The Relationships of Chewing Frequency, Eating Rates, and Food Consumption with the Body Mass Index and Gender of College Students (남녀 대학생의 체질량지수에 따른 저작횟수와 식사속도 및 식품섭취량 비교)

  • In this study, we examined chewing frequency, eating rates, and food consumption volume according to the body mass index of male and female college students. Twenty-five male college students were divided into a normal weight male group (<23 body mass index, n=14) and an obese male group (${\geq}23$ body mass index, n=11). Twenty-five female college students were divided into a normal weight female group (<23 body mass index, n=21) and an obese female group (${\geq}23$ body mass index, n=4). Subjects were provided with abundant fried rice and allowed to consume it at their own pace. Chewing frequency, total mealtime, rate of eating, consumption volume, calorie intake, and the amount of one bite were measured. There were no significant differences in chewing frequency, total mealtime, rate of eating, consumption volume, calorie intake, and the amount of one bite between the normal and obese male groups. There were also no significant differences in chewing frequency, consumption volume, calorie intake, and the amount of one bite between the normal and obese female groups. However, the obese female group had a significantly shorter mealtime and significantly higher eating rate compared with the normal group. Thus, our study suggests that females who eat a meal for a longer time may reduce their food intake, consequently inducing body weight loss.

Areca Nut Chewing and the Risk of Re-hospitalization and Mortality Among Patients With Acute Coronary Syndrome in Pakistan

  • Objectives: Areca nut is widely consumed in many parts of the world, especially in South and Southeast Asia, where cardiovascular disease (CVD) is also a huge burden. Among the forms of CVD, acute coronary syndrome (ACS) is a major cause of mortality and morbidity. Research has shown areca nut chewing to be associated with diabetes, hypertension, oropharyngeal and esophageal cancers, and CVD, but little is known about mortality and re-hospitalization secondary to ACS among areca nut users and non-users. Methods: A prospective cohort was studied to quantify the effect of areca nut chewing on patients with newly diagnosed ACS by categorizing the study population into exposed and non-exposed groups according to baseline chewing status. Cox proportional hazards models were used to examine the associations of areca nut chewing with the risk of re-hospitalization and 30-day mortality secondary to ACS. Results: Of the 384 ACS patients, 49.5% (n=190) were areca users. During 1-month of follow-up, 20.3% (n=78) deaths and 25.1% (n=96) re-hospitalizations occurred. A higher risk of re-hospitalization was found (adjusted hazard ratio [aHR], 2.05; 95% confidence interval [CI], 1.29 to 3.27; p=0.002) in areca users than in non-users. Moreover, patients with severe disease were at a significantly higher risk of 30-day mortality (aHR, 2.77; 95% CI, 1.67 to 4.59; p<0.001) and re-hospitalization (aHR, 2.72; 95% CI, 1.73 to 4.26; p<0.001). Conclusions: The 30-day re-hospitalization rate among ACS patients was found to be significantly higher in areca users and individuals with severe disease. These findings suggest that screening for a history of areca nut chewing may help to identify patients at a high risk for re-hospitalization due to secondary events.

Tobacco (Kretek) Smoking, Betel Quid Chewing and Risk of Oral Cancer in a Selected Jakarta Population

  • Purpose: This study aimed to determine the association between tobacco consumption (kretek) and betel quid chewing with oral cancer risk. Materials and Methods: A total of 81 cases of oral cancers were matched with 162 controls in this hospital-based study. Information on sociodemographic characteristics and details of risk habits (duration, frequency and type of tobacco consumption and betel quid chewing) were collected. Association between smoking and betel quid chewing with oral cancer were analysed using conditional logistic regression. Results: Slightly more than half of the cases (55.6%) were smokers where 88.9% of them smoked kretek. After adjusting for confounders, smokers have two fold increased risk, while the risk for kretek consumers and those smoking for more than 10 years was increased to almost three-fold. Prevalence of betel quid chewing among cases and controls was low (7.4% and 1.9% respectively). Chewing of at least one quid per day, and quid combination of betel leaf, areca nut, lime and tobacco conferred a 5-6 fold increased risk. Conclusions: Smoking is positively associated with oral cancer risk. A similar direct association was also seen among betel quid chewers.

