Purpose: This study aimed to explore the value of IHC4 in predicting pathological response after neoadjuvant chemotherapy in patients with hormonal receptor (HR)-positive breast cancer (BC). Materials and Methods: In this retrospective exploratory study, data for 68 HR-positive BC patients who received neoadjuvant chemotherapy were recorded. IHC4 scores were calculated based on estrogen receptors/progesterone receptors, Ki-67 and HER2 status. Logistic and ordinal regression analyses in addition to likelihood ratio test were used to explore associations of IHC4 scores and other clinico-pathological parameters with pathological complete response (pCR) and pathological stage. Results: Taking the 25th percentile as the cut-off, a lower IHC4 score was associated with an increased probability of pCR (low; 52.9% vs. High; 21.6%, OR=4.1, 95% CI=1.28-13.16, p=0.018) and a lower pathological stage (OR=3.9, 95% CI=1.34-11.33, p=0.012). When the IHC4 score was treated as a continuous variable, a lower score was again associated with an increased probability of pCR (OR=1.010, 95% CI=1.001-1.018, p=0.025) and lower pathological stage (OR=1.009, 95% CI=1.002-1.017, P=0.008). Lower clinical stage was associated with a better pCR rate that was of borderline significance (P=0.056). When clinical stage and IHC4 score were incorporated together in a logistic model, the likelihood ratio test gave a P-value of 0.004 after removal of the IHC4 score and 0.011 after removal of the stage, indicating a more significant predictive value of the IHC4 score for pCR. Conclusions: This study suggests that the IHC4 score can predict pathological response to neoadjuvant chemotherapy in HR-positive BC patients. This finding now needs to be validated in a larger cohort of patients.
This experiment was carried out to determine the optimal concentration of agricultural organic materials using sulfur and oil for the insect pest control in pepper and cherry tomato cultivation. The control value of aphids and Oriental tobacco budworm (OTB) was examined one day after spraying with sulfur preparation (SP) (0.33~0.17%), oil preparations (OP) (2.00~0.33%), SP+OP, OP+ginkgo leaf extracts (GLE), SP+OP+GLE on the "Super Manidaa"pepper. The aphid control in pepper was complete by applications of SP+OP (0.25+1.00%) in the early growth stage and the control value was above 98.1% by the application of OP+GLE (1.00+1.00 %), SP+OP+GLE (0.25+1.0+1%), SP+OP+GLE (0.25+1.0+0.5%) in the middle to late growth stage while showing 0% in the control treatment. The OTB was completely controlled by the 3 times application with the high concentration of SP+OP (0.25+1.00%) in pepper cultivation. This result indicates that the oil and the sulfur preparations should be applied at low concentration before insect pests do not appeared, and then sprayed at the high concentration after they appear at pepper plant. The greenhouse whitefly in 'Minichal' tomatoes was completely controlled by three times application of SP (0.25~0.33), OP (1.0~2.00%). and all the treatment of SP+OP. However, continuous control with intervals of 1~3 days was considered favorable in the tomato plant. By the periodical control with agricultural organic materials using sulfur and oil, the greenhouse whitefly, which is a high-temperature insect pest, several moths of OTB did not occur at all. In conclusion, SP+OP (0.17%+0.33%) treatment was the most economical combination to control the aphid, OTB, and greenhouse whitefly in pepper and tomato cultivation when considering operating cost. In addition, we recommend that SP should not be sprayed on the plant shoots during the day time from July to August because of high temperature.

Hemophagocytic lymphohistiocytosis (HLH) occurs in the primary form (genetic or familial) or secondary form (acquired). The familial form of HLH (FHL) is a potentially fatal autosomal recessive disorder that occurs because of constitutional defects in cell-mediated cytotoxicity. Here, we report a fatal neonatal case of type 2 FHL (FHL2) that involved a novel frameshift mutation. Clinically, the newborn presented with severe sepsis-like features and required mechanical ventilation and continuous venovenous hemodiafiltration. Flow cytometry analysis showed marked HLH and complete absence of intracytoplasmic perforin expression in cytotoxic cells; therefore, we performed molecular genetic analyses for PRF1 mutations, which showed that the patient had a compound heterozygous mutation in PRF1, that is, c.65delC ($p.Pro22Argfs^*2$) and c.1090_1091delCT ($p.Leu364Glufs^*93$). Clinical and genetic assessments for FHL are required for neonates with refractory fever and progressive multiple organ failure, particularly when there is no evidence of microbiological or metabolic cause.

