Bone Mineral Density and Breast Cancer Risk Factors among Premenopausal and Postmenopausal Women - A Systematic Review

  • Background: Bone mineral density (BMD) is a lifetime marker of estrogen in a woman's body and has been associated with increased breast cancer risk. Nonetheless the actual association is still debatable. Furthermore, estrogen is very crucial in maintaining human bone density and gradually decreases over age. A systematic search was conducted to assess any association of BMD with breast cancer risk factors among premenopausal and postmenopausal women. Materials and Methods: Review identification was performed through databases searching on MEDLINE, CINAHL and SCOPUS and 19 qualified studies were elected. The keywords used were "bone mineral density", "breast cancer", and "breast density". Results: A total of 19 articles showed variation with the majority of the studies focused on postmenopausal and a few focused on premenopausal women. Overall there was no concensus on effects. Conclusions: An enormous effort is being undertaken by researchers to prove that BMD might be one of the significant risk factors for breast cancer.

The Development of the Composite Index as a method of rate adjustment (의료보험수가 조정을 위한 복합지표 개발에 관한 연구)

  • The current method of rate adjustment is based on the evaluation of the financial performance of hospitals. The method has the disadvantages such as too complicated, expensive process as well as low reliability due to small sample size. This study, therefore, develops a new model for the rate adjustment with the use of the composite index. In addition to that, it examines the validity of the model by comparing the result of the new method with that of the conventional method. The idea of the new model comes from the Medicare Economic Index(MEI) on which physician fees for the Medicare patients are adjusted periodically in the United States. Medical costs are classified into three groups : labor costs, materials and other expenses. Labor costs are subdivided into physicians and other personnels. Materials are subdivided into drugs and others. Other expenses are subdivided into 5 items. Macro economic indices are selected for each cost item in order to reflect the cost inflation during the specific period. Then the composite index which integrate all items according to the ration of each item in the total costs is calculated. The result from the application of empirical data to the new model is very similar to that of the current method. Furthermore, this method is very simple and also to easy to get social concensus. This model can be replaced the current method based on the analysis of the financial performance for the adjustment of medical fees.

Vertical Distribution of Persistent Heavy metals in Core Sediments from Upo Wetland (자연습지 우포늪 퇴적물의 연도별 잔류성 중금속 축적도)

  • Sediment cores were obtained from Upo and Mokpo in Upo Wetland and core samples were divided by depth into 20 ~ 21 subsamples. The heavy metal concentrations of Fe, Mn, Zn, AS, Cu, Cd, Ni, Pb, and Cr in the sediments of each depth were determined by ICP-MS. The texture of sediemnts from Upo Wetland appeared to be clayey silt with average grain size of $7.52{\sim}11.15{\mu}m$ for physical properties. It was found to have a clear tendency of depth profile with respect to TOC and ignition loss. Organics were stabilized in the range of 0.5 ~ 0.7 % TOC and 8 ~ 9 % ignition loss in 30 years, whereas, the surficial sediments have the highest concentrations of about 3.0 % of TOC and 13 ~ 15 % ignition loss. Those are much higher than the values of the main stream, the Nakdong River, which reflects the deposit of biodegradable organics from plants and other lifes. The vertical distribution of heavy metals in two sediment cores was investigated to elucidate historical trends of heavy metals deposited into Upo wetland. The depth profile concentrations of each heavy metal were compared and discussed with the Concensus-Based Sediment Quality Guidelines for freshwater ecosystems. All the Cd data for the vertical distribution in the sediments were detected above PEC value for Cd, which predict harmful effects on sediment-dwelling organisms expected to occur frequently. The concentrations of Zn, Cu, and Cr in all sediment samples for depth profile were detected below the TEC values, which provided a basis predicting the absence of toxicity by Zn, Cu, and Cr.

