임부의 분만 자신감, 산전 우울, 분만 지식과 배우자 지지는 분만 두려움에 영향을 미치는가? (Do childbirth confidence, prenatal depression, childbirth knowledge, and spousal support influence childbirth fear in pregnant women?)

  • Purpose: This study investigated levels of childbirth fear and related prenatal factors (self-confidence for childbirth, prenatal depression, knowledge about childbirth, and spousal support) among pregnant women in South Korea. Methods: A correlational study design was used to explore levels of childbirth fear and related prenatal factors in 200 pregnant women over 28 weeks of gestation. A self-administered questionnaire was used to measure fear of childbirth and related factors, such as self-confidence for childbirth, prenatal depression, knowledge about childbirth, and spousal support. Results: One-third of the pregnant women were aged 35 years and older. Sixty-one percent of women were nulliparae, but only 26.0% had experienced prenatal education. The mean score for fear of childbirth was 66.99 out of 165. The prevalence of fear of childbirth was 72.0%, and childbirth fear was severe in 26.5% of the participants and moderate in 45.5%. Fear of childbirth was negatively related to self-confidence (r=-.45, p<.001), but positively related to prenatal depression (r=.21, p=.002). Two significant predictors were found to explain the fear of childbirth. Higher self-confidence for childbirth was associated with less severe fear of childbirth (β=-.44, p<.001), while higher prenatal depression was associated with more severe fear of childbirth (β=.13, p=.038). Conclusion: The level of fear of childbirth was higher among pregnant women with lower self-confidence and higher prenatal depression. Reasonable evidence should be provided for implementing prenatal and childbirth classes to reduce pregnant women's depression and to increase their confidence.

남편의 분만 지지와 분만참여 태도가 분만경험에 미치는 영향 (Influences of Husband's Childbirth Experiences related to Supports for their Wives during Childbirth and Attitudes toward Childbirth Presence)

  • Purpose: The purpose of this study was to identify the relations among supports during childbirth, attitudes toward childbirth presence, and the childbirth experiences in husbands. Methods: The participants in this study were 178 husbands whose wives were within 2 days after normal spontaneous vaginal delivery. Data were collected from September 1st to September 30th, 2013. Results: The significant factors affected on the childbirth experiences in husbands were attitudes toward childbirth presence (${\beta}=.61$), satisfaction for the nurse's cares during childbirth (${\beta}=.19$), psychological preparation for childbirth (${\beta}=.16$), baby weight (${\beta}=.09$), and prenatal education related to pregnancy (${\beta}=.09$). These variables explained 66% of the childbirth experiences in husbands. Conclusion: It would be necessary to develop nursing intervention programs that increasing positive attitudes toward childbirth presence, psychological preparation for childbirth, and prenatal education to increase positive childbirth experiences in husbands. Nurses should also provide husbands with satisfactory cares during childbirth.

출산장려정책으로 인한 여성의 출산의식 변화에 관한 사례연구 (A Case Study on Changes in Perceptions of Childbirth among Women in Relation to the Korean Childbirth Encouragement Policy)

  • The low birth rate in Korea reflects the declining desire to have children in this country due to rising emphasis on personal lifestyle. Since this phenomenon has accelerated, boosting the birth rate has become one of the most important policy tasks in Korea. The purpose of this research was to perform a case study based on women's lived experience to understand how the childbirth encouragement policy promoted by the government in order to boost the birth rate has affected the perception of childbirth among women. The findings are as follows : First, the effect of the childbirth encouragement policy on women's perceptions has been fairly low. Second, the social environment for encouraging childbirth is inadequate. Third, the economic burdens associated with childbirth and child care are still significant. Finally, the impact of the childbirth encouragement policy on changes in the perceptions of childbirth has been minimal. Based on these findings, various motivations for changes in the perceptions of childbirth among women were identified, leading to the following conclusion: Establishing a long-term childbirth encouragement policy is an effective way to boost the childbirth rate.

라마즈 출산교육 참여자의 분만경험 (Childbirth Experience of Participants in Lamaze Childbirth Education)

  • Purpose: This study was intended to explore the essential structure and the meanings of childbirth experiences among Korean women participated in Lamaze childbirth education. Methods: Giorgi's phenomenological method was used to analyze data collected by in-depth interviews with six primiparous women from March to July 2009. Results: Five components identified in the meanings of experience: 'Simplicity', 'Self-control', 'Uncontrollable pain', 'Spiritual maturity', 'Physiologic event'. Conclusion: The study results revealed that the childbirth experience was positive generally, partially influenced by Lamaze childbirth education. But, meanings of childbirth experience with Lamaze childbirth education were mostly physiological respect related to labor pain or one's own efforts to endure labor pain. Therefore, nursing strategies for drawing emotional and socio-environmental experiences from childbirth experience with Lamaze childbirth education is needed.

