsy, macroscopic changes were not observed in all groups including positive control groups. Histopathologically, oxonal swelling with myelin loss, focal gliosis, distention around axonal space were observed in the spinal cords of the chickens administrated with TOCP 1000 mg/kg. The lesions were distinct in the dorsal and lateral funiculi of cervical spinal cord, in the lateral and ventral funiculi of thoracic spinal cord and in ventral funiculi of lumbosacral spinal cord. Axonal swelling and mlcrogliosis were infrequently observed in the chickens of other groups including negative control one. However, they were nonspecifically distributed in the spinal cords. In this study, we concluded that the new chemical, KH-502 did not have acute delayed neurotoxicity in White Leghorn hens.
Sarcoidosis is a systemic disease of unknown etiology that may affect any organ in the body. The nervous system is involved in 5-16% of cases of sarcoidosis. Here, we report a case of intramedullary sarcoidosis presenting with delayed spinal cord swelling after laminoplasty for the treatment of compressive cervical myelopathy. A 56-year-old woman was admitted to our hospital complaining of upper extremity pain and gait disturbance. The patient had undergone laminoplasty for compressive cervical myelopathy 3 months previously. Follow-up magnetic resonance imaging revealed a large solitary intramedullary lesion with associated extensive cord swelling, signal changes, and heterogeneous enhancement of spinal cord from C2 to C7. Spinal cord biopsy revealed non-necrotizing granulomas with signs of chronic inflammation. The final diagnosis of sarcoidosis was based upon laboratory data, imaging findings, histological findings, and the exclusion of other diagnoses. Awareness of such presentations and a high degree of suspicion of sarcoidosis may help arrive at the correct diagnosis.
Swelling is a body's natural reaction to foot and ankle injury to mount a healing inflammatory response. To some degree, swelling is necessary for healing and is something that cannot be avoided following injury. However, post-traumatic swelling may have an adverse effect on wound healing and surgery can often be delayed due to preoperative swelling. We report on a unique technique of making multiple meshed stab incisions around the site of injury to reduce soft tissue swelling and promote wound healing.
Purpose: Retrobulbar hemorrhage is a rare complication followed by blepharoplasty, trauma, orbital reconstruction, and so on. Most of the cases occur within 24 hours, half of them in the first 6 hours. Some authors have reported delayed retrobulbar hemorrhage after blepharoplasty and trauma within 1 day to 9 days. However, there have been few reports of delayed retrobulbar hemorrhage resulting from the complication of orbital reconstruction. Methods: A 22-year-old male underwent orbital floor reconstruction due to the orbital floor fracture. In 84 hours after the surgery, he complained sudden onset orbital pain and decreased visual acuity immediately after defecation. Intraocular pressure was unmeasurable due to the swelling at that time. Emergency computed tomography was performed. Results: Computed tomography revealed subperiosteal hematoma on inferior orbital wall extended to the apex. Emergency decompressive surgery was performed within 1 hour. After evacuation of hematoma, orbital symptom was improved and visual acuity was restored. Conclusion: Delayed retrobulbar hemorrhage is rare but vision-threatening. Therefore early diagnosis and treatment of delayed retrobulbar hemorrhage is thought to be crucial. The cause of delayed hemorrhage was not clear, however, valsalva maneuver might be the cause of hemorrhage.
