자궁 체부암의 방사선 치료 (The Role of Radiation Therapy in Adenocarcinoma of Endometrium)

  • 1975년 5월부터 1987년 10월까지 38명의 자궁 체부암환자가 서울대학교 병원 치료방사선과에서 근치적 방사선 치료를 받았다. 이 중 32명은 수술 후 방사선 치료를, 1명은 수술 전 방사선 치료를, 5명은 방사선 치료만을 받았다. 전체 환자에서의 비만, 52세 이후의 늦은 월경, 만삭 임신이 없었던 경우, 당뇨병, 고혈압 등의 빈도는 정상 한국 여성들에서의 빈도보다 높았다. 그 중 비만, 당뇨병, 늦은 월경 등의 빈도는 유의하게 높았다. 전체 환자의 5년 생존율은 $75.6\%$였고 FIGO 병기 I, II기 및 III 기에서의 생존율은 각각 $90.0\%,\;80.8\%$$44.4\%$였고 조기병기 (I, II기)와 진행병기 (III기)의 생존율은 Cox의 다변량 분석법으로 분석한 결과 유의한 차이가 있었다. 자궁 체부암으로 사망한 것이 확인된 8예 중 원발병소의 치유실패나 재발로 인한 경우가 2예, 원격전이로 인한 경우가 3예, 두 가지 동반된 경우가 3예였다. 자궁 체부암 I, II기는 방사선 치료와 수술등의 국소적 치료방법으로 중증의 부작용없이 양호한 치료 성적을 얻었으나 III기에서는 더 적극적인 치료가 필요할 것으로 생각된다.

Prognostic Value of Lymph Node Ratios in Node Positive Rectal Cancer Treated with Preoperative Chemoradiation

  • Background: To investigate the impact of the lymph node ratio (LNR) on the prognosis of patients with locally advanced rectal cancer undergoing pre-operative chemoradiation. Methods: Clinicopathologic and follow up data of 128 patients with stage III rectal cancer who underwent curative resection from 1996 to 2007 were reviewed. The patients were divided into two groups according to the lymph node ratio: LNR ${\leq}$ 0.2 (n=28), and >0.2 (n=100). Kaplan-Meier and the Cox proportional hazard regression models were used to evaluate the prognostic effects according to LNR. Results: Median numbers of lymph nodes examined and lymph nodes involved by tumour were 10.3 (range 2-28) and 5.8 (range 1-25), respectively, and the median LNR was 0.5 (range, 0-1.6). The 5-year survival rate significantly differed by LNR (${\leq}$ 0.2, 69%; >0.2, 19%; Log-rank p value < 0.001). LNR was also a significant prognostic factor of survival adjusted for age, sex, post-operative chemotherapy, total number of examined lymph nodes, metastasis and local recurrence (${\leq}$ 0.2, HR=1; >0.2, HR=4.8, 95%CI=2.1-11.1) and a significant predictor of local recurrence and distant metastasis during follow-up independently of total number of examined lymph node. Conclusions: Total number of examined lymph nodes and LNR were significant prognostic factors for survival in patients with stage III rectal cancer undergoing pre-operative chemoradiotherapy.

Colorectal Cancer Patient Characteristics, Treatment and Survival in Oman - a Single Center Study

