저농도 조영제를 사용한 CT검사에서 저관전압 기법에 따른 유용성 평가 (Evaluation of the Low Tube Voltage in the Computed Tomography Scan Technique using a Low Concentration Contrast Agent)

  • 본 연구는 전산화단층검사(computed tomography; CT)에서 저농도 조영제와 저관전압 기법이 선량과 영상에 미치는 유용성을 팬텀으로 평가하였다. 조영제 농도별로 희석정도를 달리하여 실험팬텀에 삽입 후 관전압과 관전류를 단계적으로 변화하여 촬영하였고, 획득된 팬텀영상을 통해 선량과 HU를 평가하였다. 그 결과 표준조건(350 mgI/ml, 120 kV)보다 저농도 조영제와 저관전압 조건(300 mgI/ml, 100 kV)설정이 체적 선량평가(CTDIvol; computed tomography dose $index_{vol}$)에서 평균 21% 감소하였다. SNR은 저농도 조영제와 저관전압 조건이 팬텀에서 측정한 모든 깊이(center, 4.5cm, 2.25cm)의 조영제와 생리식염수를 1:10, 1:20으로 희석한 실험에서 각각 12.2(26%), 6.2(17%) 증가하였다. CNR은 각각 11.5(32%), 6.3(26%) 증가하였다. CT 검사에서 조영제 부작용 감소를 위한 저농도 조영제의 사용과 피폭선량 감소를 위한 저관전압 사용으로 영상에서 높은 감약계수를 통한 다양한 연구가 필요할 것으로 사료된다.

양성자치료계획을 위한 이중에너지 전산화단층촬영 잡음 제거 영상 기반 저지능비 추정 방법 (Stopping Power Ratio Estimation Method Based on Dual-energy Computed Tomography Denoising Images for Proton Radiotherapy Planning)

  • 전산화단층촬영(computed tomography, CT) 영상은 양성자 브레그 피크 위치 추정 및 치료 계획 시뮬레이션의 기초로 사용된다. Hounsfield Unit(HU) 기반의 양성자 저지능비(stopping pwer ratio, SPR) 예측 과정에서 환자의 밀도와 원소 구성의 작은 차이로 양성자 빔의 경로를 따라 브레그 피크 위치의 불확실성이 발생한다. 본 연구에서는 브레그 피크 위치 예측 불확실성 감소를 위하여 이중에너지 전산화단층촬영 영상 기반의 양성자 저지능비 예측 정확도의 잠재력을 연구를 하였다. 양성자 빔의 저지능비를 추정하기 위해 전산화단층촬영 시스템(Somatom Definition AS, Siemens Health Care, Forchheim, Germany)을 이용하여 전자밀도팬텀(CIRS Model 062M electron density phantom, CIRS Inc., Norfolk, VA, USA)의 단일에너지 및 이중에너지 영상을 획득하였다. 이를 검증하기 위해 미국 국립 표준기술 연구소(National Institute of Standards and Technology, NIST)에서 제공하는 표준 데이터를 통하여 추정한 실제 저지능비와 비교하였다. 그 결과 잡음이 제거된 이중에너지 영상 기반 방법을 통한 양성자 빔의 저지능비 예측에서 정확도 개선 가능성을 확인할 수 있었으며, 인체의 다양한 밀도와 원소 구성을 가진 대체물을 더욱 다양하게 제작하여 저지능비를 예측 할 경우 더욱 향상된 양성자의 브레그 피크 위치 예측이 가능할 것으로 사료된다.

