응급구조학과 학생의 생활스트레스에 따른 죽음인식태도와 생명윤리의식과의 상관관계에 관한 연구 (Correlation between death attitude and life ethics, according to the perception of life stress among paramedic department students)

  • Purpose: This descriptive study investigated the relationship between death awareness and life ethics awareness, according to life stress, among students in the department of paramedics. Methods: General characteristics were identified using means and frequency, and the differences between the two military models were analyzed using the χ2-test and t-test by dividing them into lower and higher groups based on the mean life stress score (99.76 points). Results: Those with high life stress had higher death awareness than those with low life stress at 114.11 points. In contrast, those with low life ethics experienced more severe life stress with a score of 145.61 points (t=-2.609, p=.010)(t=-2.953, p=.003). The death recognition attitude and bioethics according to the degree of living stress-showed a significant correlation between the low and high groups (r=.188, p=.043) (r=.201, p=.042). Conclusion: Paramedic students require education on how to cope with life stress. However, access to education is limited to people living in modern times. As a potential solution to this problem, observing videos on the Internet is recommended. Moreover, we suggest accessing Internet and smart phone applications for advertising/educational purposes.

중환자실 간호사의 좋은 죽음과 연명의료결정에 대한 인식이 임종간호태도에 미치는 영향 (Effect of the Awareness of a Good Death and Perceptions of Life-sustaining Treatment Decisions on Attitudes of Intensive Care Nurses toward Terminal Care)

  • Purpose : The purpose of this study was to identify the extent to which intensive care unit (ICU) nurses' perceptions of life-sustaining treatment decisions and "a good death" affect attitudes toward terminal care. Method : Participants included 109 ICU nurses from three university hospitals. Data were collected using structured questionnaires, and collected data were analyzed using a t-test, ANOVA, the $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and a multiple regression analysis (SPSS 24.0 program). Results : Perceptions of life-sustaining treatment decisions and a sense of closeness (a constituent for the awareness of "a good death") were positively correlated with terminal care attitudes. The factors affecting terminal care attitudes were a clinical career in ICU (${\beta}=.20$, p =.035), a sense of closeness(${\beta}=.19$, p =.041), and the perception of a life-sustaining treatment decision (${\beta}=.22$, p =.017). This finding indicates that more than 10 years of experience in ICU, a greater sense of closeness, and a higher view of life-sustaining treatment decisions results in more positive attitudes toward terminal care. The explanatory power of these variables on terminal care attitudes was 14% (F=6.84, p < .001, Adj $R^2=.140$). Conclusion : A sense of closeness and the perception of life-sustaining treatment decisions were identified as the factors affecting terminal care attitudes. Thus, various programs must be developed to raise awareness among ICU nurses of "a good death" and perceptions of life-sustaining treatment decisions.

간호대학생의 생명의료윤리의식, 품위 있는 죽음에 대한 태도 및 사전연명의료의향서에 대한 태도 (Biomedical Ethics Awareness and Attitudes toward Dignified Death and Advance Directives among Nursing Students)

  • Purpose: This study aimed conducted to investigate biomedical ethics awareness and attitudes toward dignified death and advance directives among nursing students. Methods: Data were collected through structured questionnaires from 222 nursing students with clinical practice experience, from November 7 to 23, 2015. Data were analyzed by independent t-tests, one-way ANOVA, Kruskal-Wallis test, and Pearson's correlation coefficients using SPSS WIN 22.0. Results: The scores for biomedical ethics awareness and attitudes toward dignified death were 2.89 and 3.15, respectively. Regarding attitudes toward advance directives (ADs), most students agreed with writing ADs. The main reason given for writing ADs is "I want to receive my treatment of choice." As for the range and explanation method for ADs, 45.7% of participants responded, "do not know well." Regarding willingness toward life sustaining treatment, "only pain control, no life sustaining treatment" was indicated by 83.4% and, "all information about symptoms and prognosis" by 91.9% of participants. For willingness to write ADs, 70.0% of participants responded "yes." Conclusion: Curriculum for nursing ethics should be included in clinical training courses to improve related courses and provide an opportunity to deal with practical problems, such as biomedical ethics, death with dignity, and ADs.

