Coarctation of the Aorta Associated with Chronic Thoracic Aortic Aneurysm -A case report - (만성 흉부 대동맥류를 동반한 대동맥 축착증 - 1예 보고 -)

  • A 49-year-old woman had thoracic back pain for several years. Chest CT scan and MRI angiography revealed descending thoracic aortic aneurysm with a maximum diameter of 69 mm. Thoracic aortography showed not only the aortic aneurysm, but also coarctation of descending thoracic aorta at the level of aortic hiatus of the diaphragm. Intercostal artery arising Adamkiewicz artery was found in descending thoracic aortic aneurysm just above the coarctation, The aneurysm with coarctation of the aorta was successfully repaired with prosthetic graft replacement under left atrio-femoral bypass.

Spinal Subdural Hematoma Associated with Intracranial Subdural Hematoma

  • The simultaneous occurrence of an intracranial and a spinal subdural hematoma (SDH) is rare. We describe a case of cranial SDH with a simultaneous spinal SDH. An 82-year-old woman visited the emergency room because of drowsiness and not being able to walk 6 weeks after falling down. A neurological examination showed a drowsy mentality. Brain computed tomography showed bilateral chronic SDH with an acute component. The patient underwent an emergency burr-hole trephination and hematoma removal. She exhibited good recovery after the operation. On the fourth postoperative day, she complained of low-back pain radiating to both lower limbs, and subjective weakness of the lower limbs. Spine magnetic resonance imaging revealed a thoracolumbosacral SDH. A follow-up spinal magnetic resonance imaging study that was performed 16 days later showed a significant decrease in the size of the spinal SDH. We discuss the pathogenesis of this simultaneous occurrence of spinal and cranial SDH.

Analysis for clinical feature of 130 patients with dyspepsia in a Korean Medicine hospital (소화불량증을 호소하여 1개 한방병원에 내원한 환자 130례에 대한 임상특성 분석)

  • Objective : This study aimed to explore the clinical feature of patients with dyspepsia who were treated by an Oriental internal medicine. Methods : We analyzed 130 patients who visited an Korean Medicine Hospital for their dyspepsia-related symptoms, from March 1, 2014 until to February 28, 2015 Results : The sex composition was consisted of 31.5% man and 68.5% woman, and 85.6% of outpatient and 14.4% of inpatient. The 39.2% of patients showed chronic complain (>6 months), and the main causes included stress and fatigue for 46.2% and 31.5% respectively. The accompanied symptoms were mainly fatigue (40.0%), insomnia (36.9%), abnormal stool (34.6%), upper back pain (31.5%), and headache (28.5%). The improvement rate was 68.5% while other (31.5%) showed no change of dysplasia. Conclusion : This study created an important information of patients with dysplasia, which will be critically helpful for development of Korean Medicine-based therapeutics in the future.

Prevalence, Pathophysiology, Screening and Management of Osteoporosis in Gastric Cancer Patients

  • Osteoporosis in gastric cancer patients is often overlooked or even neglected despite its high prevalence in these patients. Considering that old age, malnutrition, chronic disease, chemotherapy, decreased body mass index and gastrectomy are independent risk factors for osteoporosis, it is reasonable that the prevalence of osteoporosis in gastric cancer patients would be high. Many surviving patients suffer from back pain and pathological fractures, which are related to osteoporosis. Fractures have obvious associated morbidities, negative impact on quality of life, and impose both direct and indirect costs. In the era of a >55.6% 5-year survival rate of gastric cancer and increased longevity in gastric cancer patients, it is very important to eliminate common sequelae such as osteoporosis. Fortunately, the diagnosis of osteoporosis is well established and many therapeutic agents have been shown to be effective and safe not only in postmenopausal females but also in elderly males. Recently, effective treatments of gastric cancer patients with osteoporosis using bisphosphonates, which are commonly used in postmenopausal woman, were reported.