Odontogenic myxoma: a case report with recent image modalities

  • The odontogenic myxoma is an benign, slow growing neoplasm which is of ectomesenchymal origin. This neoplasm occurs almost exclusively in the jaw bones and comprises 0.2% to 17.7% of odontogenic tumors. The odontogenic myxoma may show a wide spectrum of radiographic appearances, unilocular, multilocular radiolucency and a distinct or diffuse border, making the differential diagnosis difficult. We present a case of the odontogenic myxoma in the maxilla with conventional and recent image modalities. Occlusal film revealed a medially extended multilocular lesion with intralesional fine and straight trabeculations from the scalloped margin and buccal expansion and thinning of cortical bone. Computed tomogram revealed lesion showed equivalent density to the muscles in the left maxillary sinus with partial cortical discontinuity of medial wall and the tennis-racket pattern with internal straight trabeculations. MRI revealed intermediate signal intensity on Tl weighted image and high signal intensity on T2 weighted image. In Gd enhanced MR image, the peripheral portions of the lesion were enhanced.

Evaluation of the Midpalatal Suture Maturation in Young Koreans Using Cone-Beam Computed Tomography

  • Purpose: The aim of this study was to evaluate the ossification and maturation of the midpalatal suture in young Koreans using cone-beam computed tomography (CBCT). Materials and Methods: The study sample consisted of 40 patients with ages from 8.2 to 23.6 years who visited the Department of Orthodontics, Chung-Ang University Dental Hospital. CBCT images were taken for diagnosis. From the CBCT image, morphological stages and Hounsfield units (HU) of midpalatal suture were obtained to evaluate the midpalatal suture maturation. Spearman's correlation coefficients were calculated to analyze relationships between chronological age, morphological stage, and HU. Result: There was a wide variation in the morphology of the midpalatal suture in each age group. It showed a modest relationship between the chronological age and the degree of morphological stages. Also there was a modest relationship between the chronological age and HU. In contrast, there was a significant relationship between morphological stages and HU of midpalatal suture (P<0.01). Conclusion: CBCT images can be used for the evaluation of midpalatal suture maturation. For the estimation of the prognosis of rapid maxillary expansion, CBCT may be reliable for the assessment of the maturation of the midpalatal suture.

Current status of dental caries diagnosis using cone beam computed tomography

  • Purpose : The purpose of this article is to review the current status of dental caries diagnosis using cone beam computed tomography (CBCT). Materials and Methods : An online PubMed search was performed to identify studies on caries research using CBCT. Results : Despite its usefulness, there were inherent limitations in the detection of caries lesions through conventional radiograph mainly due to the two-dimensional (2D) representation of caries lesions. Several efforts were made to investigate the three-dimensional (3D) image of lesion, only to gain little popularity. Recently, CBCT was introduced and has been used for diagnosis of caries in several reports. Some of them maintained the superiority of CBCT systems, however it is still under controversies. Conclusion : The CBCT systems are promising, however they should not be considered as a primary choice of caries diagnosis in everyday practice yet. Further studies under more standardized condition should be performed in the near future.

다중코어 프로세서를 이용한 전산화단층촬영의 재구성 시간 개선 (Improvement in Reconstruction Time Using Multi-Core Processor on Computed Tomography)

  • 전산화단층촬영에서 재구성 과정은 상당한 시간이 요구된다. 단면 영상의 품질을 높이기 매트릭스 크기를 증가시키면 재구성 시간이 매우 빠른 속도로 증가한다. 다중코어 프로세서는 오늘날 광범위하게 사용되고 있으며, 다중코어 프로세서의 다중 스레드를 이용하여 계산 시간을 줄이는 것이 가능하다. 본 연구는 다중 스레드로 CT의 재구성 시간을 개선하였다. 다중 스레드를 위해 Pthread와 OpenMP를 이용하였고, 재구성 과정에서 많은 시간이 소비되는 컨볼루션과 역투역 과정을 자세히 조사하였다. Pthread와 OpenMP 모두 스피드업과 효율성 측면에서 비슷한 성능을 나타내었다.

