Preliminary Study to Develop the Instrument on Pattern Identifications for Depression (우울증 변증도구 개발을 위한 기초연구)

  • Objectives: The purpose of this study is to develop a standard tool for pattern identifications in Korean Medicine for depression. Methods: The advisor committee for this study was organized by 15 Korean Medicine neuropsychiatry professors from 11 Colleges of Korean Medicine. The items and structure of the tools were based on reviews of published literature. In order to develop the tool, we took the consultation from discussions with the advisor committee twice and also incorporated additional advices from e-mail correspondences. Results: 1) We divided the symptoms and signs of depression into 11 pattern identifications. 2) We obtained the mean weights which reflected the standard deviations from each symptoms of the 11 pattern identifications which were scored on a 100-point scale by 15 experts. 3) We designed the Korean medicine pattern identification tool for depression. It was composed of 66 questions in the question- and-answer format. Conclusions: There are some points which should be considered in this study. First, we couldn't reach a complete agreement on the concept of 11 patterns. Second, each pattern identification has a different number of symptoms and signs. In addition, the items of symptoms and signs of each pattern identification are unequal. Third, as we did not set any clinical trials when using this tool, it was not possible to test its validity and reliability. Although there are some limits in this study, the development of pattern identification tools for depression through discussions with the advisor committee is meaningful. If the validity and reliability of the Korean Medicine pattern identification tools for depression are confirmed through clinical trial,s this tool is expected to be applied to the subsequent researches in the future.

Development of Instrument of Pattern Identification for Functional Dyspepsia (기능성 소화불량증 변증도구 개발 연구)

  • With the high prevalence of functional dyspepsia in the world, it was difficult to get objective diagnosis, treatment and assessment for the reason that there were many different symptoms and signs. The purpose of this study is to develop a standard instrument of pattern identification for functional dyspepsia which will be applied to clinical research. The items and structure of the instrument were based on review of published literature. The advisor committee on this study was organized by 11 oriental division of gastroenterology professors of oriental medical colleges nationwide. The experts discussed developing the instrument, and we also took professional advices by e-mail. We divided the symptoms and signs of functional dyspepsia into 6 pattern identification, such as disharmony of liver and stomach, retention of undigested food, damp-heat in the spleen and stomach, simultaneous occurrence of cold and heat syndromes, deficiency and cold of the spleen and the stomach, and insufficiency of stomach eum. We got the mean weights to each symptom of six pattern identification which had been scored on a 5-point scale ranging from 1 to 5 by the 11 experts. We made out the Korean instrument of the pattern identification composed of 45 questions for functional dyspepsia. Although there are some limitations in our study, the instrument is meaningful and certain worth of its own. We hope to improve the instrument through the further clinical studies and discussions.

The Development of Multidisciplinary Cancer Patient Education·Counseling Questionnaire and Satisfaction Survey (다직종 암환자 교육·상담에 대한 설문 개발과 환자 만족도 조사)

  • Background: Presently, a multidisciplinary team of doctors, pharmacists, nurses, and dietitians provides patient education to impart information on chemotherapy. However, studies on multidisciplinary education satisfaction are inadequate. In this study, we aimed to contribute to the improvement of quality of multidisciplinary education counseling for patients with cancer by developing a satisfaction questionnaire and analyzing the satisfaction survey. Methods: A questionnaire was developed by an expert group, and the responses were recorded using the 5-point Likert scale. After conducting a pre-test, factor analysis was performed to evaluate validity. The reliability of the questionnaire was measured by Cronbach's alpha coefficient. A satisfaction survey was conducted by self-administration method. Results: Based on the results of factor analysis, factors can be divided into two parts: "overall education" and "each team member's education" (total 14 questions). The construct validity and reliability of the questionnaire are sufficiently high. Fifty-one patients took the survey between January 2, 2018 and January 20, 2018. Twenty-six (51%) patients responded that they were "very satisfied" and 22 (43.1%) patients responded that they were "satisfied". Conclusion: By developing a questionnaire on multidisciplinary education counseling for patients with cancer, it is possible to perform evaluation and research of cancer patient education. This study will contribute to the management and improvement of quality of multidisciplinary education.