  • Recent rapid changes such as construction market reduction, lowest bidding system of construction cost, five days per week system, and post-construction sale system are inevitably accelerating the competitiveness between construction companies. Those rapid changes of construction environment require construction companies, which are main participants of construction industry, to adapt to the changes and more to lead new construction environment. However, there are many construction uncertainties which could delay construction completion. Without the proper identification of construction uncertainties, structured strategy establishment and management of construction schedule, the competitive advantages of construction duration could not be made. Thus this paper limited the research boundary to construction duration competitiveness instead of general strategy establishment. Recognizing the importance of proper calculation of construction duration, requirements of construction duration reduction and limited researches related to construction schedule strategy, we established the 25 strategies for construction duration competitivenss by internal and external construction environment analysis and SWOT analysis, and then elicited the first considerations to accomplish the strategies through concensus of all the staffs by questionnaire, brainstroming and face-to-face interviews.

A Study on the Factors Affecting the Adaption of e-SCM on Korean Hospitals (우리나라 병원의 e-SCM 도입에 영향을 미치는 요인에 관한 연구)

  • In order to achieve the purpose, the present study reviews first research papers, both domestic and foreign, related to the e-SCM; it, then, sorts out the variables, economic and otherwise, that are believed to be the most important to the adaption factors of the e-SCM on Korean Hospitals. In recent days a growing concensus has been emerging among Korean Hospitals that a e-Supply Chain Management(SCM hereafter), if properly introduced and effectively managed, could secure a competitive edge in their Hospital business. Therefore, there is an urgent heed for an study that will attempt to find out the most significant factors, and most proprice Hospital environment, affecting the implementation of e-SCM in Korean Hospitals. For this study, 101 questionnaires were collected from the employee who is work at Korean Hospitals. We will make the concept by each variable clear through manipulative definition of each variable, also we will compose questions for the characteristics by each variable to be reflected. In relation to the questions required for the data analyses, 5 points Likert scale will use. The main findings are as follows: The relationship between the hospital types, hospital size and employee size and adaption degree of e-SCM was analyzed, respondents who had higher degree of hospital size and employee size showed higher level of adaption of e-SCM Regression analyses were carried out to examine the relative contribution of three set of variables on the adaption degree of e-SCM. This study explains that hospital environment factor, partner factor and IT infra factor are major factors in predicting the adaption degree of e-SCM. This study data has its limitation that collected by internet questionnaires. To present more solid data for the purpose of improving the study of e-SCM, further study which would complement the shortcomings is needed.

Randomized Controlled Clinical Trials in the Journal of Korean Society of Chuna Manual Medicine for Spine and Nerves: A Quantitative and Qualitative Analysis (척추신경추나의학회지에 수록된 무작위대조임상 연구의 양적, 질적 분석)

  • Objectives: The aim of this study is to assess the quantity and quality of randomized controlled clinical trials (RCTs) published in the journal of Korean Society of Chuna Manual Medicine for Spine and Nerves (KSCMSN). Methods: All relevant RCTs were selected and extracted. Data extract of RCTs from all the articles published in the jounal of KSCMSN up to now, quantity assessment was made on the study design, sample size, intervention, control group and medical condition. of the consolidated standards of reporting trials (CONSORT) check list. Assessment was performed by 2 independent reviewers and disagreement was discussed based on concensus. Results: Among the 276 articles, 10 RCTs were published. 1st articles were published in 2003 and half of RCTs were published in 2011. All RCTs were parallel 2-arm designed. Average sample size was 29.2 per study and 14.6 per arm. Main intervention was consisted as acupuncture 40%, pharmacopuncture 30% and Chuna 30%. Average of adequacy of CONSORT check list was 10.3% and overall reports were insufficient. Conclusions: Though RCTs published in jounal of KSCMSN were increasing, the quality remains low. KSCMSN should make a effort to follow the CONSORT statement and improve the quantity and quality of studies.

  • Blockchain can control transactions in a decentralized way and is already being considered for manufacturing, finance, banking, logistics, and medical industries due to its advantages such as transparency, security, and flexibility. And it is predicted to have a great economic effect. However, Blockchain has a Trilemma that is difficult to simultaneously improve scalability, decentralization and security characteristics. Among them, the biggest limitation of blockchain is scalability, which is very difficult to cope with the constantly increasing number of transactions and nodes. To make the blockchain scalable, higher performance should be achieved by modifying existing consensus methods or by improving the characteristics and network efficiency that affect many ways of scaling. Therefore, in this paper, we propose a cluster-based scalable PBFT consensus algorithm called CBS-PBFT which reduces the message complexity O(n2) of PBFT to O(n), which is a representative consensus algorithm of blockchain, and the validity is verified through simulation experiments.

