To accomplish this purpose, we collected data mainly from depth interview with managers, child care workers, and children in child care institutions adopting daily two-shift roster, and analysed these data through qualitative case study approach. The results of this study are as follows. First, child care workers take the chance of recreation, their working conditions improved, they were participated in self-development activity, and they could make relationship with persons in social network. But, some participants worried about decrease of responsibility of workers. Second, one hand, possibility of high-quality care for child is increased, on the other hand, possibility of improving attachment relationship between workers and children is decreased. some children is confused by shift. But, most important strength is that partners have complementary parenting roles through discussion about parenting skills. They have developed communication skills by trial and error, and growed with children through sharing. Third, many qualified persons have applied for institution because of improvement of working conditions, thus institutions had the chance of adopting qualified workers. These workers have various abilities and resources, could mobilize resources from community, and could progress various programs and intervene for children. But, institutions had many difficulties in process adopting daily two-shift roster because of unstable financial support and previous structure. It also sought out to see which determinants makes differences between these three groups, if there are in fact any meaningful differences between these three groups. In order to achieve the aforementioned purpose, this study selected 200 child-care institutions located in Seoul and Kyung-ki Province and surveyed a total of 583 members of these child-care institutions including 118 child-care directors, 363 child-care teachers and 102 workers in the cooking staffs. The collected data were analyzed with SPSS 12.0 program. The summary of this survey is as follows: First, regarding the provision of eco-friendly meals and snacks for the children, the practice levels got lower in the order of cooking staffs, child-care directors and child-care teachers. Second, it was shown that the child-care teachers' practice level of providing eco-friendly environment to feed eco-friendly meals and snacks to the children is relevant to the type, number of teachers, position and their educational backgrounds. However, it is shown that the experience of the child-care teacher is not relevant to the practice level.

  • The present study was conducted to examine the degree of variation in length-of-stay (LOS) among health care institutions participating in 17 diagnosis-related groups (DRGs) payment system and to find out hospital characteristics affecting the variation. Electronic medical claims data for treatments of severity classification '0' of 17 DRGs provided for two $years(2003{\sim}2004)$ were collected. For each DRG, the degree of variation in average LOS among health care institutions were analyzed using the random effect model. For DRGs showing significant differences in LOS, multiple regression analyses were performed to find out factors associated with LOS. Significant variations in LOS were observed 9 DRGs including unilateral/bilateral lens procedures, adult/child tonsilectomy, other anal procedures, bilateral adult/child herniorraphy, unilateral child herniorraphy, and hysterectomy, and hysterectomy using laparoscopic procedure. Among the 9 DRGs, five DRGs were selected to investigate the factors explaining for the variation. It was observed that the location of institution was significant predictors for all five DRGs. Within the same DRGs, LOS was significantly shorter among the institutions located in Seoul than those in other areas. As compared to clinics, hospitals and general hospitals/tertiary care institutions showed significantly longer LOS for DRGs of lens procedures, tonsilectomy, and other anal procedures. It is recommended that the institutions located in other than Seoul area benchmark the strategies of the institution in Seoul in efficiently managing LOS. Also, significant variation within the same severity classification such as other anal procedures implies the imminent need for improvement of patient classification system.

Methods: The study was a descriptive study and included data from 40 institutions among the 53 university nursing programs in Korea(75.5% response rate). Data were collected using mailed semi structured questionnaires and content analysis was done. Results: Findings show that most institutions have common learning objectives for the clinical practicum; neonatal care, high risk infant care, hospitalized child care, and advanced nursing practice. The mode for theoretical credits in child health nursing was 5 to 6 and 3 to 4 credits for clinical practice. The practice settings were prepared to provide diverse experiences, including childcare centers, and community centers with various learning activities. Evaluation for learning outcomes included faculty and instructors. It was pointed out that updating evaluation based on student and faculty feedback is important for a comprehensive practicum evaluation. Conclusions: Findings suggest that there is a -need for a generalized curriculum for clinical practicum and for the expanding role of advanced nursing practice-, a need for diverse clinical settings for practice, and effective guidance and learning activities. It is significantly noted that the attitude and teaching methodologies of clinical instructor's are highly important to effective clinical learning outcomes.