Attitude toward Death in Nursing Students (간호학생의 죽음에 대한 태도)

  • Purpose: The purpose of this study was to investigate the attitude toward death in Korean nursing students. Method: The sample consisted of 365 baccalaureate nursing students. The questionnaires included questions on sociodemographics and death-related characteristics of the participants, and the Fear of Death and Dying Scale (FODS) to measure the attitude toward death. Result: The mean of the FODS score was 2.63 out of 4, so the participants had a slightly negative attitude toward death. There were statistically significant differences between gender, religion, religion activity, perceived health status, experience of parents' death, experience of friend's death, and overall FODS score. Among the four subscales of overall FODS, the score of the fear of death of self was significantly higher in the participants who experienced clinical practice and who experienced patient's death in the intensive care unit compared to the emergency room. Conclusion: Based on the study results, educational programs to change the attitude toward death are required before clinical practice. Programs need to consider nursing students' gender and religion, and give opportunity to share experiences and feelings about death of family or friend. In addition, using standardized patients and simulators is advised in the need for simulation training.

A report on the clinical efficacy of Daejo-hwan(DJH) about the dryness of vagina and sexual satisfaction in climacteric women (갱년기 여성의 질 건조증과 성만족도에 미치는 대조환(大造丸)의 임상효과 보고)

  • Purpose : To assess the clinical efficacy of DJH on the dryness of vagina and difficulty in sexual intercourse in climacteric women. Methods : 7 climacteric women who are suffering from the dryness of vagina and sexual problems received DJH: 2 pills(4g) a time, three times a day for a total 8 weeks as maintenance therapy. We evaluated ultrasonography, BMD, blood test, urine analysis, female hormone test and questionnaire, Kupperman's index plus two questions about urogenital and sexual problems in before as well as after 8 weeks of treatment. Results : The results are as follows; 1. DJH reduced climacteric symptoms.(Kupperman's index from 61.4 to 30.2 points) 2. DJH was very effective to dryness of vagina and difficulty in sexual life caused by deficiency of estrogen. 3. The affirmative efficacy of DJH improved sexual satisfaction in consequence. Conclusion : The DJH showed positive effects on improving, not only the climacteric symptoms, but also the sexual satisfaction. It shows that DJH may represent a satisfactory therapeutic choice for those climacteric women with the urogenital tract disturbances and sexual problems.

Survey of Life and Dietary Styles on Patients with Gastroesophageal Reflux Disease (역류성 식도질환 환자의 생활습관 및 식습관 조사연구)

  • Objective: It was surveyed how to help patients get better treatment for their disease by making complements based on the survey outcomes when the pharmacists guide how to administer medicine. Methods: A total of 142 gastroesophageal reflux disease (GERD) patients answered the self-answering questions consist of 15 items about their general characteristic, lifestyle and dietary style. The frequency of survey materials was analyzed to find out specific figures of surveyed patient's general characteristic, lifestyle and eating habits. Results: Based on the outcomes on analysis there was no difference between female and male patients. The characteristic based on the age group, as ones grow older, the number of GERD patients also increased. In case on the characteristic in lifestyle, many patients had improper habit to treat GERD. The items were smoking habits, stress control habit, sleeping postures and the way in wearing outfits. In case of postures after having a meal, they had right lifestyle. Patients had improper habit in eating food. They enjoyed pungent food and drinks which can hinder the treatment. In case of the time of having meal, eating habit of meat and vegetables and eating food less than an hour before go to bed, this research showed that they have the right lifestyle. Conclusion: This research indicate that pharmacists give guidance GERD patients to stop smoking that is causative of GERD and guide patients have mental stability, and patients avoid pungent food and drinks such as coffee, soda, chocolates and mint candies.

A study about the medical communication proficiency of Korean traditional medical students using standardized patients of Infertility (표준화 난임환자를 활용한 한의대생의 진료 및 의사소통 수준연구)

  • Objective : The purpose of this study is to assess the communication and medical interview level of the students in the college of korean traditional medicine using Standardized Patients(SP). Method : 35 students of 5th grade in the college who are doing their clinical practice class participated and conducted medical interview on standardized infertile female patient. SP and professor who observed the interview evaluated the students' communication skill and medical interview level using the assessment sheet composed of 23 questions. Results : 1. Satisfaction of SP was 1.66 of 3. In the communication section, students conducted well in appropriate voice and no inappropriate interruption but they didn't show good practice in considerate physical examination, understandable explanation relatively. 2. The scores that were acquired by students in informing the time required, obtaining agreement at initiating and closing, family history taking, checking mental status and physical examination 2 were low in the medical interview section. 3. There were some differences in the communication and medical interview scores by the students' sex (p<.01), age(p<.05) and medical camp experience(p<.05). But we couldn't find any evidence about the relation between character type and the communication and medical interview scores(p>.62). Conclusion : On the whole, students who participated this study had difficulties in building relationship with patient and conducting medical interview. They need to improve their ability in those fields by appropriate education before their graduation.

