A Case of Herpetic Whitlow of Toe Misdiagnosed as Bacterial Infection: A Case Report (세균성 감염으로 오인된 발가락의 헤르페스 손끝염: 증례 보고)

  • Finger infections are a common problem often caused by viruses, bacteria, or fungi. Similarly, toe infections can present with similar clinical symptoms. Prompt identification of the cause of an infection is crucial for preventing disease progression to a state necessitating immediate and appropriate medical or surgical intervention. Herpetic whitlow is characterized by erythema and painful, non-purulent vesicles and typically results from a herpes simplex virus type 1 or 2 finger infections. However, while herpes whitlow of a finger is common, cases involving a toe are rare. Consequently, a lack of experience of herpetic whitlow of the toe could lead to a misdiagnosis as a bacterial infection and potentially result in unnecessary surgical treatment. Herein, we present a case of herpetic whitlow affecting a great toe that was initially misdiagnosed as a bacterial infection and subsequently treated surgically.

  • Hepatocellular carcinoma(HCC) is the fifth most common cancer worldwide (fifth in male, seventh in female) and the third most common cause of cancer mortality. Since 2001, the various research group in the United States, Europe and Asia have published clinical practice guidelines for HCC. In Korea, a clinical practice guideline for HCC have been published by The Korean Liver Cancer Study Group in 2003, revised in 2009 and 2014. In China, oriental medicine clinical practice guideline have been published for the first time in 2014, and in the oriental medical profession of Korea, there is growing need for the guideline. This study will introduce the methods of diagnosis and the medical therapeutics which is commonly utilized for HCC in Korea, and existing korean medicine clinical practice guideline for Disease Analysis and Treatment(辨證論治) and herbal therapy of HCC. Further clinical research about various herbal medicines are needed to develop more advanced guideline of HCC.

  • Introduction: Vertigo is one of the common symptoms that we can see often clinically. It is hallucination to motion of oneself or surroundings. Vertigo include not only simple whirling sensation but also leaning or falling down sensation. Particularly in vestibular neuritis, the principal symptoms is dizziness and accompanied by nystagmus, gait imbalance, nausea, vomiting. Cause of vestibular neuritis is inflammatory disease such as common cold. Objective: The aim of this study was to estimate the efficacy of oriental-treatment on vestibular neuritis patient Subjects: We diagnosed one patient who had severe vertigo and gait imbalance as "dizziness retention of phlegm"(담훈) and treat orientally. Conclusion: After oriental-treatment for 15 days, walk balance was improved and no more vertigo was appeared. We could assume that in the vertigo and gait inbalance due to vestibular neuritis, the acupuncture and herbal medicine can be used.

Prevalence of Corneal Diseases of Dogs in Korea (국내 개에서 각막질환의 발생양상)

  • This study was performed to investigate the signalment, cause and clinical feature of corneal diseases of dogs in Korea. One hundred eighty-four dogs(273 eyes) with corneal diseases were examined and classified into congenital, inflammatory and metabolic diseases. Inflammatory diseases were subclassified into nonulcerative keratitis and ulcerative keratitis. The most prevalent corneal disease type was inflammatory disease(93.7%) and nonulcerative keratitis(58.6%) is more prevalent than ulcerative keratitis(35.1%) in the inflammatory diseases. The main cause of nonulcerative keratitis was keratoconjunctivitis sicca(19.0%) whereas that of ulcerative keratitis was trauma(9.9%). The most common breed with corneal disease was Shih Tzu(49.5%). The affected mean age was $6.4{\pm}4.1$ years old. The prevalence rate of corneal diseases were slightly higher in female(56.8%) than in male(43.2%). The data of corneal diseases in dogs obtained from this study will be utilized in prevention, diagnosis and treatment of corneal diseases in dogs and client education.

BAG3 mutation in a patient with atypical phenotypes of myofibrillar myopathy and Charcot-Marie-Tooth disease

  • Bcl2-associated athanogene 3 (BAG3) mutations have been reported to cause the myofibrillar myopathy (MFM) which shows progressive limb muscle weakness, respiratory failure, and cardiomyopathy. Myopathy patients with BAG3 mutation are very rare. We described a patient showing atypical phenotypes. We aimed to find the genetic cause of Korean patients with sensory motor polyneuropathy, myopathy and rigid spine. We performed whole exome sequencing (WES) with 423 patients with sensory motor polyneuropathy. We found BAG3 mutation in one patient with neuropathy, myopathy and rigid spine syndrome, and performed electrophysiological study, whole body MRI and muscle biopsy on the patient. A de novo heterozygous p.Pro209Leu (c.626C>T) mutation in BAG3 was identified in a female myopathy. She first noticed a gait disturbance and spinal rigidity at the age of 11, and serum creatine kinase levels were elevated ninefolds than normal. She showed an axonal sensory-motor polyneuropathy like Charcot-Marie-Tooth disease (CMT), myopathy, rigid spine and respiratory dysfunction; however, she did not show any cardiomyopathy, which is a common symptom in BAG3 mutation. Lower limb MRI and whole spine MRI showed bilateral symmetric fatty atrophy of muscles at the lower limb and paraspinal muscles. When we track traceable MRI 1 year later, the muscle damage progressed slowly. As far as our knowledge, this is the first Korean patient with BAG3 mutation. We described a BAG3 mutation patient with atypical phenotype of CMT and myopathy, and those are expected to broaden the clinical spectrum of the disease and help to diagnose it.

