Evaluation of Deep Learning Model for Scoliosis Pre-Screening Using Preprocessed Chest X-ray Images

  • Scoliosis is a three-dimensional deformation of the spine that is a deformity induced by physical or disease-related causes as the spine is rotated abnormally. Early detection has a significant influence on the possibility of nonsurgical treatment. To train a deep learning model with preprocessed images and to evaluate the results with and without data augmentation to enable the diagnosis of scoliosis based only on a chest X-ray image. The preprocessed images in which only the spine, rib contours, and some hard tissues were left from the original chest image, were used for learning along with the original images, and three CNN(Convolutional Neural Networks) models (VGG16, ResNet152, and EfficientNet) were selected to proceed with training. The results obtained by training with the preprocessed images showed a superior accuracy to those obtained by training with the original image. When the scoliosis image was added through data augmentation, the accuracy was further improved, ultimately achieving a classification accuracy of 93.56% with the ResNet152 model using test data. Through supplementation with future research, the method proposed herein is expected to allow the early diagnosis of scoliosis as well as cost reduction by reducing the burden of additional radiographic imaging for disease detection.

Outpatient Drainage Therapy with a Thoracic Vent for Traumatic Pneumothorax due to Bull Attack

  • Outpatient drainage therapy is generally indicated for spontaneous pneumothoraces. A 63-year-old man, who had been attacked by a bull sustaining injuries on the right side of his chest, was referred to the emergency room with dyspnea. His chest X-ray showed a small pneumothorax. The next day, a chest X-ray demonstrated that his pneumothorax had worsened, although no hemothorax was identified. Outpatient drainage therapy with a thoracic vent was initiated. The air leak stopped on the third day and the thoracic vent was removed on the sixth day. Thoracic vents can be a useful modality for treating traumatic pneumothorax without hemothorax.

  • Congenital Cystic Adenomatoid Malformation [C.C.A.M. is rare, cause acute respiratory distress in the newborn infants. The histologic features are cystic areas and marked proliferation of terminal respiratory structures. On case 1, the patient was 8-month-old male, and suffered from acute respiratory distress and cyanosis. The pulmonary cystic lesion was detected in right lung at birth and has been evaluated since birth. The study for diagnosis were chest x-ray and chest CT. A right upper lobectomy was urgently performed and he was discharged with a satisfactory postoperative course. On case 2, the patient was 20-year-old female, and suffered from cough and sputum for 2 months. The study for diagnosis were chest x-ray, chest CT, and pulmonary angiography. The cystic lesion was detected in left lung and difficult to distinguish from pulmonary sequestration. A left lower lobectomy was performed and she was discharged with a satisfactory postoperative course. We report two cases of C.C.A.M. with differential clinical course.

Intralobar Pulmonary Sequestration; A Report of 3 Cases (내엽형 폐격리증;3례 보고)

  • Pulmonary sequestration is a congenital malformation in which a mass of pulmonary tissue is detached from the normal lung and receives its blood supply from a systemic arterys from the thoracic aorta or the abdominal aorta, or occasionally from an intercostal artery. We have experienced the three cases of the intralobar pulmonary sequestration. In the first case a 5 year old male was admitted of productive cough and abdominal distension, and chest film showed pneumonic infiltration in RLL. Second case was 26 year old male patient complaining Rt. chest discomfort and hemoptysis and chest X-ray revealed infiltration in Rt. LLF. Third case was 26 year old male patient whose complaint was hemoptysis. Chest x-ray showed hazy density in Lt. lower lung field. In the all cases, the aortograms were performed and the confirmed diagnosis was intralobar pulmonary seqeustration.

Mediastinal lipoma and liposarcoma (종격동 지방종 및 지방육종: 3례 보고)

  • The lipomatous of the mediastinum are fairly uncommon tumors. Since 1971, two patients with proven mediastinal lipoma and one primary liposarcoma of the mediastinum have been treated at the Seoul National University Hospital. This report reviews our experience with the review of literature. Case 1.: A 3 year old body revealed a huge round homogenous mass density in the posterior mediastinum in routine chest X-ray. The tumor mass, lipoma, was successfully removed and postoperative course was uneventful. Cases 2.: An asymptomatic 24 year old male was operated on with the preoperative diagnosis of mediastinal lipoma. His preoperative chest X-ray and CT films showed a huge anterior to middle mediastinal tumor, right, with fat density. There is no postoperative problem after successful removal of the tumor mass. Case 3. : A 24 year old female was hospitalized with the complaints of cough and chest pain. A mediastinal mass was removed, which proved to be a liposarcoma on pathologic examination. About one year later, she was found to have recurrent liposarcoma in the right chest area at the OPD follow-up. She was lost to follow-up since then.

