  • Quality management is a recent phenomenon. Advanced civilizations that supported the arts and crafts allowed clients to choose goods meeting higher quality standards than normal goods. There are many methods for quality improvement. Health care, refers to the treatment and management of illness, and the preservation of health through services offered by the medical, dental, complementary and alternative medicine, pharmaceutical, clinical laboratory sciences (in vitro diagnostics), nursing, and allied health professions. Health care embraces all the goods and services designed to promote health, including "preventive, curative and palliative interventions, whether directed to individuals or to populations. The overall impact of managed care remains widely debated. Proponents argue that it has increased efficiency, improved overall standards, and led to a better understanding of the relationship between costs and quality. Practices can solicit feedback from patients in a variety of ways: phone surveys, written surveys, focus groups or personal interviews. What do I do with the results? While you don't have to act on every suggestion that your patients give you, you should take action on the key items that are causing dissatisfaction.

  • Statement of Problem. As a standard means of diagnostics, an orthopantomogram(OPT) permits to measure the vertical and mesiodistal dimension of available bone at the desired implant site with the help of suitable radioopaque references. Based on the clinical investigation of the dentition and the edentulous sites, information upon the width of the implant site can be obtained and documented in the dental scheme. Both findings permit together systematic primary planning for endosteal implants. Purpose of Study. Contents of the present article are the representation of a semiquantitative classification of available bone with the aim to simplify the primary phase of a systematic implant planning. Results. Thus the ASCIi- system permits a clear protocol of bone findings for the implant case with all information available during the primary appointment for treatment planning as a basis of further diagnostic and therapeutic measures. Conclusion. With the ASCIi system, important parameters such as alveolar height and sub-crestal alveolar width can be documented systematically, easily and time saving in the dental scheme as a basis for exact treatment planning.

Non-invasive Transcutaneous pCO2 Gas Monitoring System for Arterial Blood Gas Analysis

  • Monitoring the carbon dioxide concentration in arterial blood is vital for the evaluation and prevention of pulmonary disease. Yet, domestic pure arterial blood carbon dioxide sensor technologies are not being developed, instead all sensors are imported. In this paper, we develop a real time monitoring system for arterial blood partial pressure of carbon dioxide($pCO_2$) gas from the wrist by using a carbon micro-heater. The micro-heater was fabricated with a thickness of 0.3 ${\mu}m$ in order to collect the carbon dioxide under the skin. The micro-heater has been designed to perform temperature compensation in order to prevent damage to the skin. Two clinical trials of the system were undertaken. As a result, we demonstrated that a portable, transcutaneous carbon dioxide analysis($TcpCO_2$) device produced domestically is possible. In addition, this system reduced the analysis time significantly. Carbon films could reduce the unit price of these sensors by replacing the gold film used in foreign models. Also, we developed a real time monitoring system which can be used with optical biosensors for medical diagnostics as well as gas sensors for environmental monitoring.

The Role of Korean Medicine for Treatment and Management for Lung Cancer (폐암 환자의 치료 및 관리에서 한의학적 치료의 역할)

  • Objectives : Lung cancer is the leading cause of death in cancer patients. Many studies reflect the growing application of integrative korean and western medicine for lung cancer, but its efficacy remains largely unexplored. The purpose of this study is to show that the role of Korean medicine for treatment and manage for lung cancer patients. Methods : In the present study, we searched all the clinical studies of Korean Medical therapies for lung cancer by electronic and manual searching in two databases including MEDLINE, OASIS. Results : Our study showed that acupuncture has strong immunomodulatory effect. Further analysis revealed that acupuncture and herb medicine alleviate the conventional therapy-induced bone marrow supression in lung cancer patients, as well as desease nausea and vomiting. In this study, we showed that acupuncture and herb medicine can improve immediate tumor response, quality of life of lung cancer patients. Conclusions : Korean medicine including that acupuncture and herb medicine is found to be effective in lung cancer treatment and management.

Fatigue by Analyzing Heart Rate Variability and Its Association Factors among Workers in the same industry (심박변이도 측정을 통한 피로요인 분석 : 동일 사업장의 근로자 대상으로)

  • Objective : The aim of this study was to investigate fatigue-related factors including socio-demographic variables and life styles among workers in the automobile manufacturing industry. Method : The subjects were 22,937 individuals who had received health examination by Korean Medical doctors and responded to questionnaires for 8 months from March to November in 2011. The fatigue index was measured by heart rate variability (HRV) in an automobile manufacturing industry. A multivariate analysis with SPSS version 19.0 was used to examine the relationship between fatigue and other factors including ages, smoke consumption, drinking frequency, exercise frequency, occupational category, conjugal condition. Results : In comparison between normal and very bad level of fatigue, factors of ages, smoking, drinking, and exercise showed significant difference(p<0.05). In addition, when normal and bad fatigue were compared, ages, smoking, exercise also produced significant difference(p<0.05). Furthermore, in normal and good level of fatigue comparison, occupation and smoking yielded significant difference(p<0.05). Finally, in other comparisons between normal and very good fatigue, exercise, smoking and occupation revealed significant difference(p<0.05). Conclusion : The results of this study suggest that age increase affects fatigue negatively, whereas exercise and drinking of once or twice per a week can have positive correlations.

