The protein truncation caused by fusion of PEP-1 peptide and protective roles of transduced PEP-1-MsrA in skin cells

  • PEP-1 peptide has been used for transduction of native protein into mammalian cells. This work describes the findings that the fusion of PEP-1 to target proteins led to protein truncation likely in a non-protein-specific manner. Approximately 75% of PEP-1-MsrA fusion protein was truncated in the N-terminal region of MsrA between Lys-27 and Val-28 during expression in Escherichia coli and purification. This large protein truncation was also observed in another PEP-1 fused protein, PEP-1-MsrB2, in the N-terminal region of MsrB2. The full-length PEP-1-MsrA protein was rapidly transduced into keratinocyte cells within 15 min. The transduced PEP-1-MsrA was functionally active and could protect skin cells against oxidative stress- and ultraviolet radiation-induced cell death. Collectively, our data demonstrated the protective roles of MsrA in skin cells and, moreover, may raise a concern of protein truncation caused by fusion of PEP-1 about the general use of this peptide for protein transduction.

  • In the point of view, this study aims to examine the life of remarriaged old people after divorce and bereavement. therefore, the purpose of this study is to increase the understanding of married couple's relationship and remarriage in old age and help old people who want to be abused about their remarriage as a coping response of lire for widowhood in old aged. the subject were 14 old people who married over 53 years old bereavement and divorce residing in Chonju, Chonbuk and they had indepth interviews. 1. Most of subject would married with help of matchmaker under the consent a children and relative 2. The main motivation of remarriage was lone lies, however, for men being taken care and for women economic security were also important. 3. Premarriage experience seemed to help spouse adjustment between couples. 4. They matintained close relationship with there biological children. 5. There show a tendency to the bereavement woman remarriage life was happier than divorce woman. 6. When one of the couple had regular income, little economic conflict was there. In most case, wifes were younger than husband and economic stability after husband's death was their main concern and when the husband didn't show definite will to inherit asset to his wife, conflict got bigger.

Congenital heart disease in the newborn requiring early intervention

  • Although antenatal diagnostic technique has considerably improved, precise detection and proper management of the neonate with congenital heart disease (CHD) is always a great concern to pediatricians. Congenital cardiac malformations vary from benign to serious conditions such as complete transposition of the great arteries (TGA), critical pulmonary and aortic valvular stenosis/atresia, hypoplastic left heart syndrome (HLHS), obstructed total anomalous pulmonary venous return (TAPVR), which the baby needs immediate diagnosis and management for survival. Unfortunately, these life threatening heart diseases may not have obvious evidence early after birth, most of the clinical and physical findings are nonspecific and vague, which makes the diagnosis difficult. High index of suspicion and astute acumen are essential to decision making. When patent ductus arteriosus (PDA) is opened Widely, many serious malformations may not be noticed easily in the early life, but would progress as severe acidosis/shock/cyanosis or even death as PDA constricts after few hours to days. Ductus dependent congenital cardiac lesions can be divided into the ductus dependent systemic or pulmonary disease, but physiologically quite different from each other and treatment strategy has to be tailored to the clinical status and cardiac malformations. Inevitably early presentation is often regarded as a medical emergency. Differential diagnosis with inborn error metabolic disorders, neonatal sepsis, persistent pulmonary hypertension of the newborn (PPHN) and other pulmonary conditions are necessary. Urgent identification of the newborn at such high risk requires timely referral to a pediatric cardiologist, and timely intervention is the key in reducing mortality and morbidity. This following review deals with the clinical presentations, investigative modalities and approach to management of congenital cardiac malformations presenting in the early life.

The Experiences of Young Adult with Hypertension (성인 전기 고혈압 환자의 경험)

  • Purpose: The purpose of this study was to explore the experiences of young adults with hypertension, using phenomenology in order to describe the essence of such experiences and to understand them from the patients' point of view. Methods: Participants in this study were 4 hypertension patients with ages ranging from 20 to 35 years old, living in Seoul city. Data were collected from in-depth interviews done from October to November in 2005. Colazzi method(1978) was used to analyze the data. Results: The 12 themes were extracted in this study ; 'Discovery by accident', 'Denial of the diagnosis', 'A disease unworthy of the young men', 'Guilty feeling toward their parents', 'Fear of death', 'Unconvinced reason', 'Worry about the future', 'Attribute all symptoms to the disease', 'Will of not life-long medication', 'Concern for fertility problem', 'Not convinced with current treatment method', 'Becoming unimportant'. Conclusion: The results of this study show that the young adult with hypertension will be in a high risk group of non compliance, so helpful program for them should be developed and the nurses need to pay more attention to the psychosocial aspect of nursing.

The Poor Children's Status and Its Investigation (빈곤한 아동의 현황 및 고찰)

  • Poor children are a field of causing of various Issues and become a subject of sympathy, concern and anger, against adult's poor such as sleeper outdoors and unemployed to be made a subject of discussion. This study has theoretically investigated the poverty concept, family trait around poor children. And also for understanding the above status, poverty circumstances have been focused children family and shelter-care children. The conclusions are as follows. First, the family of children head was formed with children under fifteen years of age. It was reported that solo families were 26.4% and living together families with relatives were 64.7%. As status of producing causes, parents death was shown the most rate as 49% and abscondence or missing was 31.9%. Second, In the shelter-care aspect, the highest case is a parents-divorce with 42.5%, while children-head case is just shown 0.5%. Therefore, in a child right viewpoint, it needs more systematical/long-range study on children poverty, and some policies for protecting the poverty-children must be established in social policy.