This study was conducted to describe the attitude toward pressure ulcer prevention, knowledge and non-compliance risk for pressure ulcer prevention practice and degrees of nursing performance, as well as to examine the relationship among these variables. To accomplish this, we invited 397 nurses from four hospitals to complete surveys of attitude toward pressure ulcer prevention, knowledge and certainty for pressure ulcer prevention practice, and degrees of nursing performance. Non-compliance risk for pressure ulcer prevention practice was derived from the difference between knowledge and certainty for pressure ulcer prevention. The data were collected from March to June 2017 and analyzed by descriptive statistics, independent t-tests, ANOVA, and partial Pearson's correlation coefficient testing. The correct answer rate regarding the knowledge of pressure ulcer prevention practice was 79%, and the degree of pressure ulcer prevention nursing performance was $2.46{\pm}0.31$. The highest item of the non-compliance risk for pressure ulcer prevention practice was incontinence diapers prevent incontinence-associated dermatitis. There were significant negative correlations between non-compliance risk for pressure ulcer prevention practice and knowledge (r=-0.25, p<0.001), and degree of pressure ulcer prevention nursing performance (r=-0.13, p=0.009). Continuous education and furnishing practical guidelines for pressure ulcer prevention should be implemented to improve knowledge and certainty of pressure ulcer prevention practice.

This study examined the effect of cognitive reserve on the cognitive function in adults. In addition, this study investigated the direct and indirect effects of factors, such as subjective memory decline, depression, and quality of life on the cognitive function. The participants consisted of 253 adults who were 40 to 80 years of age. The assessment tools included the cognitive function scale, depression scale, quality of life scale, subjective memory decline, and cognitive reserve scale. The data results were analyzed using SPSS 25.0, and PROCESS macro for SPSS Version 3.3 and bootstrapping were used. The results of multiple mediating of the cognitive reserve and cognitive function in adults did not have a mediating effect in depression and subjective memory decline, and the quality of life showed a complete mediating effect in the relationship between the cognitive reserve and subjective cognitive function. Based on the results of this study, adults must accumulate cognitive reserve continuously through various activities and continuous training so that the cognitive function can lead to a positive direction. Nevertheless, more studies on cognitive reserve and cognitive function in healthy adults are needed.

Purpose: We should prepare proper medical service for disaster control as South Korea is not free from terrorism and war, as we experienced through the two naval battles of the Yeonpyeong, one in 1999 and the other in 2002, the sinking of Cheonan in 2010, and the attack against the border island of Yeonpyeong in 2010. Moreover, North Korea's increasingly bellicose rhetoric and mounting military threats against the world demand instant action to address the issue. The aim of this article is to describe our experience with three patients with combat-related gunshot and explosive injuries to their extremities and to establish useful methods for the management of patients with combat-related injuries. Methods: Three personnel who had been injured by gunshot or explosion during either the second naval battle of the Yeonpyeong in 2002 or the attack against the border island of Yeonpyeong in 2010 were included in our retrospective analysis. There were one case of gunshot injury and two cases of explosive injuries to the extremities, and the injured regions were the left hand, the right foot, and the right humerus. In one case, the patient had accompanying abdominal injuries, and his vital signs were unstable. He recovered after early initial management and appropriate emergency surgery. Results: All patients underwent emergent surgical debridement and temporary fixation surgery in the same military hospital immediately after their evacuations from the combat area. After that, continuous administration of antibiotics and wound care were performed, and definite reconstructions were carried out in a delayed manner. In the two cases in which flap operations for soft tissue coverage were required, one operation was performed 5 weeks after the injury, and the other operation was performed 7 weeks after the injury. Definite procedures for osteosynthesis were performed at 3 months in all cases. Complete union and adequate functional recovery were achieved in all cases. Conclusion: The patient should be stabilized and any life-threatening injuries must first be evaluated and treated with damage control surgery. Staged treatment and strict adherence to traditional principles for open fractures are recommended for combat-related gunshot and explosive injuries to the extremities.
Removal of hexavalent chromium from artificial groundwater (AGW) by granular activated carbon (GAC) was investigated in batch and continuous-flow column studies. Experimental parameters that were examined included solution pH, presence of dissolved oxygen (DO), and GAC pretreatment with Fe(II). As the solution pH increased from 4 to 7.5, the amount of Cr(VI) removed by both GACs decreased significantly. Exclusion of DO from the experimental systems resulted in greater removal of Cr(VI) from solution, possibly as a result of reduction to Cr(III). However, pretreatment of the GAC with a reductant (Fe(II)) did not improve Cr(VI) removal. Equilibration With 0.01 M $K_2$$HPO_4$[to extract adsorbed Cr(VI)] followed by a wash with 0.02 N $K_2$$HPO_4$[to remove precipitated/sorbed Cr(III)] proved to be a viable approach for the regeneration of carbons whose Cr(VI) removal capacities had been exhausted. The performance of the regenerated carbons exceeded that of the virgin carbons, primarily because of the favorable adsorption of Cr(VI) at lower pH values and the reduction of Cr(VI) to Cr(III), The presence of Cr(III) in acid wash solutions provides direct evidence that Cr(VI) is reduced to Cr(III) in GAC systems under relatively acidic conditions. GAC performance over five complete cycles was consistently high, which suggests that such a system will be able to function over many operation cycles without deleterious effects.

