Risperidone Monotherapy in Children and Adolescents with Autism Spectrum Disorders : A Naturalistic Study

  • Objectives : We retrospectively investigated the efficacy and tolerability of risperidone monotherapy in subjects with autism spectrum disorder (ASD). In addition, we did mixed effect model analysis of the effects of risperidone in patients with ASDs naturalistically treated in a routine clinical setting to determine whether the clinical effects were maintained and the side effects were tolerable. Methods : This retrospective study assessed children and adolescents with ASD, who were on risperidone monotherapy from July 2010 to July 2011 at the Child and Adolescent ASD Clinic at Seoul National Hospital. Outcome measures included the Clinical Global Impression-Severity of Illness (CGI-S) and the CGI-Improvement (CGI-I) scales along with other clinical indices: dosage, target symptoms, and side effects. Results : The mean dose of risperidone in 47 children and adolescents with ASD (40 males, 7 females; age range 5-19 years) who were on risperidone monotherapy was $1.6{\pm}0.8mg/day$, and the mean duration of the treatment period was $20.2{\pm}17.3months$. Aggressive behavior, stereotypic behavior, irritability, and self-injurious behavior were the most frequent target symptoms of risperidone. The most common side effects were weight gain followed by somnolence and extrapyramidal symptoms. In a mixed effects model analysis of CGI-I scores, the mean CGI-I score at the 1 month follow-up was significantly different from the mean CGI-I score of the 3-month follow-up (p=.046), and the CGI-I scores were equally maintained over 3 to 48 months [F(6, 28.9)=4.393, p=.003]. Of the 47 patients, 33 patients (70.2%) were identified as the response group, showing an end point CGI-I rating of 3 or under and having continued risperidone treatment for at least 6 months. The baseline CGI-S score showed significant association with clinical response to risperidone (p=.005), the mean baseline CGI-S was higher in the response group compared to the non-response group. Conclusion : In this study, clinical improvement of risperidone stabilized around 3 months and was equally maintained up to 48 months with tolerable side effects, supporting maintenance of risperidone treatment in children and adolescents with ASDs.

  • The purpose of this study was to provide basic data on preschool children for accident prevention and improve their health through sound, safe living environment with safety education established through more systematic method. Data were collected from 300 preschoolers(150 preschooler are assigned to experimental group and 150 preschoolers are assigned to control group) from 4 to 6 years old using APP paper test which consists of questions and drawings. To experimental group, safety education were done 4 times within the time of 30 minutes per 1 time using education books, video, OHP, slide. The findings of this study are as follows: 1. There were significant difference in behavioral character between experimental group and control group(χ2=11.690, p= 0.003). So, safety education have effect on the behavioral character of preschooler. 2. In the accident proneness on preschooler between experimental group and control group according to general characteristics, it proved significant difference in the case of accident prevention education were done, movement stability(χ2=8.844, p=0.012) and behavioral character(χ2=6.699, p=0.035), in the case of housing pattern is mixed-type, behavioral character(χ2=10.37, p=0.006), in the case of subjects' age is 4 years old, watchfulness(χ2=9.525, p=0.009), in the case of subjects' age is 5 years old, behavioral character(χ2=7.324, p=0.026), in the case of children's order is second, behavioral character(χ2=14.31, p=0.001), in the case of children's sex is boy, living safety(χ2=7.981, p=0.018), movement speed (χ2=6.661, p=0.036), bihavioral character(χ2=8.837, p=0.012), in the case of children's sex is girl, reasoning power(χ2 =9.78, p=0.008), in the case of childrens have no past accidental experience, behavioral character(χ2=9.862, p=0.007), in the case of nuclear family, movement speed(χ2=6.341, p=0.042) and behavioral character(χ2=9.326, p=0.009), in the case of mothers' age is under thirty behavioral character(χ2=16.40, p=0.000), in the case of mothers' school career is under high school graduate, behavioral character(χ2 =8.375, p=0.015), in the case of mothers' school career is beyond college graduate, reasoning power(χ2=9.803, p=0.007) and behavioral character(χ2=6.205, p=0.045), in the case of mothers' job is part time, movement speed(χ2=10.99, p=0.004), in the case of mothers have no job, movement stability(χ2=8.490, p=0.014) and behavioral character(χ2=10.11, p=0.006). The difference of accident proneness between experimental group and control group according to general characteristics, it also showed that there were significant difference in behavioral character compared to other area.. From this findings, we can guess that safety education change and guide preschoolers' behavioral character to desirable direction.