Complete denture satisfaction by the subjective oral health state among the elderly (노인의 주관적인 구강건강상태에 따른 총의치 만족도)

  • Objectives : This study aims to identify factors affecting satisfaction among the elderly with complete denture to help the elderly with complete denture perform oral hygienic care and to contribute to improvement in oral health. Methods : Interviews were performed with 247 people having complete denture, who were over 65 years and resided in Daejeon and South Chungcheong Province, from March 5 to 31, 2009. Results : 1. As for satisfaction in areas of complete denture by the number of brushing, chewing, pronunciation, and aesthetic functions were all high in over three times of brushing, showing statistically significant differences in satisfaction with chewing (p=0.1825) and aesthetic (p=0.005) functions. 2. As for satisfaction in areas of complete denture by the period of using denture, chewing, pronunciation, and aesthetic functions were all high in less than 7 years, showing statistically significant differences in satisfaction with chewing (p=0.030) and aesthetic (p=0.000) functions. 3. As for satisfaction in areas of complete denture by the subjective oral health state, the better subjective oral health state, the higher satisfaction with chewing, pronunciation, and aesthetic functions, showing statistically significant differences (p=0.000). 4. As for relevance between the subjective oral health state and satisfaction with complete denture, there was 30.1% explanation power in satisfaction with complete denture by the subjective health state. Conclusions : As for satisfaction with complete denture among the elderly, more than three times of brushing and less than 7 years of using denture were related to higher satisfaction; the better subjective oral health state, the higher satisfaction with chewing, pronunciation, and aesthetic functions. Therefore, it is necessary to have the elderly with complete denture receive education about oral hygienic care and perform the care at the same time.

The Effects of Some Halitosis Removal Methods on the Reduction of Intraoral Volatile Methyl Mercaptan Concentrations (수종 구취제거법이 구강내 휘발성 메틸머캅탄 감소에 미치는 영향)

  • In order to evaluate the effectiveness of tooth brushing, mouth gargling and gum chewing in reducing halitosis, 84 individuals ranging in age from 22 59 28 years old were examined. These individuals had no gross oral abnormalities, other than mild gingival inflammation, dental caries, nasopharyngeal disorder, or systemic diseases that were associated with halitosis. They were divided into a tooth brushing group, a mouth garging group, a gum chewing group and a control group that did not use any halitosis removing method. Each of the groups included 21 persons, B.B. Checker (Tokuyama Soda Col, LTDl, Japan) was used to measure the concentrations of intraoral volatile methyl mercaptan of each group. The concentrations of intraoral volatile methyl mercaptan were measured before and after lunch, and after removing halitosis by toothe brushing, mouth gargling and gum chewing. The obtained results were as follows : 1. The average concentration of intraoral volatile methyl mercaptan before lunch was 1.79ppm and after lunch it was 2.02ppm, an increase of 12.9%. 2. In the tooth brushing group the average concentration of intraoral volatile methyl mercaptan was 0.61ppm, in the mouth gargling group it was 1.15ppm, in the gum chewing group it was 1.64ppm and in the control group it was 1.92ppm. It decreased 69.5% in the tooth brushing group, 43.8% in the mouth gargling group, 18.4% in the gum chewing group and 5.4% in the control grop (p<0.05). 3. There were significant differences between the tooth brushing and control group, tooth brushing and gum chewing group and between mouth gargling and control group in concentrations of intraoral volatile methyl mercaptan after using the halitosis removing methods (p<0.05). According to the above results, tooth brushig and mouth gargling are effective ways to reduce halitosis.

  • This study was performed to investigate the muscular activity of the complete denture wearers compare with subjects with natural teeth. For the study, 10 subjects with natural dentition and 18 upper and lower complete denture wearers selected and the Bio-electric Processor EM2(Myo-tronics Reaserch, Inc., U.S.A.) with the surface electrodes was used to record electromyographic activity from the right and left middle of masseter and anterior temporal muscles of each subject during mandibular postural rest position, tapping of teeth from postural rest position, maximal clench, and right and left gum and raw carrow chewing. This results of this study were as follows : 1. In mandibular postural rest position, the denture wearers produces high muscular activity in contrast to natural objects(P<0.05) but, there was no difference between the state of denture removal and insertion, and the muscle activity of the anterior temporal muscle was high than the middle of masseter muscle in natural objects and denture wearers. 2. In tapping of teeth, there was no difference in muscle activity between natural objects and the state of denture removal of denture wearers. 3. In maximal clench, there was markedly lower denture wearers than natural objects in muscle activity, and the ratio of mean voltages was about 36 percentages. 4. In gum and raw carrow chewing, the activity was lower than natural object, the ratio was about 59 percentages. 5. In chewing, the mean voltages of the middle of masster muscle on the chewing side was highest, followed by the anterior temporal on the chewing side, the anterior temporal and masster muscles on the non-chewing side.