Endobronchial Hamartoma - 1 case - (기관지내 발생한 폐과오종 - 1례 보고 -)

  • Pulmonary hamartoma has been considered as rare disease, which consists of lung tumor less than 1 %. Originally described by Albrecht in 1904, hamartoma is tumor like malformation-abnormal mixing of the normal components of organ-and is applied also to tumor found in many organs other than the lung. Lately, the major conclusions are that pulmonary hamartoma is neoplastic rather than developmental error in origin. Because pulmonary hamartoma frequently mimics lung cancer, especially in cancer-risk age groups, its clinical significance is great. Recently, we experienced 1 cases of endobronchial hamartoma which located at the right main stem bronchus. The patient was a 54 year old male who was admitted due to symptoms of fever 4 chilliness and dyspnea. Radiologic studies such as chest x-ray, chest tomogram and chest C-T scan revealed that the nearly total haziness of the right lung was caused by endobronchial tumor. The mass was considered as a benign by bronchoscopic exam, so we removed it surgically by tracheobronchotomy without pulmonary resection. Postoperative chest x-ray revealed satisfactory reexpansion of previous collapsed right lung. The patient discharged uneventfully.

Image Quality Enhancement for Chest X-ray images (흉부 엑스레이 영상을 위한 화질 개선 알고리즘)

  • The initial X-ray images obtained from a digital X-ray machine have a wide data range and uneven brightness level than normal images. In particular, in Chest X-ray images, it is necessary to improve naturally all of the parts such as ribs, spine, tissue, etc. These X-ray images can not be improved enough from conventional image quality enhancement algorithms because their characteristics are different from ordinary images'. This paper proposes to eliminate unnecessary background from an input image and expand the histogram range of the image. Then, we adjust the weight per frequency band of the image for improvement of contrast and sharpness. Finally, jointly taking the advantages of global contrast enhancement and local contrast enhancement methods we obtain an improved X-ray image suitable for effective diagnosis in comparison with the existing methods. Experimental results show quantitatively that the proposed algorithm provides better X-ray images in terms of the discrete entropy and saturation than the previous works.

Analysis of the Influence of Examination Gowns on the Image and the Suitable Fabrics for Chest AP Examinations on DR X-ray Systems (디지털 X-선 시스템에서 흉부 전·후 방향 검사 시 검사복이 영상에 미치는 영향과 적정 검사복 원단의 분석)

  • The purpose of this study was to analyze fabrics suitable for use as examination gowns to determine whether examination gowns affect imaging during anterior to posterior chest examinations(Chest AP) on a digital X-ray system. Examination gowns in use at five medical centers in Seoul were collected and included modal, tencel, cotton, and rayon fabrics. The selection of fabrics was based on studies that reported fabrics with good tactile, absorbent, stretchable, and wrinkle resistance. Phantoms of five hospital gowns and four fabrics, arranged in overlapping layers from one to eight, were created and examined on a digital X-ray system in both Chest AP examination. The images examined were subjected to a first-step profile analysis, a second-step signal intensity averaging analysis, and a third-step microscopic analysis. The results showed that all nine materials had an increasing impact on the image as the number of layers of fabric increased, with the modal fabric having the least impact on the image in the first, second, and third analyses. In conclusion, as the resolution of digital x-ray systems increases, the impact of examination clothing on the image will increase, and research to find suitable materials for examination clothing will continue to be necessary.

A Method for Region-Specific Anomaly Detection on Patch-wise Segmented PA Chest Radiograph (PA 흉부 X-선 영상 패치 분할에 의한 지역 특수성 이상 탐지 방법)

  • Recently, attention to the pandemic situation represented by COVID-19 emerged problems caused by unexpected shortage of medical personnel. In this paper, we present a method for diagnosing the presence or absence of lesional sign on PA chest X-ray images as computer vision solution to support diagnosis tasks. Method for visual anomaly detection based on feature modeling can be also applied to X-ray images. With extracting feature vectors from PA chest X-ray images and divide to patch unit, region-specific abnormality can be detected. As preliminary experiment, we created simulation data set containing multiple objects and present results of the comparative experiments in this paper. We present method to improve both efficiency and performance of the process through hard masking of patch features to aligned images. By summing up regional specificity and global anomaly detection results, it shows improved performance by 0.069 AUROC compared to previous studies. By aggregating region-specific and global anomaly detection results, it shows improved performance by 0.069 AUROC compared to our last study.

  • This paper describes a feature extraction in digitized chest X-ray image and CT head Image. There are Extraction, Thresholding, Region G rowing, Split-Merge and Relaxation in feature extraction technique. In this study, Region Growing System was realized and Fuzzy Set Theory was applied in order to extract the vague region which the conventional method has difficulties in extracting. The performance of proposed algorithm was proved by being applied to chest X-ray image and CT head image.
