Triangular Resection of the Upper Lateral Cartilage for Middle Vault Deviation

  • Objectives. Middle vault deviation has a significant effect on the aesthetic and functional aspects of the nose, and its management continues to be a challenge. Spreader graft and its modification techniques have been focused, but there has been scarce consideration for removing surplus portion and balancing the upper lateral cartilage (ULC). This study aimed to report the newly invented triangular-shaped resection technique ("triangular resection") of the ULC and to evaluate its efficacy for correcting middle vault deviation. Methods. A retrospective study included 17 consecutive patients who presented with middle vault deviation and underwent septorhinoplasty by using triangular resection at a tertiary academic hospital from February 2014 and March 2016. Their outcomes were evaluated pre- and postoperatively including medical photographs, acoustic rhinometry and subjective nasal obstruction using a 7-point Likert scale. Results. The immediate outcomes were evaluated around 1 month after surgery, and long-term outcomes were available in 12 patients; the mean follow-up period was 9.1 months. Nasal tip deviation angle was reduced from $5.66^{\circ}$ to $2.37^{\circ}$ immediately (P<0.001). Middle vault deviation also improved from $169.50^{\circ}$ to $177.24^{\circ}$ (P<0.001). Long-term results were $2.49^{\circ}$ (P=0.015) for nasal tip deviation and $178.68^{\circ}$ (P=0.002) for middle vault deviation. The aesthetic outcome involved a complete correction in eight patients (47.1%), a minimally visible deviation in seven patients (41.2%) and a remaining residual deviation in two patients (11.8%). Pre- and postoperative minimal cross-sectional areas (summation of the right and left sides) were 0.86 and 1.07, respectively (P=0.021). Fifteen patients answered about their nasal obstruction symptoms and the median symptom score had alleviated from 6.0 to 3.0 (P=0.004). Conclusion. Triangular resection of the ULC is a simple and effective method for correcting middle vault deviation and balancing the ULCs without complications as internal nasal valve narrowing.

  • This study evaluates the impact of an information security policy sanction, a perceived threat, and the perception of the information security climate on a compliance behavioral intention. The research method was structured with a cross-sectional study design for the prospect and goal orientation. The variables used in the analysis are information security policy sanction, perceived threat, perception of information security climate, and compliance behavioral intention. Progress in this research consists of measuring the prospect and goal orientation, and then measuring the four variables. As a result, the prospect had a significant effect on the perception of the information security climate, and it was found that the influence of the gain-based condition was greater than the loss-based condition. Goal orientation had a significant effect on the information security policy sanction, the perceived threat, and the compliance behavioral intention, and the influence of the development-based condition was greater than the stability-based condition. Both prospect and goal orientation had an interactive effect on the compliance behavioral intention. The exploration model was verified as a mediation model. In addition, the discussion includes the appropriate implications for information security based on these research results.

Juvenile Polyps in Bangladeshi Children and Their Association with Fecal Calprotectin as a Biomarker

  • Purpose: Colonoscopy is considered the most reliable method for the diagnosis of juvenile polyps. However, colonoscopic screening is an invasive and expensive procedure. Fecal calprotectin (FCP), a marker of intestinal inflammation, has been shown to be elevated in patients with polyps. Therefore, this study aimed to evaluate FCP as a screening biomarker for the diagnosis of juvenile polyps. Methods: This cross-sectional, observational study was conducted at the Pediatric Gastroenterology and Nutrition Department, Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh. For children with polyps, colonoscopic polypectomy and histopathology were performed. FCP levels were analyzed before and 4 weeks after polypectomy in all patients. Information was recorded in a datasheet and analyzed using the computer-based program SPSS. Results: The age of the children was between 2.5 and 12 years. Approximately 93% of the polyps were found in the rectosigmoid region. Children with juvenile polyps had elevated levels of FCP before polypectomy that subsequently normalized after polypectomy. The mean FCP levels before and after polypectomy were 277±247 ㎍/g (range, 80-1,000 ㎍/g) and 48.57±38.23 ㎍/g (range, 29-140 ㎍/g) (p<0.001), respectively. The FCP levels were significantly higher in patients with multiple polyps than in those with single polyps. Moreover, mean FCP levels in patients with single and multiple polyps were 207.6±172.4 ㎍/g and 515.4±320.5 ㎍/g (p<0.001), respectively. Conclusion: Colonic juvenile polyps were found to be associated with elevated levels of FCP that normalized after polypectomy. Therefore, FCP may be recommended as a noninvasive screening biomarker for diagnosis of colonic juvenile polyps.