A Survey about the recognition regarding the Korean acupuncture method and research direction (한국 침법에 대한 인식도 및 연구방향에 관한 설문 조사)

  • Obejectives : This survey was accomplished to find out how Korean medical doctors think that the recognition regarding the Korean acupuncture method and research direction Methods : The survey questions were developed by the consensus from the professors who major in acupuncture and moxibustion. The questionnaire was given the 2731 Korean medical doctors at the mending education site; In addition, it was given to 793 doctors working at the 105 Korean medical hospitals listed on the National Korean Medical Hospital Associations Address book by postal mail; and also it was given to the 142 public health care Korean medical doctors who attended the conference for the municipal and provincial representatives of the public health service Korean medical doctors. Total of 1277 questionnaires were retrieved out of the 3666 subjects. Results : 1. Eleven hundred eighth seven out of 1277 Korean medical doctors think motion and bone and joint disease which has an effect in acupuncture treatment(92.9%) 2. Nine hundred fourteen out of 1277 Korean medical doctors used "Body acupuncture method"(71.5%) and 902 out of 1277 Korean medical doctors think of the korean acupuncture method which is "Sa Am acupuncture method"(70.6%) 3. Eight hundred forty out of 1277 Korean medical doctors want verification which leads a clinical research to know that acupuncturist have acupuncture and moxibustion treatment(65.7%). Conclusion : Korean medical doctors think motion and bone and joint disease which has an effect in acupuncture treatment Korean medical doctors most used "Body acupuncture method" but think of the korean acupuncture method which is "Sa Am acupuncture method". Korean medical doctors want verification which leads a clinical research to know that acupuncturist have acupuncture and moxibustion treatment.

A Case Study the Relationship Between Five Woon Constitutions And Five Zhang Symptoms Based on 286 Clinical Data of Asian Women near L.A (운기체질과 오장병증의 상관관계 - 캘리포니아에 거주하는 아시아 여성 286명의 임상연구 -)

  • Objective: To relate with significant relationship between symptoms and Five Zhang(五臟)symptoms in Five Woon Constitutions and to characterize Five Woons of Asian women who live at Los Angeles area in U.S.A. Methods: 19 diagnostic questions were given to 286 Asian females subjects who were treated for their symptoms at an Asian medical clinic near Los Angeles in USA. The participants were given their constitutions, one of the Five Woons based on their birthdays. The Cross tabulation analysis was also applied to look for the effectiveness of Five Woons on the symptoms. Results: 1. The cross tabulation analysis showed the significant effectiveness of the Five Woons on the five symptoms including dizziness, hot and cold, lower body edema and face fever(p<0.1). 2. The dizziness and the hot and cold were related with Liver diseases, the lower body edema was related with Kidney diseases and the face fever was related to Heart diseases. Conclusions: This study can explain the relationship between the significant symptoms of Five Woons and Five Zhang(五臟)symptoms with reference which is the book by Yoon, ChoChang who lived 1700 years. Further clinical research is necessary to develop more delicate questionnaires and look for ways of characterizing the Five Woons. The diagnosis and corresponding treatment based on the Five Woons should be further studied in view of the Asian medicine.

Report on the Survey for the Necessity of Korean Standard Differentiation of the Symptoms and signs for the Stroke (한국형 중풍변증진단 표준안의 필요성에 대한 설문조사 보고)

  • This survey was accomplished in order to find out the necessity of creation of Korean standard differentiation of the symptoms and signs for the stroke(KSDS) in clinical practice. The survey questions were developed by the consensus from the professors who major in internal traditional Korean medicine(TKM). May 11th to July 31th 2005, the questionnaire was given to 800 TKM doctors in the whole country of Korea listed on the National Korean Medical Hospital Associations address book dy postal mail. Total of 358(44.7%) questionnaires were retrieved out of the 800 subjects. The 311 (86.9%) of 358 TKM doctors given an answer to the survey responded to need development of KSDS, and 176(56.6%) of 311 TKM doctors who answered to need development of KSDS responded to have to consider consensus of clinical experts on the stroke for creation of KSDS. These results support a role for new KSDS might be usefully applied in diagnosis and treatment of the stroke.
