Studies on the Analysis of Tobacco Leaf Alkaloids (잎담배 Alkaloid분석에 관한 연구)

  • This study was carried out to develop the method of alkaloids analysis and investigate the kinds and contents of alkaloids in flue-cured, burley and oriental tobacco leaves using developed analytical method. The developed analytical method of alkaloids was as followed : Tobacco sample was treated with acid(pH 2∼3) and extracted with chloroform to remove chemical components except alkaloids. Sample solution was treated with alkali(pH 12∼ 13) and was extracted with chloroform to obtain alkaloids from sample solution. After extraction of alkaloids from tobacco leaves, alkaloids were separated and identified by GC, GC/MS using SE-54 fused silica capillary column. Nicotine, nornicotine, myosmine, 9-nicotyline, anabasine, anatabine, 2, 3-bipyridyl, cotinine, formyl nornicotine, acetyl nornicotine and formal anatabine were isolated and identified from the extracts of tobacco leaves. The contents of alkaloids were burley > flue-cured > oriental tobacco leaves, but oriental tobacco leaves were higher nornicotine, cotinine, formyl nornicotine and acetyl nornicotine contents than those of flue-cured tobacco leaves. The burley tobacco harvested in Korea was higher nornicotine contents by 2-6 times in the portions of cutter, leaf and tips position than that of burley tobacco(B3F) harvested in V. S. A.

Development and Evaluation of a Web-based Education Program to Prevent Secondary Stroke (뇌졸중 환자와 가족을 위한 웹 기반 뇌졸중 이차예방 교육 프로그램 개발 및 평가)

  • Purpose: This study was conducted to develop an e-Learning program that assists nursing students' clinical Purpose: This study was conducted to develop and evaluate a web-based education program for secondary stroke prevention. Methods: A web-based secondary stroke prevention education program was developed using the system's life cycle methods and evaluated by comparing the effects of education among three groups, a web group, a booklet group and a control group. Results: Knowledge level of both patients and family, as well as some health behavior compliance in the web-based and booklet education groups were significantly higher than those of the control group. Family support in the web-based and booklet education groups was significantly higher than that of the control group after 12 weeks. The urine cotinine level in the web-based education group was significantly lower than that of the control group after 12 weeks. Medication adherence, blood pressure and perceived health status were not statistically different among the three groups at any time. Conclusion: Webbased and booklet education programs were equally effective regarding the level of knowledge of patients and their families, family support, health behavior compliance, and urine cotinine level. These results demonstrate the potential use of a webbased education program for secondary stroke prevention.

A Case of Green Tobacco Sickness (GTS) in Elderly Patient (담뱃잎농부병으로 진단된 고령 환자 1례)

  • Green tobacco sickness is an illness caused by dermal exposure to nicotine. The common symptoms of the disease include dizziness, headache, nausea, vomiting, severe general weakness, fluctuations of blood pressure or heartbeat, abdominal cramping, chills, increased sweating, salivation, and difficulty breathing. A 79-year-old female arrived at the emergency room for an evaluation of sudden onset dizziness. Magnetic resonance imaging and angiography of the brain did not show any relevant abnormal findings. Four days later, with supportive care, she said that she had harvested green tobacco for six hours on the day of admission and the tobacco harvest was the first time in her life. She sweated excessively during the hot and humid weather and the tobacco leaves were wet from rain the night before. The serum cotinine tested at five days of admission was 16ng/ml. She was diagnosed with acute nicotine poisoning by her clinical symptoms and the half-life of cotinine in the blood.

Detoxicating Effects of Oriental Herb Extract Mixtures on Nicotine and Dioxin (생약재 추출물의 nicotine 및 dioxin 해독효과)

  • In this report, we investigated the detoxication effects of Saururus chinenis, Geranium nepalense, Lonicera japonica, Cassia obtusifolia, Glycyrrhiza uralensis, or their mixtures by employing acute toxicity tests for nicotine and dioxin. When fatal doses $(LD_{100}\;=\;42\;mg/kg)$ of nicotine were injected into the abdominal cavities of ICR mice, those treated with OHEM showed delayed paralysis, half the duration of hyperactivity, and a 73 % survival rate. The results revealed the strong detoxicating effects of the mixtures. We also measured the amount of the degradation product of nicotine and cotinine in humans. Consumption of OHEM promoted (he more specific) the metabolic pathways of nicotine, increasing continine excretion by 1.5 times. As a result the amount of cotinine in urine was reduced to less than 5% after treatment with OHEM. In order to test the toxicity of dioxin, we used TcnN(SD)BR rats exposed to TCDD. While TCDD treatment reduced the blood levels of hemoglobin and platelet, OHEM consumption relieved these effects and, furthermore, helped to recover the number of platelet to the normal level (p<0.05). Moreover, neutrophils (%) and monocytes (%), which were reduced by the injection of TCDD, recovered to normal levels upon treatment with OHEM. The amount albumin reduced by TCDD (p<0.05) normalized, while the activities of GOT and GTP increased by TCDD were reduced. Increases in total cholesterol and neutral fatty acids induced by TCDD were also reduced by OHEM injection (p<0.05). In the kidney, TCDD-induced rises in creatinine were suppressed by OHEM treatment, while decreases in iron levels from TCDD were raised to normal. The treatment of TCDD had more toxic effects in the blood and pancreas than on the liver, kidney and heart. On the other hand, the detoxication of OHEM had significant effects on the liver and pancreas. The normalization by OHEM of various clinical abnormalities induced by TCDD demonstrates the detoxicating effect of OHEM that ameliorates systemic metabolism not properly functioning.

