Purpose: The earlobe is one of the areas which are most vulnerable to trauma. Various auricular diseases need compression treatment. We report a new compression method using magnetic disks. Methods: Seventeen patients with auricular diseases were treated from October 2002 to September 2006. The mean age was 29.1 years. The diseases details were osteochondroma in 2 patients; cauliflower's ears in 2 patients; acute otohematoma in 1 patient; and hypertrophic scars in 11 patients. The most common cause of their disease was ear piercing. The mean follow-up period was 8.9 months. All surgical procedures were performed under local anesthesia. To compress immediately, a pair of magnetic disks was applied to the anterior and posterior surface of the earlobe. Results: The results were generally good. Major complications, such as recurrence, necrosis, dehiscence, or infection, did not occur. Conclusion: A pair of magnetic disks are useful compression tool in various auricular diseases.
In industrialized countries, low back pam is very common, with up to 60 to 80% of the population being affected at some time in their lives. Herniated lumbar intervertebral disc is one of principal cause of low back pain and radiculopathy. The therapy of the disease is classified into conservative treatment or surgical intervention. The majority of these cases successfully heal with conservative managemant, even though it was claimed that the henirated disc material would not resolved spontaneously without surgical removal. It was reported that there was no significant difference in outcome and cost effectiveness between conservative and surgical therapy group. Also after conservative therapy the resorption of herniated disc with satisfactory improvement of symptom was reported. Conservative treatment includes bed rest, heat, drug therapy, therapeutic exercises, physical therapy such as traction and manipulation, and acupuncture. These managements show favorable outcome in the patients with herniated lumbar intervertebral disc. So conservative management should be applied in the therapy of herniated intervertebral disc, unless there exist aggravation during the first 3 weeks of treatment or apparent surgical care indication. And further investigation to improve effectiveness of conservative therapy should be ensued.

Nemaline myopathy is a genetically heterogeneous neuromuscular disorder and one of the most common congenital myopathies. The clinical manifestations usually vary depending on the age of onset. Neonatal nemaline myopathy has the worst prognosis, primarily due to respiratory failure. Several genes associated with nemaline myopathy have been identified, including NEB, ACTA1, TPM3, TPM2, TNNT1, CFL2, KBTBD13, KLHL40, KLHL41, LMOD3, and KBTBD13. Here, we report a neonatal Korean female patient with nemaline myopathy carrying compound heterozygous mutations in the gene KLHL40 as revealed using next generation sequencing (NGS). The patient presented with postnatal cyanosis, respiratory failure, dysphagia, and hypotonia just after birth. To identify the genetic cause underlying the neonatal myopathy, NGS-based gene panel sequencing was performed. Compound heterozygous pathogenic variants were detected in KLHL40: c.[1405G>T];[1582G>A] (p. [Gly469cys];[Glu528Lys]). NGS allows quick and accurate diagnosis at a lower cost compared to traditional serial single gene sequencing, which is greatly advantageous in genetically heterogeneous disorders such as myopathies. Rapid diagnosis will facilitate efficient and timely genetic counseling, prediction of disease prognosis, and establishment of treatments.
This study was performed to investigate transport activities and strategize improvement of 119 rescue. We reviewed emergency care records of users who were transported by 119 rescue of six agencies in Chungnam from July 13, 1998 to August 8, 1998. The results were as follows ; 1. In sex distribution of users, the male was 65.0%. And the highest age group among users was above sixties(21.6%), then thirties in second order(19.3%). Accident was 50.8% as occupied first cause of transport, and then acute disease 22.8%. The highest requester for 119 rescue call was patient's families(47.1%) and average number of 119 rescue users per day was 20.9. 2. The nonurgent state of users was 58.9%. The frequency of users was 26.0 persons at sunday in most frequently, weekend and holiday was more common than ordinary day, and most frequent weather state was cloudy(23.8 persons). 3. Total running distance of 119 ambulance was 7.0km in average. Call time by users was 20-24 hours most commonly(21.9%). In then running time by each transport stage, 8 minutes were taken from 119 call receipt to scene arrival, 13 minutes from scene arrival to hospital. The kinds of pre-hospital care by 119 rescuer was vital sign check(81.2%), wound dressing or fracture fixation(41.2%), airway maintenance and O2 supply(30.4%).
두통은 전체 인구중 상당수가 일생에 한번 이상 겪게되는 질환으로, 계속하여 논의되고 개정되어 현재까지 국제적인 분류법이 마련되어 왔다. 원발두통은, 다른 원인질환에 의하지 않은 통증을 의미하며, 다음과 같이 분류할 수 있다: 1) 편두통 2) 긴장형두통 3) 군발두통과 기타 삼차자율신경두통 4) 기타 원발두통. 한편 턱관절장애에 기인한 두통 및 약물 과용 두통은 두통의 원인이 기질적인 경우로 이차성 두통에 분류된다. 본 종설에서는 국제두통질환분류 제3판의 베타판(ICHD-3 beta)에 근거한 두통의 진단에 대하여 고찰해보고자 한다.
