중소규모 사업장 고혈압환자의 치료순응과 관련요인 (Factors Related to Therapeutic Compliance of Hypertensive Patients in Small and Medium Scale Industry)

  • In order to evaluate the factors related to therapeutic compliance of hypertensive patients in small and medium scale industry, the questionnaire survey and the blood pressure measurement were performed on 138 hypertensive patients who were reported to have C or $D_2$ result of hypertension at the workers' periodic health examination from March to November 2001. The contents of questionnaire included the informations of factors related to therapeutic compliance of hypertensive patients such as, age, sex, marital status, income, education levels, scale, occupation, work duration, smoking and drinking habits, exercise, family history, stress and A type personality, employer's concern, organizational culture, health status, awareness, knowledge and attitude toward the needs of hypertension treatment, and experience of health education. The results of the study were as follows: 1. The proportion of those who were compliant to the treatment of hypertension was 41.3% of subjects. Among small scale industry less than 50 employees, the rate of therapeutic compliance group was 27.5% and therapeutic noncompliance group was 72.5%. Among medium scale industry more than 50 employees, the rate of therapeutic compliance group was 60.3% and therapeutic noncompliance group was 39.7%. 2. Among therapeutic noncompliance group, 95.1% of patients were aware of the fact that workers have hypertension, and 77.8% of patients got 6-10 marks of hypertension related knowledge. For health education, 27.2% were experienced and 34.6% said no intention to participate. And for hypertension treatment, 9.9% said no need to get the treatment and 44.4% said have no idea whether get treatment or not. 3. The significant factors related to therapeutic compliance of hypertensive patients in small scale industry were work duration, A type personality(anger), health status, attitude toward the needs of hypertension treatment, and experience of health education. 4. The significant factors related to therapeutic compliance of hypertensive patients in medium scale industry were age, occupation, subjective symptom, attitude toward the needs of hypertension treatment, and experience of health education. In consideration of above findings, it was suggested that in order to improve the therapeutic compliance in small and medium scale industry hypertensives, it be necessary to change attitude, perception, knowledge and treatment of hypertension by various methods such as effective health education and individually consulting programs by occupational health professional.

물벼룩을 이용한 일부 염색폐수의 만성 수질독성 특성 연구 (Chronic Toxicities of Effluents from Dye Industry using Daphnia magna)

  • 본 연구를 통한 결론은 다음과 같다. 5개의 염색폐수배출업소에서 각각 3회씩 채취한 최종 방류수를 대상으로 한 생물독성시험 결과 미생물독성시험에서는 B시료와 E시료에서 독성이 관찰되었고 급성 물벼룩시험에서는 반수치사농도가 C시료에서만 관찰되어 염색폐수의 독성이 시험 생물종별로 다르게 나타날 수 있음을 제시하였다. 동일 생물종을 대상으로 실시한 급성 및 만성시험에서는 급성반수치사가 5개 방류수 중 C시료에서만 비교적 낮은 농도에서 관찰이 된 것에 비해 만성반수치사는 3개 방류수에서 나타났고 $EC_{50}$도 급성영향의 $1/2\~1/5$ 수준에서 결정되었다. 특히 만성노출에 의한 생식 및 성장에 미치는 영향은 치사에 미치는 영향농도보다 더 낮은 농도에서 나타나는 경향을 보이고 있다 따라서 방류수가 연속적으로 수계에 배출되는 특성을 고려하면 만성노출에 의한 영향을 평가하는 것이 중요한 의미를 갖는다. 한편 염색폐수의 경우 동일업종 또는 동일한 시료에서도 독성평가방법에 따라 서로 다른 정도의 독성이 관찰되는 등 독성발현이 매우 다양하게 나타났다. 그러므로 염색폐수가 수중생태계에 미치는 영향을 정확하게 파악하고 제어하기 위해서는 물고기나 조류 등 다른 영양단계에 있는 생물종에 미치는 영향 등의 연구가 요구된다.

