Stress Experience of Nursing Students to Clinical Practice : Q-methodological Approach (간호학생의 임상실습 스트레스 경험유형 : Q방법론적 접근)

  • This study was designed to determine the schemata and their characteristics of stress experience the subjectivity of stress experience(structure of subjectivity) would be a basic step for the effective clinical education through the stress management for characteristics of these types. Q-methodological method was used for that purpose. The research method statements were collected prior to the study through indepth interviews. For the study, 31 Q-statements were selected. There were 34 nursing students as subjects for the research. The 34 nursing students sorted the 31 statements using the principal of forced normal distribution. The principle of forced normal distribution, which has 9 scales to measure the individual opinions, was called. Q-factor analysis by using PC QUANL program supply the material. According to the outcomes of this study, there were 3 types of special opinion about the stress experience of clinical practice. The first type is called 'Influence of practical atmosphere type'. Members of this type experienced stress by an inadequate orientation and undesirable role model of nursing. The second type is called 'conflict of nursing role type'. Members of this type experienced stress by an inadequate orientation and undesirable role model of nursing. The third type is called 'Lack of confidence type'. Members of this type experienced stress because of a lack of confidence for their own nursing knowledge and skill. As a result, we now need further study to identify individual psychological aspects of stress for clinical practice. The findings will guide the development in effective approaches for clinical education. Finally, the result of the study will provide us the need for developing systematic and integrated practice education program for students and active involvement of clinical instructor.

Structural Equation Modeling on Self-Care Behavior and Quality of Life in Older Adults with Diabetes Using Citizen Health Promotion Centers (시민건강증진실을 이용하는 노인 당뇨환자의 자가관리 이행 및 삶의 질 예측모형)

  • Purpose: The purpose of this study was to construct and test a structural equation model for Diabetes self-management (DSM) behavior and Quality of life (QoL) in older adults with diabetes who use Citizen Health Promotion Centers. The theory used this study was a combination of the Information-Motivation-Behavioral Model (IMB) and Self-Determination Theory (SDT) to reflect autonomous characteristics of participants. Methods: Data were collected from April 20 to August 31, 2015 using a self-report questionnaire. The sample was 205 patients with type 2 Diabetes who regularly visited a Citizen Health Promotion Center. SPSS 22.0 and AMOS 22.0 programs were used to analyze the efficiency of the hypothesized model and calculate the direct and indirect effects of factor affecting the participants' DSM behavior and QoL. Results: The supported hypotheses were as follows; 1) The variable that had a direct effect on QoL was health behavior adherence (${\gamma}=.55$, p=.007). 2) The variables that had a direct effect on DSM behavior were DSM information (${\gamma}=.15$, p=.023), DSM confidence (${\gamma}=.25$, p<.001), and autonomous motivation (${\gamma}=.13$, p=.048). 3) The variable that had a direct effect on DSM confidence was autonomy support (${\gamma}=.33$, p<.001). Conclusion: The major findings of this study are that supporting patient's autonomous motivation is an influential predictor for adherence to DSM behavior, and integrative intervention strategies which include knowledge, experience and psychosocial support are essential for older adults with diabetes to continue DSM behavior and improve QoL.

  • Among data mining techniques, the association rule is one of the most used in the real fields because it clearly displays the relationship between two or more items in large databases by quantifying the relationship between the items. There are three primary quality measures for association rule; support, confidence, and lift. We evaluate association rules using these measures. The approach taken in the previous literatures as to estimation of association rule number has been one of a determination function method or a regression modeling approach. In this paper, we proposed a few of non-linear regression equations useful in estimating the number of rules and also evaluated the estimated association rules using the quality measures. Furthermore we assessed their usefulness as compared to conventional regression models using the values of regression coefficients, F statistics, adjusted coefficients of determination and variation inflation factor.

  • Purpose: The purpose of this study was to identify predictive factors of Brest Self-Examination practice of clinical nurses. Method: The subject for this study were 277 nurses in 8 university hospitals in Busan. The data were collected from September 21 to October 20, 2001 by means of a structure questionnaire. The instruments used for this study were Choi's BSE knowledge scale. Kim's BSE attitude scale and Jung's BSE practice scale. The data were analyzed using frequency, percentage, mean, Peason Correlation, t-teat, ANOVA, scheffe's test, and multiple stepwise Regression using SPSS program. Result: 1. The mean score of BSE practice for the total sample was 7. 25${\pm}$4.62. 2. Statistically significant factors influencing the BSE Practice among social demographic characteristics were age(F=2.734, P=0.44), Married status(t=2.598, p=0.010). 3. Statistically significant factors influencing the BSE Practice among BSE relating characteristics were enlisting the help of significant peers(t=3.34, P=0.00), Intention of Practice for BSE(t=10.462, p=0.00), performance of BSE(t=7.800, P=0.00), frequency of performance in BSE(F=13.932, p=0.00), confidence in Knowledge of BSE technique(F=5.350, p=0.00), confidence in finding breast nodule(F=7.204, p=.00), asking client's BSE (t=3.153, P=0.01). 4.The mild correlation between nurse's BSE knowledge and practice was found(r=0.366,p=0.000). 5. There were significant predictors of BSE Practice. Performance of BSE was the best significant predictive factor(R2=.383, p=.000) Another significant predictive factors were knowledge, intension of practice, married status, frequency of performance. Conclusion: Degree of nurses' performance of BSE was average. It is necessary to develope the nurses' educational program for BSE with its focus on above predictive factors of performance of BSE.