  • According to recent data more than 80% of people suffer from back pain, due to an aging and a poor posture, at least once in their life time, and 7~10% of them have chronic spine illness. Researchers over the years have studied on various spinal traction devices that utilize the force of traction, and have also reported clinical test results. However, most existing devices are too complicated and too expensive. In order to solve these problems, we have developed a new device in which the frame moves up and down and at the same time tilting angle of the frame is adjusted. We have analyzed the forces applied on the body as a function of tilting angle. And the result shows that the new device has achieved the force of traction, which is known to be effective for spinal rehabilitation.

Effects on the Lumbar & Quadriceps Muscle Activities when Getting in and Out of RV · and Passenger Cars

  • Objective: The objective of this study is to compare muscle activity levels of the lumbar and rectus femoris muscle when entering and exiting RV and passenger vehicles. Background: Three varieties of RVs and two varieties of passenger vehicles were selected for this study. Method: Twenty individual(10 male and 10 female) in their twenties and thirties with no chronic back pain or any other illness participated in this study. Participants performed entering and exiting the three models of RVs and two models of passenger cars. Appropriate rest time was given to participants between entering and exiting each vehicle in order to recover from muscle fatigue while performing their tasks. Results: Results from this study show that muscle activity levels were highest for entering RVs and exiting passenger vehicles. There were no significant differences in muscle activity levels for left and right side muscles in regards to entering and exiting vehicles. Application: These results can be used to provide baseline information for reduce muscle fatigue when getting in and out of vehicles.

  • The prevalence of discogenic pain among patients with chronic low back pain is estimated to be about 40%. Lumbar discectomy is being performed as a treatment according to the studies done so far. Recently IDET- Intradiscal electrothermal therapy which is minimally invasive technique is being introduced. This study will investigate important factors of this procedure such as the temperature of heat source, loading times, and the temperature distribution within the intervertebral disc. This study utilized finite element analysis and experiment. It was able to analyze the temperature range of inner intervertebral disc by two mechanisms which are known to alleviate pain clinically. As a result, verification of temperature distribution to 15.6mm($\geq$45$^{\circ}C$) (Mechanism 1-coagulation inner annulus by heat) and 9mm($\geq$6$0^{\circ}C$) (Mechanism 2- contraction inner nucleus by heat) from the heat source was done.

The Application of Ryodoraku in the U-health Care System (유헬스케어(U-health Care)에서 양도락의 활용 방안)

  • Objectives : The purpose of this study was to propose the utilization plan of Ryodoraku in the U-health care systems. Methods : Computerized literature searches were performed for Ryodoraku related articles using the following databases: KISS, RISS, DBPIA, NDSL from 1990 to Oct 2010. Search terms were '양도락' or 'Ryodoraku' or 'U-health'. Due to Ryodoraku coming from Japan, additional literature review(articles published by 2008) on Japanese journal of Ryodoraku medicine was done for compensation. Results : 1. Introduction of U-health : As rapid progress of population aging and strong interest in health, the demand for the traditional Korean medical service is increasing. Until now healthcare service has provided post treatment by face-to-face manner. But according to related researches, proactive treatment is resulted to be more effective for preventing diseases. Particularly, the existing healthcare services have limitations in preventing and managing chronic geriatric degenrative diseases such as metabolic syndrome, CVA, coronary heart diseases, parkinson's diseases, degenerative joint disease, spondylosis, etc., because the cause of the above is complex and even related to life habit. As the advent of ubiquitous technology, patients with the chronic geriatric degenrative diseases can improve life habit such as poor eating habits and physical inactivity without the constraints of time and space through u-healthcare service. Therefor, lots of researches for u-healthcare service focus on providing the personalized healthcare service for preventing and managing that. To cope with this situations, The concept of u-healthcare service should be adopted in the traditional korean medicine and diagnostic devices suitable for it should be also devised and developed based on traditonal korean medine. 2. Review of existing Ryodoraku related articles for applying to U-health : articles investigating feasibility applying Ryodoraku to meridian diagnosis and raising problems of it, articles providing recent research trends of Ryodoraku, Ryodoraku related articles considering usefulness for U-health, and articles confirming the repeatability and reproducity of Ryodoraku were included. Based on the review of the above Ryodoraku related articles, several application of Ryodoraku in the U-health care system. Conclusions : To make preparations for the increasing need of traditional Korean medicine due to rising morbidity rate of chronic geriatric degenerative diseases, it is necessary to appropriately apply Ryodoraku to the U-health care system. The application of Ryodoraku is as follows. 1. To use Ryodoraku additionally to the established diagnostic device of metabolic syndrome, CVA, coronary heart diseases, parkinson's diseases, degenerative joint disease, spondylosis. 2. To apply Ryodoraku to the symptoms or diseases having a tendency to be diagnosed by correlation between the affected meridian and the lesion such as headache, nuchal pain, shoulder pain, low back pain, sciatica, HNP, etc. 3. To secure the repeatability and reproducity of Ryodoraku. 4. To devise and develop Ryodoraku appliance in order to overcome the known drawbacks and to improve error of measurement.