실내공기질 공정시험법 중 VOCs 측정방법의 문제점 고찰 및 개선방안에 관한 연구 (Critical Evaluation of and Suggestions for the VOCs Measurement Method Established as the Korean Indoor Air Quality Standard Method)

  • During the last two decades, indoor air quality and volatile organic compounds (VOCs) have been of concern in Korean society due to their nature of potential health impacts. In order to investigate the pollution levels of VOCss in indoor environments, establishment of a solid test method for monitoring the airborne VOCss is essential. In Korea, a method based on adsorbent sampling and GC analysis coupled with thermal desorption was proclaimed as the Korea Standard Method for Indoor Air Quality Test. This study was carried out to examine some inherent problems of the VOCs measurement method. The VOCs method does not describe in detail preparing the standard samples. The standard samples may be prepared by impregnation of either liquid standard solutions or a mixture of standard gases. In this study, we investigated the optimal temperature condition for transferring the liquid standards onto a standard adsorbent tube. As a result, keeping the impregnation temperature at $250^{\circ}C$ will be recommended in regard of the boiling points of multiple target analytes and the thermal stability of the adsorbent. We also demonstrated some problems associated with handling of a syringe used for transferring the standard solutions onto the adsorbent tubes, and a best way to get rid of the syringe problems was suggested. Finally, a number of field works were conducted to evaluate the performance of adsorbent sampling methods. Comparison of different adsorbent tubes, i.e. tube packed with single sorbent (Tenax) and double sorbents (Tenax with Carbotrap), revealed that 30 to 40% differences between the two groups, implying that sampling efficiency is depending on the volatility and the strength of adsorbents. However, duplicate precisions for VOCs sampling with a same type of adsorbent and at same flow rates appeared to be satisfactory to be all within 20%, which is a quality control guideline. Distributed volume precisions were also found to be within a guideline value, 25%, although the precision was in general inferior to the duplicate precision. The Korea indoor VOCs test method should be more refined and improved in many aspects, particularly procedure and instrumentation for preparing the standard samples and specification of quality control assessment.

사료이용율 증가에 따른 비반추가축의 분뇨에 의한 공해발생 감소에 관한 연구 (A Study on the Reducing Pollutants in Non-Ruminant Manure by Increasing Feed Utilization)

  • 가축사육이 집단화되면서 가축배설물의 축적은 집중화 현상을 초래하게 되었고 또 계속적인 분뇨 분비물의 유출은 환경공해의 요인이 되고 있으며 이는 공공의 걱정거리로 대두되고 있다. 가축분뇨 중에서 질소와 인이 환경공해요인이 되는 가장 중요한 영양소이지만 가축이나 축산시설물에서 발생되는 냄새 역시 공해의 요인이 되고 있다. 급여되는 사료의 영양소효율을 높이는 길이 환경공해를 감소시키는 방법이다. 가장 많은 실험은 닭과 돼지를 대상으로 하여 이루어졌다. 사료첨가제의 이용, 사양방법의 수정 등으로 사료중의 영양소 이용성을 높임으로서 질소와 인의 배출감소는 물론 냄새와 분의 건물함량을 줄일 수 있었다. 1) 합성아미노산 제제를 사료 중에 첨가하고 단백질 수준을 감소시켰을 때 육계의 경우 10∼27%의 질소배출을 기대할 수 있었으며 산란계의 경우 18∼35%, 돼지의 경우 62%, 돼지에서 생성되는 냄새를 9∼43%까지 감소시키는 효과를 기대 할 수 있었다. 2) 사료 중에 효소제를 첨가하였을 때 육계 분의 건물함량을 12∼15% 감소시킬 수 있었다. 3) phytase를 첨가할 경우 분으로 배설되는 인의 함량은 양계의 경우 25∼35%,돼지의 경우 9∼43%까지 감소가 가능했다. 4) 성장촉진제를 이용할 경우 돼지에서 질소의 배설 양을 5∼30% 감소시킬 수 있었으며 냄새는 53∼55%까지 감소가 가능했다. 5) 사료배합을 요구량에 가깝게 하여 급여하였을 때 양계나 양돈의 경우 질소와 인은 각각 10∼15% 감소를 기대 할 수 있었으며 돼지의 냄새는 28∼79%까지 감소가 가능했다. 6) 기별 사양을 철저히 실시함으로서 양계와 양돈의 경우 질소는 10∼33%, 인산은 10∼13%, 그리고 성장 비육돈의 경우 49∼79% 감소를 기대 할 수 있다. 7) 닭이나 돼지의 사료를 배합시 소화율이 높은 곡류를 이용하면 질소와 인의 배설을 5% 감소시킬 수 있다.

