재가 노인의 주관적 신체 증상, 일상생활수행능력 및 건강관련 삶의 질 (A Study on Physical Symptom, Activity of Daily Living, and Health-Related Quality of Life (HRQoL) in the Community-Dwelling Older Adults)

  • Purpose: This study aimed to identify the relationships among physical symptoms, activities of daily living, and health-related quality of life (HRQoL) in community-dwelling older adults. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Physical symptoms were measured using the Physical Health Questionnaire (PHQ), activities of daily living using the Late-Life Functional and Disability Instrument (LLFDI), and HRQoL using the Medical Outcomes Short-Form Health Survey (SF-36) in 242 community-dwelling elderly Korean people. Results: The HRQoL correlates with the physical symptoms (r=-.31) and the function component (r=.59). Of the two disability parts of the LLFDI, the limitation dimension correlates higher (r=.57) with HRQoL than the frequency dimension (r=.42). The HRQoL is significantly associated with the function component, and disability limitation in capability which explained 44.4% of variance in physical health. Conclusion: These results may contribute to a better understanding of physical symptoms, activities of daily living, and HRQoL in community-dwelling older adults. Therefore, health programs for prompting older adult's health should be planned based on results of the study.

취약계층 당뇨병노인을 위한 건강멘토링프로그램의 효과 (The Effects of a Health Mentoring Program in Community-dwelling Vulnerable Elderly Individuals with Diabetes)

  • Purpose: This study aimed to estimate the effects of a health mentoring program on fasting blood sugar, total cholesterol, triglyceride, physical activity, self care behavior and social support changes among community-dwelling vulnerable elderly individuals with diabetes. Methods: A non-equivalent control group pre-post-test design was used. Participants were 70 community-dwelling vulnerable elderly individuals with diabetes. They were assigned to the experimental (n=30) or comparative (n=30) or control group (n=28). The experimental group participated in the health mentoring program, while the comparative group participated in health education program, the control group did not participate in any program. Data analyses involved a chi-square test, Fisher's exact test, a generalized linear model, and the Bonferroni correction, using SPSS 23.0. Results: Compared to the control group, the experimental and comparative groups showed a significant decrease in fasting blood sugar, total cholesterol, and triglyceride. Compared to the comparative and control groups, the experimental group showed significant improvement in self care behavior. However, there were no statistical differences in physical activity or social support among the three groups. Conclusion: These findings indicate that the health mentoring program is an effective intervention for community-dwelling vulnerable elderly individuals with diabetes. This program can be used as an efficient strategy for diabetes self-management within this population.

Screening Sarcopenia in Rural Community-Dwelling Older Adults in Korea

  • Purpose: Several screening tools have been developed to identify sarcopenia in rural community-dwelling older adults. We aimed to compare the diagnostic accuracy of two such tools, namely the SARC-F and SARC-CalF assessments. Methods: This cross-sectional study on 388 community-dwelling older adults comprised 254 women and 134 men with a mean age of 77.8 ± 6.26 year in Korea. We assessed muscle mass, muscle strength, and physical performance using a bioimpedance analysis device, hydraulic hand dynamometer, and 4 m gait speed test, respectively. Three widely-used diagnostic criteria [the Asian Working Group for Sarcopenia (AWGS), European Working Group on Sarcopenia in Older People, and the International Working Group on Sarcopenia] were applied. Sensitivity and specificity analyses were performed on the SARC-CalF and SARC-F tests. We used receiver-operating characteristic curves and the area under the curves (AUCs) to compare the diagnostic accuracy of the assessments with regard to sarcopenia. Results: An analysis using four sets of diagnostic criteria showed that the prevalence of sarcopenia was 27.6% to 41.0%. Using the AWGS 2019 criteria as a reference standard, the SARC-CalF had a sensitivity of 83.02% and a specificity of 53.71% in the entire study population, whereas the SARC-F had a sensitivity of 79.87% and a specificity of 41.92%. The AUCs for the SARC-CalF and SARC-F tests were 0.725 (95% confidence interval 0.678-0.769) and 0.645 (95% confidence interval 0.595-0.693), respectively (p<001). In the analyses using the other three diagnostic criteria, similarity was also confirmed. Conclusion: SARC-CalF showed better sensitivity than did SARC-F when diagnosing sarcopenia in rural community-dwelling older adults. Further studies are needed to verify this finding in different populations.