A 47-year-old man with end-stage renal disease due to diabetic nephropathy underwent a peritoneal scintigraphy to evaluate the cause of recently developed scrotal swelling. Two liters of dialysate mixed with 111 MBq of Tc-99m sulfur colloid were administered into the peritoneal cavity via the dialysis catheter. Various anterior images of the abdomen and pelvis were obtained at 15 min, 2 hr and 4 hr after the tracer instillation. At 15 min, anterior images of the abdomen and pelvis demonstrated linear tracts of activity through both inguinal canals, which were more prominent in the right side (A). Images at 2 hr revealed a passage of the radioactive fluid into the right hemiscrotum. At the same time, there was a considerable accumulation of activity in the right inguinal canal (B). In the delayed image, there was a progressive accumulation of activity in the inguinal canals and a prominent passage of the tracer into the scrotum (C). Both abdominal and inguinal hernias are commonly associated with continuous ambulatory peritoneal dialysis (CAPD). Overall incidence of CAPD-induced hernia ranges from 2.7% to 25%.$^{1)}$ Inguinal hernias were frequently manifested as scrotal swelling. Leakages of dialysate fluid into the scrotum has been noted in CAPD patients with scrotal swelling, with or without clinical findings of inguinal hernia.$^{1,2)}$ In the present case, the right side had leakage from a clinical inguinal hernia and the left side, leakage from a subclinial inguinal hernia. A subclinical inguinal hernia was easily demonstrable with peritoneal scintigraphy. Peritoneal scintigraphy is extremely helpful in the evaluation of scrotal swelling in a patient on CAPD.
Retaining a drug in the stomach by some means is sometimes necessary to extend the G1 absorption time of the drug more than 6-8 hrs. Hydrogel has often been examined for its feasibility as a dosage form, so called platform, that could be retained in the stomach due to its excellent swelling properties in the gastric fluid. In this study, polyvinylpyrrolidone (PVP) hydrogel crosslinked by albumin or acrylated albumin was synthesized in a tablet form and evaluated for its possibility as the platform. The synthesis of the hydrogel was performed by $^{60}Co\;{\gamma}-ray$ irradiation of N-vinyl-2-pyrrolidone (monomer) in the presence of a crosslinking agent: aqueous solution of albumin or acrylated albumin. Synthetic conditions such as radiation dose, dose rate and concentration of crosslinking agent were varied in order to optimize the swelling and mechanical properties of the hydrogels. Degree of swelling of albumin-crosslinked PVP (Al-PVP) was highly dependent on radiation dose, dose rate and albumin concentration: it was decreased as they increased. On the other hand, that of acrylated albumin-crosslinked PVP (Acryl-PVP) was almost independent on them except dose rate: it was decreased as the radiation dose rate increased. The compressive strength of the two hydrogels was decreased as the dose rate increased. Digestion of both PVP in artificial gastric fluid containing pepsin was delayed by the ${\gamma}-ray$ irradiation. In conclusion, Al-PVP and Acry-PVP with diverse swelling and mechanical properties could be obtained by controlling synthetic conditions, mainly the irradiation dose rate.
As many crude drugs are used in the oriental medical field problems on the side effects of these drugs come to the front. To conduct delayed-type hypersensitivity we selected 29 kinds of drugs used frequently for therapeutic agents in oriental medical hospitals (Table I). The cell-mediated immune response was evaluated by measuring the foot pad swelling reaction and humoral immune response by measuring the antibody formation to these crude drugs. Mice were given these drugs intraperioneally for sensitization and challenged with same drug as used for sensitization respectively by intral dermal injection on the left and righ hind foot pad 4 days after senstization and then the foot pads were measured with the dial micrometer. The results were as follow; 1) Gentianae Scabrae Radix, Arecae Semen, Corydalis Tuber, and Paeoniae Radix were significant as delayed-type hypersensitivity inducers. 2) None of the crude drugs tested had effect on the induction of humoral immune response.
The development of delayed-type hypersensitivity(DTH) reaction to Staphylococcus aureus in mice was studied, Mice received 3 injections of $10^8$ viable S. aureus subcutaneously showed a marked footpad swelling when mice were challenged with $10\;{\mu}g$ staphylococcal protein antigen into footpad(The percent increase of footpad thickness at 24 h after challenge wsa 35% approximately). Histological observation of footpad of immunized mice showed a marked thickness of subcutaneous tissue due to edematous reaction and massive infiltration of lymphocytes and neutrophils which are characteristic cells in DTH reaction. Intensity of DTH reaction of mice immunized with viable bacteria was much higher than that of mice immunized with staphylococcal protein or heat-killed bacteria. The DTH reaction to S. aureus could be transferred to normal recipient mice by both spleen cells and lymph node cells.