  • Background: Colorectal cancer is the most common gastrointestinal cancer in Oman with an increasing incidence. We here report the presenting features, treatment outcomes and survival in a University hospital in Oman and compare our data with regional and international studies. Materials and Methods: Medical records of patients with colorectal cancer were reviewed retrospectively between June 2000 and December 2013 and were followed until June 2014. Results: A total of 162 patients were diagnosed with colorectal cancer. The majority were males (58.6%), with a median age of 56 years. Rectum was involved in 29.6% of patients, followed by ascending and sigmoid colon. The majority of patients had stage III (42.6%) and stage IV (32.7%) disease at presentation. K-Ras status was checked for 79 patients, and 41 (51.9%) featured the wild type. Median relapse free survival was 22 months. Median overall survival for all patients was 43 months. Observed 5 year overall survival (OS) for stages I, II and III was 100%, 60% and 60% respectively. On Log rank univariate analysis, age, BMI, diabetes, hypertension, metformin use, stage, clinical nodal status for rectal cancer, pathological T and nodal status, site of metastasis, surgical intervention, chemotherapy, radiotherapy, chemotherapy regimen, no of cycles of chemotherapy, response, RFS, site of recurrence and administration of $2^{nd}$ line chemotherapy were significant factors affecting OS. On Cox regression multivariate analysis none of the factors independently affected the OS. Conclusions: The majority of patients present with advanced disease and at young age. The survival rates are comparable to the published regional and international literature.

오약순기산가미방의 항염작용과 Collagen 유발 관절염의 발생억제 및 치료효과 (Anti-inflammation, Anti-Development and Curative Effects of Oyaksunki sangamibang on the Collagen-Induced Arthritis in Rats)

  • Objectives : This study was carried out to investigate the anti-inflammation, anti-development and curative effects of Oyaksunki-sangamibang (OSKM) on collagen-induced arthritis in Wistar rats and ICR mice. Materials & Methods : D experiment part II, the inhibitory effects of nitric oxide synthesis, pro-inflammatory cytokines, and cyclooxygenase were studied. In experiment part II, paw eduma volume and thickness of ankle joint were measured at 0, 10, 15, and 20 days after immunization. The incidence and arthritis score were evaluated 14 days after immunization, At 15 days after immunization, serum $TNF-\alpha$ was analyzed. In experiment part III paw edema volume and thickness of ankle joint were measured at 0, 10, and 15days after treatment. At 15 days after treatment, serum $TNF-\alpha$ was analyzed. Results : In experiment part I: 1. Nitric oxide synthesis ·md pro-inflammatory cytokines were inhibited significantly by OSKM extract. 2. Cyclooxygenase 2 (COX-2) was inhibited by OSKM extract. In experiment part II: Paw edema volume, thickness of ankle joint and serum $TNF-\alpha$ level of the teated group were significantly decreased compared with the control group at 20 days after immunization. In experiment part III: Incidence of arthritis was $70\%$. OSKM-treated group had no significant change on paw edema volume, thickness of ankle joint and serum $TNF-\alpha$ level. Conclusions : These results indicated that OSKM has anti-inflammation effects on the ICR mouse, and higher inhibitory effects on the onset but lower inhibitory effects on the progression of collagen-induced arthritis in rats.

An Outbreak of Trichinosis with Molecular Identification of Trichinella sp. in Vietnam

  • The 5th outbreak of trichinosis occurred in a mountainous area of North Vietnam in 2012, involving 24 patients among 27 people who consumed raw pork together. Six of these patients visited several hospitals in Hanoi for treatment. Similar clinical symptoms appeared in these patients within 5-8 days after eating infected raw pork, which consisted of fever, muscle pain, difficult moving, edema, difficult swallowing, and difficult breathing. ELISA revealed all (6/6) positive reactions against Trichinella spiralis antigen and all cases showed positive biopsy results for Trichinella sp. larvae in the muscle. The larvae detected in the patients were identified as T. spiralis (Vietnamese strain) by the molecular analysis of the mitochondrial cytochrome c oxidase subunit III (cox3) gene.

