A Study on Physician Performance Measures for Financial Compensation in Academic Medical Centers (대학병원 의사들의 보상결정 기준으로서의 성과 측정지표에 대한 연구)

  • An increasing number of hospitals are seeking for new or mixed compensation strategies to improve the productivity of their medical staff in the struggle to provide high quality medical services at low costs amid the economic hardship. To motivate physicians toward the right direction, it is necessary to effectively evaluate their performance that provides a basis for compensation. However, productivity has been historically difficult to measure, particularly for physicians in academic medical centers who are expected to engage in research, education, and patient care simultaneously. The objectives of this study were to define performance measures of physicians and clinical departments in academic medical centers. to examine correlations between the measures. and to investigate factors affecting the measures. The performance data of 212 faculty physicians in 17 clinical departments in two university teaching hospitals affiliated to one medical school during the fiscal year 1994 was used for analyses. Patient care revenue, net profit. and adjusted number of patients were defined to measure the performance in patient care. and number of articles published in academic journals and research grant were defined for research activities. Both individual physicians' performance measures and per physician measures of clinical departments were analyzed. All measures defined to evaluate individual physicians' performance were positively related to each other. Clinical department and rank of faculty position were statistically significant predictors of revenue. and hospital. clinical department. and rank were significant predictors of net profit. journal publication. and research grant. Patient care measures defined to evaluate clinical departments were related to each other. so were research measures. and no significant correlations were found between patient care measures and research measures. Also found were large differences in department. ranks when clinical departments were evaluated by absolute per physician performance measures and evaluated by annual rate of changes in performance measures. These findings suggest that departmental performance measures opposed to individual performance measures are relatively free from problems of factors affecting the performance measures that are not in control of clinical departments or individual physicians. Results from the correlation analysis of departmental performance measures indicates that measures of research performance should be included in the evaluation to promote research activities in academic medical centers.

The Effects of Preceptors' Transformational Leadership on Job Stress and Clinical Performance among New Graduate Nurses (프리셉터의 변혁적 리더십이 신규간호사의 업무스트레스와 업무성과에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of preceptors' transformational leadership on job stress and clinical performance among new graduate nurses. Methods : The study subjects were 180 new nurses in three University Hospitals. General characteristics, leadership, job stress and clinical performance were collected using self-reported questionnaires. Transformational leadership consisted of charisma, intellectual stimulation and individual consideration. T-test, ANOVA, pearson correlation and multiple regression analysis were performed to access the effect of leadership on job stress and clinical performance. Results : Of 180 subjects, 94.4% were female. The mean scores of transformational leadership, charisma, intellectual stimulation and individual consideration were $3.9{\pm}.46$, $3.9{\pm}.46$, $3.9{\pm}.57$ and $4.0{\pm}.58$, respectively. In multiple regression analysis, overall transformational leadership(${\beta}$=.154, p=.006) and charisma(${\beta}$=.388, p=.008) significantly increased the job stress. On the other hand, individual consideration significantly decreased the job stress (${\beta}$=-.671, p=.048) and increased the clinical performance(${\beta}$=2.472, p=.024). Conclusions : Charisma of preceptors was associated with the increase of job stress, and individual consideration was associated with the decrease of job stress and improvement of clinical performance. Therefore, the preceptors' leadership focusing on individual consideration rather than charisma may reduce job stress and improve clinical performance in the University hospital organization.

  • Objectives : Clinical data in traditional medicine, such as Korean medicine, traditional Chinese medicine have a long history of accumulating evidence and these rich data are recorded in classic literature. We have conducted a study of developing an algorithm that support clinical diagnosis with composing both users knowledge and data obtained from literature. In order to define necessary information and required steps in diagnosis procedure, we have established a clinical diagnostic procedure including a step of collecting patients symptoms, a step of determining candidates, a step of diagnostic decisions, a step of deciding of treatment and a step of adjusting medicinal treatment. Methods : Our study have been based on the following premises. 1. Using data obtained from literature contributes to accurate diagnosis 2. Displaying the data before users request contributes to accurate conclusion. Displaying before users request enable users to recognize their overlooking a fact on purpose or not. 3. Checking symptoms that are commonly accompanied with a group of diseases that accompany symptoms appealed by a patient contributes to accurate conclusion. These symptoms are worthy of checking. 4. Comparing more than two candidates contributes to accurate conclusion. Users can compare their accompanied symptoms with patients symptoms and this helps users to make a decision. Results : Based on the above premises, we have developed an literature based algorithm to provide various functions, such as recommending symptoms to check, comparing groups of symptoms, differential diagnosis, recommending medicinal materials to prescribe, and more. Conclusions : By the results of simulation with virtual diagnostic scenario, we concluded this algorithm is useful helping clinician in diagnosis procedure.