도시 지역 거주 노인의 연령집단별 삶의 만족감에 영향을 미치는 가족 요인과 지역사회 요인: 사회적 자본의 중요성을 중심으로 (Family and Community Factors Associated with Life Satisfaction of the Urban Community-dwelling Elderly across Age Groups: Focusing on the Importance of Social Capital)

  • Purpose: The aim of this study was to identify differences and influencing factors in the level of life satisfaction among the urban community-dwelling elderly by age group. Methods: The study was conducted utilizing the secondary data of 2017 Seoul Survey in a cross-sectional design. Of 42,688 participants in total, the data of 7,927 adults aged 65 or older were analyzed. The data were analyzed using descriptive statistics, independent t-test, chi-square test, Pearson's correlation coefficients, and multiple linear regression. Results: There were significant differences between age groups, and it was found that the old elderly groups had significantly higher life satisfaction than the oldest elderly group (t=8.37, p<.011). In common, family and community factors influencing life satisfaction in the two age groups were companion animals (old elderly: β=.03, p=.002; oldest elderly: β=.06, p=.021), social network (old elderly: β=.10, p<.001; oldest elderly: β=.08, p=.008), and social support (old elderly: β=.05, p<.001; oldest elderly: β=.08, p=.005). Conclusion: Based on these results, social welfare and nursing care services focusing on social capital and age group-specific interventions are needed to improve life satisfaction of the elderly. This study might provide the possibility and evidence for a program to improve life satisfaction for the urban community-dwelling elderly, including social capital elements.

지역사회 거주 노인의 금연의도 영향요인 (Factors Contributing to the Intention to Quit Smoking in Community-dwelling Elderly Smokers)

  • Purpose: The purpose of this study was to investigate factors associated with the intention to quit smoking in community-dwelling elderly smokers. Methods: From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES-IV), the researchers selected 448 elderly smokers over the age of 65. Data were analyzed with the $x^2$ test and multiple logistic regression using the SPSS/WIN 18.0 program. Results: Of the respondents, 45.5% intended to quit. The factors associated with the intention to quit were relative youth, having limit of activity day living and having past attempts to quit. Conclusion: Population-based smoking cessation programs, especially those targeted at the elderly, should take these predictors into consideration in the design of interventions.

도시형 코하우징 관심자의 거주요구 (Dwelling Needs of Residents Interested in Urban Cohousing)

  • The purpose of this study is to gain an in-depth understand of the dwelling needs of residents who are interested in Cohousing. To do this, we interviewed 18 urban residents who wanted a community life; investigated consciousness of neighbor relationships; and analyzed their consciousness of Cohousing residences, preference types, and spatial needs. At first the research has revealed that the neighbor relationships of the participants were not very close and that there was a difference in awareness levels between a 'good neighbor' and 'strange neighbor' with regard to neighbors, though all of the participants required a vitalization of these relationships. Second, Cohousing residence types could be divided into the 'life preferred type' and 'space preferred type', depending on the differences of pursued dwelling interests. These could be further segmented into the 'leisure life preferred type' and 'daily life preferred type', 'public space preferred type' and 'individual space preferred type'. Thirdly, preferences for Cohousing types varied depending on differences in priority with regard to important concepts of the social-physical environment (e.g., community property, effectiveness, naturalness, economic feasibility, and individuality). Finally, we found that the greatest needs were for public gardens and exercise space as aspects of the common spaces. Agreements pertaining to special use were considered as most important during the design process of the common spaces.

