Anterior Talofibular Ligament and Superior Extensor Ankle Retinaculum Thicknesses: Relationship with Balance

  • Purpose: This study determined if anterior talofibular ligament (ATFL)/superior extensor ankle retinaculum (SEAR) thicknesses are related to dynamic balance in individuals with chronic ankle instability (CAI). Materials and Methods: The subjects were 14 males and 15 females (age=24.52±3.46 years). Ankle instability was assessed using the Cumberland Ankle Instability Tool (CAIT) with a cut off score of 25 to define two groups. SonoSite MTurbo (Fugifilm Sonosite, Inc.) musculoskeletal ultrasound (MSKUS) unit was used to assess ATFL and SEAR thicknesses. Dynamic balance was measured with the Y Balance Test (YBT) and two NeuroCom balance tests. Results: There were no significant differences in the average ATFL thickness between stable and unstable ankles in those subjects with CAI (0.25±0.03 cm and 0.21±0.05 cm, respectively) or in the SEAR thickness (0.09±0.04 cm and 0.10±0.03 cm, respectively). There were also no significant differences in the right and left ATFL thicknesses (0.23±0.07 cm and 0.21±0.04 cm, respectively) or the SEAR thicknesses (0.09±0.01 cm and 0.09±0.01 cm, respectively) in those without CAI. There were no differences between limbs in composite scores on YBT in those with CAI (p=0.35) and those without CAI (p=0.33). There was a moderate correlation between the left SEAR thickness and the large forward/backward perturbations on the NeuroCom (Natus) motor control test (r=0.51, p=0.006 and r=0.54, p=0.003, respectively). Conclusion: There were no differences in the ATFL/SEAR thicknesses or balance measures between or within the groups, likely because CAI is multi-factorial and related to mechanisms other than tissue changes alone. More sensitive technology and a better definition of the measurement process may provide more definitive results.

Love : A Concept Analysis for Nursing Theory Development (간호이론 개발을 위한 개념분석 : 사랑)

  • Since nursing has come of age both as a profession and as a scholarly discipline, there has been increasing concern with delineating its theory base. In 1978 Chinn and Jacobs asserted that “the development of theory is the most crucial task facing nursing today.” The basic building blocks of theories are concepts. Concept formation begins in infancy, for concepts help us to categorize and organize our environmental stimuli. Concepts help us to identify how our experiences are similar or equivalent by categorizing all the things that are alike about them. concepts can be primitive, concrete, or abstract. Concept Analysis is a strategy that examines the attributes or characteristics of a concept. It is a formal, linguistic exercise to determine those defining attributes. It encourages communication. Its basic purpose is to distinguish between the defining and irrelevant attributes of a concept similarities. It is a process of determining the differences between concepts and it is useful for several reasons. It is an excellent way to begin examining information in preparation for research or theory construction and results in an operational definition and a list of defining attributes and antecedents. It provides the scientist with an excellent beginning for a new tool, is an excellent way of evaluating an old one and is useful in evaluating existing instruments. The steps of concept analysis are : 1. Select a concept, 2. Determine the aims or purposes of the analysis, 3. Identify all uses of the concept that you can discover, 4. Determine the defining attributes, 5. Construct a model case, 6. Construct borderline, re-lated, contrary, invented, and illegitimate cases, 7. Identify antecedents and consequences, 8. Define empirical referents. In this paper, the concept selected for analysis was Love. The concept of love is of great interest to nursing because loving care is considered vital to the nursing care of patients. The aims of the concept analysis of love were to clarify the meaning of love, to develop an operational definition for it and to contribute to existing nursing theory. Love influences the quality of life which is the goal of nursing according to Parse in her Human Becoming Theory. Lived experiences are the entities for study in Parse's Research Methodology. Human caring, human understanding, and human becoming are the most important issues in lived experiences. In this research, dictionaries and literature from nursing philosophy and other human disciplines were used to identify the concept of love. As many different instances of the concept as could be found were examinned. The model case was a real life example of the use of the concept. Next borderline, related, invented, and contrary cases were constructed for the purpose of providing examples of “not the concept” and for promoting further understanding of the concept being discussed. The defining attributes of the concept of love were concern, responsibility, respect, understanding and dedication. Love was defined as showing concern and understanding. relating with mutual respect and dedicating oneself responsibly to others. Concept analysis is a highly creative activity and may add significant new information to a given area of interest. It is a strategy for developing a concept based on observation or other forms of empirical evidence. The purpose of concept analysis is to generate new ideas. It provides a method of examining data for new insights that contribute to theoretical development. This concept analysis suggests that a nurse’s love for patients is shown in the process of giving oneself in mutual relationships of responsibility and respect and in continuously providing understanding and quality human care for them.

