Mucormycosis Management in COVID-19 Era: Is Immediate Surgical Debridement and Reconstruction the Answer?

  • Background Excessive use of corticosteroids therapy along with gross immunocompromised conditions in the novel coronavirus disease 2019 (COVID-19) pandemic has raised the risks of contracting opportunistic fungal infections. Here, we describe our experience with the implementation of a surgical protocol to treat and reconstruct rhino-orbital-cerebral mucormycosis. Methods A retrospective review of our prospectively maintained database was conducted on consecutive patients diagnosed with mucormycosis undergoing immediate reconstruction utilizing our "Mucormycosis Management Protocol." All patients included in this study underwent reconstruction after recovering from COVID-19. Wide local excision was performed in all cases removing all suspected and edematous tissue. Reconstruction was done primarily after clear margins were achieved on clinical assessment under a cover of injectable liposomal amphotericin B. Results Fourteen patients were included. The average age was 43.6 years and follow-up was 24.3 days. Thirteen patients had been admitted for inpatient care of COVID-19. Steroid therapy was implemented for 2 weeks in 11 patients and for 3 weeks in 3 patients. Eight patients (57.1%) had a maxillectomy and mucosal lining resection with/without skin excision, and six patients (42.8%) underwent maxillectomy and wide tissue excision (maxillectomy and partial zygomatic resection, orbital exenteration, orbital floor resection, nose debridement, or skull base debridement). Anterolateral thigh (ALT) flaps were used to cover defects in all patients. All flaps survived. No major or minor complications occurred. No recurrence of mucormycosis was noted. Conclusion The approach presented in this study indicates that immediate reconstruction is safe and reliable in cases when appropriate tissue resection is accomplished. Further studies are required to verify the external validity of these findings.

Key success factors for implementing modular integrated construction projects - A literature mining approach

  • Modular integrated construction (MiC) is an innovative construction method where components of a building are manufactured in an offsite factory, trucked to the job site in sections, set in place with cranes, and assembled together to form a whole building. Where circumstances merit, favorable conditions exist and implemented effectively; MiC improves project performance. However, several key factors need to converge during implementation to realize the full benefits of MiC. Thus, a thorough understanding of the factors which are critical to the success of MiC projects is imperative. Drawing on a systematic review of 47 empirical studies, this research identified 25 key success factors (KSFs) for MiC projects. Of these, the five topmost cited KSFs for MiC projects include effective working collaboration and communication among project participants; standardization, optimization, automation and benchmarking of best practices; effective supply chain management; early design freeze and completion; and efficient procurement method and contracting. The study further proposed a conceptual model of the KSFs, highlighting the interdependences of people, processes, and technology-related KSFs for the effective accomplishment of MiC projects. The set of KSFs is practically relevant as they constitute a checklist of items for management to address and deal with during the planning and execution of MiC projects. They also provide a useful basis for future empirical studies tailored towards measuring the performance and success of MiC projects. MiC project participants and stakeholders will find this research useful in reducing failure risks and achieving more desired performance outcomes. One potential impact of the study is that it may inform, guide, and improve the successful implementation of MiC projects in the construction industry. However, the rigor of the analysis and relative importance ranking of the KSFs were limited due to the absence of data.

The Comparative Analysis of the Present and Expected Level on Meal-Price and Facilities Investment Cost Perceived by Foodservice Managers of Contract-Managed Highschools in Seoul (서울시내 고등학교 위탁급식의 급식비와 투자비의 실태 및 위탁업체의 기대수준 비교 분석)

  • The purposes of this study were to a) find out the operational characteristics of the contract-managed highschool foodservice in Seoul, b) investigate the expected level of meal-price and facilities investment cost perceived by contract-managed highschool foodservice managers c) compare the present level and expected level of meal-price and facilities investment cost. From October 12 to November 13 in 2001, the questionnaires were mailed to 249 high schools which was managed by contract foodservice company with respondent rate 40.2%. Data were analyzed using SPSS Win(10.0) for descriptive analysis and one group paired t-test. The results of this study were as follows ; 1. The student enrollment of highschools run by contract-managed foodservice was 1,243 with 72.6% participation rate of school lunch program. The average meal-price was 2,138 won. The average annual period of school foodservice operation was 156.78 days per year. The average contracting period was about 3 years. 2. The average cost concerned in the facilities investment amounts 169,578,180 won at the initial investment and 25,204,092 won at the repairs and maintenance cost in the course of operation. 3. The present level of meal-price and facilities investment cost were respectively 2,136won/meal and 171,157,336.72 won. And expected level of meal-price and amount of facilities investment cost were 2,418.75 won and 121,353,215.19 won. Comparing the present level with the expected level of the meal- price and facilities investment cost, expected level of meal-price was significantly higher than the present level of meal-price(p<.001) and expected level of facilities investment cost was significantly lower than present level of facilities investment cost(p<.001).