  • Purpose: This study was conducted to evaluate the effect of a one-session spouse-support enhancement childbirth education on childbirth selfcefficacy and perception of childbirth experience. Methods: The design of this study was a nonequivalent control group pretest-posttest experiment. The participants in the study were 31 couples in the experimental group and 30 in the control group. The experimental couples were provided with one session on spouse-support enhancement childbirth education the night before delivery. Data were collected at two hours after delivery using the Childbirth Self-Efficacy Inventory (CBSEI) and perception of childbirth experience scale. Data were analyzed using PASW statistics 18 program. Frequencies, percentage, mean, $x^2test$, t-test were used for data analysis. Results: Childbirth self-efficacy significantly increased in the experimental group as compared to the control group, but there was no significant difference in perception of childbirth experience in the experimental group compared to the control group. Conclusion: The results suggest that a one-session childbirth education has beneficial effects on enhancing childbirth self-efficacy in pregnant couples. A one-session spouse-support enhancement childbirth education is recommended as an effective nursing intervention to promote couple's childbirth self-efficacy and it is also recommended that modifications of program to promote women's childbirth experience should be considered.

일 지역 보건계열 대학생을 대상으로 한 희망자녀수, 출산정책, 출산저하 원인이 출산인식에 미치는 영향 (Influence of the Hoping Number of Children, the Policy of Childbirth, and the Causes of Reduced Childbirth to the Perception of Childbirth in Health-Line College Students)

  • 목적: 본 연구는 미래의 출산주체가 될 보건계열 대학생들의 희망자녀수, 출산정책, 출산저하 원인이 출산인식에 미치는 영향을 살펴 본 결과 다음과 같다. 결과: 첫째, 출산인식에 관한 태도는 개인 삶의 질과 자녀양육, 출산지원 부족, 후손 번식이 출산에 가장 높은 영향을 미쳤으며, 희망자녀수에 관한 태도에서는 경제적 이유, 양육비 및 교육비 증가 원인과 출산인식 등이 영향을 미치는 것으로 조사되었다. 둘째, 인구사회학적 특성이 출산에 영향을 미치는 요인을 분석 결과 성별에 모든 요인에서 유의하였고, 성장지역에서 희망자녀수, 경제수준에서는 출산정책을 제외한 모든 요인에서, 부모직업에서는 희망자녀수에서 유의하였다. 셋째, 상관관계를 분석한 결과 출산저하원인과 경제적 요인에서 희망자녀수, 개인적 요인에서는 출산정책, 출산제도요인에서는 개인적요인, 가족적 요인에서 출산저하원인과 경제적 요인을 제외한 모든 영역에서 상관관계가 있는 것으로 나타났다. 넷째, 희망자녀수, 출산정책, 출산저하 원인이 출산인식에 미치는 영향을 살펴 본 결과 출산인식에 희망자녀수인 무자녀 또는 1자녀를 원하는 경우, 2자녀 이상, 출산정책, 출산저하원인이 통계적으로 유의미한 정(+)의 영향을 미치는 것으로 나타났다. 결론: 출산 주체자인 대학생들의 미래 출산인식을 전환하기 위한 정부차원에서 출산정책 입법제정과 출산지원정책, 출산인식의 가치관 형성을 위한 교육프로그램 개발과 운영, 현재 출산지원정책의 효과를 분석하여 새로운 지원책이 마련된다면 출산율 향상으로 이어 질 수 있을 것으로 판단되며, 그에 따른 효과를 검증 할 수 있는 후속 연구가 필요 할 것으로 사료된다.

코칭 출산교육 프로그램이 초임부의 불안과 출산 자기효능감에 미치는 효과 (Effects of a Coaching-based Childbirth Program on Anxiety and Childbirth Self-efficacy among Primigravida Women)

  • Purpose: Childbirth self-efficacy plays an important role in women's ability to cope with labor and delivery. Coaching has been gaining popularity as a way to promote cognitive, emotional and behavioral change. This study aimed to test the effects of a Coaching-based childbirth program on anxiety and childbirth self-efficacy among primigravida women. Methods: The study design was a quasi-experimental pre-post design. A coaching-based childbirth program was developed synthesizing concepts and techniques from the literature on coaching and was verified by an expert panel. It consisted of four weekly 2-hour small group sessions. Pregnant women were recruited from H hospital in Seoul. Childbirth self-efficacy and state anxiety were measured before and after the program. Results: Although there was a no significant reduction in anxiety, there were statistically significant increase for childbirth self-efficacy in the experimental group when compared to the control group. Conclusion: The program appears to increase childbirth self-efficacy for pregnant women. Future studies may benefit from using mixed coaching modalities and consider measuring health behaviors and obstetric outcomes to gain insights on its long-term impact.