A Study of the Effects of Health Contracting on Compliance with Health Behaviors in Clients with Hypertension (자가간호증진을 위한 건강계약이 고혈압자의 건강행위 이행에 미치는 영향)

  • It is generally accepted that the delivery of health care is undergoing many changes specially those related to acute, contagious disease care and to the increase of chronic illnesses which can not be cured but are controlable. The health care practitioner can not be soley responsible for the control of their clients' care. Because the clients will play a vital role in controlling their illnesses, long term participation by both the health care provider and the client is necessary. Since most individuals with hypertension do not experience signs or symptoms, the disease is difficult to detect and even when diagnosed, clients do not comply well with their hypertension regimens. The noncompliant client is at increased risk for compliants involving the heart, brain, kidney and other organs. In an effort to explore methods of increasing patient participation in and adherence to treatment programs for hypertension, the researcher used health contracting to promote self care. The research questions are; 1) Will the health contracting increase compliance in health behavior and reduce the blood pressure\ulcorner 2) If clients comply with their regimens will this reduce their blood pressure\ulcorner The research design utilized in this study was a quasi-experimental design. A purposive sample, was abtained from two churches in the 1. area, consisting of 64 clients with hypertension. The data was collected from the middle of January to the 1st of September 1985. Randomization was only of the two church groups into experimental and control groups. Compliance with health behavior related to the hypertensive regimen, blood pressure and body weight were measured, compared and analyzed. In the experimental group measurements were made 6 times; one month before the education program after education program when health contracting was done and 4 more times once a month for 4 months. In the control group measurements were made 3 times; one month before the education program after the education program, and once 4 months later. There was no health contracting. The data were analyzed by t-test, Pearson correlation and ANOVA according to purpose of the study. The result of this study may be summarized as follows: The result related to the hypothesis on the effect of health contracting are as follows: H$_1$; “The hypothesis that the experimental group, with a health contractual agreement will demonstrate increased compliance levels for health behavior than the control group” was supported(t=-5.29, df=62, p=.000). H$_2$; “The hypothesis that the experimental group, with a health contractual agreement, will demonstrate a greater reduction in blood pressure than the control group” was supported (for systolic blood pressure t=2.72, df=62, p=.009, for diastolic blood pressure t=1.95, df=62, p=.050). H$_3$; The hypothesis that the greater the compliance of the client with health behavior the lower the client's blood pressure will be was partially supported (for systolic pressure r=-.2981, p=.008, for diastolic pressure r=-.1720, p=.087). From the examination of the results of this study it can be concluded that the interaction between the nurse and the client, contracting to define goals and reinforcing compliant behavior, leads to improved compliance with health care behaviors and thus to an increase in the effectiveness of nursing care. Further consideration need to be given to the inclusion of the concept of health contracting in primary nursing and to further research in this area.

Development of Novel Method of Seismic Slope Stability Analysis (신(新) 유사정적 사면안정해석 기법 개발)

  • The seismic slope stability is most often evaluated by the pseudo-static limit analysis, in which the earthquake loading is simplified as static inertial loads acting in horizontal and/or vertical directions. The transient loading is represented by constant acceleration via the pseudostatic coefficients. The result of a pseudostatic analysis is governed by the selection of the value of the pseudostatic coefficient. However, selection of the value is very difficult and often done in an ad hoc manner without a sound physical reasoning. In addition, the maximum acceleration is commonly estimated from the design guideline, which cannot accurately estimate the dynamic response of a slope. There is a need to perform a 2D dynamic analysis to properly define the dynamic response characteristics. This paper develops a new hybrid pseudostatic method that links the modified one-dimensional seismic site response analysis and the pseudostatic algorithm. The modified site response analysis adjusts the density of the layers to simulate the change in mass and weight of the layers of the slope with depth. Multiple analyses were performed at various locations within the slope to estimate the change in seismic response of the slope. The calculated peak acceleration profiles with depth from the developed procedure were compared to those by the two-dimensional analyses. Comparisons show that the two methods result in remarkable match. The calculated profiles are used to perform pseudostatic analysis. The results show that use of peak or a fraction of acceleration at the surface can seriously underestimate or overestimate the factor of safety, and that the proposed procedure significantly enhances the reliability of a standard procedure.