In situ Delivery of Therapeutic Proteins by Recombinant Lactococcus lactis

  • Chronic inflammatory bowel disease (IBD) such as Crohn's disease or ulcerative colitis, affects around 2 in every 1000 individuals in western countries and its incidence, particularly amongst children, is increasing. IBD shows extreme morbidity with impact on all aspects of quality of life. If left untreated, IBD can lead to death. Conventional treatment of IBD involves powerful immunosuppressive chemotherapies and surgical intervention. Long-term anti-inflammatory medication is required and so patients are often subject to a spectrum of unpleasant side effects. Interleukin-10 (IL-10) is a cytokine that acts to suppress inflammation. When however administered by injection, the high levels of IL-10 that are distributed throughout the body also lead to side effects. Lactococcus lactis can be genetically engineered to secrete biologically active cytokines. When applied to the mucosa, these L. lactis can actively deliver such cytokines. By use of this principle we developed a new therapeutic approach for IBD. Administration of L. lactis that secretes murine IL-10 cures and prevents IBD in mice. The use of the engineered L. lactis gets around the problem of delivering IL-10, allowing dramatic reduction of the effective dose. A sincere concern exists about the possible dangers of uncontrolled, deliberate release of genetically modified microorganisms, such as could occur following application in healthcare. We engaged in the establishment of adequate means for biological growth control of engineered L. lactis by targeted gene exchange between thyA and hIL-10.

Investigating the Time Lag Effect between Economic Recession and Suicide Rates in Agriculture, Fisheries, and Forestry Workers in Korea

  • Previous studies on the vast increase in suicide mortality in Southeast Asia have indicated that suicide rates increase in parallel with a rise in unemployment or during periods of economic recession. This paper examines the effects of economic recession on suicidal rates amongst agriculture, fisheries, and forestry workers in Korea. Monthly time-series gross domestic product (GDP) data were linked with suicidal rates gathered from the cause of death records between1993-2008. Data were analyzed using generalized additive models to analyze trends, while a polynomial lag model was used to assess the unconstrained time lag effects of changes in GDP on suicidal rate. We found that there were significant inverse correlations between changes in GDP and suicide for a time lag of one to four months after the occurrence of economic event. Furthermore, it was evident that the overall relative risks of suicide were high enough to bring about social concern.

Improving Patient Safety and Control in Operating Room by Leveraging RFID Technology

  • Patient safety has become a growing concern in health care. The U.S. Institute of Medicine (IOM) report "To Err Is Human: Building a Safer Health System" in 1999 included estimations that medical error is the eighth leading cause of death in the United States and results in up to 100,000 deaths annually. However, many adverse events and errors occur in surgical practice. Within all kinds of surgical adverse events, wrong-side/wrong-site, wrong-procedure, and wrong-patient adverse events are the most devastating, unacceptable, and often result in litigation. Much literature claims that systems must be put in place to render it essentially impossible or at least extremely difficult for human error to cause harm to patients. Hence, this research aims to develop a prototype system based on active RFID that detects and prevents errors in the OR. To fully comprehend the operating room (OR) process, multiple rounds of on site discussions were conducted. IDEF0 models were subsequently constructed for identifying the opportunity of improvement and performing before-after analysis. Based on the analysis, the architecture of the proposed RFID-based OR system was developed. An on-site survey conducted subsequently for better understanding the hardware requirement will then be illustrated. Finally, an RFID-enhanced system based on both the proposed architecture and test results was developed for gaining better control and improving the safety level of the surgical operations.

Disinfection Characteristics of Waterborne Pathogenic Protozoa Giardia lamblia

  • Giardia lamblia is a parasitic protozoa which is transmitted in the form of a cyst through untreated water and also treated drinking water. Since its presence in water has led to frequent outbreaks of giardiasis and death in many countries, the removal and disinfection of this protozoan cyst from the water supply are of great concern for public health. This study examined the disinfection characteristics of G. lamblia cysts isolated from a Korean patient with giardiasis. When using sodium hypochlorite including 5 or 10 ppm chlorine, the killing rate was initially rapid, however, the disinfection slowed down and a 3log reduction could not be achieved even after 2h. The disinfection effectiveness was also reduced at a lower temperature, thereby implying that the risk o a giardiasis outbreak will be higher in the winter season. A CT (concentration$.$time) curve was constructed based on the results with sodium hypochlorite for use in designing and predicting disinfection performance. The organic chlorination disinfectant SDIS (sodium dichloroisocyanurate) produced a lower pH and a much higher residual effect than sodium hypochlorite. The disinfection of cysts by SDIC continued steadily throughout 2h of contact, although the initial killing rate was lower than that with sodium hypochlorite.

  • Bisphosphonate(BP) significantly influence the hone remodeling process. Increasing number of patients with osteoporosis and metastatic bone disease need high dose or long term bisphosphonate therapy. Major adverse effect is jaw bone necrosis and now the bisphosphonate-related necrosis of the jaw(BRONJ) is the major concern of dental practitioner. This study intended to perform the review of the current understandings concerning the pathophysiology of BRONJ. Even though pathophysiological mechanism of BRONJ is not clearly elucidated but now suggested as largely two different concepts; so-called "inside-out" or "outside-in" theory. Inside-out theory emphasize the osteonecrosis of the jaw is the initial major event and subsequent infection and inflammation is the second event that accompanies bone exposure and death of overlying mucosa. However, in "outside-in" theory, infection or inflammation initiated by traumatized oral epithelium is the major event of BRONJ. Both theory would be partially explain BRONJ. Recent research reveals the immune modulating effect and influence of microcrack accumulation by BP. These findings and those of others might explain the missing part of outside-in theory.
