Contralateral Superior Cerebellar Artery Syndrome : A Consequence of Brain Herniation

  • Vascular compromise is a well-known consequence of brain herniation syndromes. Transtentorial brain herniation most often involves posterior cerebral arteries. However, isolated involvement of contralateral superior cerebellar artery (SCA) during unilateral impending brain herniation is reported only once and we present another case of this exceedingly rare entity. A 24-year-old man was referred to us with impending herniation due to a multiloculated hydrocephalus, and during the course of illness, he developed an isolated SCA ischemia in the opposite side of the most dilated entrapped horn. In the current article we discuss the probable pathophysiologic mechanisms of this phenomenon, as well as recommending more inclusive brain studies in cases suspected of Kernohan-Woltman notch phenomenon in unilateral brain herniation. The rationale for this commentary is that contralateral SCA transient ischemia or infarct might be the underdiagnosed underlying pathomechanism of ipsilateral hemiparesis occurring in many cases of this somehow vague phenomenon.

  • Retained austenite in matrix of austempered ductile iron has been well-known as a parameter in controlling mechanical properties, but investigation to obtain quantitative relationship with mechanical properties lack. Therefore, this study executed austempering treatment at various temperatures on ductile iron alloyed with Mo, Ni, Cu. In consequence, microstructure of retained austenite transformed coarse, and quantity increased according as austempering temperature increased. After heat-treatment, microstructure of specimen alloyed with Ni was fine, and toughness improved. At austempering temperature up to $400^{\circ}C$, carbide precipitation started in retained austenite. In consequence, afforded cause of hardness increase, a lot of increase did not arise for coarse structure.

  • This paper presents and online scheme for fuzzy modelling of nonlinear systems, based on the model adjustment technique and the genetic algorithm technique. The fuzzy model is characterized by fuzzy "if-then" rules which represent locally linear input-output relations whose consequence parts are defined as subsystems of a nonlinear sysem. The discrete-time model for each subsystem is obtained to deal with initalization and unmeasurable signal problems in online estimation and the final output of the fuzzy model is computed from the outputs of the discrete-time models. Then, the parameters of both the premise and consequence parts of the fuzzy model are adjusted by a genetic algorithm. A set of simulation works is carried out to demonstrate the effectiveness of the proposed method.ed method.

  • The majority of the SKYBIO will be used in the treatment of water in food processing plants, swimming pools and cooling towers, in the manufacture fabric softeners in Australia. Most will eventually be released into domestic sewage system as a consequence of product use. The SKYBIO is not readily biodegradable (0% over 28 days), and is expected to have a low partition coefficient and high water solubility (285 g/L), all indicating that the material would be mobile in both aquatic and terrestrial compartment. The PEC/PNEC ratio for the aquatic environment is 56. This value is significantly greater than 1, indicating an immediate concern to the aquatic compartment. However as a consequence of it's cationic character, the SKYBIO will be expected to associate to negatively charged organic matter in soil and sediment.

A Case Study of Explosion Accident at MEK-PO Factory using Consequence Analysis (결과분석을 이용한 MEK-PO 제조공장의 폭발사고 사례연구)

  • In this case study, results of the explosion accident at MEK-PO factory were analysed by using the consequence analysis of quantitative hazard assessment and the explosion energy, the burst pressure of vessel, and overpressures at the explosion center and at 300m distance from the explosion center were estimated, respectively. As a result, we found that a cause of accident was the runaway reaction of product(MEK-PO) because of the molecular expansion in vessel and that the possibility of the runaway reaction was classified the mechanical failure(the obstacle of refrigerator or the shutdown valve), design error, and operating error by lack of thermochemical knowledge. Also, the evasive action to prevent accident was suggested.

  • This article aims to provide a design method of warning thresholds for active safety systems based on the performance-based approach considering driver behaviors. Both positive and negative consequences of warnings are considered, and the main idea is to choose a warning threshold where the positive consequence is maximized, whereas the negative consequence is minimized. The process of the performance-based approach involves: Defining the operating scenarios; setting the trajectory models, including human characteristics; estimating the alert and nominal trajectories; estimating the performance metrics; generating a performance-metric plot; and determining the alert thresholds. This paper chose a lane-departure warning system as an example to show the usefulness of the performance-based approach. Both human and sensor characteristics were considered in the system design, and this paper provided a quantitative method to include human factors in designing active safety systems.

  • This paper traces the issue of key account management success which has regarded as a pressing concern of many companies' sales efforts at the organizational business-to-business context. Based on the extensive review of extant literatures, we introduced a theoretical framework that covers the antecedents and consequence of key account management success. We theorized the conditions under which organizational and relational factors influence the success of key account management approach. We endeavor to develop research propositions for each construct and provide necessary suggestions to isolate a platform for future empirical research. Beyond this span, an improved understanding from this framework will help developing policies for successful key account management approach.

Identification of Prevailing Risk Attitudes in Various Risk Situations (다양한 위험상황에서의 지배적 위험태도의 파악)

  • Previous researches on risk attitudes or on the typical utility functions have mostly focused on how the risk attitude of decision maker varies when changes are made in one or two lottery reference points such as consequence domain and magnitude of probability under assumed risk situations represented by simple lotteries. It is, however, very difficult to forecast dominant risk attitudes under risk situations which exhibit a complex combination of many reference points. In this study, twelve risk situations which a decision maker may confront in real decision-making situations were formulated by combining in various ways three reference points, that is, magnitude of probability, consequence domain, and magnitude of gain or loss. Then through a questionnaire dominant risk attitudes under every assumed risk situation were investigated, and the general shape of utility function implied by the experimental results were derived. Results of the present study show that none of the three reference points have dominant effect over the others due to complicated interaction between them, and given the twelve risk situations the observed risk attitude widely varies from strong risk taking to strong risk aversion.

  • Performance based design is a recent evolutionary step in the process of designing fire protection systems. In essence, it is a logical design process resulting in a solution that achieves a specified performance. Sometimes the prescriptive solutions presented in various codes and standards are too expensive or inflexible. Often the solutions do not and enables optimization of a solution for cost and function. In this study, performance based design was carried out to determine the extent of passive fire protection for oil terminal facilities. The results of performance based design were compared with those of prescriptive code based design. Performance based design is not always more economic than prescriptive code based design but provides more reliable and effective design that is fit for the purpose.

Severe Diarrhea-induced Acute Kidney Injury and Its Consequence in an Elderly

  • Methods: This study presents a comprehensive case study of an elderly male diagnosed with acute kidney injury (AKI) resulting from severe dehydration, supported by an extended follow-up with laboratory findings. Results: An 83-year-old male patient experienced severe diarrhea overnight, leading to hospitalization due to symptoms of dehydration and hypotension. His laboratory results displayed a typical AKI pattern, including a significant increase in creatinine levels (5.19 mg/dL) and the presence of hyperkalemia and hyponatremia. Following general treatments, including the administration of an herbal drug (Bulhwangeumjeonggi-san), the estimated glomerular filtration rate (eGFR) improved from 10 ml/min (Stage 5) to 34 ml/min (Stage 3) within five days when he was discharged. Although subsequent eGFR tests, conducted one and two months later as an outpatient, revealed an improvement of 42 ml/min, the patient still experienced mild chronic dysfunction as a consequence. Conclusion: This study presents a noteworthy case of acute kidney injury attributed to severe dehydration, emphasizing the importance of medical awareness regarding diarrhea-induced kidney function impairment, especially in the elderly population.