The Effects of a Mupirocin Smear in the Nasal Cavity Against Nosocomial Infections (비강 내 약물 도포가 병원 감염 예방에 미치는 효과)

  • This study was done to analyze the effects of a smear in the nasal cavity against nosocomial Infection. The smear used was mupirocin, and the study centered on infection which constitutes the majority of nosocomial infections called MRSA. The data were collected between March 23 1998 and June 31 of the same year in a university hospital in the Kyongi Province, and is made up of a experimental group of 14 patients who were given nasal cavity smears and a control group of 16 patients who were not given nasal cavity smears. The data were analyzed through frequency and the Chi-square tests and gave forth these results. 1. Of the experimental group 28.6% developed nosocomial infections while 62.5% of the control group developed infections. This difference was significant. 2. In the experimental group, all of the patients developed infections within the first week in the ICU, while 80% of the control group developed infections in the first week and 20% in the second week. The difference was not as marked here. 3. In the experimental group the DM group 66.7% contracted MRSA while 18.2% developed it in the non-DM group showing that the DM group had infection rate was 3.7 times higher than the non-DM groups. In the control group the DM group had a 100% infection rate while 50% of the non-DM group developed it. Overall the DM group's rate infection was 2.4 times higher than the non-DM group. 4. In the experimental group, 37.55% of the patients who had a tracheostomy developed it while 16.7% of the patients who did not have a tracheostomy developed infections. In the control group, 62.5% of the patients who had tracheostomy, and 37.5% of the patients who did not have tracheostomies developed infections. Those who had tracheostomies, and the control group had double the rate contracting infections. From these results we can see that nasal cavity smears are effective against nosocomial infections. In spite of the smears, patients with the diabetes mellitus had a high MRSA infection rate, which requires new alternative treatments.

Risk Assessment of Escherichia coli Infection from Use of Interactive Waterscape Facilities (수경시설 물놀이에 따른 대장균 감염의 위해도 분석)

  • Objectives: With the goal of quantifying the risk of children contracting gastroenteritis while playing at interactive waterscape facilities and evaluating the adequacy of current water quality regulations, risk assessment was performed with Escherichia coli as pathogen. Methods: Abundances of E. coli in the waters of interactive water features in South Korea were acquired from survey reports. A gamma distribution describing the volume of water swallowed by children during swimming activities was adopted. Exposure rate and risk were calculated by Monte Carlo simulation and dose-response models for various pathogenic E. coli. Results: E. coli was detected in 25 out of 40 facilities, with range of ~1,600 CFU/100 ml. The abundance fitted an exponential distribution. Simulated exposures ranged ${\sim}1.9{\times}10^{10}$ CFU, varying greater along E. coli abundance than the volume of water. Risk of children being infected by enterohemorrhagic E. coli was high, with range of ~0.85. When E. coli abundance was <200 CFU/100 ml, which is the current government threshold, the risk decreased to <0.43. Although the guideline successfully reduced the risk of adults being infected by a less virulent E. coli strains (<0.03), the risk for children could not be quantified due to lack of dose-response models for those pathogens for children. Conclusions: Under the current guideline, children are at risk of being infected if water is contaminated with by enterohemorrhagic E. coli. For other E. coli strains, the risk appears to be considerably less. The result warrants need for developing dose-response models for children for each pathogenic E. coli strain.

Nurses' Knowledge about and Attitudes toward AIDS and Patients with AIDS (간호사의 AIDS에 대한 지식과 태도)