Multivariate Analysis of Prognostic Factors in Male Breast Cancer in Serbia

  • Background: The aim of this study was to analyze the demographic and clinical characteristics of male breast cancer patients in Serbia, and furthermore to determine overall survival and predictive factors for prognosis. Materials and Methods: In the period of 1996-2006 histopathological diagnosis of breast cancer was made in 84 males at the Institute for Oncology and Radiology of Serbia. For statistical analyses the Kaplan-Meier method, long-rank test and Cox proportional hazards regression model were used. Results: The mean age at diagnosis with breast cancer was $64.3{\pm}10.5$ years with a range from 35-84 years. Nearly 80% of the tumors showed ductal histology. About 44% had early tumor stages (I and II) whereas 46.4% and 9.5% of the male exhibitied stages III and IV, respectively. Only 7.1% of male patients were grade one. One-fifth of all patients had tumors measuring ${\leq}2cm$, and 14.3% larger than 5 cm. Lymph node metastasis was recorded in 40.4% patients and 47% relapse. Estrogen and progesterone receptor expression was positive in 66.7% and 58.3%, respectively. Among 14.3% of individuals tumor was HER2 positive. About two-thirds of all male patients had radical mastectomy (66.7%). Adjuvant hormonal (tamoxifene), systematic chemotherapy (CMF or FAC) and adjuvant radiotherapy were given to 59.5%, 35.7% and 29.8% patients respectively. Overall survival rates at five and ten years for male breast cancer were 55.0% and 43.9%, respectively. According to the multivariate Cox regression predictive model, a lower initial disease stage, a lower tumor grade, application of adjuvant hormone therapy and no relapse occurrence were significant independent predictors for good overall survival. Conclusions: Results of the treatment would be better if disease is discovered earlier and therefore health education and screening are an imperative in solving this problem.

ABO Blood Group, Epstein-Barr virus Infection and Prognosis of Patients with Non-metastatic Nasopharyngeal Carcinoma

  • Background: A prior study showed blood type A/AB to be associated with an increased risk of nasopharyngeal carcinoma (NPC) compared to subjects with blood type O. However, the relationship between ABO blood groups and prognosis of NPC patients is still questionable. In addition, whether Epstein-Barr virus (EBV) infection is associated with prognosis of NPC patients with different ABO blood groups is unclear. Materials and Methods: We conducted univariate and multivariable Cox regression analyses based on a consecutive cohort of 1,601 patients to investigate the above issues. Results: There was no significant difference in overall survival (OS) between different ABO blood groups (p=0.629), neither between A vs. non-A blood groups (p=0.895) nor AB vs. non-AB blood group (p=0.309) in univariate analyses and after adjusting for other factors. Interaction tests revealed that high immunoglobulin A against Epstein-Barr virus viral capsid antigen (VcA-IgA) level was associated with a favorable prognosis in male patients with UICC stage II disease who had an A blood type (p=0.008), compared with those with non-A blood type. In addition, male patients with an A blood group with a high blood lymphocyte level showeda tendency towards better survival in UICC stage III (p=0.096). Conclusions: ABO blood group status is not associated with the prognosis of patients with NPC. Additionally, blood group A male NPC patients with high VcA-IgA level or high blood lymphocyte counts might be correlated with a favorable prognosis in UICC stage II or III, respectively.

Risk factors for cancer-specific survival in elderly gastric cancer patients after curative gastrectomy