수질유해물질 배출허용기준 설정에 따른 배출시설 비용영향 분석사례 연구: 1,4-Dichlorobenzene을 중심으로 (Case Study of Cost Effect Analysis for Toxic Compounds to Developing Effluent Limitation Standards : Focus on 1,4-Dichlorobenzene)

  • Recently, regulations on toxic compounds in aquatic environment have been strengthened in korea due to the increasing public awareness of the water quality. Typically, these regulations include introduction of emerging toxic compounds and stricter effluent limitations for the already regulated compounds. However, too strict regulations may cause excessive burden on the industry. Therefore it is also important to assess the economic impacts when the new effluent limitation guidelines are introduced. The estimation of the additional cost for the wastewater dischargers to meet the new guidelines are based on the selected treatment technology to handle the hazardous substances and the regulatory levels for effluent limitations. To explore the procedures for cost estimation in enforcing new effluent limitations, a case study was performed specially for 1,4-dichlorobenzene. The pollutants of concern are surveyed for different industrial categories and various treatment technologies. For a given pollutant, the general performances of the treatment technologies are surveyed and a representative technology is selected. For a given technology, the capital cost and annual Operation and Maintenance (O&M) cost was calculated. The calculation of baseline costs to operate ordinary treatment technologies is also important. The ratio between the cost for introducing new treatment process and the baseline cost required for conventional technology was used to evaluate the economic impact on the industry. For 1,4-dichlorobenzene, steam stripping and activated carbon processes were selected as the specific treatment technologies. The cost effects to the regulation of the compound were found to be 6.4% and 14.5% increase in capital cost and O&M cost, respectively, at the flow rate over $2,000m^3/d$ for the categories of synthetic resin and other plastics manufacturing industry. For the case of petrochemical basic compounds manufacturing industry, the cost increases were 5.8% and 12.4%, respectively. It was suggested that cost effect analysis to evaluate the economic impacts of new effluent limitations on the industry is crucial to establish more balanced and reasonable effluent limitations to manage the industrial wastewater containing emerging toxic compounds in the wastewater.

염화수은(II)과 이온화 방사선 처리에 따른 토양 내 환형동물의 DNA 손상 측정 (Evaluation of DNA Damage Induced by Mercury Chloride (II) and Ionizing Radiation in the Earthworm)

  • 각종 유전독성학적 물질로 인한 생물체내의 영향을 평가해보기 위해 E. fetida를 대상으로 본 연구를 수행하였다. 염화수은에 대한 DNA 손상을 알아보는 실험에서는 노출 시간에 상관없이 노출 농도에 비례한 유전자의 손상이 나타났다. 방사선이 지렁이의 DNA 손상에 미치는 영향을 알아본 실험에서도 역시 방사선 총 선량의 증가에 따라 DNA 손상이 증가하는 경향을 보였다. 염화수은에 48시간 동안 노출시키고 방사선을 조사한 지렁이의 세포를 comet assay하면, 수은 단독 처리군이나 방사선 단독 처리군에 비해 DNA의 손상이 유의적으로 크게 나타났다. 염화수은과 방사선 모두에 복합처리 된 지렁이의 DNA 손상치는 각각 단독 처리한 지렁이의 DNA 손상치를 합한 값보다 크게 나타나 두 요인의 상승작용이 확인되었다. 본 연구를 통해 지렁이의 세포내에서 수은과 방사선이 야기하는 DNA 손상을 측정하고, 두 인자의 복합처리에 따른 유전독성 상승효과를 관찰할 수 있었다. 이는 중금속과 방사선의 복합적인 효과를 나타낸 기존의 여러 연구결과와도 비교가 가능한 연구라고 사료되며, 향후 이를 보완하고 더 정확한 평가를 위해 지렁이 세포 내에서의 스트레스 반응 측정이나 효소 활성 실험등을 추가로 수행하여야 할 것이라고 생각된다.

