Nursing Need and Satisfaction of Patients in Hospice Ward (호스피스병동 입원 환자의 간호요구도와 간호만족도)

  • Purpose: This study was conducted to explore the nursing needs and satisfaction of patients who are hospitalized at hospice palliative care institutions. Methods: This study included 80 hospice patients who were in-patients of the hospice department of two general hospitals in S city. The collected data were analyzed by descriptive statistics, t-test, one-way ANOVA, and Pearson's correlation coefficients using the IBM SPSS 20.0 program. Results: The mean score of nursing needs and satisfaction were 3.58 (${\pm}0.31$) and 3.25 (${\pm}0.20$) respectively. Significant differences in hospice nursing needs and satisfaction were observed in terms of general characteristics including gender, education level, occupation, religion, support for medical expense and functional status. Significant positive correlation was found between the overall nursing needs and nursing satisfaction. Conclusion: Based on the results of this study, in order to improve the quality of hospice service, further study is necessary in a comprehensive manner to develop nursing intervention to meet physical, psychological/social, spiritual, and educational/referral nursing needs of hospice in-patients.

Factors Influencing Nursing Practices of Physical Restraints Use by nursing students (간호대학생의 신체적 억제대 사용 관련 간호실무 예측 요인)

  • Although the use of physical restraints is controversial, it is still commonly used in hospitalized patients to prevent patient accidents in clinical practice. It is, therefore, necessary to evaluate the knowledge, attitude, perception, and nursing practices related to the use of physical restraints. This was a cross-sectional descriptive study in which self-administered questionnaires were collected from 144 nursing students at two Universities. Descriptive statistics, including t-test, one-way ANOVA, Pearson correlation coefficients, and multiple regression, were applied to analyze the data collected. Our results reveal that the mean knowledge score of nursing students regarding the use of physical restraints was 11.55±2.81, the mean attitude score was 60.01±7.24, the mean perception score was 59.97±10.68, and the mean nursing practice score was 38.04±4.15. Significant positive correlations were observed between nursing practice and knowledge, attitude, and perception. Multiple regression analysis showed that the factors significantly related to nursing practice and the use of physical restraints were perception (β=.23, p=.005) and attitude (β=.21, p=.009), having a total explained variance of 13.0%. The findings from this study suggest that perception is the strongest predictor of the use of physical restraints in nursing practice. Thus, it is necessary to develop and apply an evidence-based educational intervention program to ensure that nursing students correctly understand the need for physical restraints and later use them appropriately in clinical practice.

Cross Validation of Attention-Deficit/Hyperactivity Disorder-After School Checklist

  • Objectives: This study aimed to evaluate the efficacy of the attention-deficit/hyperactivity disorder (ADHD)-After School Checklist (ASK) by comparing the results of the Comprehensive Attention Test (CAT) and Clinical Global Impression-Severity (CGI-S) Scale and then by calculating the area under the receiver operating characteristic (ROC) curve. Methods: We performed correlation analyses on the ASK and CAT results and then the ASK and CGI-S results. We created a ROC curve and evaluated performance on the ASK as a diagnostic tool. We then analyzed the test results of 1348 subjects (male 56.8%), including 1201 subjects in the general population and 147 ADHD subjects, aged 6-15 years, from kindergarten to middle school in Seoul and Gyeonggi province, South Korea. Results: According to the correlation analyses, ASK scores and the Attention Quotient (AQ) of CAT scores showed a significant correlation of -0.20--0.29 (p<0.05). The t-test between ADHD scores and CGI-S also showed a significant correlation (t=-2.55, p<0.05). The area under the ROC curve was calculated as 0.81, indicating good efficacy of the ASK, and the cut-off score was calculated as 15.5. Conclusion: The ASK can be used as a valid tool not only to evaluate functional impairment of ADHD children and adolescents but also to screen ADHD.

Do Saudi Nurses in Primary Health Care Centres have Breast Cancer Knowledge to Promote Breast Cancer Awareness?

  • Background: Breast cancer is one of the leading causes of death among cancer patients in Saudi Arabia. It is known that nurses play a key role in promoting breast cancer awareness among women in any society. Nurses in primary health care centres (PHCC) have more direct contact with general population. This study aimed to investigate nurse knowledge of breast cancer and practice of early screening in PHCC in Jeddah city. Methods: A cross-sectional study was conducted using a self-administered questionnaire to assess the general knowledge of breast cancer, risk factors, and practice of 210 PHCC nurses. Data were analysed using SPSS v.16. Results: The mean age of the PHCC Nurses was 36.9 (SD ${\pm}8.6$). Only 11% percent scored <50% of the total score for general epidemiological knowledge on breast cancer, about 35% scored <50% of the total score for breast cancer risk factors. Sixty seven percent scored >75% of the total score on breast cancer signs. Majority of nurses 62.8% practiced BSE, but only 4% practiced regularly every month. Some 28% practiced once per year. About 81% of the nurses had not had a clinical breast examination and only 14% had had a mammography. The results of the study failed to show any correlation between the knowledge scores with age, education, year of experience, family history of BC and marital status of the nurses. Conclusion:The results from the study reflect that there is a need to provide continuing nursing education programmes for PHCC nurses to improve their breast cancer knowledge and practice.