The Influence of General Characteristics, Temperament, and Attachment on the Development of Young Children in Institutional Care (양육시설 영유아의 일반적 특성과 기질 및 애착안정성이 발달에 미치는 영향)

  • Objective: This study examined that general characteristics such as gender, age, period of stay at an institution, size of facility, and temperament and attachment affect young children's development under institutional care. Methods: A total of 94 young children (ages 2-4years), who live in 32 institutions in Seoul were assessed on their development using the Korean-Age & Stage Questionnaire. Pearson productmoment correlation analysis, and multiple regression analyses were used for data analysis. Results: The major findings of this study were as follows: first, a longer period of stay at an institution had more positive effects on the development of communication. Facilities with more than 60 children had lower communication, gross motor, and fine motor development compared to facilities with less than 60 children. Second, social temperament had positive effects on communication, problem solving, and personal-social development. Emotional temperament had no effects on all development areas. Third, attachment had positive effects on all development areas. Lastly, social temperament and attachment had interactive effects on communication, gross motor, problem solving, and personal-social development. Conclusion: These results imply that caretakers in institutions should give developmental mediation services for considering temperament and facilitation of attachment.

Severe recurrent nocturnal hypoglycemia during chemotherapy with 6-mercaptopurine in a child with acute lymphoblastic leukemia

  • Various endocrine dysfunctions occur during chemotherapy, including hypoglycemia. However, reports of hypoglycemia associated with 6-mercaptopurine (6-MP) are rare. Herein, we report an 8-year-old boy with severe symptomatic hypoglycemia likely due to 6-MP during chemotherapy. He had been diagnosed with acute lymphoblastic leukemia 3 years previously and was in the maintenance chemotherapy period. Treatment included oral dexamethasone, methotrexate, and 6-MP, of which only 6-MP was administered daily. Hypoglycemic symptoms appeared mainly at dawn, and his serum glucose dropped to a minimum of 37 mg/dL. Laboratory findings showed nothing specific other than increased serum cortisol, free fatty acids, ketone, alanine aminotransferase, and aspartate aminotransferase. Under the hypothesis of hypoglycemia due to chemotherapy drugs, we changed the time of 6-MP from evening to morning and recommended him to ingest carbohydrate-rich foods before bedtime. Hypoglycemia improved dramatically, and there was no further episode during the remaining maintenance chemotherapy period. To the best of our knowledge, this is the first report of this type of hypoglycemia occurring in an Asian child including Korean.

Current use of safety restraint systems and front seats in Korean children based on the 2008-2015 Korea National Health and Nutrition Examination Survey

  • Purpose: The use of proper safety restraint systems by children is vital for the reduction of traffic accident-related injury and death. This study evaluated the rates of use of safety restraint systems and front seats by Korean children. Methods: Based on data from the National Health and Nutrition Examination Survey from 2008 to 2015, I investigated the frequencies of safety restraint systems and front seat use by children under six and 12 years of age, respectively. Results: The percentage of respondents who said they always use safety restraint systems increased from 17.7% in 2008 to 45.0% in 2015. The rate of children who did not use the front seats at all was 47.3 % in 2008 compared to 33.4% in 2015. Multivariate logistic regression analysis showed a decrease in safety-restraint-system use as age increased (odds ratio, 0.63; 95% confidence interval [CI], 0.51-0.77). The use rate of front-passenger seat belts by the mother is significantly correlated with the safety-restraint-system use rate by children (odds ratio, 2.14; 95% CI, 1.12-4.06). Conclusion: Although the rate of safety-restraint-system use for children is increasing annually, it remains low. Additionally, the use rate of front passenger seats for children is high. To reduce the rates of injury and death of children from traffic accidents, it is necessary to educate on the appropriate use of safety restraint systems according to age and body size and to develop stronger regulations.