유해 무기질의 자연정화 : 지화학적 고찰 (NATURAL ATTENUATION OF HAZARDOUS INORGANIC COMPONENTS: GEOCHEMISTRY PROSPECTIVE)

  • While most of regulatory communities in abroad recognize ' 'natural attenuation " to include degradation, dispersion, dilution, sorption (including precipitation and transformation), and volatilization as governing Processes, regulators prefer "degradation" because this mechanism destroys the contaminant of concern. Unfortunately, true degradation only applies to organic contaminants and short- lived radionuclides, and leaves most metals and long-lived radionuclides. The natural attenuation Processes may reduce the potential risk Posed by site contaminants in three ways: (i)contaminants could be converted to a less toxic form througy destructive processes such as biodegradation or abiotic transformations; (ii) potential exposure levels may be reduced by lowering concentrations (dilution and dispersion); and (iii) contaminant mobility and bioavailability may be reduced by sorption to geomedia. In this review, authors will focus will focul on "sorption" among the natural attenuation processes of hazardous inorganic contaminants including radionuclides. Note though that sorption and transformation processes of inorganic contaminants in the natural setting could be influenced by biotic activities but our discussion would limit only to geochemical reactions involved in the natural attenuation. All of the geochemical reactions have been studied in-depth by numerous researchers for many years to understand "retardation" process of contaminants in the geomedia. The most common approach for estimating retardation is the determination of distrubution coefficiendts ($K_{d}$) of contaminants using parametric or mechanistic models. As typocally used in fate and contaminant transport calculations such as predictive models of the natural attenuation, the $K_{d}$ is defined as the ratio of the contaminant concentration in the surrounding aqueous solution when the system is at equilibrium. Unfortunately, generic or default $K_{d}$ values can result in significant error when used to predict contaminant migration rate and to select a site remediation alternative. Thus, to input the best $K_{d}$ value in the contaminant transport model, it is essential that important geochemical processes affecting the transport should be identified and understood. Precipitation/dissolution and adsorption/desorption are considered the most important geochemical processes affecting the interaction of inorganic and radionuclide contaminants with geomedia at the near and far field, respectively. Most of contaminants to be discussed in this presentation are relatively immobile, i.e., have very high $K_{d}$ values under natural geochemical environments. Unfortunately, the obvious containment in a source area may not be good enough to qualify as monitored natural attenuation site unless owner demonstrate the efficacy if institutional controls that were put in place to protect potential receptors. In this view, natural attenuation as a remedial alternative for some of sites contaminated by hazardous-inorganic components is regulatory and public acceptance issues rather than scientific issue.

음식점 비흡연 종업원의 간접흡연 노출량 지표로써 이산화질소 이용 (Use of Nitrogen Dioxide as Exposure Marker of Passive Smiking for Non-smoking Service-workers at Restaurants)