  • Purpose: This study compared panoramic radiography and cone beam computed tomography (CBCT) for evaluating the relationship between the maxillary sinus floor and the roots of maxillary molars. Materials and Methods: Paired panoramic radiographs and CBCT images from 97 subjects were analysed. This analysis classified 388 maxillary molars according to their relationship to the maxillary sinus floor on panoramic radiograph and CBCT. Correlations between these two radiographic techniques were examined. Results: Maxillary molar roots that were separate from the sinus floor showed the same classification in 100% of the cases when using these two imaging techniques. The corresponding percentage for such roots that were in contact with the sinus floor was 75%. When roots overlapped the maxillary sinus floor on panoramic radiographs, only 26.4% of maxillary first molars and 60.0% of second molars showed protrusion of roots into the sinus with CBCT. Conclusion : The results of the study suggest that roots projecting into the sinus on panoramic radiographs require a three-dimensional image in order to analyze the proximity of their apex to the sinus floor. 조영증강 전산화단층촬영검사에서 보이지 않는 이하선의 다형선종 (Invisible Parotid Gland Pleomorphic Adenoma on Contrast Enhanced-Computed Tomography)

  • Pleomorphic adenomas is the most common tumor of parotid gland and usually located and confined in superficial lobe of parotid gland. Computed tomography (CT) is commonly used to initially evaluate salivary gland lesion, but contrast-enhanced CT may sometimes fail to reveal lesions in spite of a high clinical suspicion. For this reason, ultrasonography (US) can be used as the first-line image work-up in some cases of parotid gland benign tumors. We experienced a case of a 60-year-old woman without underlying disease presenting a palpable parotid mass in which the initial CT examination was reported as 'no obvious mass detected'. However, the lesion was revealed in US and histologically confirmed as pleomorphic adenoma. The patient underwent superficial parotidectomy through modified facelift incision. To the best of our knowledge, this is the first report of invisible major salivary gland tumor on CT in the Korean literature.

Experimental evaluation of fuel rod pattern analysis in fuel assembly using Yonsei single-photon emission computed tomography (YSECT)

  • The purpose of this study was to verify the possibility of fuel rod pattern analysis in a fresh fuel assembly using the Yonsei single-photon emission computed tomography (YSECT) system. The YSECT system consisted of three main parts: four trapezoidal-shaped bismuth germanate scintillator-based 64-channel detectors, a semiconductor-based multi-channel data acquisition system, and a rotary stage. In order to assess the performance of the prototype YSECT, tomographic images were obtained for three representative fuel rod patterns in the 6 × 6 array using two representative image-reconstruction algorithms. The fuel-rod patterns were then assessed using an in-house fuel rod pattern analysis algorithm. In the experimental results, the single-directional projection images for those three fuel-rod patterns well discriminated each fuel-rod location, showing a Gaussian-peak-shaped projection for a single 10 mm-diameter fuel rod with 12.1 mm full-width at half maximum. Finally, we successfully verified the possibility of the fuel rod pattern analysis for all three patterns of fresh fuel rods with the tomographic images obtained by the rotational YSECT system.

Cone-beam computed tomography in endodontics: from the specific technical considerations of acquisition parameters and interpretation to advanced clinical applications

  • The implementation of imaging methods that enable sensitive and specific observation of anatomical structures has been a constant in the evolution of endodontic therapy. Cone-beam computed tomography (CBCT) enables 3-dimensional (3D) spatial anatomical navigation in the 3 volumetric planes (sagittal, coronal and axial) which translates into great accuracy for the identification of endodontic pathologies/conditions. CBCT interpretation consists of 2 main components: (i) the generation of specific tasks of the image and (ii) the subsequent interpretation report. A systematic and reproducible method to review CBCT scans can improve the accuracy of the interpretation process, translating into greater precision in terms of diagnosis and planning of endodontic clinical procedures. MEDLINE (PubMed), Web of Science, Google Scholar, Embase and Scopus were searched from inception to March 2023. This narrative review addresses the theoretical concepts, elements of interpretation and applications of the CBCT scan in endodontics. In addition, the contents and rationale for reporting 3D endodontic imaging are discussed.