Secular change in waist circumference and waist-height ratio and optimal cutoff of waist-height ratio for abdominal obesity among Korean children and adolescents over 10 years

  • Purpose: This study aimed to evaluate the time trends of waist circumference (WC) and waist-height ratio (WHR), and to present WC and WHR distributions with optimal WHR cutoff for abdominal obesity in Korean children and adolescents. Methods: We performed a retrospective cross-sectional analysis of data from 13,257 children and adolescents (6,987 boys and 6,270 girls) aged 6-18 years who were included in the third to sixth Korea National Health and Nutrition Examination Survey (KNHANES, 2005-2015). Linear regression analyses were used to identify secular changes in WC and WHR by age, sex, and KNHANES waves. A receiver operating characteristic curve analysis was used to determine the optimal WHR cutoff values for abdominal obesity and cardiometabolic risk factors. Results: The mean WC and WHR distributions from 2005 to 2015 showed no significant secular changes between the KNHANES 4 waves (P for trend ${\geq}0.05$ in all ages and both sexes). The mean WCs in the present study were lower than those in the 2007 Korean National Growth Charts. The mean WHR at ages <13 years was statistically higher in the boys than in the girls, but did not significantly differ between the sexes among those aged 13 to 18 years. The optimal WHR cutoff for abdominal obesity was 0.48 (area under the curve, 0.985; 95% confidence interval, 0.985-0.985) in the 13- to 18-year-old adolescents. Conclusion: WC and WHR showed no secular changes over 10 years. The optimal WHR cutoff for abdominal obesity of 0.48 is useful for diagnosing and managing obesity and thus preventing obesity-related cardiometabolic complications in 13- to 18-year-old Korean adolescents.

The Effect of Children's Age on Married Women's Career Reinterruption (자녀 연령이 기혼여성의 경력 재단절에 미치는 영향)

  • Purpose - The main purpose of this paper is to examine the effect of children's age on maternal labor supply in Korea using survival analysis. Specifically, we focus on the career re-interruption of women having children under age 12, which has rarely been studied in the existing literature. Research design, data, and methodology - We use micro data from the Korea Labor and Income Panel Study (KLIPS) surveyed from 1998 to 2016. Instead of using a pre-school child dummy or the number of young children as an explanatory variable, 9 children's age dummies are included to capture the effect of nurturing 0 to 9 years old children. This study estimates the hazard of a woman's exiting the labor market after her first experience of the career interruption, rather than the hazard of the first career interruption itself. A Cox proportional hazard model is applied to numerically capture the impact of children's age on behavioral changes in maternal labor supply. The sample used in this analysis is women between 15 and 54 years old. Most of all, we restrict the sample to women who had at least a child between 0 and 12 years old at the time of quitting their jobs. Results - The Cox proportional hazard model estimates show a strong negative effect of a 0-year-old child on maternal labor supply. Mothers with newborns have a high hazard ratio of labor force exit after the re-entry. The hazard of women with infants is three times higher than those with children aged 10 to 18. Additionally, the results show that not only newborns, but also children in the age of school-entry have a negative impact on their mother's labor supply. Conclusions - The findings reveal that children's ages need to be properly expanded and included when analyzing the effect of children and their ages on married women's labor supply, especially on women's career re-interruption. A large negative effect of 7-year-old children on maternal labor supply found here indicates that supporting mothers with school age children as well as pre-school children is necessary to prevent mothers from leaving the labor market.

A Study on Chest X-ray Using Ancillary Device for Child Radiography (방사선촬영 보조기구를 이용한 어린이 흉부 엑스선 검사에 관한 연구)