The Effect of Life Smoking Cessation Program- multidisciplinary approach (다 학제간 접근방식을 이용한 생활금연 프로그램개발과 그 효과에 관한 연구)

  • This study was designed to develop the life moking cessation program with multidisciplinary approach using the Bandura's theory of self-efficacy and confirm the effect of it on the self-efficacy and smoking amount in college students. For this purpose non-equivalent control group, pretest-posttest design was used. The participating subjects in this study were 46 male college freshmen, 23 in experimental and 23 in control group. The experimental group received the 8 sessions of lecture and small group discussions for 4 weeks and a telephone coaching program for a period of 8 weeks. The control group received 5 sessions of lecture without further treatments. The data was collected from March 15 to June 11, 1999, and analysed by SAS/PC program with $\chi$$^2$test, simple t, paired t test. The results were as follows. 1. The score of self-efficacy was significantly increased over time in the experimental group than those of the control group. 2. The amount of urine cotinine was decreased over time in the experimental group, number of cigarettes smoked a day were significantly decreased, and they were significantly less than those of the control group. In conclusion it was found that the Life Smoking Cessation Program with Multidisciplinary Approach was an effective nursing intervention for increasing self efficacy and reducing the amount of smoking in male college students. Therefore, future smoking cessation programs should always consider the concept of self-efficacy and ways to positively reinforce it.

A Study on the Dietary Nutrient Intakes and Blood Profiles of Smoking Teenage Girls Living in a Rural Community in Korea (일부 농촌 지역 흡연 여고생의 영양 섭취 및 혈액 성상 조사)

  • Cigarette smoking has a negative effect for nutrient intakes and nutritional status of antioxidant nutrients including vitamin C. This study has been performed to compare the dietary nutrient intakes, serum vitamin C concentration and serum cotinine concentration of smoking teenage girls(n=104) with those of non-smoking teenage girls(n=791). The subjects were 895 high school girls(15-19 years of age) living in a rural community in Korea. Of the subjects, 11.6% stated that they were currently smoking. The frequency of taking alcohol, soda and coffee was higher in smokers than in non-smokers, respectively. The usage rate of vitamin/mineral supplements was higher in smokers than in non-smokers. The smokers tended to skip breakfast and lunch and to take snack more often than did the non-smokers. The dietary intakes of calorie, protein, vitamin A, vitamin B, vitamin C, calcium and iron were lower in smokers than in non-smokers. Daily mean intakes of these nutrients were less than the 2/3 of the Korean RDAs in smokers while these intakes were similar to or more than the RDAs in non-smokers. The ratio of under weight or lean tended to be higher in smokers. Serum vitamin C concentration was lower in smokers than in non-smokers. These findings show that smokers have unsound eating habits and poor nutirtional status. In particular, nutritional status of vitamin C is significantly poor in smokers, and this tendency can be found more notably in heavy smokers. Therefore, oxidative stress may be present in young teenage girls with short smoking history as evidence by above results. (Korean J Nutrition 34(3) : 338∼347, 2001)

The Relationship between Urinary 8-hydroxydeoxyguanosine and Polycyclic Aromatic Hydrocarbons Exposure in Working Environment

  • To investigate the exposure effect of polycyclic aromatic hydrocarbons (PAHs), we analyzed the relationship between urinary 8-hydroxydeoxyguanosine (8-OHdG) concentration and PAHs exposure. The study population contained 44 workers in steel-pipe coating and paint manufacture industries. We measured airborne total PAHs as an external dose, urinary 1-hydroxypyrene (1-OHP) as an internal dose of PAHs exposure, and urinary 8-OHdG as an effective dose of oxidative DNA damage. There was significant correlation between the urinary concentration of l-OHP and the environmental concentration of PAHs, pyrene, urinary cotinine, AST, and GGT. The mean of urinary 8-OHdG was $17.07\pm1.706{\mu}g/g$ creatinine in workers exposed to airborne PAHs. There was significant correlation between the urinary concentration of 8-0HdG and the airborne concentration of PAHs. From the results of stepwise multiple regression analysis about 8-OHdG, significant independents was total PAHs. In this study, there were significant correlation between the urinary concentration of 8-OHdG and the airborne concentration of PAHs. The urinary 1-OHP was effective index as a biomarker of airborne PAHs in workplace. But it was influenced by non-occupational PAHs source, smoking and biomarkers of liver function test.