The Effects of Clinical Education Program for Nurses in Regional Public Hospital (공공보건의료인력 임상교육효과 평가: 지역거점공공병원 간호사 대상)

  • Background: The purpose of this study is to evaluate the outcomes of clinical education program for nurses in regional public hospital, utilizing the Kirkpatrick's model. Methods: Kirkpatrik's 4-level model was applied to this study. Trainees were asked to fill out questionnaires in the middle and at the end of the program. Also administrators of excellent trainees were asked to fill out the questionnaires regarding nursing management performance after 1-2 months from the end of the training course. Results: All trainees had positive reactions to the clinical education program. Not only the results of individual level (satisfaction and achievement scores, academic achievement scores, practical application rate, and educational transition factors) but also the scores of organization level (nursing management performance scores) are improved. Conclusion: By showing a correlation between the effectiveness factors we need to verify the relationship between these factors in a future study. In addition, development of quantitative and qualitative performance indicators are needed. To establish a long-term education system, it is required to applying the excellent trainee's successful experiences.

Safety Evaluation of Sankhaholi (Evolvulus alsinoides Linn.) in the Management of Essential Hypertension: A Randomized Standard Control Trial

  • Background: Hypertension is one of the major risk factors for stroke, heart attack, heart failure and kidney failure, thereby causing deaths and disability world-wide. The most predominant type of HTN is essential hypertension (HTN). Unani scholars have mentioned about the clinical manifestations and management of the hypertension and documented it in the context of 'Imtila'. The drug Sankhaholi (Evolvulus alsinoides Linn.) is one of the widely prescribed medicines for the management of essential hypertension in Unani medicine. Material and Methods: The present clinical study was carried out to evaluate the safety of Sankhaholi (Evolvulus alsinoides Linn.) in the management of stage-1 essential hypertension. Newly diagnosed 41 patients of Essential Hypertension (22 patients were in the test group and 19 patients in the control group) were enrolled for the study. All the patients in the test group were given with the test drug 3 g powder of Sankhaholi twice a day for 6 weeks orally. Patients in the control group were given standard drug Ramipril 5 mg once a day for the same duration. Clinical as well as hematological parameters were recorded before and after the treatment. Results: No significant changes are recorded in safety parameters viz. CBC, Haemogram, LFT and KFT. Clinically no adverse effect of the drug has been reported during the course of treatment. Also, significant effect on the systolic blood pressure (p<0.001) were recorded in test group. The drug Sankhaholi was also found effective on the symptoms associated with hypertension. Conclusion: The finding of the study revealed that the test drug Sankhaholi (Evolvulus alsinoides Linn.) is safe and has substantial efficacy as an antihypertensive drug.

  • Antithrombotic agents, including antiplatelet agents and anticoagulants, are widely used in Korea because of the increasing incidence of cardiocerebrovascular disease and the aging population. The management of patients using antithrombotic agents during endoscopic procedures is an important clinical challenge. The clinical practice guidelines for this issue, developed by the Korean Society of Gastrointestinal Endoscopy, were published in 2020. However, new evidence on the use of dual antiplatelet therapy and direct anticoagulant management has emerged, and revised guidelines have been issued in the United States and Europe. Accordingly, the previous guidelines were revised. Cardiologists were part of the group that developed the guideline, and the recommendations went through a consensus-reaching process among international experts. This guideline presents 14 recommendations made based on the Grading of Recommendations, Assessment, Development, and Evaluation methodology and was reviewed by multidisciplinary experts. These guidelines provide useful information that can assist endoscopists in the management of patients receiving antithrombotic agents who require diagnostic and elective therapeutic endoscopy. It will be revised as necessary to cover changes in technology, evidence, or other aspects of clinical practice.

  • Bioinformatics is a rapidly emerging field of biomedical research. A flood of large-scale genomic and postgenomic data means that many of the challenges in biomedical research are now challenges in computational sciences. Clinical informatics has long developed methodologies to improve biomedical research and clinical care by integrating experimental and clinical information systems. The informatics revolutions both in bioinformatics and clinical informatics will eventually change the current practice of medicine, including diagnostics, therapeutics, and prognostics. Postgenome informatics, powered by high throughput technologies and genomic-scale databases, is likely to transform our biomedical understanding forever much the same way that biochemistry did a generation ago. The paper describes how these technologies will impact biomedical research and clinical care, emphasizing recent advances in biochip-based functional genomics and proteomics. Basic data preprocessing with normalization, primary pattern analysis, and machine learning algorithms will be presented. Use of integrated biochip informatics technologies, text mining of factual and literature databases, and integrated management of biomolecular databases will be discussed. Each step will be given with real examples in the context of clinical relevance. Issues of linking molecular genotype and clinical phenotype information will be discussed.