A Study on the Yin-Yang Theory in 『Tongsokanuihagwollon(通俗韓醫學原論)』 (통속한의학원론(通俗韓醫學原論) 음양편(陰陽篇)에 관한 고찰(考察))

  • Cho Heon Yeong's 'Tongsokanuihagwollon' is an introductory and essential book on Traditional Korean Medicine(TKM), and now we can nearly take it as classic. It contains almost whole contents of TKM : physiology, pathology, herbology, meridian & acupoint, internal medicine, diagnostics, formula science, etc. He wanted to help people get TKM services easily on their own through this book. In the first chapter following the introduction, he inserted Yin-Yang theory, and his lecture on Yin-Yang theory continues for 88 pages, taking 17% of the whole book. Yin-Yang theory chapter is composed of 14 parts which tells about concept, definition and meaning of Yin-Yang, change of Yin-Yang according to time, season, constitution, body region, meridian, herb, etc. Last part refers to disharmony & dysfunction of Yin-Yang in body, so he showed both theoretical and clinical view of Yin-Yang theory. He wrote some quotations from the book 'Complete Works of Jingyue(景岳全書)', 'Huangdi's Internal Classic(黃帝內徑)', 'Introduction to Medicine(醫學入門)', etc. He tried to explain easily about Yin-Yang theory with modern but rough language of science. He seemed to already know clearly that without modern science TKM cannot progress and will soon be overwhelmed by western medicine, and acted his own way to spread spirit of TKM in the period of rapid change & conflict between two civilizations.

  • Human astrovirus (HAstV) is a major cause of acute diarrhea among children, resulting in outbreaks of diarrhea and occasionally hospitalization. Improved surveillance and application of sensitive molecular diagnostics have further defined the impact of HAstV infections in children. These studies have shown that HAstV infections are clinically milder (diarrhea, vomiting, fever) than infections with other enteric agents. Among the 8 serotypes of HAstV identified, serotype 1 is the predominant strain worldwide. In addition to serotype 1, the detection rate of HAstV types 2 to 8 has increased by using newly developed assays. HAstV is less common compared with other major gastroenteritis viruses, including norovirus and rotavirus; however, it is a potentially important viral etiological agent with a significant role in acute gastroenteritis. A better understanding of the molecular epidemiology and characteristics of HAstV strains may be valuable to develop specific prevention strategies.

  • The most important progress in diagnostic sciences is the increased sensitivity and specificity in diagnostic procedures due to the development of newer micromethodologies and increasing availability of immunological and molecular biological reagents. The outcome of researches in this field has already provided DNA probes and antibodies which can be used for diagnosing various kinds of diseases including inherited ones. This development can be also applied to diagnose diseases in oral and maxillofacial regions. Technological advances have yielded highly sensitive test methodologies so that low analyte concentration and small sample volume are no longer limiting factors. Therefore, saliva can be useful test fluid for an array of analytes. Salivary constituents of diagnostic significance include steroid hormones, antibodies, drugs, and tumor markers. Of the proteins present in saliva, viral-specific immunoglobulins are of the greatest diagnostic interest. The development of conjugates and antigens by recombinant DNA technique and peptide synthesis is necessary for clinical application. Several kits developed for the purpose of blood testing should be modified to permit their application to saliva. The final practical outcome of researches in diagnostic sciences will be various diagnostic agents which can be used for detection of bacteria and viruses, screening and diagnosis of diseases, genetic screening for forensic individual identification. For these purposes, collaboration researches and development between institutions and companies are essential.

A Clinical Study of the Live Blood Condition of Women′s Shoulder Measurement (여성 견비통 환자의 생혈액 형태에 대한 임상적인 연구)

  • Objectives : To study the Live Blood Condition of chronic shoulder measurement. Methods : Twenty-one patients were studied. The observation items of the Live Blood Condition are the form's abnormality and cohesion of red blood cells, the abnormality of hemoglobin and the toxicity in plasma. Results : 1. The form's abnormality and cohesion of red blood cells observed were erythrocyte aggregation (8 persons), Rouleau (3 persons), target cells (7 persons), ovalocytes (3 persons), poikilocytes (2 persons). Double conditions observed were erythrocyte aggregation & target cells (3 persons), erythrocyte aggregation & poikilocytes (1 person), target cells & ovalocytes (l person). 2. The abnormality of hemoglobin and the toxicity in plasma observed were cholesterol crystals (5 persons), atherosclerotic plaque (3 persons), chylous (6 persons). 3. The form normality of red blood was generally observed in one woman. The normality of hemoglobin and the non-toxicity in plasma were generally observed in seven women. Conclusions : According to this study of the Live Blood Condition of women's shoulder measurement, these results suggest that the twenty-one patients evidence the conditions of extravasated blood and phlegm.

  • Objectives : The purpose of this study is to review the Seven Acupoints for Stroke and reveal the meaning of the composition of that. Methods : 1. The authors reviewed several literatures and studies related with the Seven Acupoints for Stroke. 2. We investigated the composition of Seven Acupoints for Stroke and found the mechanism of the function and clinical usefulness of that. Results and Conclusions : 1. Seven Acupoints for Stroke is composed of 7 acupoints, i.e. 1 acupoint of Governer Vessel, 2 acupoints of Yang Myoung meridian, and 4 acupoints of So Yang meridian construct that. 3. Seven Acupoints for Stroke can be used for many of central nervous disease related with stress.