With recent interest organic farming the use of legumes including vetch and clover to provide N to adjacent crops is increasing in Korea. In the present studies, we conducted a trial to investigate the effects of the application of N rate on nitrogen fixation and transfer from vetch to barley in mixed stands. The experiment was arranged in a randomized complete block design with three replications. Four different N rates(0, 75, 113 and 150/ha) was used and vetch+barley was broadcasted manually on 1.5 $\times$2 m plot in Oct. 2001. Half of urea and K$_{2}O, 200 Phosphate and 75 kg potash per ha were applied as basal dressing md half of N md 75 potash were used for topdressing to soil surface on MarctL 2002. The equivalent of 1kg ha$^{-1}$ at($^{15}$NH$_{4}$)$_{2}$SO$_{4}$ solution at 99.8 atom $\%$$^{15}$N excess was applied to the microplot in mid April. Forage was harvested from each plot at ground level and separated into barley and vetch. Total N content and It values of samples were determined using a continuous flow stable isotope ratio mass spectrometry(IsoPrime-EA. Micromass, UK.). The percentage of legume H fixed from atmospheric N2 were 95.0, 93.8, 94.4 and $84.8\%$ with increment of N levels. The percentage of N transfer from vetch to barley by N-difference method with increment of N fertilizer were from 58 to$49\%$ while 39 to $23\%$ in $^{15}$N-dilution method. The amount of transfer from vetch to barley were 87 to 68 kg/ ha with N level by N-difference moth여 and 58 to -56/ha with N application levels by $^{15}$N dilution method. The amount of nitrogen fixation per ha were from 150 kg / ha to 219 kg / ha by different method, but on the other side 49 to 105kg/ha by N$^{15}$-dilution.
At the spots of enterprises experiencing rapid environmental changes, necessity for ceaseless managemental innovation is raised. We can't deny that management quality is also a part of such managemental innovation. Introduction of management quality by an organization is different from introduction of other individual innovative techniques. Management quality is not one time project but complete historical change program to be pursued continuously. Furthermore, it must be understood as a strategic 'Quality Journey' looking for far future of an organization. Malcolm Baldrige Model has been increasingly used in Korea as a global standard for management quality. The model, however, has shown a number of limitations in application due to nonsystematic elements in the implementation process. The deployment process of the model should be detailed through a real world case study. In this research, we proposed a quality evaluation standard model which is suitable for the Korean public enterprise based on the Malcolm Baldrige Non-profit Criteria, and examined the model. A 7-Point Likert Scale was used based on the seven categories within the 2007 Malcolm Baldrige Non-profit Criteria: Leadership, Strategic Planning, Customer and Market Focus, Management Analysis and Knowledge Management, Workforce Focus, Process Management, and Results. Furthermore, we analysed the validity and causal relationship among the factors within the model. Through the above case study, following common factors for successful Malcolm Baldrige Model have been derived. The following is a summary of the results of this study. First, for Malcolm Baldrige Model management, needs powerful support Leadership. Second, for Malcolm Baldrige Model management, needs systematic Strategic Planning. Third, for Malcolm Baldrige Model management, needs the continuous Customer and Market Focus. Fourth, for Malcolm Baldrige Model management, needs systematic Measurement Analysis and Knowledge Management. Five, for Malcolm Baldrige Model management, needs systematic Workforce Focus. Six, for Malcolm Baldrige Model management, needs systematic Process Management. The finding are surely necessary factors to arrange the human resource management for upgraded management quality, those factors help to explain why all enterprises consider human resource as the strategic view.