Effect of Allergic Rhinitis and Asthma on Oral Health

  • Background: This study aimed to investigate allergic rhinitis and the relationship between allergic diseases, such as asthma and allergic rhinitis, and dental caries and periodontal disease using the raw data from third year of the 6th Korean National Health and Nutrition Examination Survey. Methods: A total of 3,729 subjects aged over 30 years who underwent examination for allergic diseases and an oral health checkup were selected. The data were analyzed using SPSS IBM SPSS ver. 25.0 (IBM Corp., USA). Composite sample cross correlation and composite sample logistic regression analyses were performed using the composite sample general linear model. Results: On examining the socioeconomic characteristics of the subjects suffering from allergic disease and the relationship between allergic disease and oral health, allergic rhinitis was more frequently found in younger subjects with a higher level of education. Periodontal disease and dental caries were more frequent among female, older age groups, lower income earners, and subjects with a lower level of education (p<0.05). On examining the relationship between the oral health characteristics of the subject and allergic diseases and oral health, allergic rhinitis was more common in subjects with a good oral health status perception than those who answered "bad" to the oral health status question. The community periodontal index of treatment needs (CPITN) score was higher in subjects who answered "poor" to the oral health status question, lower frequency of brushing, and higher in subjects using secondary oral hygiene products; the decayed, missing and filled teeth (DMFT) index was higher in subjects with a perception of poor oral health status (p<0.05). The DMFT index was high in the asthma group, and the CPITN score was high in the group who answered "no" to allergic rhinitis. Conclusion: There is a relationship between asthma and allergic rhinitis and the DFMT index and CPITN score. Corresponding oral programs for allergic patients need to be developed.

Does Clinical Experience Help Oncology Nursing Staff to Deal with Patient Pain Better than Nurses from other Displines? Knowledge and Attitudes Survey Amongst Nurses in a Tertiary Care in Malaysia

  • Background: Successful implementation of pain management procedures and guidelines in an institution depends very much on the acceptance of many levels of healthcare providers. Aim: The main purpose of this study was to determine the level of knowledge and attitudes regarding pain among nurses working in tertiary care in a local setting and the factors that may be associated with this. Materials and Methods: This cross-sectional research study used a modified version of the Nurses' Knowledge and Attitudes Survey (NKAS) regarding pain. Basic demographic data were obtained for further correlation with the level of pain knowledge. Results: A total of 566 nurses, 34 male and 532 female, volunteered to participate in this study. The response rate (RR) was 76%, with an overall mean percentage score of $42.7{\pm}10.9$ (range: 5-92.5). The majority of participants were younger nurses below 40 years of age and more than 70% had worked for less than 10 years ($6.6{\pm}4.45$). Up to 92% had never had any formal education in pain management in general. The total mean score of correct answers was $58.6{\pm}9.58$, with oncology nursing staff scoring a higher percentage when compared with nurses from other general and critical care wards ($63.52{\pm}9.27$, p<0.045). Only 2.5% out of all participants obtained a score of 80% or greater. The majority of the oncology nurses achieved the expected competency level (p<0.03). Conclusions: The present findings give further support for the universal concern about poor knowledge and attitudes among nurses' related to the optimal management of pain. The results reflected that neither number of years working nor age influenced the level of knowledge or attitudes of the practising nurses. Oncology nursing staff consistently scored better than the rest of the cohort. This reflects that clinical experience helped to improve attitudes and knowledge concerning better pain management.

Maternal Perception of Children's Weight, Maternal Body Shape Satisfaction, and Maternal Feeding Styles in Preschool-Aged Children (학령전기 아동 체중에 대한 어머니의 인식과 자기 체형만족도 및 식이관리방식)

  • Purpose: The purpose of this study was to explore the correlates of maternal perception (identification and satisfaction) of children's weight, maternal body shape satisfaction, and maternal feeding styles in Korean preschool-aged children. Methods: A cross-sectional survey design was utilized. Participants consisted of 287 pairs of preschool-aged children (3-6 years) and their mothers. Data were analyzed by ${\chi}^2$-test, Fisher's exact test, ANOVA, and the Pearson correlation coefficient. Results: Among the 287 mothers, 47.7% underestimated their children's weight, while 15.7% overestimated it. 46.7% of the mothers wished their children weighed more, while 11.1% of them wished their children weighed less. The mean score of maternal body shape satisfaction was $83.75{\pm}28.77$. The mean score of parent-centered feeding styles was $2.95{\pm}0.54$, and the mean score of child-centered feeding styles was $3.33{\pm}0.42$. There were statistically significant correlations between maternal identification of children's weight and children's body mass index (BMI) (r=-.366, p<.001). In addition, there were statistically significant correlations between maternal satisfaction of children's weight and children's BMI (r=-.484, p<.001), maternal BMI (r=-.126, p=.033), and maternal body shape satisfaction (r=-.127, p=.031). Conclusion: The results of the study suggest that intervention programs for mothers to develop more accurate perception of their child's weight should be established.