요양병원 근무자의 호스피스 완화돌봄 지식과 인식, 임종돌봄 태도, 죽음에 대한 인식이 임종돌봄 수행에 미치는 영향 (The Impact of Nursing Hospital Workers' Hospice·Palliative Care Knowledge and Awareness, End-of-Life Care Attitude and Death Awareness on Their End-of-Life Care Performance)

  • 목적: 본 연구는 요양병원 근무자를 대상으로 호스피스 완화돌봄에 대한 지식과 인식 및 임종돌봄 태도, 수행도, 중요도, 죽음에 대한 인식하고 임종돌봄 수행에 영향을 미치는 요인을 파악하기 위한 서술적 조사연구이다. 방법: K도에 소재한 의료기관평가인증을 받은 요양병원에 근무하는 113명의 근무자를 대상으로 자가 보고식 설문지를 이용하여 자료를 수집하였다. 호스피스 완화돌봄 지식, 호스피스 완화돌봄 인식, 임종돌봄 태도, 임종돌봄 수행도 및 중요도, 죽음에 대한 인식 등의 변수를 사용하였다. IBM SPSS 21.0을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient, Multiple regression로 분석하였다. 결과: 요양병원 근무자의 임종돌봄 수행도에 영향을 미치는 요인은 임종돌봄 중요도와 결혼유무이었으며, 이는 38.2%의 설명력을 보였다. 결론: 요양병원간호사의 임종돌봄 수행도를 향상시키기 위하여 임종돌봄 중요도에 대한 교육이 필요하며, 그들이 중요하다고 생각되는 것을 잘 수행할 수 있도록 효율적인 인력배치 등의 행정적인 방안을 수립하고 임종돌봄 수행도를 향상시킬 수 있는 교육프로그램 개발 및 효과검증을 하는 연구 수행을 제안하는 바이다.

수술실 의료진의 뇌사자 장기기증 태도 관련 요인 (Factors Affecting Attitudes toward Brain Death Organ Donation among Nurses and Doctors in an Operating Room)

  • Purpose: This study was conducted to identify the factors affecting the attitudes toward brain death organ donation among nurses and doctors in an operating room. Methods: A descriptive research was used. The participants included 90 nurses and 30 doctors who had experience of operating organ transplantation for brain death organ donation. Data were collected from March 12 to May 23, 2020 in the one tertiary general hospital. The outcome measures were perception and attitude of death and attitude towards brain death organ donation. Results: Attitudes toward brain death organ donation was influenced by type of occupation, intention of organ donation and attitude toward death. In addition, the explanatory power of the total variance was 52.1%. Conclusions: Based on the results, it is necessary to prepare an intervention to improve awareness of the brain death and the brain death organ donation.

The Meaning of Death for Korean in View of Novel and End Stage Cancer Patient

  • Every one experiences death one day, however no one can hows exactly what it is because people can not experience death until it comes, it is therefore impossible to judge correctly on the phenomenon of the death. On the whole, man experiences indirect death through the mass communications such as TV drama, fiction, magazine etc because those methods can easily access by every one. In addition to this, people usually acquire the negative awareness of death through the dramatic change of story like dying of cancer for dramatic effect by giving scare and fear to the cancers. The purpose of this study is to provide basic information on the spiritual care that enables the facing death patients to accept death as a part of life and divert hope from scare about after death by comparing and analyzing of two aspects of death meaning i.e, Korean fiction and the end stage cancer patients. Additionally, for medical staff to understand the facing death cancer patients by making to aware patients correctly and provide the better quality of care. The study was performed from September 28, 2002 to February, 28, 2003. The materials of this study were collected by direct data obtained from observation, interviews, note and diary of end stage of cancer patients and written materials acquired from Korean contemporary fiction. Participants of this study were 4 end stage cancer patients including 2 lung cancer patients, 1 liver cancer patient and 1 esophagus cancer patient. The methodology used in this study was divided into two types; Huberman & Miles methodology was used for fiction to find and categorize subject, and Colaizzi, one of phenomenological methodology was used for end stage cancer patients to find the major meaning, subject and categorization. 죽음준비교육이 일반인의 연명치료중단 및 호스피스·완화의료에 대한 인식에 미치는 효과 (Effects of Death Preparation Education on Awareness of Hospice Palliative Care and Withdrawing Life Sustaining Treatment in City Dwellers)