Randomized Controlled Clinical Trials in the Journal of Oriental Rehabilitation Medicine : A Quantitative and Qualitative Analysis(1) (한방재활의학과학회지에 수록된 무작위대조임상연구의 양적, 질적 분석(1))

  • Objectives : The aim of this study is to assess the quantity and quality of randomized controlled clinical trials(RCTs) published in the journal of oriental rehabilitation medicine(JORM). Methods : After searching RCTs from all the articles published in the JORM from the inception(1991) to Oct, 2012, quantity assessment were made on the study design, sample size, main intervention, intervention versus control design and medical condition. Quality assessment were made on the cochrane risk of bias(RoB) check list. Assessment was performed by 2 independent reviewers and disagreement was discussed based on concensus of all authors. Results : Among the 1013 articles, total 59 RCTs were published. First RCT was published in 1994. 49 RCTs were parallel 2-arm designed. Average sample size was 35.1 per study and 16.3 per arm. The most common medical condition is musculoskeletal disease(30.5%). Quality from cochrane RoB was generally low. Conclusions : Though RCTs published in JORM were increasing, the quality remains low. Researchers should make a effort to follow the RoB checklists and improve the quantity and quality of studies.

Cloning and High Expression of Nattokinase Gene from Bacillus subtilis BB-1 (Bacillus subtilis BB-1으로부터 나토키나아제 유전자 크로닝 및 대량발현)

  • A fibrinolytic enzyme gene was isolated from Bacillus subtilis BB-1 by PCR method. Primers for PCR cloning were designed according to pre-identified gene for fibrinolytic enzymes from B. subtilis. The primer sequences were 5'-CGG ATC CGT GAG AGG CAA AAA GGT G-3' and 5'-TGA ATT CTT AAT GTG CTG CTG CTT GTC C-3' as concensus sequences of the fibrinolytic genes of Bacillus species. The PCR product was 1,145 bp and the sequence homology was 99% with nattokinase gene isolated from Japanese natto. The cloned fibrinolytic gene was reconstructed in Bacillus-E. coli shuttle vector, pEB for bulk-production. The fibrinolytic enzyme was purified by FPLC from the cloned B. subtilis 168. The optimum pH and temperature of the enzyme were 7.0 and $35^{\circ}C$, respectively. The fibrinolytic enzyme did not show any activity toward to skim milk, gelatin, casein and blood agar plate. The enzyme specific polyclonal antibody was prepared in rabbit for further assays such as detection of the gene expression in plant cells. This means that the enzyme may be used for health-care such as thrombosis without any hamful effects in the blood vessel.

Cloning and Characterization of Replication Origins from Misgurnus mizolepis (미꾸라지로부터의 복제원점 클로닝 및 그 특성에 관한 연구)

  • The nuclear matrix was isolated from Misgumus mizolepis liver nuclei by low salt extraction and restriction enzyme treatment. The structure was digested with proteinase K. After centrifugation, matrix attachment regions (MARs) were obtained by RNase treatment and phenol-chloroform extraction. The result leads to the appearance of smeared bands in the range of about 0.3-15 kb. pURY19 vector was constructed by inserting 2.13 kb Eco47 III fragment of the yeast uracil 3 gene into the unique Ssp I site of pUC19 plasmid vector as a selection marker. This vector is unable to be maintained in Sacrharomyces cerevisiae by itself since it cannot replicate as an extrachromosomal element. Using this system, we attempted cloning the ARS (autonomously replicating sequence) from M. mizelepis to develop an efficient expression vector for the transgenic fish. pURY19N_{l-62}$ were constructed by inserting MARs in pURY19 plasmid vector and transformation of E. coli $DH5\alpha$. Replication origins (ARS) of M. mizolepis were isolated, which enabled the vector to replicate autonomously in S. cerevisiae. The cloned DNA fragments were sequenced by Sanger's dideoxy-chain termination method. All clones were AT-rich. $pURY19N_6$, one of the clones, expecially contained ARS consensus sequence, Topoisomerase II consensus, near A-box and T-box.