  • There is increasing evidence suggestion that passive smoking increases the risk of lung cancer and other disease, though the potential health effects of exposure to environmental tobacco smoke (ETS) is a controversial subject. Since smoking in restaurant is prevalent in Korea, the concern on passive smoking exposure of non-smoking service-workers has been requested. ETS exposure of non-smoking service-workers at restaurant was assessed because they hare spent their times in restaurant indoors. The purpose of this study was feasibility of nitrogen dioxide($NO_2$) as exposure marker of ETS. The results of the study were as follows; 1. Average $NO_2$ concentrations in indoor and outdoor t restaurants were 57.1ppb(${\pm}12.4$) and 54.29ppb(${\pm}9.54$), respectively. Comparing office-workers, service-workers at restaurants were exposured highly. 2. The personal $NO_2$ measurement as exposure marker of ETS could cause the exposure error because $NO_2$ can be generated by combustion appliances in indoor. 3. Service-workers spent their most time(86.6%) in indoor. Mean time spent at restaurant indoors and at home was 9.4 hours and 10.9 hours, respectively. 4. Personal $NO_2$ levels correlated with indoor $NO_2$ concentrations of restaurant (r=0.70) and of their home (r=0.52) rather than of outdoor $NO_2$ concentration of restaurant (r=0.35). The cause of personal $NO_2$ exposure of non-smoking service-workers were considered as smoking of guests and combustion appliance indoors. 5. personal $NO_2$ exposures were estimated using Monte-Carlo simulation and time-weighted model. Estimated personal $NO_2$ level was 47.25ppb(${\pm}8.3$).

계분을 원료로 한 퇴비의 퇴비화 과정 중 동물용 항생물질 농도저감 (Decline in Extractable Veterinary Antibiotics in Chicken Manure-Based Composts during Composting)

  • Release of veterinary antibiotics (VSs) to agricultural environment through application of animal manure and/or animal manure-based composts to soils is of concern. The current study was conducted to examine decline of VAs during composting the chicken manure. For this, antibiotics free chicken manure (20 kg) and sawdust (10 kg) were added to the bench-scale composting apparatus and then the mixed material was spiked simultaneously with three VAs (chlortetracycline, CTC; sulfamethazine, SMZ; tylosin, TYL) at two different levels (10 and $20mg\;kg^{-1}$). Then the decline of VAs was determined using Charm II system during 53 composting period. For comparison, composting only chicken manure was included at VAs concentration of $10mg\;kg^{-1}$. During composting, the concentration of all three different VAs declined below the prospective guideline values ($0.8mg\;kg^{-1}$ for CTC, $0.2mg\;kg^{-1}$ for SMZ, and $1.0mg\;kg^{-1}$ for TYL) except CTC at $20mg\;kg^{-1}$ spiking when the chicken manure was composted together with sawdust. Interestingly, CTC at $10mg\;kg^{-1}$ spiking appeared to be declined under the guideline value without sawdust while SMZ was resistant to be declined without sawdust. Unlike CTC and SMZ, TYL showed immediate decline right after spiking TYL to composting materials regardless the spiking concentration and existence of sawdust. Appropriate composting procedure of chicken manure was able to decline the residual VAs in the manure below the prospective guideline value and the importance of organic substances on this decline was perceived.

간질 환자에서 보이는 불안의 임상적 특징 (Clinical Characteristic of Anxiety in People with Epilepsy)

  • 연구목적 : 불안은 간질 환자에서 흔히 동반되는 증상이며, 간질의 치료와 경과에 영향을 주는 요인으로 알려져 있다. 본 연구는 간질 환자의 불안과 간질의 관련성을 알아보고자 시행하였다. 방법 : 2008년 1월부터 7월까지 80명의 간질 환자를 대상으로 하였고 상태-특성 불안 척도(State-Trait Anxiety Inventory ; STAI)를 이용하여 불안을 평가하였다. 대조군으로 113명의 정상 성인을 포함시켰다. 결과 : 상태불안과 특성불안 점수의 평균은 환자군과 대조군에서 차이가 없었다. 환자군 내에서 직업이 없는 환자와 우울이 있는 환자는 상태불안, 특성불안이 모두 높은 점수를 보였고 전조증상이 있는 환자들은 특성불안의 점수가 높았다. 결론 : 비록 간질 환자의 불안 정도가 정상 대조군보다 높다는 결과를 얻지는 못했지만, 간질 환자의 불안이 증상과 예후에 미칠 가능성이 있음을 알 수 있었다.
