A Surgical Case of Lymphocytic Hypophysitis with Progressive Visual Worsening

  • Lymphocytic hypophysitis is a clinically rare disease, and it has been known to be an autoimmune disease which mainly affects pregnant women at the end of gestation or right after delivery. The authors experienced a case of lymphocytic hypophysitis in a 29-year-old pregnant woman with rapid progressing visual disturbance. Sella MRI showed a mass-like lesion of hypophysis and hypertrophy of pituitary stalk with evidences of hypopituitarism. Cesarean section was done and then TSA was performed. The pathologic diagnosis was lymphocytic hypophysitis. After TSA, visual acuity was improved and visual field defect was recovered. She was given thyroid hormone replacement therapy because of transient partial hypopituitarism for 6 months after surgery. One must consider the probability of lymphocytic hypophysitis, if there are alteration of visual acuity and visual field defect which aggravate rapidly during pregnancy due to mass effect, decreased serum hormonal levels shown in hypopituitarism and sella MRI findings of hypertrophy of pituitary stalk and enlargement of pituitary gland.

A Study on the Fetal Amino Acids Nutrition at Term of Human Pregnancy (임신말 태아의 아미노산영양에 대한 연구)

  • This study was performed to investigate the placental transfer and the human fetal utilization of amino acids at term of pregnancy. The plasma levels of 23 free amino acids in both the umbilical circulation ( umbilical vein and artery ) and the uterine circulation ( uterine vein and iliac artery ) of 34 pregnant women were measured at delivery by the cesarean section. In the umbilical circulation, 9 amino acids ( alanine, lysine, valine, leucine, arginine, isoleucine, ornithine, cystine, ${\alpha}-aminobutyrate$ ) were significantly higher and 2 amino acids ( glutamate, aspartate ) were significantly lower in the umbilical vein than in the umbilical artery. In the uterine circulation, alanine, tyrosine and methionine were significantly lower in the uterine vein than in the iliac artery. Glutamate was significantly lower in the uterine vein than in the iliac artery. According to these results, the origin of fetal plasma amino acids was discussed in terms of the metabolic conversions which would occur in the Placenta and the fetal utilization of amino acids was estimated.

Acute Type II Aortic Dissection with Severe Aortic Regurgitation and Chronic Descending Aortic Dissection in Pregnant Patient with Marfan Syndrome

  • Aortic dilatation and dissection are severe complications during pregnancy that can be fatal to both the mother and the fetus. The risks of these complications are especially high in pregnant patients with Marfan syndrome; however, incidents of descending aortic dissection are very rare. This case report involves a successful Bentall procedure for and recovery from a rare aortic dissection in a pregnant Marfan patient who developed acute type II aortic dissection with severe aortic regurgitation and chronic descending aortic dissection immediately after Cesarean section. Regular follow-up will be needed to monitor the descending aortic dissection.

  • Chylothorax in the neonatal period is a rare cause of respiratory distress. Surgical ligation of the thoracic duct is rarely necessary in congenital chylothorax. A 3 day-old newborn delivered by the cesarean section showed signs of respiratory distress suddenly and diagnosed as chylothorax on the right hemithorax. Conservative management such as multiple thoracenteses and tube thoracostomy drainage with nutritional support failed to close the leakage. At age of 60 days, we performed a supradiaphragmatic mass ligature of the thoracic duct visualized after injection of methylene blue into the thigh subcutaneously. Postoperatively, chylous effusion occurred in the left hemithorax and successfully treated with chest tube drainage for several days.

  • 5 years Poodle was referred to the Veterinary Teaching Hospital, Chonnam National University in January 1997, with progressive swelling on the rear xiphoid process after cesarean section last year in local-vet clinic. Physical, radiographic, ultrasonographic and intraoperative findings that were obtained from this case were as follow; Physical findings were known as no pain, no fever in the hernia] sal at palpation. Radiographic findings revealed the decreased density of swelling contents more than peritoneum and did not identify the abdominal wall defect line. Ultrasonographic findings showed abdominal wall defect line of 3mm and intra-abdominal originated fat contents with mixed echo(hypoechoic). During the operation, falciform ligament into the hernial sac was observed and not excised, manipulated back into the cavity. The dog was given an herniorraphy with no recurrence and infection.

Determinants of Length of Hospital Stay by Insured and Non-insured Patients (의료보험환자와 일반환자의 재원기간에 관련되는 요인분석)

  • In order to determine the factors affecting the length of stay by pay status, a total of 961 in-patients medical records with appendectomy. cholecystectomy and Cesarean section discharged from the January 1979 to December 1981 from the University hospital were reviewed. Average length of stay showed no statistically significant difference by year between the insured and the non-insured patients, however multiple diagnoses and surgical complication were significantly different from single diagnosis and non-complicated cases. Surgical complication explained the length of stay mostly, and physician in discharge, multiple diagnoses, and accommodation in order for insured patients. Surgical complication, admission route, physician in charge and age in order explained the length of stay for non-insured patients.

Plasma Progesterone Concentrations during Gestation in Companion Bitches (반려견에서 임신기 동안 혈중 Progesterone 농도)

  • The aim of this study was to compare the plasma progesterone concentrations during gestation in 4 breeds of companion bitches. When Day 0 was timed the first male acceptance, plasma progesterone were 1.8 ± 0.4 ng/ml (Mean ± SD) at Day 0 and increased constantly with 58.5 ± 8.9 ng/ml a peak at Day 20. Thereafter plasma progesterone concentrations was decreased below 1.0 ng/ml with 0.7 ± 0.2 g/ml at Day 65. There were no statistically significant differences of plasma progesterone concentrations in the each day of gestation and in litter size among the companion bitches (p < 0.01). These results indicated that the plasma progesterone concentrations was useful method for estimating optimal breeding time, ovulation time, the time of onset of parturition, properly planned elective cesarean section and abortion time in companion bitches.

  • Herein, we report an exceptionally rare case of a 25-year-old woman with cloacal exstrophy/omphalocele-exstrophy-imperforate anus-spinal defects (OEIS) syndrome achieving a viable pregnancy despite many gastrointestinal and genitourinary malformations and multiple respective corrective operations. The patient was born with two vaginas, two uteruses, four ovaries, an imperforate anus, a large omphalocele including bowel and bladder exstrophy, and diaphysis of the pubic rami. This patient is the only documented OEIS patient not to have tethered spinal cord as an anomaly, perhaps contributing to her successful pregnancy. After experiencing preeclampsia with severe features at 35 weeks, the baby was born via cesarean section.

Twin anemia polycythemia sequence in a dichorionic diamniotic pregnancy: a case report

  • Complications related to the vascular anastomosis of the placental vessels in monochorionic twins are fatal. The clinical syndromes of feto-fetal transfusion include twin anemia polycythemia sequence (TAPS), twin-twin transfusion syndrome, and twin reversed arterial perfusion sequence. We present an extremely rare case of TAPS in a dichorionic diamniotic pregnancy. A 36-year-old woman, gravida 0, para 0, was referred to our hospital with suspected preterm premature membrane rupture. Although her pelvic examination did not reveal specific findings, the non-stress test result showed minimal variability in the first fetus and late deceleration in the second one. An emergency cesarean section was performed. The placenta was fused, and one portion of the placenta was pale, while the other portion was dark red. The hemoglobin level of the first fetus was 7.8 g/dL and that of the second one was 22.2 g/dL.