  • Lateral canal benign paroxysmal vertigo (BPPV) causing dizziness is a common cause is not found while continuing to appeal for vertigo is a typical disease. It is characterized by acute stand up, brief and rotatory vertigo attacks provoked by change in head position. Treatment requires only one treatment visit in most patients. However, there are significant numbers of patients who require multiple treatment visits for relief. The purpose of this study benign paroxysmal positional vertigo treatment of type affect is to analyze the cause. Dizziness and vertigo patient's in patients admitted to the dizziness center of lateral canal benign paroxysmal positional vertigo were classified. In patients with lateral canal benign paroxysmal positional vertigo and accompanying lateral 15 treatment affects disease were investigated. March 2008 to November 2010 lateral canal benign paroxysmal positional vertigo 166 people cure rate of patients was investigated. First time the success rate of 74.1%, twice times the success rate of 12.0%, three times the success rate of 9.6%, more than three times the success rate was 4.2%. Affecting factor treatment of benign paroxysmal positional vertigo in post-traumatic, medicine disease, headache, cerebral infarction, small vessel disease, vestibulopathy, (p<0.05). Statistical analysis using SPSS (version 12K) in coefficient measure through descriptive statistical of cross table.

  • Despite its declining incidence, gastric cancer is globally, still, the third most common cause of cancer-related mortality. Gastric cancer is a heterogeneous disease with diverse pathogenesis and molecular backgrounds. Therefore several systems have been proposed to aid in the classification of gastric adenocarcinoma based on the macroscopic, microscopic and anatomical features of the tumor. However, these classifications did not reflect the pathogenesis of the disease. Recently, genomic analysis has identified several subtypes of gastric adenocarcinoma and a detailed understanding of the molecular biology behind the neoplastic phenotype is possible to develop of more effective therapies. We will describe the existing various classification of gastric cancer and the recently introduced molecular biology and immunological classification.

Diet components can suppress inflammation and reduce cancer risk

  • Epidemiology studies indicate that diet or specific dietary components can reduce the risk for cancer, cardiovascular disease and diabetes. An underlying cause of these diseases is chronic inflammation. Dietary components that are beneficial against disease seem to have multiple mechanisms of action and many also have a common mechanism of reducing inflammation, often via the $NF{\kappa}B$ pathway. Thus, a plant based diet can contain many components that reduce inflammation and can reduce the risk for developing all three of these chronic diseases. We summarize dietary components that have been shown to reduce cancer risk and two studies that show that dietary walnut can reduce cancer growth and development. Part of the mechanism for the anticancer benefit of walnut was by suppressing the activation of $NF{\kappa}B$. In this brief review, we focus on reduction of cancer risk by dietary components and the relationship to suppression of inflammation. However, it should be remembered that most dietary components have multiple beneficial mechanisms of action that can be additive and that suppression of chronic inflammation should reduce the risk for all three chronic diseases.

  • Currently, diabetes is the most common chronic disease in the world, affecting 23.7% of the population in the Kingdom of Saudi Arabia. Diabetes may be the cause of lower-limb amputations, kidney failure and blindness among adults. Therefore, diagnosing the disease in its early stages is essential in order to save human lives. With the revolution in technology, Artificial Intelligence (AI) could play a central role in the early prediction of diabetes by employing Machine Learning (ML) technology. In this paper, we developed a diagnosis system using machine learning models for the detection of type 2 diabetes among adults, through the adoption of two different diabetes datasets: one for training and the other for the testing, to analyze and enhance the prediction accuracy. This work offers an enhanced classification accuracy as a result of employing several pre-processing methods before applying the ML models. According to the obtained results, the implemented Random Forest (RF) classifier offers the best classification accuracy with a classification score of 98.95%.

  • Idiopathic peripartum cardiomyopathy is an uncommon malady disease. Making the diagnosis is often difficult and it is always necessary to exclude other prior heart disease and other causes of left ventricular dysfunction in pregnant women. Heart failure in these women ensues when the cardiovascular demands of normal pregnancy are further amplified when the common complications of pregnancy complications superimposed upon these underlying conditions that cause compensated ventricular hypertrophy. This may be aggravated by making a late diagnosis and providing inappropriate treatment. We experienced a 38-year-primigravida who has diagnosed with idiopathic peripartum cardiomyopathy and underwent elective cesarean section with general anesthesia.