  • 목적: 본 연구에서는 일반인을 대상으로 죽음준비교육 프로그램이 연명치료중단 및 호스피스 완화의료에 대한 인식에 미치는 효과를 규명하기 위해 수행되었다. 방법: 본 연구의 죽음준비교육 프로그램은 총 5주에 걸쳐 진행되었으며 연구 대상자는 서울시 소재 일개구청과 대학교의 협력사업으로 진행한 죽음준비교육 참여자로 실험군 35명, 대조군 40명으로 총 75명이었고 사전 사후 측정은 자가 보고식 설문지를 이용하여 자료를 수집하였다. 결과: 죽음준비교육 후 호스피스 완화의료에 대한 인식은 실험군과 대조군에서 유의한 차이를 보이지 않았으나 연명치료중단 인식정도는 실험군이 대조군보다 유의하게 높은 결과를 보였다. 결론: 인간은 의미를 추구하는 존재로서 늘어나는 기대수명에 맞춰 삶의 질을 높이려는 욕구 또한 증가하고 있으며 자신의 삶의 마지막을 결정할 권리가 있는데, 본 연구결과는 인생의 마지막 순간에 자신의 삶을 결정하는 것에 대한 이론적 근거가 되었다고 본다. 또한, 일반인들에게 호스피스 완화의료에 대한 인식에 긍정적으로 영향을 줄 수 있는 죽음준비교육 프로그램 개발과 적절한 교육과정과 기간 및 교육방법에 대한 평가연구가 계속되어야겠다.

요양병원 간호사의 연명치료중단에 대한 태도에 미치는 영향 요인 (Factors affecting the attitude toward withdrawal of life-sustaining treatment of nurses working at long-term care hospitals)

  • Purpose: This study was a descriptive study to identify factors affecting the attitude toward life-sustaining treatment of nurses working at long-term care hospitals. Methods: Data were collected through structured questionnaires from August 2nd to 27th, 2019. Study participants consisted of 163 nurses who were working for at least 6 months from 7 long-term care hospitals in B and K city. Data were analyzed using t-test, ANOVA, Scheffe' test, Pearson's correlation coefficients, and hierarchical regression with SPSS WIN v 21.0. Results: There were significantly positive correlations between awareness of good death (r=.46, p<.001) and perception of patients' rights (r=.32, p<.001). The factors affecting participants' attitude toward life-sustaining treatment were awareness of good death (β=.35, p<.001) and their own view of death (β=.24, p=.001), which explained about 27.0% of the attitude toward life-sustaining treatment. Conclusion: Based on these results, it is necessary to develop nursing educational materials that can establish values for deaths, and cultivate legal and ethical knowledge related to attitude toward life-sustaining treatment. In addition, since the severity of a patient's condition varies and the characteristics of the institution vary depending on the type of hospital, a study is needed on the relevance of variables considering the hospital environment.

Attitudes towards Death, Perceptions of Hospice Care, and Hospice Care Needs among Family Members of Patients in the Intensive Care Unit

  • Purpose: This study aimed to identify the relationships among attitudes towards death, perceptions of hospice care, and hospice care needs as perceived by family members of patients in the intensive care unit (ICU). Methods: This study used a descriptive correlational method. A structured questionnaire was used to collect data from 114 participating families in the ICU at Dong-A University Hospital, from October 10 to November 1, 2019. The data were analyzed in terms of frequency, percentage, and mean and standard deviation. The t-test, one-way analysis of variance, and Pearson correlation coefficients were also conducted. Results: Perceptions of hospice care showed significant differences according to age (F=3.06, P=0.031) and marital status (t=3.55, P=0.001). However, no significant differences in attitudes towards death or hospice care needs were found. A significant positive correlation was found between perceptions of hospice care and hospice care needs (r=0.49, P<0.001). Conclusion: In order for families to recognize the need for hospice care and to receive high-quality palliative care at the appropriate time, it is necessary to increase public awareness of hospice care through various educational and awareness-raising efforts, thereby providing opportunities for families of terminally ill patients to request hospice care.

노인의 죽음 준비교육이 죽음의 불안도에 미치는 요인분석 연구 (A study on the influence of the preparative education on the Elderly's attitude for death)

  • This study attempt to analyze the influence of the preliminary education on the Elderly's attitude for death and to present basic data for the death-preliminary education. The data were collected by administerial the Questionnaire interview with 169 the elderly who was 200 the elderly over 60 year old in silver colleges. The Questions was consists the awareness recognition of death, character, attitudes toward for death. The statistical methods used for the analysis were t-test, factor analysis. The results were the follows. There was no statistically significant relations between the fear of death and the general characteristics the elderly, but the old women felt more anxiety than old men. Especially, more aged, unhealthy the elderly felt it more and the lower educated or the single felt it more severely. The change in the attitude for death: They attitude for death was considerably changed after the preliminary education. Fears and anxiety about death were more reduced and the inevitability of death was accepted positively. This result showed the influence of the preliminary death education had positive affliction of the elderly's attitude for death. Accordingly, with the practice of the preliminary education we can release the elderly from the fear for death and guide them to live meaningly.
