When participating in problem-based learning (PBL), it is important for medical students to generate claims and provide justifications for their claims in small group discussions. This study aimed to investigate the effect of argumentation instruction on medical student learning experiences with PBL. A total of one hundred first-year preclinical students from Inje University College of Medicine, who had attended argumentation instruction, participated in this study. All of the participants completed a 5-point Likert scale questionnaire regarding their learning experiences with PBL, before and after the argumentation instruction. The questionnaire comprised 22 items with eight subcategories: argumentation activity, reflection, integration of basic and clinical science, identification of lack of knowledge, logical thinking, self-directed study, communication, and attitude toward discussion. The collected data were analyzed through a paired-sample t-test. The results of this study found that the argumentation instruction promoted the preclinical students' experiences with argumentation activities, reflection, an integration of basic and clinical science, the identification of their lack of knowledge, logical thinking, and self-directed study, and it increased positive attitudes toward group discussion. The findings suggest argumentation instruction can enhance medical student group discussions and help students achieve the objectives of PBL, including acquisition of basic and clinical science knowledge and development of clinical reasoning and self-directed learning abilities, which can highlight the meaningful learning experiences students have with PBL.

Objectives : The aim of this study is to summarize and compare acupuncture points used for dental disorder in the classics with those used in recent clinical trials. Methods : We searched the data for acupuncture points used and rationale of acupuncture in dentistry. Following two sources were searched: 1) seven Classics of traditional medicine and 2) clinical trials through Pubmed from January 2000 to March 2013 with no language restriction. Results : Dental pain was the most common disorder in the dentistry section of the Classics of traditional medicine. We found many similarities of acupuncture points used between literatures and clinical trials. From the meridian perspective, large intestine meridian(LI), stomach meridian(ST), triple energizer meridian(TE), and gallbladder meridian(GB) were frequently used in the Classics of traditional medicine and the clinical trials. From the acupuncture point perspective, acupuncture points were selected according to syndrome. The specific points such as five transport points, source points, and connecting points were also used. In the clinical trials, combination of acupuncture points usually consisted adjacent points and distant points. Among them, LI4 was used in all of the studies included in this review. Conclusions : For well-designed clinical trial, appropriate intervention is essential. To establish appropriate acupuncture treatment, we have to suggest reasoning for treatment based on literature and/or expert consensus. Our review only focused on pain relief in dentistry and had many limitations. Further studies based on the literatures such as the Classics of traditional medicine are required to ensure the rationale of acupuncture treatments in various dental disorders.

Neural networks have recently attracted considerable attention in the field of classification and other areas. The purpose of this study was to demonstrate an experiment using back-propagation neural network model applied to nursing diagnosis. The network's structure has three layers ; one input layer for representing signs and symptoms and one output layer for nursing diagnosis as well as one hidden layer. The first prototype of a nursing diagnosis system for patients with stomach cancer was developed with 254 nodes for the input layer and 20 nodes for the output layer of 20 nursing diagnoses, by utilizing learning data set collected from 118 patients with stomach cancer. It showed a hitting ratio of .93 when the model was developed with 20,000 times of learning, 6 nodes of hidden layer, 0.5 of momentum and 0.5 of learning coefficient. The system was primarily designed to be an aid in the clinical reasoning process. It was intended to simplify the use of nursing diagnoses for clinical practitioners. In order to validate the developed model, a set of test data from 20 patients with stomach cancer was applied to the diagnosis system. The data for 17 patients were concurrent with the result produced from the nursing diagnosis system which shows the hitting ratio of 85%. Future research is needed to develop a system with more nursing diagnoses and an evaluation process, and to expand the system to be applicable to other groups of patients.