  • BACKGROUND/OBJECTIVES: This study aimed to investigate cancer-specific survival (CSS) and associated risk factors in elderly gastric cancer (EGC) patients. SUBJECTS/METHODS: EGC patients (≥ 70 yrs) who underwent curative gastrectomy between January 2013 and December 2017 at our hospital were included. Clinicopathologic characteristics and survival data were collected. Receiver operating characteristic (ROC) analysis was used to extract the best cutoff point for body mass index (BMI). A Cox proportional hazards model was used to determine the risk factors for CSS. RESULTS: In total, 290 EGC patients were included, with a median age of 74.7 yrs. The median follow-up time was 31 (1-77) mon. The postoperative 1-yr, 3-yr and 5-yr CSS rates were 93.7%, 75.9% and 65.1%, respectively. Univariate analysis revealed risk factors for CSS, including age (hazard ratio [HR] = 1.08; 95% confidence interval [CI], 1.01-1.15), intensive care unit (ICU) admission (HR = 1.73; 95% CI, 1.08-2.79), nutritional risk screening (NRS 2002) score ≥ 5 (HR = 2.33; 95% CI, 1.49-3.75), and preoperative prognostic nutrition index score < 45 (HR = 2.06; 95% CI, 1.27-3.33). The ROC curve showed that the best BMI cutoff value was 20.6 kg/m2. Multivariate analysis indicated that a BMI ≤ 20.6 kg/m2 (HR = 2.30; 95% CI, 1.36-3.87), ICU admission (HR = 1.97; 95% CI, 1.17-3.30) and TNM stage (stage II: HR = 5.56; 95% CI, 1.59-19.43; stage III: HR = 16.20; 95% CI, 4.99-52.59) were significantly associated with CSS. CONCLUSIONS: Low BMI (≤ 20.6 kg/m2), ICU admission and advanced pathological TNM stages (II and III) are independent risk factors for CSS in EGC patients after curative gastrectomy. Nutrition support, better perioperative management and early diagnosis would be helpful for better survival.

한국 성인의 녹차 섭취와 대사증후군과의 연관성: 한국인 유전체 역학 조사사업 자료를 기반으로 (Association between green tea consumption and metabolic syndrome among Korean adults: results from the Health Examinees study)

  • 본 연구는 40-69세 한국 성인 남녀를 대상으로 녹차섭취량에 따른 대사증후군 발생 위험을 조사하였으며, 연구 결과로 남녀 모두 녹차 섭취량이 증가할수록 대사증후군과 관련지표의 발병률이 감소하는 경향을 보였다. 특히 추가 보정을 진행한 결과에서 여성에서 대사증후군발생이 유의하게 감소하였다. 따라서 본 연구의 결과를 토대로 녹차 섭취가 대사증후군에 긍정적인 영향을 미치며, 이러한 결과를 기반으로 대사증후군 예방을 위한 녹차 섭취 수준에 대한 추가 연구가 필요하다. 한편 국내 녹차 시장은 녹차를 습관적으로 우려 마시는 사람들이 적고 녹차를 가공한 형태인 말차 (가루녹차)와 발효차 등 섭취 방법이 다양하므로 녹차의 효능 정보를 효과적으로 식단에 활용하기 위해서는 녹차를 활용한 가공품과 보조제에 대한 추가 연구가 필요하다.

Endometrial Adenocarcinoma: Clinicopathologic and Survival Characteristics in Yazd, Iran

  • Background: Endometrial adenocarcinoma is the most common gynecological cancer in the Western world and its incidence appears to be rising. However, population-based studies on endometrial cancer providing survival estimates by age, histology, and stage in Asia have been sparse. The aim of this study was to evaluate the clinicopathological data and survival for patients with endometrial adenocarcinoma treated at three institutions in Yazd, Iran. Materials and Methods: Medical and anatomicopathological records at the Department of Pathology and Radiotherapy of the Shahid Sadoughi University of Medical Sciences and Madar private hospital, between 2005 and 2012 were reviewed. All cases of endometrial adenocarcinoma were included. The Kaplan-Maier method was used for survival analysis and Cox proportional hazards model for multiple regression analysis. Results: The study included 84 patients. Stages I, II, III, and IV were identified in 65.4%, 21.5%, 11.9% and 1.2%, respectively. Disease-free survival rate was $73.9{\pm}3.77$ months (95% confidence interval, 64.51-83.22 months) and relapse occurred in 12.3% of the patients. The overall survival rate was $78.2{\pm}3.65$ months (95% confidence interval, 71.0-85.3 months). A multivariate analysis revealed that stage and grade were associated with overall survival. Conclusions: In this survival analysis of patients with endometrial cancer, we found that the prognosis of endometrial cancer was fair but strongly varied by stage and grade, and moderately varied by histology and age.
