Oculomotor Nerve Palsy Associated with Rupture of Middle Cerebral Artery Aneurysm

  • Oculomotor nerve palsy (ONP) with subarachnoid hemorrhage (SAH) occurs usually when oculomotor nerve is compressed by growing or budding of posterior communicating artery (PcoA) aneurysm. Midbrain injury, increased intracranial pressure (lCP), or uncal herniation may also cause it. We report herein a rare case of ONP associated with SAH which was caused by middle cerebral artery (MCA) bifurcation aneurysm rupture. A 58-year-old woman with clear consciousness suffered from headache and sudden onset of unilateral ONP. Computed tomography showed SAH caused by the rupture of MCA aneurysm. The unilateral ONP was not associated with midbrain injury, increased ICP, or uncal herniation. The patient was treated with coil embolization, and the signs of oculomotor nerve palsy completely resolved after a few days. We suggest that bloody jet flow from the rupture of distant aneurysm other than PcoA aneurysm may also be considered as a cause of sudden unilateral ONP in patients with SAH.

  • This paper proposes a robot driving system and sensor implementation for use with an education robot. This robot has multiple functions and was designed so that children could use it with interest and ease. The robot recognizes the location of a user and follows that user at a specific distance when the robot and user communicate with each other. In this work, the robot was designed and manufactured to evaluate its performance. In addition, an embedded board was installed with the purpose of communicating with a smart phone, and a camera mounted on the robot allowed it to monitor the environment. To allow the robot to follow a moving user, a set of sensors combined with an RF module and ultrasonic sensors were adopted to measure the distance between the user and the robot. With the help of this ultrasonic sensors arrangement, the location of the user couldbe identified in all directions, which allowed the robot to follow the moving user at the desired distance. Experiments were carried out to see how well the user's location could be recognized and to investigate how accurately the robot trackedthe user, which eventually yielded a satisfactory performance.

  • Old people and patients with cardiovascular disease could die of a heart attack in the bath with heated water for a long time. Various researches have been studied to prevent these accidents from happening such as measuring the ECG signal when taking bath. However, these devices are hard to use and the higher price is not easily accepted by the public. In this paper, a low-cost and use-friendly, real time high precision water temperature monitoring device to prevent heart attack in the bath was developed. The device with waterproof design that lets the device can float on the surface of the water, and an accurate way to make water temperature measurement method was proposed by this paper that is immerging the sensor into water with 4cm depth to measure the temperature of underwater. The manufactured device was conducted to two experiments; one was to verify the basic functions of the device, and another one was for compare the proposed device with commercial products for monitoring the water temperature in the bathtub. As the experimental results shown, the proposed device has stable performance for the water temperature measurement and communicating with laptop in wireless.

Ruptured Sinus of a Valsalva Aneurysm into the Left Ventricle with the Rupture Site Communicating with the Left Coronary Sinus and the Left Noncoronary Sinus (좌관상동맥동과 비관상동맥동이 좌심실로 파열된 발살바동 동맥류)

  • We report here on a case of a ruptured sinus of a valsalva aneurysm into the left ventricle with the rupture site communicating with both the left coronary sinus and the noncoronary sinus in a 37-year-old male who presented with symptoms of congestive heart failure. Echocardiography showed a sac-like structure around the sinus of valsalva, an enlarged left ventricle (LV) and severe aortic regurgitation, which all suggested a ruptured sinus of a valsalva aneurysm or an aortic-left ventricular tunnel. The operative findings revealed that both the left coronary sinus and the noncoronary sinus had an opening into the left ventricle. The proximal opening into the LV was closed with bovine pericardium and the aortic root was replaced with a composite graft (a 21 mm St. Jude Epic Supra tissue valve and a 24 mm Hemashild graft) by the modified Bentall procedure. The patient was discharged on the 15th postoperative day, and he was regularly followed up for 2 months. We report on this case due to its rarity and to describe the surgical repair techniques.