  • In this study, We developed a Ancillary device for child radiography for X-ray of children under 5 years old and verified its effectiveness. Chest X-rays of children younger than 5 years of age were performed by Supine method at the position of Table detector, Short - Source to Image Receptor Distance(SID). Existing Supine and Short -SID imaging methods cause many problems, such as errors in image reading and excessive radiation exposure dose to patients, but the use of an Ancillary device for child radiography(ADCR) solves these problems. A total of 160 children were divided into the Upright group using ADCR and Supine group without ADCR. The chest X-ray image was visually evaluated by two radiologists with reference to the European Commission's List of Quality Criteria for Diagnostic Radiographic Images in Pediatrics. The total score of the qualitative evaluation was 5.15% higher in the chest upright method using ADCR than in the chest supine method without ADCR, and the chest upright method score was higher than that of the chest supine method in items 1 to 7. whether infants have deep inspiration or not, 4.87% higher for item 1, whether infants rotate or not and the degree of tilting, 0% higher for the item 2, the reproduction of image from just above apices of lungs to T12/L1, 0% for the item 3, reproduction of the vascular pattern in central 2/3 of the lungs, 6.92% higher for the item 4, reproduction of the trachea and the proximal bronchi, 12.9% higher for the item 5, visually sharp reproduction of the diaphragm and costo-phrenic angles, 10% higher for the item 6, reproduction of the spine and paraspinal structures and visualisation of the retrocardiac lung and the mediastinum, and 3.65% higher for the item 7. Items 2 and 3 showed no statistically significant differences(P > 0.05), and items 1, 4, 5, 6, and 7 showed statistically significant differences(P < 0.05). In conclusion, Upright method using ADCR in pediatric chest X-ray is considered as a good alternative to existing Supine method.

A Study of Sexual Assaults on Children and Adolescents: Based on Data from a One-Stop Service Center (소아.청소년 성폭력 피해의 실태 : 원스톱 지원센터 대상자를 중심으로)

  • Objectives: The aim of this study was to identify characteristics of children and adolescents who were victims of sexual assault in Korea. Methods: The subjects were 60 children and adolescents who visited the one-stop center as victims of sexual assault. The medical records of victims of sexual trauma were retrospectively reviewed. We studied the demographic data of the victims, their relationship to their perpetrator, the characteristics of the assault (frequency, duration, place, type), and the process from sexual assault to treatment. We also paid special attention to how the characteristics of the victims or perpetrators affected the characteristics of the assault or follow-up treatment. Results: There were several differences between sexual assaults committed by strangers and those committed by acquaintances. Sexual assaults committed by acquaintances lasted for a longer period of time than those committed by strangers. In addition, it took more time for victims of sexual assaults committed by acquaintances to seek treatment than those who were victims of sexual assaults committed by strangers. The majority (55.0%) of victims were between 10 and 15 years of age. Forty percent of the perpetrators were teenagers, and two of them were under the age of 10. Voluntary discontinuation of treatment was more frequent in adolescents than in children. Conclusion: All teenage victims of sexual assault need some sort of urgent intervention. In addition, approachable methods are needed in order to prevent sexual abuse by strangers or acquaintances.

Disintegration and Reconstruction of the Family/Kinship Structure Among the Rural Families of Korea (가족/친족 구조의 해체와 재구성 II : 농촌지역 실태조사를 중심으로)

  • This study focused on the disintegration and reconstruction of the family/kinship structure among the rural families of Korea. For this study, detailed data about the value related to the family/kinship, the family relationship, The attitude of divorce and remarriage, the social network, and the family/kin rites are gathered A total 593 subjects completed structured questionnaires. Major conclusion of the study are as follows : First, most of the rural respondents are found to support familism and boy preference slightly, and not to agree the reward of child value. So, their attitudes toward traditional values are changing slowly than urban residents. Second, the rural respondents shared the common perceptions that spousal and parent-child relationships has been changed toward the direction that the positions of wives and children are respected and the their influences are increased in the past 10 years. In addition, generational differences in the perceptions of relationship change and appropriate roles of wives and husbands are discovered. However, sex differences previously revealed in Seoul study were not found in the case of rural respondents. Third, generally, there are both remaining and changing aspects of conservative attitude toward divorce and remarriage, the level of change is different according to age and sex. And the difference by age is stronger than by sex. Fourth, the social networks of the respondents is characterized by two distinct trends, namely, strong parent-adult child ties and the close relationships between neighbors. fifthly, in the family/kin rites, traditional aspects coexist with changing aspects under the influence of industrialization and westernization. But the aspects of attitudes toward rites, it is showed the non-traditional tendency. Respondents who support westernization and socialization of family rites are young, highly eamed, and Christian.