Body Composition of Male College Students by Standard Guideline for Korean Dietary (식생활 실천 지침에 따른 남자 대학생의 체성분 분석)

  • To study the correlation with increasing body fat mass for the Standard Guideline for Korean Dietary Life, a nationwide, cross-sectional survey was performed with 178 male college students in december 2005. Among the student subjects, 76 were analyzed for body protein mass, body mineral mass, body fat mass, percent body fat, waist-hip ratio(WHR), body mass index(BMI) and obesity degree using Inbody 3.0(Biospace Co, Seoul, Korea). Body protein mass was at a high level of 92.1%, body mineral mass was at a low level of 94.7%. Body fat mass was at a high level of 28.9%, percent body fat was at a high level of 37.3%, WHR was at a high level of 31.6%, and obesity degree was at a high level of 38.2%, BMI was at a high level of 24.3% and BMR was observed at a high level of 41.8%. WHR was correlated with '7. Prepare food proper amount sanitarily.' and obesity degree, while BMl was were correlated with '4. Increase movement and eat proper amount.' positively. BMR was correlated with '6. Enjoy 3 meal a day regularly.' ArmCircle was correlated with '4. Increase movement and eat proper amount.' and fitness score was corrected with '4. Increase movement and eat proper amount', and '8. Enjoy Korean rice food style.' positively. Body protein mass and body mineral mass were negatively correlated with fruit, ArmCircle was positively correlated with fish, obesity degree was positively correlated with soup and BMI and ArmCircle were positively correlated with sweet. '8. Enjoy Korean rice food style.' was negatively significantly correlated with overweight, over percentage body fat, obesity degree > 120 and BMI. Male students were not practicing the Standard Guideline for Korean Dietary Life well with an average score of 37.7%. However, male students who are within the normal range of body composition analysis value have to notice that over range of weight, protein, percentage body fat, WHI and obesity degree was observed above average score of 30%.

Relationship between Visceral Adipose Tissue and Self-Esteem in Korean Pre-menopausal Obese Women (폐경전 비만여성에서 내장지방과 자존감의 연관성)

  • Objectives Obesity is not only a matter of accumulation of adipose tissue but also a projection of self-cognition. This study examined the association between low self-esteem and visceral obesity; visceral adipose tissue (VAT) and visceral adipose tissue /subcutaneous adipose tissue ratio (VSR). Methods This cross-sectional study was performed in pre-menopausal obese(BMI $\geq\;25\;kg/m^2$) women in Seoul, from 2007 to 2008 (n=54). Simple anthropometry including BMI and waist circumference and Computed Tomography (CT) including VAT and VSR were done. To measure self-esteem, Rosenberg self-esteem scale (SES) questionnaire was administered. Subjects were given written consent and this study was performed under the permission of institutional review board of Kyung-Hee East-west Neo Medical Center. Results There was a significant relationship self esteem (SES score) with visceral obesity (VAT and VSR). 1. SES was correlated with VAT (r=-0.377, p<0.01) and VSR (r= -0.400, p<0.01) significantly by Pearson Correlation. 2. VAT and VSR could be predicted from SES by Simple linear regression. VAT = -1.701 ${\times}$ (SES score) +161.191 ($R^2$=0.142) VSR = -1.09${\times}$$10^{-2}$ ${\times}$ (SES score) +0.858 ($R^2$=0.160) Conclusions This study proves that low self-esteem might contribute to visceral obesity in Korean pre-menopausal obese women. Self-esteem and psychological factor should be considered in treatment of visceral obesity in adult-women.

Its Relationship with Symptoms Index, Self Efficacy and Family Support in Hemodialysis Patients (혈액투석환자의 증상지수, 자기효능감과 가족지지와의 관계)

  • This study was conducted to investigate the prevalence, severity or impact of symptoms in hemodialysis patients and its relationship with self efficacy and family support to identify factors influencing their symptoms. A cross-sectional design was used and the subjects were 243 patients undergoing hemodialysis at local units. The patients were Dialysis Symptom Index (DSI), a questionnaire about the presence and severity of symptoms. The data were analyzed by descriptive statistics, independent t-test, Pearson coefficient correlation and multiple regression. There were significant differences in the total score of symptoms according to age and duration of hemodialysis. Symptoms score had significant negative correlations with family support (r=-.35, p<.001) and self-efficacy (r=-.16, p=.006). Family support explained 16.0% of the variance in symptoms score. Hemodialysis patients experience many distressing symptoms, which have negative effects on quality of their life. Promoting family support can help to reduce and control distressing symptoms in hemodialysis patients.