  • With the increasing incidence of AIDS, it is probably inevitable that nurses working in hospital and community settings will come in contact with patients with AIDS. Nurses, more than any other health care profession, are on the front line of AIDS patient care. The purposes of this descriptive study were to gather information about registered nurses' knowledge and attitude regarding AIDS. and to provide a basis for the development of continuining education programs for the nurse. Data was gathered in 1988 using self-administered questionnaires given to a convinience sample of two hundred seventy registered nurses from two university hospitals, school nurses and postgraduate students of nursing. Data was analyzed with SAS. The results were as follows. Above 90% of the nurse knew well about definition of AIDS and routes of transmission but nurses relatively lacked knowledge about transmission of HIV in breast milk(69.3%). Less than half knew that drug abuser(44.1%) and sexual partner with IV drug abuser(39.6%) are at high risk for contracting AIDS. Above 70% of the nurse showed reluctance to provide care for surgery. delivery of child birth and hemodialysis of patients with AIDS. The results showed that, given a choice. 41.7% of the nurse would refuse to care for AIDS patients and 48.3% claimed that they should have a right refuse to care for AIDS patients. Reluctance of nursing patient with AIDS appeared to be principally associated with general fear of becoming infected with HIV. 41.8% exhibited a sympathetic attitude toward individual AIDS patient. The study findings suggest that it is necessary to examine the correlations between knowledge and attitude and to develop continuing education programs that alleviate the fear of contagion of the nurse.

Development of an Organic Dairy Complex by Contract Farming with an Enterprise: A Case Study of Gochang-gun, Jeollabuk-do (계약생산에 의한 기업 주도형 유기낙농단지의 형성: 전라북도 고창군을 사례로)

  • The objective of this study was to review how contract farming started in Gochang-gun and identified its characteristics among the contracting parties and the consequent changes in the region's dairy industry with regard to the contract production of organic milk in Gochang-gun, Jeollabuk-do, Korea in conjunction with the Maeil Dairy Industry Corporation. The contract was established in keeping with the product diversification strategy of the company after a crisis due to opening of the market, taking advantage of the clean environment of Gochang-gun and positive attitudes of the local government and dairy farmers. The expansion of the participating farms led to a production gap among dairy farms in the region; however, the overall concern over the conventional practice of contract farming was mitigated as the organic farming spread with the conversion of the contract farms to organic farms.

  • Global electronic commerce, driven by the development of the Internet and computer, premises to be an important engine for growth for the world economy in the 21st century. Electronic commerce offers considerable new opportunities for businesses and citizens in all regions of the world by enhancing productivity across all sectors 6f our economies and encouraging trade in both goods and services. Specially in relation to contract, electronic commerce requires a coherent, coordinated approach internationally on key issues such as a validity, a legality, consumer protection. In order to ensure the stable growth of electronic commerce in Korea, the government needs to construct a predictable legal and commercial environment, suitable to the situation in Korea, for business conduct on the Internet and other electronic method. Electronic commerce, which breaks down national boundaries and widens the gap between the place where services are performed and the place where they are consumed, requires a new paradigm when making an agreement between contracting parties.

Globalization Development Plans of the Convergence Systems and Policies for Domestic Fire Industry (화재피해자의 긴급복구를 위한 민·관 융합형 지원시스템 방안)

  • This study is related with support system plans of public-private convergence for emergency recovery services to fire victims. The current issues are analysed from the condition and problem analysis results of emergency recovery activities for fire victims. And building plans of an integrated support system based on the government and private sector cooperation are proposed. Overall, the system shows that the government is responsible for the functionality of management and supervision for the system operation, and the private is responsible for the functionality of specialized agencies for practical emergency recovery. Consequently, support system plans include with infrastructure maintenance of laws and regulations, contracting-out of the support system, stable supply of operating budget and function strengthening of public-private convergence for emergency recovery. Especially, plan for stable supply of operating budget in detail including with establishment of the fund and self-financing composition of private corporations for emergency recovery services is proposed. The results of this study will lead strengthening of fire safety welfare and social safety nets at future.

Mosquito Species Composition and Plasmodium vivax Infection Rates on Baengnyeong-do (Island), Republic of Korea

  • Vivax malaria is a significant military and civilian health threat in the north of the Republic of Korea (ROK). The island of Baengnyeong-do is the westernmost point of the ROK and is located close to the southwestern coast of the Democratic People's Republic of Korea (DPRK). Mosquitoes were collected using a black light trap on Baengnyeong-do, and Anopheles spp. were assayed by PCR, to identify the species, and screened for sporozoites of Plasmodium vivax. Of a subsample of 257 mosquitoes, Anopheles lesteri was the most frequently collected (49.8%), followed by Anopheles sinensis (22.6%), Anopheles pullus (18.7%), Anopheles kleini (7.8%), and Anopheles belenrae (1.2%). The overall sporozoite rate was 3.1%, with the highest rates observed in An. kleini (15.0%), An. sinensis (5.2%), and An. lesteri (1.6%). No sporozoite positive An. pullus or An. belenrae were observed. The results extend our knowledge of the distribution and potential role in malaria transmission of An. kleini, An. lesteri, and An. sinensis, for an area previously considered to be at a low risk for contracting vivax malaria.
