Effectiveness and Approach of Rehabilitation in Patients With Acute Heart Failure: A Review

  • Acute heart failure is associated with high mortality and frequent rehospitalization, resulting in enormous healthcare costs and declining physical function, activities of daily living, and quality of life. Cardiac rehabilitation has been recommended as one of the non-pharmacologic treatments for patients with heart failure. However, much of the evidence for cardiac rehabilitation interventions reported to date has been limited to chronic heart failure. In recent years, the effectiveness of rehabilitation intervention in patients with acute heart failure has been reported, led by the Rehabilitation Therapy in Older Acute Heart Failure Patients (REHAB-HF) trial. This review overviews the recent evidence of rehabilitation in patients with acute heart failure.

Beta Blockers in Contemporary Cardiology: Is It Better to Cast Them Out?

  • Beta blockers are one of the commonest prescription drugs in medicine and they have been thought to revolutionize the treatment of heart failure (HF) with reduced ejection fraction (HFrEF) in the last century. In addition to HFrEF, they are prescribed for a variety of diseases in cardiology from hypertension to HF, angina, and stable coronary artery disease (CAD). The increased prescription of beta blockers in conditions like HF with preserved ejection fraction (HFpEF), and stable CAD may be doing more harm than good as per the data we have so far. The available data shows that beta blockers are associated with increased stroke risk and atrial fibrillation (AF) in hypertension and in patients with HFpEF, they have been associated with decreased exercise capacity. In patients with stable CAD and patients with myocardial infarction with normal systolic functions, beta blockers don't offer any mortality benefit. In this article, we critically review the common indications and the uses of beta blockers in patients with HFpEF, CAD, hypertension and AF and we propose that beta blockers are overprescribed under the shadow of their beneficial effects in patients with HFrEF.

Exploring the Effect of Qihyul-therapy for Recovery of Qi Circulation in Body (인체에서 기혈(氣穴) 순환 회복을 위한 기혈테라피의 효과 탐색)

  • Purpose: The purpose of this study was to investigate whether Qihyul-therapy was effective against subjects of bad Qihyul circulation who had narrowed muscles of the buttocks, a distorted body shape, or pain. Methods: Qihyul-therapy used. Results: The muscles of the buttocks remained almost untouched. The elasticity of the muscles restored. The skin tone brightened, and the anal muscle strengthened after Qihyul-therapy. The subject's body was shifted to the right by -7°. The right arm was about 6 cm longer than the left arm. But after correction, the right arm was about 5 cm shorter. The distorted body was almost balanced. After neck and shoulder correction, the distance between the collarbone and neck was 7~10 cm wider. After the pelvic correction, the pain in the buttocks was relieved, and the position of the hip joint returned correctly. The right leg that was turned 45° to the left was corrected, and the length became the same. Subjects had no knees attached, their right knees were up 5 cm above the left, and their ankles bent at an angle of 30 ° relative to the left, but after correction, the knees of the two legs stably attached. The height of the knee became the normal angle. Conclusion: If the Qihyul-therapy, the spine correction Qihyul-exercise and the pelvic correction Qihyul-exercise performed steadily, the deformed body shape restored adequately, and it is useful to prevent and cure related diseases by alleviating the pains.

Double Outlet Right Ventricle in a Maltese Dog (말티스견에서 발생한 양대혈관 우심실기시)

  • An 8 month old female Maltese (body weight 3.6 kg) was referred with primary complaints of dyspnea and exercise intolerance. Diagnostic imaging studies revealed marked cardiomegaly and prominent main pulmonary trunk dilation on thoracic radiography, abnormally arisen aortic roots (toward right ventricle) with left-to right shunted perimembraneous ventricular septal defect located underneath of aortic root, aortic root was located to predominantly to the right ventricle and pulmonary regurgitation (peak velocity 4.7 m/s, pressure gradient ~88 mmHg) from pulmonary over-circulation and hypertension on echocardiography, indicating double outlet right ventricle (DORV). The dog was treated with furosemide (1 mg/kg, BID) for reducing volume overload at right ventricle, spironolatcone (1 mg/kg) and enalapril (0.5 mg/kg) for minimizing deleterious cardiac remodeling, and sildenafil (1 mg/kg) for lessening pulmonary over-circulation and hypertension. The clinical condition of this dog was improved after 1 week of medical treatment. The dog is currently survived and regularly monitored.

Aorto-Coronary Bypass Graft -A Case Report- (관상동맥 회로술 치험 1예)

  • Occlusive coronary artery disease presents a potential threat to a significant population in the United States. According to many case reports, the increasing incidence of coronary artery disease due to atherosclerosis is noted in Korean, recently. Operative vascular procedures have increased steadily in number over the past 20 years in the United States. There have been many isolated case reports about coronary artery surgery, but these had little clinical impact. Nowadays, major three coronary bypass surgery has developed principally at three cardiac centers in the United States since 1967. Among three coronary bypass operation, the aorta-coronary artery saphenous vein bypass graft was first demonstrated by Favolaro and Effler at the Cleveland Clinic in 1967. We experienced one case of coronary artery disease, which was treated successfully by aorta-coronary saphenous vein graft with mild hypothermia under extracorporeal circulation in May, 1977. Saphenous vein was removed from above the knee and was 2.5 mm in diameter. The left ventricle was not vented for the left ventricle was not overdistended. Temporary artificial pace-maker-Medtronic-was implanted for the prevention and treatment of post-operative arrhythmia and heart block in post-operative first day. He is a 57 year old male businessman who had been suffered from hypertension [200 mmHg in systolic pressure] since 4 years ago, who had intermittent conservative treatment at local clinic. He had been afflicted with severe chest pain with choking sensation for 50 days. This symptom was aggravated exposing cold weather, or cold water, but was respond to rest. Pre-operative ECG revealed no any other ischemic sign except sinus bradycardia. Significant S-T segment depression was noted at lead II, AVF after double 5 minutes exercise, indicating positive Master`s test. Serum cholesterol was slight elevated to 253 mg/dl. Final pre-operative diagnosis was made by coronary arteriogram, which showed about 1.0-cm segmental 90 % occlusive atherosclerotic lesion in the proximal part of right coronary artery above the origin of acute marginal artery. Left coronary artery revealed good patency and there was no collateral circulation between right and left coronary artery .Hospital course was not eventful. He was discharged with good result on the post-operative day. He has been free from chest pain for longer than 2 years. And also the arterial flow in the coronary bypass graft is auscultated with the pocket-sized ultrasonic velocity detector, which shows the patency of the coronary bypass graft good.