Effect of Joint Mobilization and Insole on Pain, Pelvic Angle, and Foot Pressure in Patient with Sacroiliac Joint Pain : A Randomized Controlled Pilot Trial (관절가동술과 깔창적용이 엉치엉덩관절통증환자의 통증과 골반경사각, 족저압에 미치는 효과: 무작위배정예비임상시험)

  • This study was to compare the effects of joint mobilization, treadmill with insole, and joint mobilization and treadmill with insole on pain, pelvic angle, and foot pressure in patients with sacroiliac joint pain. 24 patients randomly assigned to joint mobilization group(n=8), treadmill with insole group(n=8), or joint mobilization and treadmill with insole group(n=8). Each groups were conducted 30 minutes a day, two days a week for four weeks. Pain was evaluated using visual analogue scale and pelvic angle was measured using palpation meter and foot pressure(fore/rear ratio) was measured using Gateview AFA-50 before intervention and after 4 weeks. All groups were significant differences pain in intragroup(p<.01). In pelvic angle, the joint mobilization group was statistically significant in the anterior tilt only, the joint mobilization and treadmill with insole group showed statistically significant improvement in both anterior and posterior tilt(p<.01), and the treadmill with insole group did not show any statistically significant change(p>.05). Also the joint mobilization and treadmill with insole group were significant differences in foot pressure(p<.01). All the interventions reduce sacroiliac joint pain and joint mobilization and treadmill with insole training are most effective changes in pelvic angle and foot pressure. This study can be used as a basic data for prevention of injury, posture correction and gait training in patients with sacroiliac joint pain, as well as chronic low back pain and plantar pressure problem.

Two-Case Report on Chronic Renal Failure Treated with Herbal Enteroclysis (한약 관장법으로 호전된 신부전 환자 2례)

  • Chronic renal failure (CRF) is defined as a permanent reduction in glomerular filtration rate (GFR) sufficient to produce detectable alterations in well-being and organ function. This usually occurs at GFR below 25 ml/min. In this report, is a 75-year-old woman who had back pain and diarrhea as chief complaints. She was diagnosed with CRF on March. 2008. is a 62-year-old man who had whole body weakness, anorexia and dyspepsia as chief complaints. He was diagnosed with CRF on 2008. We used herbal enteroclysis solution. which consists of Radix et Rhizoma Rhei. Concha Ostreae and Radix Glycyrrhizae or Radix Salviae Miltiorrhizae. Herbal enteroclysis was done for 15 min once a day. Enteroclysis solution was controlled at 37-39 $^{\circ}C$ and inserted 15-20cm depth for intubation. We found the patient's BUN, creatinine and GFR were improved through enteroclysis. In both and , the patient's BUN and creatinine decreased, and GFR increased favorably. This result suggests that herbal enteroclysis used for CRF is effective in clinical therapy.