An Experimental Study on the Performance of Expandable Steel Pipe Pile (확장형 강관말뚝의 성능에 대한 실험적 연구)

  • Expandable steel pipe piles are installed by inserting expansion equipment to increase the cross-sectional area of steel pipes, which can improve the pile performance compared to micro-piles. In this paper, a hydraulic expansion device was developed to expand steel pipe piles in practice. A series of laboratory and field tests were conducted to verify the performance of the developed expansion device to expand steel pipes. The expansion capability and expandable range was evaluated by measuring the strain and expansion time at the maximum pressure of the hydraulic expansion device. The thinner steel pipe, the larger strain but longer expansion time required in the test. For example, the 4.0-mm-thick steel pipe showed strain reduction by 30% and a decrease in the required expansion time by 40% compared to the 2.9-mm-thick steel pipe. In addition, in-situ expansion tests were performed to verify the expandability of steel pipes under the ground, and the exhumed specimen showed clear expanded sections. The structural integrity was determined by comparing the material performance the original and expanded specimens.

  • EHA (Electro-Hydrostatic Actuator) is more energy efficiency than a centralized hydraulic system. In particular, the EHA used for aircraft has a redundant design in preparation for failure scenario. Also, due to the aircraft's internal space limitation, the actuator's length must be optimized. Therefore, a series configuration of double rod and single rod cylinder is advantageous. However, due to the asymmetry of the cross-sectional area of the piston, the force fighting phenomenon between the two cylinder areas occurs during redundant operation with a general control system. Analysis of High-Temperature Corrosion of Heat Exchanger Tubes in Biomass Circulating Fluidized Bed Boiler (바이오매스 순환유동층 보일러의 열교환기 고온 부식 특성)

  • This paper presents the research results of analyzing the high-temperature corrosion characteristics of three currently commercialized heat exchanger tube materials under actual operating conditions of a biomass power plant. In order to precisely analyze the high-temperature corrosion characteristics of these materials, a high-temperature corrosion evaluation device was installed in the power plant equipment, which allows for adjusting the surface temperature of the heat exchanger tubes. Experiments were conducted for approximately 300 hours under various temperature and operating conditions. In this study, the commercialized heat exchanger tube materials used were SA213T12, SA213T22, and SA213T91 alloys. In order to objectively analyze the high-temperature corrosion characteristics of each material, an international standard-based process to remove corrosion products was applied to obtain the weight change of the specimens, and the average thickness loss and corrosion rate were derived. Thus, the high-temperature corrosion results for each condition were quantitatively compared and analyzed. In addition, in order to increase the reliability of the high-temperature corrosion evaluation method introduced in this study, the surface and cross-sectional corrosion of the specimens were confirmed by using scanning electron microscopy and energy-dispersive X-ray analysis. Based on these analysis results, it was found that the corrosion resistance of the commercial heat exchanger materials increases as the content of chrome and nickel in the composition increases. Additionally, it was found that the corrosion phenomenon is rapidly accelerated as the surface temperature increases. Finally, the replacement period (lifetime) of the heat exchanger tubes under each condition could be inferred through this study.

The Relationship among Attitude toward DNR Orders, Depression and Self-esteem in the Elderly (노인의 심폐소생술 금지(DNR)에 대한 태도와 우울 및 자아존중감과의 관계)

  • This cross-sectional descriptive study was performed to investigate the relationship among attitude toward DNR orders, depression, and self-esteem in the elderly. Method: The participants of this study were 99 elderly individuals who were hospitalized in four university hospitals in Seoul and Kangwon-do from October 1, 2006 to October 21, 2006. The data were collected using self-administered questionnaires. Results: The mean scores were 3.99 for attitude toward DNR orders(range of 1-5), 6.64 for depression(range of 1-15), and 26.83 for self-esteem(range of 10-40). Self-esteem was significantly correlated with attitude toward DNR orders(r=.200, p=.047). About half of the participants(49.5%) responded that the proper time for obtaining DNR consent was when they were healthy and could express their own intentions and make the decision by themselves. Most of the participants showed a positive attitude toward DNR orders. The participants preferred to make the DNR decision when they were healthy. Therefore, health care providers working with the elderly should try to discuss the DNR decision with their patients when they are conscious and able to make the DNR decision by themselves rather than leaving the decisions up to the patient's family members

Effects of Neuromuscular Electrical Stimulation on Muscle Wasting with Cancer Patient: A Systematic Review (신경근전기자극치료가 암 환자의 근육 손실에 미치는 영향: 체계적 문헌고찰)