Background and Objectives: Deep neck infections, which affect soft tissues and fascial compartments of the head and neck and their contents, have decreased after the develop ment of chemotherapeutic agents and antibiotics. However they may still result in significant morbidity and mortality despite the use of chemotherapeutic agents and antibiotics. Materials and Methods : A retrospective study was performed on 66 deep neck infections in patients admitted for diagnosis and treatment at Asan medical center from June 1994 to December 2000. Results : Age of the patients varied from 1 to 86-year-old and sex ratio of male to female was 1.2:1. Most frequently involved site was submandibular space (21.2%). Most common cause of infection was dental disease (28.8%). The isolated pathogenic organisms were Streptococcus species in 19 cases, Staphylococcus species in 7 cases, Klebsiella in 5 cases, mixed infection of Staphylococcus and Klebsiella in 3 cases and a case of Corynebacterium. 51 cases were treated surgically, 15 cases were medically. Mean duration of admission was 9.6 days in cases of single space infection, 17.5 days in multiple spaces, 8.1 days when the infection resulted in cellulitis, 13.4 days in abscess, 7.9 days when the infection treated medically and 13.4 days when treated surgically. Conclusion Early diagnosis and treatment is important to manage deep neck infection and the duration of admission was increased when the infection involved multiple spaces.
Premature contraction arrhythmia is the most common disease among arrhythmia and it may cause serious situations such as ventricular fibrillation and ventricular tachycardia. Most of arrhythmia clasification methods have been developed with the primary objective of the high detection performance without taking into account the computational complexity. Also, personalized difference of ECG signal exist, performance degradation occurs because of carrying out diagnosis by general classification rule. Therefore it is necessary to design efficient method that classifies arrhythmia by analyzing the persons's physical condition and decreases computational cost by accurately detecting minimal feature point based on only QRS features. We propose method for personalized specific classification of premature contraction arrhythmia based on QRS features in smart healthcare environments. For this purpose, we detected R wave through the preprocessing method and SOM and selected abnormal signal sets.. Also, we developed algorithm to classify premature contraction arrhythmia using QRS pattern, RR interval, threshold for amplitude of R wave. The performance of R wave detection, Premature ventricular contraction classification is evaluated by using of MIT-BIH arrhythmia database that included over 30 PVC(Premature Ventricular Contraction) and PAC(Premature Atrial Contraction). The achieved scores indicate the average of 98.24% in R wave detection and the rate of 97.31% in Premature ventricular contraction classification.
Cytomegalovirus (CMV) is currently the most common agent of congenital infection and the leading infectious cause of brain damage and hearing loss in children. Symptomatic congenital CMV infections usually result from maternal primary infection during early pregnancy. One half of symptomatic infants have cytomegalic inclusion disease (CID), which is characterized by involvement of multiple organs, in particular, the reticuloendothelial and central nervous system (CNS). Moreover, such involvement may or may not include ocular and auditory damage. Approximately 90% of infants with congenital infection are asymptomatic at birth. Preterm infants with perinatal CMV infection can have symptomatic diseases such as pneumonia, hepatitis, and thrombocytopenia. Microcephaly and abnormal neuroradiologic imaging are associated with a poor prognosis. Hearing loss may occur in both symptomatic and asymptomatic infants with congenital infection and may progress through childhood. Congenital infection is defined by the isolation of CMV from infants within the first 3 weeks of life. Ganciclovir therapy can be considered for infants with symptomatic congenital CMV infection involving the CNS. Pregnant women of seronegative state should be counseled on the importance of good hand washing and other control measures to prevent CMV infection. Heat treatment of infected breast milk at $72{^{\circ}C}$ for 5 seconds can eliminate CMV completely.
Respiratory diseases are common in swine industry and have great economic importance. Respiratory disorders cause substantial losses to the swine industry. Losses associated with respiratory disease vary considerably between herds and seasons. In a survey was carry out to investigate the lesion of red internal organs in slaughtered pigs and provided assistant data which are useful for each farm. From november 2012 to december 2013, 1,680 pigs out of 84 farms were sampled in Jeonbuk province. Gross lesions such as swine enzootic pneumonia (SEP), pleuritis, pleuropneumonia, pericarditis, liver milk spot were examined for the pigs. Overall prevalence of SEP was 55.7%. According to season, the incidence occurred higher in fall than winter, spring, and summer. The mean SEP score was 0.91, the highest incidence occurred in fall. The prevalence of pleuropneumonia, pleuritis, pericarditis, and milk spot was 36.4%, 49.7%, 2.3%, and 8.8%, respectively. The positive rate of PRRS and PCV2 was 2.9% and 70.0% by PCR analysis.
Obstructive sleep apnea (OSA), most common respiratory disorder of sleep, is characterized by intermittent partial or complete occlusions of the upper airway due to loss of upper airway dilating muscle activity during sleep superimposed on a narrow upper airway. Termination of these events usually requires arousal from sleep and results in sleep fragmentation and hypoxemia, which leads to poor quality of sleep, excessive daytime sleepiness, reduced quality of life and numerous other serious health consequences. Untreated OSA may cause, or be associated with, several adverse outcomes, including daytime sleepiness, increased risk for motor vehicle accidents, cardiovascular disease, and depression. Various treatments are available, including non-surgical treatment such as medication or modification of life style, continuous positive airway pressure (CPAP) and oral appliance (OA). Skeletal surgery for obstructive sleep apnea (OSA) aims to provide more space for the soft tissue in the oropharynx to prevent airway collapse during sleep. Conventional surgical techniques include uvopalatopharyngoplasty(UPPP), genioglossus advancement (GA), and maxillomandibular advancement (MMA). Surgical techniques, efficacy and complications of skeletal surgery are introduced in this review.