A Study on Correlation of Saccharin QLFT and CNC QNFT for Respirators (호흡기보호구에 대한 Saccharin QLFT와 CNC QNFT간 상관성에 관한 연구)

  • A quantitative fit test, condensation nuclei counting(Portacount 8025, TSI), was performed concurrently with a qualitative fit test, sodium saccharin(FT-10, 3M) to evaluate FF values and to determine the correlation between two methods. Two brands of full facepieces, T, S and two brands of half masks, T, S, were fit tested, respectively, on 103 wearers one time. The FF values obtained by CNC QNFT were lognormally distributed. The FF values for T brand of respirators were statistically very much higher than those for S brand of respirators. For a full facepiece wearer combination having a $FF{\leq}100$, as determined by CNC QNFT, the point estimate of the probability of that combination not being rejected by the saccharin QLFT was found to be 0.09 with 90% confidence that this statistic is not expected to exceed 0.25. For a half mask wearer combination having a $FF{\leq}10$, as determined by CNC QNFT, the point estimate of the probability of that combination not being rejected by the saccharin QLFT was found to be 0.10 with 90%, confidence that this statistic is not expected to exceed 0.23. The uncertainty associated with each estimate, however, is large due to the small number of study subjects with inadequately fitting respirators. This result indicates that saccharin QLFT may be more suitable for adequately fitting respirators than inadequately fitting respirators.

The Effect of Acute Coagulopathy in Profoundly Traumatic Patients on Acute and Early Deaths (고도 중증외상 환자에서 급성 혈액응고장애가 초기 및 조기 사망에 미치는 영향)

  • Purpose: Numerous studies have investigated the pattern of traumatic death with a focus on the injury mechanism, the severity of the injury and the presence of hemorrhage. Acute coagulopathy has been treated as only one of many complications. The purpose of this study was to investigate the influence of acute coagulopathy on acute and early death due to trauma. Methods: A retrospective analysis of trauma patients with injury severity score (ISS)${\geq}25$ who had been treated between January 2011 and December 2012 was conducted. Based on the time of injury, traumatic death was categorized into acute (within 48 hours) and early (from 3 to 7 days). The correlations between various parameters within 24 hours after injury and time of death were analyzed. Results: A total of 124 patients were enrolled. Of them, 8.1% (n=10) of the patients experienced acute mortality. For those patients, significant differences in initial systolic blood pressure, coagulopathy score, amount of transfusion, abbreviated injury scale of the head and neck, the abdomen and the extremities were noted. Early mortality was experienced by 7.0% (n=8) of the patients, only coagulopathy score was found to be a significant independent risk factor for acute (odds ratio: 3.127; 95% confidence interval: 1.185-8.252; p=0.021) and early mortality (odds ratio: 2.470; 95% confidence interval: 1.029-5.929; p=0.043). Conclusion: Acute traumatic coagulopathy has an important role in the mortality, even after the acute phase. Early management and prevention of acute coagulopathy may improve survival of trauma patients.

Factors affecting suicidal ideation of athletes in middle and high school (중.고등학교 운동선수들의 자살생각에 영향을 주는 관련요인)

  • Purpose: The purpose of this study is to analyze factors affecting suicidal ideation of athletes in middle and high school. Methods: The data in this study came from the survey which conducted to 500 students of physical education high school athletes and elite athletes in general school from the beginning of December 2009 to the end of January 2010 in Seoul and Kyunggi. Finally 436 cases was used. The dependant variable is suicidal ideation in this study. And the independent variables are socio-demographic factors, exercise-related factors, school and family environments, stress and depression factor. The analysis method is logistic regression analysis. Finally 436 cases was used. Results: First, the rate of suicidal ideation of athletes in middle and high school was 28.9%. Second, there were significant differences between stress and sex, grades, athletic career, kind of sport, sport skill level, the average exercise time per day, family circumstances, life satisfaction, future expectations. Third, there were significant differences between depression and sex, academic achievements, the average exercise time per day, family circumstances, life satisfaction, future expectations. Fourth, there were significant differences between suicidal ideation and sport skill level, academic achievements, the average exercise time per day, family circumstances, life satisfaction, future expectations. Fifth, the higher level of stress and degree of depression, the more increased the risk of suicidal ideation. Conclusions: Considering the close relationship between suicidal ideation and suicidal attempt, to reduce the suicidal ideation of athletes in middle and high school it is necessary to adjust exercising time properly and to promote life satisfaction. Also to overcome depression and stress support system should be established and to promote self-efficacy cognition strength program should be introduced in the school. So athletes in middle and high school should have confidence and self-confidence on the future.