Analysis of the Conceptions of Science Achievement in Major Reform Documents in the United States and Korea (과학교육 개혁운동에 관련된 과학성취 개념의 비교 분석-미국과 한국의 연구 보고서 분석-)

  • This paper reviews and analyzes the conceptions of science achievement in the United States and Korean reform documents, including those on science content standards(NSES, Porject 2061, and Korean 7th science curriculum), performance standards(New Standards, and Survey of Ecucational Achievement in Korean Elementary and Secondary Schools), and large-scale assessment framworks(1996 NAEP, TIMSS, The National Assessment of Science Inqury Abilities, The National Assessment of Science Knowledge, and The National Assessment of Affective Characteristics related to Science). The analysis of these documents indicates that there is an overall agreement on the conceptions of science achievement. The documents consistently emphasize high achievement in terms of knowledge and abilities in scientific, technological, social, and environmental perspectives. In addition, these documents define science achievement at different developmental levels and at certain depths of knowledge and abilities for all students. Despite the overall agreement, there are also noticeable variations among the documents because of different contexts and purposes. There is a difference in the balance of representations or emphases among content and process standards in the documents. The conceptions of science achievement in the Korean documents are not as comprehensive or inclusive as those in the United States documents. There is no representation of the mathematical world, the nature of science, historical perspectives, unifying concepts, or scientific communication. From these results, two conclusions are drawn. First, more coherent conceptions of science achievement are needed for common understanding among educators and the public. Second, efforts are needed for developing more comprehensive and inclusive conceptions of science achievement in Korea.

Secondary Science Teachers' Thoughts on 'Good' Science Teaching ('좋은' 과학 수업에 관한 중등 과학 교사들의 사고)

  • The purpose of this study is to explore the characteristics of secondary science teachers' thoughts on 'good' science teaching and to find a concept that can represent the way the teachers think. Participants were twenty pre- and in-service teachers who were enrolled in a graduate course I taught at a university located in Seoul in the first semester, 2011. The participating teachers collected and analyzed a variety of data and created portfolios while they were trained, as part of the course, on qualitative research methods with the same research questions as those of this study. In the current study, the process in which the teachers and I found answers to the research questions was narratively reconstructed based on the teachers' portfolios and my field notes. It was revealed that science teaching was perceived as a task aiming at realizing some kind of values and that because the teachers pursued various values in the science classroom and there exist conflicting relationships among different values, it was hard to define 'good' science teaching. It was also discussed that science instruction was inherently accompanied with the ongoing process of selecting values as the relationships among the values were ever-changing within the contexts of the classroom. This multi-faceted and dynamic structure of the teachers' thoughts on 'good' science teaching was conceptualized analogically as 'Foucault's pendulum,' which has multiple planes of oscillation. Implications for science teacher education and science education research were suggested.

Argument and Argumentation: A Review of Literature for Clarification of Translated Words (논변, 논의 그리고 논증: 개념의 명료화를 위한 문헌조사 연구)

  • It has been a decade since argument and argumentation were introduced in science education literature in South Korea. The word "argument" has been translated into three different Korean terms in literature. The purpose of this study was to clarify those translated terms by examining how the terms were defined and used in Korean education research literature. From a philosophical perspective on the diversity of translation, we examined definitions of argument and argumentation, research topics in papers published in major international journals on science education, and reviewed relevant science education papers published in South Korean journals. We reviewed 79 papers published since the year 2000 in major international journals on science education, whose titles have terms argument and/or argumentation and 37 Korean science education papers whose titles have terms translated from the two English words. Findings showed that Korean researchers defined argument and argumentation either in a general sense or in a specific sense such as science investigation or group work aspects, depending on research contexts. Researchers in Korea rarely mentioned the diversity of translation. If they mentioned it, justification for a specific translation of the term was not provided except for in one case. When the same foreign literature was reviewed to define "argument" or "argumentation, different Korean words were used to translate the same two terms. This indicated to the researchers that the translation of the terms was not related to their definitions of them. On the other hand, each research group used a certain translation of the term consistently, indicating that translations might be based on research tradition. Based on the findings, a position on translation of the term is proposed in terms of professionalism and communication between research groups.