Size and Location of Ruptured Intracranial Aneurysms

  • Objective : The aim of study was to review our patient population to determine whether there is a critical aneurysm size at which the incidence of rupture increases and whether there is a correlation between aneurysm size and location. Methods : We reviewed charts and radiological findings (computed tomography (CT) scans, angiograms, CT angiography, magnetic resonance angiography) for all patients operated on for intracranial aneurysms in our hospital between September 2002 and May 2004. Of the 336 aneurysms that were reviewed, measurements were obtained from angiograms for 239 ruptured aneurysms by a neuroradiologist at the time of diagnosis in our hospital. Results : There were 115 male and 221 female patients assessed in this study. The locations of aneurysms were the middle cerebral artery (MCA, 61), anterior communicating artery (ACoA, 66), posterior communicating artery (PCoA, 52), the top of the basilar artery (15), internal carotid artery (ICA) including the cavernous portion (13), anterior choroidal artery (AChA, 7), A1 segment of the anterior cerebral artery (3), A2 segment of the anterior cerebral artery (11), posterior inferior cerebellar artery (PICA, 8), superior cerebellar artery (SCA, 2), P2 segment of the posterior cerebral artery (1), and the vertebral artery (2). The mean diameter of aneurysms was $5.47{\pm}2.536\;mm$ in anterior cerebral artery (ACA), $6.84{\pm}3.941\;mm$ in ICA, $7.09{\pm}3.652\;mm$ in MCA and $6.21{\pm}3.697\;mm$ in vertebrobasilar artery. The ACA aneurysms were smaller than the MCA aneurysms. Aneurysms less than 6 mm in diameter included 37 (60.65%) in patients with aneurysms in the MCA, 43 (65.15%) in patients with aneurysms in the ACoA and 29 (55.76%) in patients with aneurysms in the PCoA. Conclusion : Ruptured aneurysms in the ACA were smaller than those in the MCA. The most prevalent aneurysm size was 3-6 mm in the MCA (55.73%), 3-6 mm in the ACoA (57.57%) and 4-6 mm in the PCoA (42.30%). The more prevalent size of the aneurysm to treat may differ in accordance with the location of the aneurysm.

  • This paper proposes the parallel operation control algorithm of a power conditioning system (PCS) for a distributed Fuel Cell power generation system. A proposed control algorithm is made good a drawback of the conventional control algorithm. The controller must also supervise the total PCS operation while communicating with the fuel cell system controller. Simulation results are presented to performance of a proposed control algorithm for the PCS.

A clinical pathogenetic study of broncholithiasis (기관지 결석증의 임상적 연구)

  • Broncholithiasis is defined as a cor9ition in which a concretion is present within a bronchus or a cavity in the lung communicating with a bronchus. The usual causes of broncholithiasis are known as tuberculosis, histoplasmosis, silicosis, aspirated calculi, and a few fungal infections. It is generally accepted that the constant motion created by respiration and beating of the heart may cause the peribronchial calcified lymph node to erode into the tracheobronchial tree and to form broncholith. After the analysis of our 6 cases of broncholithiasis which were treated surgically in the Department of Thoracic Surgery, Seoul National University Hospital from 1960 to December, 1985, we could suggest that intrinsic formation of calculi should be regarded as the pathogenesis of broncholithiasis in addition to the extrinsic formation of calculi.

  • The mathematics education community is seeking to change a teacher-centered class-room culture to a student-centered culture. However, the real transition is not easy, even for teachers who are eager and willing to teach differently. The challenge for teachers is to use the social structure of the classrooms to nurture students' development toward mathematical ways of thinking and communicating as well as their under-standing of mathematical concepts and processes. By introducing an elementary teacher's teaching practice and professional develop-ment along with her classroom episodes, this paper is to make strides toward an enriched understanding of the culture of the elementary mathematics classrooms in which students may have a lot of opportunities to develop conceptual under standing and math-ematical disposition. This paper first provides a detailed description of the classroom flow in terms of general social norms and sociomathematical norms in order to explore how the teacher and the students have established such a student-centered math-ematics microculture. This paper then analyzes the teacher's teaching approach and professional development.

Telemedicine for Real-Time Multi-Consultation

  • We introduce a new multimedia telemedicine system which is called Telemedicine for Real-time Emergency Multi-consultation(TREM), based on multiple connection between medical specialists. Due to the subdivision of medical specialties, the existing one-to-one telemedicine system needs be modified to a simultaneous multi-consulting system. To facilitate the consultation the designed system includes following modules: high-quality video, video conferenceing, bio-signal transmission, and file transmission. In order to enhance the operability of the system in different network environment, we made it possible for the user to choose appropriate data acquisition sources of multimedia data and video resolutions. We have tested this system set up in three different places: emergency room, radiologist's office, and surgeon's office. All three communicating systems were successful in making connections with the multi-consultation center to exchange data simultaneously in real-time.

  • The purpose of this research was to identify the difference between expected and actual corporate image in the market and to verify the possibility of green-oriented corporate identity (CI) visual design as a useful method for communicating with consumers. In addition, this research suggests how green-oriented CI visual design can effectively convey corporate image to consumers. The methods of research used to achieve this aim were case studies and questionnaire surveys. In regard to the results, the difference between consumers'perceived favourable corporate image and companies' expectation about corporate image was found. Moreover, the results show that green-oriented visual identity (VI) design can improve corporate image. In turn, the gap between the expected and actual perception of corporate image can be decreased through green-oriented design. Based on these results, a method of effective development for green-oriented VI design is recommended.