Investigating the Relationship of Sustainability Factors with Project Management Success

  • One of the main concerns of the present age, is attending to the issue of sustainability that in recent decades, the concept of it has grown in terms of recognition and importance. Sustainability have defined further the development of the welfare and living standards of the present generation, without the lives of future generations is at stake. One of these tasks of the business is project management and today, "green or sustainable project management" as one of the most important global trends of project management have been identified. The issue of present study was to investigate the relationship of sustainability factors with success of a project. The population of the research consists of directors of oil and gas in the South Pars region including 150 people that according to Cochran formula, the number of samples were obtained 108 people. In this study, data collection was conducted based on a structured questionnaire that from a sample set of questions was used for sustainability impact on 6 scale of project management success. For this purpose, the whole five-item Likert has been used. Also for statistical analysis, PLS software was used. The results of path analysis at confidence level of 95%, showed a significant relationship exists between "sustainability factor" and "project success criteria." Since the correlation coefficient is positive, so this type of relationship is positive. Thus all the hypotheses at confidence level of 95 percent was confirmed and it became clear that there is a significant positive relationship between "sustainability factors" and "project success criteria."

Adherence to the Clean Intermittent Catheterization Following a Customized Intensive Education Program for Patients with Emptying Failure

  • Purpose: The purpose of this study is to investigate adherence to the clean intermittent catheterization (CIC) and influencing factors on the adherence following a customized intensive education program (CIEP). Methods: This work is a retrospective descriptive study. The subjects were 226 emptying failure patients who learned in a CIEP from January 2012 to July 2014. The program developed in 2011 and consisted of 1) customized theoretical education; based on the results of urologic tests, underlying disease, or surgery of the patients, 2) hands-on practice until the trainees were able to master the techniques, 3) questions about the process of catheterization and what he/she needs to know. 4) at follow-up, the survey about adherence and barrier to CIC. Clinical characteristics; the level of satisfaction, understanding, and self-confidence; the barrier in medical records were reviewed. Results: The short-term adherence rate (median 22 days) is 87.6% and the long-term adherence rate (median 112 days) is 50.4%. The biggest obstacle is time management. The levels of satisfaction, understanding, and self-confidence are very high. The variable of Income is the only factor that has influence on adherence. Conclusion: Despite the CIEP, the adherence rate is relatively low. In addition to the education, emotional and psychological supports and regular follow-up are needed to improve long-term adherence.

Prediction for the Lifetime Effective Dose and Radon Exposure Risk by using Dose Conversion Convention: Base on the Indoor Radon Concentration of Lecture Room in a University (선량 환산 관례를 이용한 생애유효선량 및 라돈피폭 위험도 예측: 대학 강의실 라돈농도 중심으로)

  • The indoor radon concentration was measured in the lecture room of the university and the radon concentration was converted to the amount related to the radon exposure using the dose conversion convention and compared with the reference levels for the radon concentration control. The effect of indoor radon inhalation was evaluated by estimating the life effective dose and the risk of exposure. To measure the radon concentration, measurements were made with a radon meter and a dedicated analysis Capture Ver. 5.5 program in a university lecture room from January to February 2018. The radon concentration measurement was carried out for 5 consecutive hours for 24 hours after keeping the airtight condition for 12 hours before the measurement. Radon exposure risk was calculated using the radon dose and dose conversion factor. Indoor radon concentration, radon exposure risk, and annual effective dose were found within the 95% confidence interval as the minimum and maximum boundary ranges. The radon concentration in the lecture room was $43.1-79.1Bq/m^3$, and the maximum boundary range within the 95% confidence interval was $77.7Bq/m^3$. The annual effective dose was estimated to be 0.20-0.36 mSv/y (mean 0.28 mSv/y). The life-time effective dose was estimated to be 0.66-1.18 mSv (mean $0.93{\pm}0.08mSv$). Life effective doses were estimated to be 0.88-0.99 mSv and radon exposure risk was estimated to be 12.4 out of 10.9 per 100,000. Radon concentration was measured, dose effective dose was evaluated using dose conversion convention, and degree of health hazard by indoor radon exposure was evaluated by predicting radon exposure risk using nominal hazard coefficient. It was concluded that indoor living environment could be applied to other specific exposure situations.