Characteristics of Social Security Contained in Mecius's People-Care Theory(保民論) (맹자의 보민론(保民論)이 지닌 사회보장적 성격)

  • This paper aims to define Mecius's people-care theory(保民論), and aims to recognize the characteristics of social security contained in Mecius's people-care theory. Mecius considered to the public relief as King's obligation. So to speak, the King protects to the property of the people. Especially his people-care theory implicate to continuity of safety life, a relief fund on the property, protection of disaster damage. Continuity of safety life means to enabling the people procurable food, clothing and bury dead persons without difficulties. It's may be said that implicated to the theory and system of modern social-welfare. It is reason for his opinion that a state should construct in the minimum social safety network by taking responsibility for basis needs of life. He was a humanist so much. He thought about the pursuit of the human happiness and the improvement in quality of life. Therefore his idea nearly means to the social security system in a modern sense. His thought is not only an ideal background on the public assistance and a social relief security in the contemporary but also a relief activity system such as Jindaebup called historical prototype of social welfare. It is an important fact that his people-care theory implicate to social security system.

Inducing Apoptosis of NCI-H157 Human Lung Carcinoma Cells via Activation of Caspase Cascade by Combination Treatment with Arsenic Trioxide and Sulindac (NCI-H157 폐암 세포주에서 Caspase Cascade 활성을 통한 Arsenic Trioxide와 Sulindac 병합요법의 세포고사효과)

  • Arsenic trioxide($As_2O_3$) was introduced into the treatment of refractory or relapsed acute promyelocytic Ieukemia. Some investigators have reported that arsenic trioxide had induced apoptosis in a variety of solid human tumor cell lines, including non-small cell lung cancer. Non-steroidal anti-inflammatory drugs(NSAIDs) are powerful chemopreventive agents for gastrointestinal cancers and the growth of established tumors are reduced by inducing apoptosis. It's also reported that NSAIDs enhanced tumor response to chemotherapeutic drugs or radiation. In this study, we aimed to determine whether combination of arsenic trioxide with sulindac augmented its apoptotic potential in NCI-H157 human lung cancer cells. The human lung cancer cell line NCI-H157 was treated with arsenic trioxide and sulindac. Cell viability was measured by the MTT assay. Apoptosis was measured by nuclear staining and flow cytometric analysis. The catalytic activity of the caspase families were measured by the fluorogenic cleavage of biosubstrates. The western blotting were also performed to define the mechanical basis of apoptosis. Combination treatment of arsenic trioxide and sulindac decreased the viability of NCI-H157 human lung cancer cells in a dose-dependent manner. The catalytic activity of caspase-3, 8 and 9 proteases were increased after combination treatment. Consistently PARP was cleaved from 116kDa to 85kDa fragments, and the expression of ICAD was decreased by time-dependent manner. Also combination treatment increased the expression of Fas and Fas/L. Combination therapy of arsenic trioxide with sulindac augments cell death and induces apoptosis via the activation of caspase cascade in NCI-H157 human lung carcinoma cells.

A Study on the Recognition Differences about Using the Private Forests and Conflicts among the Stakeholders related with Mt. Jiri National Park (지리산국립공원 내 사유림이용에 있어서 이해당사간의 갈등과 산림이용에 대한 인식의 차이에 관한 연구)

  • There has been a conflict over the use of private forest in Mt. Jiri National Park among the stakeholders. Therefore, the purpose of this study is to define each stakeholders' recognition difference about value and conflict of Mt. Jiri National Park and find the possibility of creating agreement point for solving conflict. For the purpose, the study performs factor analysis on the value of national park and conflict factor and abbreviates them to 4 factors respectively. The study classifies and compares the recognition difference among the stakeholders with t-test and Duncan multiple comparison. The result of this study is that village residents, Korea Forest Service and local autonomy share the same recognition about the value of national park but National Park has different recognition. Regarding the conflict, National Park, Korea Forest Service and local autonomy share the same recognition but village residents have different recognition. Regarding the organization of conference as a direction to solve conflict and its reason, all of the stakeholders share the same recognition. It is necessary to adopt clear standard for the use of forest and apply the different execution of regulation to each area.