Effectiveness of a Smoking Cessation Program for Adolescents

  • Purpose. The purpose of this study was to test the effectiveness of a comprehensive smoking cessation program for Korean adolescents. Method. The study design was quasi-experimental with one pre and three post-tests. The three posttests were done immediately after, three months later, and six months after the completion of the program. A total of 43 high school students who smoked participated in the study with 22 in the experimental group and 21 in the control group. The smoking cessation program consisted of 9 sessions with content on enhancement of self-efficacy, stress management, correction of distorted thoughts, consciousness raising, and assertiveness training. The study variables were urine cotinine levels, self-efficacy, stress, and stages of changed behavior. Results. Urine cotinine levels significantly decreased in the experimental group after the program (F=3.02, p=.06) but significantly increased in the control group (F=6.32, p=.004). Self-efficacy and the degree of stress did not change in either group. The stages of smoking cessation behavior tended to change when compared with raw data for the experimental group. For most participants, the stages of change had been precontemplation and contemplation, but changed to action and maintenance stage among the experimental group. Conclusion. The program was effective in smoking cessation and influencing stages of change but did not change psychosocial factors such as self-efficacy and stress. It is suggested a program should be developed to change psychosocial variables on a long-term basis. It is also desirable to involve peers and families of adolescents who smoke when planning programs to enhance social support.

Validity of Expired Carbon Monoxide and Urine Cotinine Using Dipstick Method to Assess Smoking Status (호기 중 일산화탄소와 소변 코티닌 검사의 흡연상태 타당도 분석)

  • Objectives : We investigated the validity of the dipstick method (Mossman Associates Inc. USA) and the expired CO method to distinguish between smokers and non-smokers. We also elucidated the related factors of the two methods. Methods : This study included 244 smokers and 50 ex-smokers, recruited from smoking cessation clinics at 4 local public health centers, who had quit for over 4 weeks. We calculated the sensitivity, specificity and Kappa coefficient of each method for validity. We obtained ROC curve, predictive value and agreement to determine the cutoff of expired air CO method. Finally, we elucidated the related factors and compared their effect powers using the standardized regression coefficient. Results : The dipstick method showed a sensitivity of 92.6%, specificity of 96.0% and Kappa coefficient of 0.79. The best cutoff value to distinguish smokers was 5-6ppm. At 5 ppm, the expired CO method showed a sensitivity of 94.3%, specificity of 82.0% and Kappa coefficient of 0.73. And at 6 ppm, sensitivity, specificity and Kappa coefficient were 88.5%, 86.0% and 0.64, respectively. Therefore, the dipstick method had higher sensitivity and specificity than the expired CO method. The dipstick and expired CO methods were significantly increased with increasing smoking amount. With longer time since the last smoking, expired CO showed a rapid decrease after 4 hours, whereas the dipstick method showed relatively stable levels for more than 4 hours. Conclusions : The dipstick and expired CO methods were both good indicators for assessing smoking status. However, the former showed higher sensitivity and specificity and stable levels over longer hours after smoking, compared to the expired CO method.

The Effect of Self-Efficacy Promotion Smoking Cessation Program for Middle School Students (흡연 중학생에 대한 자기효능감증진 금연프로그램의 효과)

  • The purpose of this study was to test the effect of a self-efficacy promotion smoking cessation program for middle school students. This program was redesigned on the basis of Shin Sung Rye( 1997)' s Self-Efficacy Promoting Program for this study. The design of this paper was quasi-experimental. equivalent control group pre-post test. time series design. The subjects of this study were 53 smoker adolescents in D Middle School in the city of Busan. The results were summarized as follows: 1) The Hypothesis 1 was accepted: The self efficacy of the experimental group was higher than that of the control group after 1 week (expected efficacy t=2.20, p<.05. expected outcomes t=-2.58. p<.05) 4 weeks after education (expected efficacy t=- .19. p<.001, expected outcomes t=-2.586. p<.05). 2) The Hypothesis 2 was accepted: The amount of smoking of the experimental group was reduced more than that of the control group after 1 week (t=2.05, p<.05) and after 4 weeks (t=2.03. p<.05). 3) The Hypothesis 3 was accepted: The positive urine cotinine of the experimental group was less than that of the control group after 1 week after education($x^2$=8.57. p<.01) after 4 weeks ($x^2$=22.49. p<.001). In conclusion. a self-efficacy promotion smoking cessation program for middle school students was an effective smoking cessation program and then it will be valuable for stopping the smoking among the adolescents.