재가노인의 자아존중감, 자가간호와 생활만족도간의 관계 (The Relations between Self-esteem, Self-care and Life Satisfaction in the Home-dwelling Elderly)

  • Purpose: The present study was conducted to identify the relations between self-esteem. self-care and life satisfaction in the home-dwelling elderly. Methods: The subjects were 106 persons aged over 65 in J city. Data were collected from April to June 2005 by a questionnaire survey. The instruments used in this study are the self-esteem scale developed by Rosenberg(1965), the self-care scale by Hwang (2000) and the life-satisfaction scale by Choi (1986). Data were analyzed through t-test, ANOVA. least significant difference, Pearson's correlation coefficient and stepwise regression analysis using SPSS-Win 10.0. Results: Self-esteem and self-care were in a positive correlation with each other (r=.464, p=.000). Self-care and life-satisfaction showed a significant correlation with each other (r=.222 p=.031), and a significant correlation was found between self-esteem and life-satisfaction (r=.506, p=.000). The most significant predictors influencing life-satisfaction were self-esteem, self-care and perceived health condition, and the three factors accounted for 47.7% of variance in life satisfaction in the home-dwelling elderly. Conclusion: These results suggest that self-esteem, self-care and life-satisfaction can be important factors the quality of life of the elderly in long-term care. Therefore nurses can plan nursing interventions to promote self-esteem, self-care and life satisfaction of the home-dwelling elderly.

치매 환자의 격앙행동과 주간호자의 대처행동 (Agitation in Home-dwelling Persons with Dementia and Coping Behaviors in Primary Care-givers to the Agitation)

  • Purpose: The purpose of this study was to investigate agitation in home-dwelling persons with dementia and coping behaviors of primary family care-givers to agitation. Methods: The research was designed as a descriptive study. A total of two hundred and five subjects had participated in this study. To measure agitation in persons with dementia, Cohen-Mansfield Agitation Inventory was used. To measure coping behaviors of primary family care-givers to the agitation, a questionnaire was used. Descriptive statistics, t-test, ANOVA, Scheff$\acute{e}$ test were used to answer the research objectives. Results: 'Repetitive mannerisms' and 'repeating sentences' were the most commonly occurred agitation in home-dwelling persons with dementia. 'Leaving the situation', 'allowing certain time', and 'soothing by talking' were the most common coping behaviors occurred in primary care-givers. Conclusion: The findings of the current study would provide meaningful data to develop nursing programs to control agitation for home-dwelling persons with dementia and education programs for primary care-givers to cope with persons' agitation.

Group Locomotor Imagery Training-Combined Knowledge of Performance in Community-Dwelling Individuals With Chronic Stroke: A Pilot Study

  • We evaluated whether group locomotor imagery training-combined knowledge of performance (KP) lead to improvements in gait function in community dwelling individuals with chronic stroke. Ten adults who had suffered a hemiparetic stroke at least 6 months earlier participated in group locomotor imagery training-combined KP for 5 weeks, twice per week, with 2 h intensive training. Dynamic gait index scores increased significantly after the group locomotor imagery training-combined KP. However, times for the timed up-and-go test did not improve significantly after the training. Group locomotor imagery training-combined KP may be a useful option for the relearning of gait performance for community dwelling individuals with chronic hemiparetic stroke.

지역사회거주 노인의 연명치료 중단에 대한 자기결정 영향 요인 (Factors Influencing Self Determination for Withdrawing Life-Sustaining Treatment of the Community Dwelling Elderly)

  • Purpose: This study was conducted to identify factors that influence on self-determination for withdrawal of life-sustaining treatment in the community dwelling elderly. Methods: This study used a descriptive correlational design. A convenience sample of 201 elderly were recruited from a welfare center located in Seoul, Korea. Data collection was done from September 1 to September 18, 2015. Four survey questionnaires were used: Multidimensional Scale of Perceived Social Support (MSPSS), Life Satisfaction Index-Z (LSI-Z), Attitude on Do Not Resuscitate (DNR), and Self-determination for Withdrawing Life-sustaining Treatment. Data analysis was done by t-test, ANOVA, Pearson's correlation, and hierrarchical multiple regression. Results: Hierarchial multiple regression showed that the factors predicting the level of self-determination for withdrawing life-sustaining treatment were educational level, DNR experience in family, life satisfaction, and DNR attitude. These factors explained 44% of the self-determination for withdrawing life-sustaining treatment in community dwelling elderly. Conclusion: The results of this study suggest developing educational programs aiming at changing elderly's DNR attitude positive ways so that they can enhance self-determination for withdrawing life-sustaining treatment. Further study is needed with more elderly population in extended areas.