Relationship between periodontal diseases and quality of life (치주질환자의 구강보건특성과 삶의 질의 관계)

  • Objectives : The purpose of this study was to evaluate the chewing ability, oral-health related quality of life and factors in periodontal disease patients. Methods : Subjects were 111 patients having peridontal disease in 10 dental clinics in Busan and Gyeongsangnamdo. Questionnaire survey was carried out from July to August 2010. Questionnaire consisted of demographic characteristics, oral health factors, chewing ability and quality of life. Data were analyzed by descriptive analysis, t-test, ANOVA, correlation analysis, and multiple regression analysis using SAS(Ver 9.2) set at p<0.05. Results : Chewing ability was $44.85{\pm}8.8$, and their quality of life was $41.7{\pm}8.8$. Low level of quality of life was closely related to those factors as education, missing of more than 5 posterior teeth, certain subjective periodontal symptoms including swollen gums, sore gums, drifting gums, bad breath and toothache. The effective factors to the quality of life was chewing ability, bad breath and toothache. Conclusions : Chewing ability and quality of life were closely related each other. To enhance the quality of life, chewing ability should be improved. Proper treatment and effective method of management should also be properly considered to prevent the subjective periodontal symptoms and to minimize tooth loss.

Effectiveness of low-level laser therapy and chewing gum in reducing orthodontic pain: A randomized controlled trial

  • Objective: The purpose of this study was to evaluate the effects of chewing gum and low-level laser therapy in alleviating orthodontic pain induced by the initial archwire. Methods: Patients with 3-6 mm maxillary crowding who planned to receive non-extraction orthodontic treatment were recruited for the study. Sixty-three participants (33 females and 30 males) were randomly allocated into three groups: laser, chewing gum, and control. In the laser group, a gallium aluminum arsenide (GaAlAs) diode laser with a wavelength of 820 nm was used to apply a single dose immediately after orthodontic treatment began. In the chewing gum group, sugar-free gum was chewed three times for 20 minutes-immediately after starting treatment, and at the twenty-fourth and forty-eighth hours of treatment. Pain perception was measured using a visual analog scale at the second, sixth, and twenty-fourth hours, and on the second, third, and seventh days. Results: There were no statistically significant differences between the groups at any measured time point (p > 0.05). The highest pain scores were detected at the twenty-fourth hour of treatment in all groups. Conclusions: Within the limitations of the study, we could not detect whether low-level laser therapy and chewing gum had any clinically significant effect on orthodontic pain. Different results may be obtained with a higher number of participants or using lasers with different wavelengths and specifications. Although the study had a sufficient number of participants according to statistical analysis, higher number of participants could have provided more definitive outcomes.

  • The purpose of this study is to identify the effect of depression on chewing problems and the importance of oral aids through a point-in-time analysis. The data are the 4th(2008, 2nd year) and the 7th(2018, 3rd year) of the National Health and Nutrition Survey. The analysis method was frequency analysis and regression analysis using the SPSS/WIN 24.0 program. Studies have shown that depression has affected chewing problems(p<.001). Chronic diseases and the use of oral aids affected the chewing problem. The prevalence and treatment of depression affected the chewing problems(p<.001). In addition, private health insurance coverage increased in 2018 compared to 2008. In the future, the comparative studies considering various subjects and chronic diseases, the educational methods for the use of oral aids are needed.

Impact of Chronic Hepatitis B and Hepatitis C on Adverse Hepatic Fibrosis in Hepatocellular Carcinoma Related to Betel Quid Chewing

  • The pathogenesis of hepatocellular carcinoma (HCC) related to habitual betel quid (BQ) chewing is unclear. Risk of HCCis increased with adverse hepatic fibrosis. This study aimed to assess the impact of chronic viral hepatitis on adverse hepatic fibrosis in HCC related to BQ chewing. This hospital-based case-control study enrolled 200 pairs of age- and gender-matched patients with HCC and unrelated healthy controls. Serologic hepatitis B surface antigen (HBsAg), antibodies to hepatitis C virus (anti-HCV), ${\alpha}$-fetoprotein (AFP), and surrogate markers for significant hepatic fibrosis were measured. Information on substance-use habits was obtained with a questionnaire. By analysis of surrogate markers for hepatic fibrosis, the prevalence of significant hepatic fibrosis in patients chewing BQ was between 45.8% and 91.7%, whereas that for patients without BQ chewing was between 18.4% and 57.9%. The difference was significant (P <0.05 for each surrogate marker). Multivariate analysis indicated that cirrhosis with Child-Pugh C (odds ratio (OR) = 3.28; 95% confidence interval (CI), 1.29-8.37), thrombocytopenia (OR = 3.92, 95% CI, 1.77-8.68), AFP >400 mg/L (OR = 2.21, 95% CI, 1.05-4.66) and male gender (OR = 4.06, 95% CI, 1.29-12.77) were independent factors associated with habitual BQ chewing. In conclusion, adverse hepatic fibrosis and severe liver damage play important roles in the pathogenesis of BQ-related HCC, which could be aggravated by chronic hepatitis B and hepatitis C. BQ-cessation programs and prevention of chronic HBV/HCV infection are needed to prevent HCC related to BQ chewing.