출산에 영향을 미치는 개인 가치관과 출산장려정책에 대한 사회적 인식 (Individual Values on Childbirth and Social View on Childbirth Encouragement Policy)

  • The purpose of this study was to examine whether childbirth encouragement policy is actually helping those childbirth, and also discuss the possible directions of this policy to unfold in the future, should take. For this, We have surveyed 412 married and 437 unmarried men and women in order to investigate the individual values that influence childbirth, From these respondents and we interviewed and surveyed recipients of grants (244people), administrative officers in charge (41people), and experts (34people) to investigate their social view on the Korean government's impact. The results of this study are as follows: Firstly, most of the people (unmarried 70.3%, married 69.1%) surveyed said that important life objectives for them. Secondly, most of the people(male 44.4%, female 73.8%) surveyed said that once married shied away from having babies, (a) due to the finance and childcare burden is heavy of running a family while both parents are fulltime employed (b) because it is difficult to get pregnant due to health problems, and (c) foremost because having several children exacerbates their financial burden. Thirdly, grant recipients(75.4%), administrative officers in charge(65.9%), and experts(53.0%) all said that childbirth encouragement policy currently being implemented has little impact on their childbirth. Finally, it was established that the incentives to encourage childbirth is best suited for individuals that favor having many children.

출산준비 교육이 초산모 배우자의 지식 및 분만참여에 미치는 효과 (Effects of a Prepared Childbirth Education on the Knowledge and Delivery Participation Levels of the Spouses of Primiparas)

  • Purpose: This study was conducted to identify the effects of a prepared childbirth education on the knowledge and delivery participation levels of the spouses of primiparas. Method: The study's subjects were a convenience sample of 126 spouses, consisting of an experimental group of 74 educated for prepared childbirth for 10 hours. This study was measured by using a 41-item measurement instrument for knowledge of childbirth that was devised from Kang's(1981) instrument and a 23-item measurement instrument for measuring delivery levels which was developed through clinical experiences. The data gathered as analyzed by using SPSS WIN10.0. Result: There were no significant differences between the experimental group and the control group in homogeneity concerning the general characteristics of the spouses of primiparas. The first hypothesis, which stated that the level of knowledge of childbirth in the experimental group who received prepared childbirth education would be higher than in control group, was statistically supported by the Compared Independent Sample t-test(t=17.92, p=.000). The second hypothesis, which stated that the level of participation of delivery in the experimental group who received prepared childbirth education would be higher than in the control group, was also statistically supported (t=28.87, p=.000). The third hypothesis, which stated that there would be a correlation between the level of knowledge and the level of participation in the experimental group who received prepared childbirth education, showed a high positive correlation(r=.810, p=.000). Conclusion: The above results indicate that the increase of knowledge through prepared childbirth education in the spouses of primiparas has a high correlation with delivery partici- pation levels which engenders physical and moral support for primiparas at delivery. Additionally, a varied prepared childbirth education acts as an effective means to increase under- standing and support for primiparas couples who experience tension, anxiety and fear due to an unprepared delivery in Korea.

출산교실에 참여한 임부의 교육 평가 및 교육 (The Evaluation of Childbirth Class and Eductional Need of Pregnant Women)

  • This study was conducted to identify the educational need of pregnant women, and to evaluate the effect of the 2-hour childbirth class provided by researchers. The subjects were composed of 183 pregnant women who participated voluntarily in the 2-hour childbirth class, and the data were collected by self reporting structured questionnaire designed by researchers. The data were analyzed with descriptive statistics, paired t test, and McNemar test by SPSS pc program. The results were as follows ; 1. Mean age of th subjects was 27 and most subjects graduated from the high school and the college. 80.3% of the subjects had nuclear family. 82.5% of the subjects were full time housewives. 2. 85.8% of the subjects were ipara. Though 71.6% of the subjects might have antepartal care in the clinic, 67.2% of the subjects had no chance to participate in the childbirth class before this study. 3. Subject's health and lifestyle, especially related to eating habits, were changed in the antepartum period. We found that the pregnancy affected positively on health and lifestyle of the women. Over 75% of the subjects became to understand the progress of labor, breath methods during the labor, and perineal exercise. Most subjects changed to think that they could control their body more actively and positively during the labor after participating in the childbirth class. 93.4% answered that 'childbirth class was necessary and important'. Most subjects wanted to be taught the contents, such as postpartum care, breast feeding, and infant care in the childbirth class. In conclusion, pregnant women to attend the childbirth educational program with their husband, and the 2-hour childbirth class provided by the researchers very effective in many aspects. Therefore we anticipated that the childbirth class on basis of pregnant women's needs will contribute to improvement of pregnant women's health and well-being during the labor.