A Literature Review on Prevention and Management of Acne (여드름의 예방 및 관리에 대한 문헌적 고찰)

  • Objectives : This study was conducted to review about a prevention and management of acne Methods : We searched Korean research database by using keyword 'acne' and 'prevention' or 'management'. Korean research databases were Korean Studies Information Service System(KISS), Research Information Service System(RISS), Korean Medicine Information System(OASIS), National Digital Science Library(NDSL) and Jeonju Municipal Library. We classified studies identified by year, method, contents Results : Among 19 studies, 10 studies were articles and 9 studies were books. The proportion of studies published in 1990s was 26.3%, published in 2000s was 46.3%, published in 2010s was 26.3%. We analyzed 19 studies by classifying 4 categories that consist of Diet Management, Skin Management, Life Management, Exercise Management. In Diet Management, intake of fruits and vegetables was highly recommended, and intake of oily foods and high-fat foods was restricted. In Skin Management, thorough skin cleansing was highly recommended and skin contact was restricted. In Life Management, enough sleep and rest were highly recommended, and smoking was restricted. In Exercise Management, some of exercises to promote blood circulation were recommended. Conclusions : Various methods have been used for prevention and management of acne. It is necessary to systematically study the prevention and management of acne.

  • With the recent rapid increase in obesity worldwide, metabolic syndrome (MetS) has gained significant importance. MetS is a cluster of obesity-related cardiovascular risk factors including abdominal obesity, atherogenic dyslipidemia, high blood pressure and impaired glucose tolerance. MetS is highly prevalent and strongly associated with an increased risk of developing diabetes and cardiovascular disease, putting a great burden on human society. Therefore, it is very important to reduce MetS risk, which can improve patients' cardiovascular prognosis. The primary and most effective strategy to control each component of MetS is lifestyle change such as losing body weight, keeping regular exercise, adopting a healthy diet, quitting smoking and alcohol drinking in moderation. Many studies have shown that lifestyle modification has improved all components of MetS, and reduces the incidence of diabetes and cardiovascular disease. Here, the Korean Society of CardioMetabolic Syndrome has summarized specific and practical methods of lifestyle modification in the management of MetS in the healthcare field.

  • A unhealthy 58-year-old male patient required extraction of left upper second molar due to advanced periodontitis. Lidocaine contained 1 : 100000 epinephrine for left posterior superior alveolar nerve block was administered in the mucobuccal fold above the second molar to be treated at the local private dental clinic. After four hours of posterior superior alveolar block anesthesia, patient feeled double vision and discomfort of eyeball movement. At next day, he complained difficulty of left eyeball movement, vertigo and diplopia. He was referred to our department via local clinic and department of ophthalomology of our hospital. He was treated by medication and eyeball exercise, and then follow up check. The double vision and medial rectus muscle palsy disappeared patially after 2 months of block anesthesia. We described herein an ocular complication of diplopia and inferior rectus muscle palsy after posterior superior alveolar nerve block for extraction of left upper second molar, and review the cause or origin of this case. The autonomic nervous system is presented as the logical basis for the untoward systems of ophthalmologic sign likely to diplopia and inferior rectus muscle palsy, rather then simple circulation of anesthetic solution in the vascular network.

  • 1. At the Menstruation Period, the uterus is easy to be invaded by a toxin because of its patency. Take cleanliness and hygiene into consideration. 2. At the Menstruation Period, much attention must be paid at the regulation of coldness and heat to harmonize the Menstruation. 3. At the Menstruation Period, excessive exercise must be avoided. Appropriate labor can help the circulation of vital energy & blood and reinforce the immunity of body. 4. The nourishing, plain, warming, digestible food is good at the Menstruation Period. Hot, stenched, stimulating food must be avoided and nuts & water fruit must not be overeaten. 5. At the Menstruation Period, sharpness & excessiveness of emotion must be avoided and a comfortable mood must be maintained. Especially premenstrual or postmenstrual period, peaceful spirit and sufficient sleep must be kept. 6. Women generally feel uncomfortable at the Menstruation Period, but it disappears spontaneously after menstruation. Don't take an overdose of medicine because it can disturb the normal condition.

  • In modem city life, the citizen get insufficient exercise and has high levels of stress. Increased Stress causes such minor things as tiredness, disease and mental fatigue, and increase brain blood pressure too. In this paper trying to design the multi-functional blood pressure monitor with airo-dynamic brain hemokinesis improvement function. So this system have developed for the improvement of flows in the capillary blood vessel of head and limbs. Also This system is able to measure brain blood pressure(BBP) which need diagnosis circulation state of brain blood. So, we extracted correlativity of blood pressure(BP) and BBP through clinical experiment. We confirmed that compare factor of BP with BBP is more useful factors for diagnosis state of brain hemokinesis.