  • Objectives The purpose of this study is to systematically review the effects of Neuromuscular Electrical Stimulation treatment on muscle wasting in cancer patients. Methods Randomized controlled trials (RCTs) were searched from seven online databases (PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure, Oriental Medicine Advanced Searching Integrated System, Korean studies Information Service System, Research Information Sharing Service. The selected RCTs were evaluated for methodological quality through the Cochrane RoB. Results A total of 126 articles were identified, and 4 randomized controlled trials were selected for systematic review. In one study, it was found that there were statistically significant improvements in the Health-Related Quality of Life (FACTH&N total score, p<0.001). Additionally, significant effects were observed in measurements that represented the size of the quadriceps muscle (cross-sectional area of Vastus lateralis and Rectus femoris, p=0.004), maximum muscle strength, the twitch response of resistance muscles, and voluntary activation (p<0.001). However, no significant differences were observed between the intervention and control groups in terms of quadriceps muscle strength in two other studies (p>0.05). Lastly, while one study showed no significant differences in muscle fiber characteristics between the two groups, it did report significant improvements in measurements related to mitochondria within muscle tissue and muscle strength in the intervention group (p<0.05). Conclusions Neuromuscular electrical stimulation can be a method used to improve muscle strength in muscle wasting of cancer patients, but it is difficult to see its effects as significant compared to other treatments.

Prevalence and Associated Factors of Vertebral Fractures in Children with Chronic Liver Disease with and without Liver Transplantation

  • Purpose: To evaluate the prevalence of vertebral fracture (VF) in children with chronic liver disease (CLD) with and without liver transplantation (LT) and to determine the associated factors. Methods: A cross-sectional study was conducted. Patients aged 3-21 years with CLD both before and after LT were enrolled in the study. Lateral thoracolumbar spine radiographs were obtained and assessed for VF using Mäkitie's method. Clinical and biochemical data were collected. Results: We enrolled 147 patients (80 females; median age 8.8 years [interquartile range 6.0-11.8]; 110 [74.8%] in the LT group and 37 [25.2%] in the non-LT group). VF was identified in 21 patients (14.3%): 17/110 (15.5%) in the LT group and 4/37 (10.8%) in the non-LT group (p=0.54). Back pain was noted in only three patients with VF. In the univariate analysis, a height z-score below -2.0 (p=0.010), pre-LT hepatopulmonary syndrome (p=0.014), elevated serum direct and total bilirubin levels (p=0.037 and p=0.049, respectively), and vitamin D deficiency at 1-year post-LT (p=0.048) were associated with VF in the LT group. In multivariate analysis, height z-score below -2.0 was the only significant associated factor (odds ratio, 5.94; 95% confidence interval, 1.49-23.76; p=0.012) for VF. All VFs in the non-LT group were reported in males. Conclusion: In children with CLD, VF is common before and after LT. Most patients with VF are asymptomatic. Screening for VF should be considered in patients with a height z-score below -2.0 after LT.

Research trends in Journal of The Korean Society for School & Community Health Education on Vulnerable Populations from 2000 to 2023: Based on the elderly and people with disabilities (한국학교·지역보건교육학회지 2000년~2023년 취약 계층 연구 동향: 노인과 장애인을 중심으로)

  • Purpose: This study aims to identify research trends in papers related to the elderly and the disabled published in the journal of Korean society for school & community health education from 2000 to 2023 and seek the direction of the academic development of this journal in the future. Method: A total of 26 articles related to the elderly and the disabled, who are vulnerable groups, were analyzed by year by analyzing the specific subjects, research themes, research design, data collection methods, and keywords of papers published from 2000 to 2023. Results: Looking at the research subjects, studies on the elderly (18 studies) accounted for a larger proportion than studies on the disabled (8 studies). Research themes in the field of healthy living practices for the elderly (44.4%) and research in the field of mental health management (37.5%) for the disabled accounted for a high proportion. The design of research were mostly quantitative and cross-sectional studies. Data collection is mostly based on secondary data. In studies targeting the elderly, keywords appeared in the following order: 'Health' and 'Elderly'. And research targeting the disabled appeared in the following order: 'Disabilities', 'Health', and 'COVID-19'. Additionally, research on the elderly and the disabled has recently shown an increasing trend. Conclusion: Research on the elderly and the disabled has been conducted in line with the purpose of the Korean society for school & community health education, However, In terms of quantitative expansion and qualitative research, research themes, research designs, and data collection methods must be diversified. Methods, public perception. Additionally, research on vulnerable groups that fit the public health promotion and health education paradigm is needed.
