Acute Spontaneous Spinal Subdural Hematoma with Vague Symptoms

  • Spinal subdural hematoma is a rarely reported disease and spontaneous spinal subdural hematomas (SSDH) without underlying pathological changes are even rarer. The patients usually show typical symtoms such as back pain, quadriplegia, paraplegia or sensory change. But rarely, patients may show atypical symptoms such as hemiparesis and misdiagnosed to cerebrovascular accident. We recently experienced a case of SSDH, where the patient initially showed vague symptoms, such as the sudden onset of headache which we initially misdiagnosed as subarachnoid hemorrhage. In this case, the headache of patient improved but the neck pain persisted until hospital day 5. Therefre, we conducted the MRI of cervical spine and finally confirmed SSDH. The patient was managed conservatively and improved without recurrence. In this case report, we discuss the clinical features of SSDH with emphasis on the importance of an early diagnosis.

  • A 34-year-old man presented with a severe headache, fever, and cervical lymphadenopathy followed by generalized tonic-clonic seizure. Evaluations showed splenomegaly, elevated liver enzymes, and 380 white blood $cells/mm^3$ in the cerebrospinal fluid. Two weeks after admission, he developed sudden vertigo. Examination revealed spontaneous horizontal-torsional nystagmus to the right and bithermal caloric tests documented left canal paresis. A cervical lymph node biopsy disclosed subacute necrotizing lymphadenitis. We report a case of aseptic meningitis and unilateral vestibulopathy associated with histiocytic necrotizing lymphadenitis (Kikuchi's disease).

Surgical Treatment of Thoracic Outlet Syndrome (흉곽 출구 증후군 수술치험 1례)

  • Thoracic outlet syndrome refers to compression of the subclavian vessels and brachial plexus at the superior aperture of the thorax. A 35-year old female was evaluated for right hand numbness. This patient had a history of headache, right shoulder pain, and right hand numbness during 10 months.Preoperative angiography, EMG, and NCV was performed. First rib and cervical rib resection was done with transaxillary approach. After operation, right hand numbness and right shoulder pain were disappeared. Postoperative course was uneventful.

  • Kikuchi disease(subacute necrotizing lymphadenitis), first reported by Kikuchi and Fujimoto in 1972, is a benign self-limiting illness characterized by fever, neutropenia and cervical lymphadenopathy and develops predominantly in young women, especially in Asia. The cause of Kikuchi disease is unknown, but postinfectious(virus or bacteria) hyperimmune reaction has been suggested. Few pediatric cases have been reported. We experienced a case of Kikuchi disease associated with Yersinia pseudotuberculosis infection in 12-year-old girl. After she was admitted with cervical lymphadenopathy and headache, followed by sustained fever with leukopenia. All symptoms and signs did not seem to be responded to antimicobial treatment. Open biopsy of cervical lymph node was performed and showed findings consistent with subacute necrotizing lymphadenitis. Serologic studies were all negative except for Y. pseudotuberculosis. Fever subsided and lymphadenopathy improved after administration of oral steroid for 1 week.

Two Cases of Atypical Facial Pain Using Treatment of Acupuncture at Cervical and Facial region : A Case Report (경추부 중심의 침치료로 호전된 비정형 안면통의 치험 2례)

  • Atypical facial pain is persistent facial pain in the absence of clinical neurological deficit. We experienced two patient who are diagnosed atypical facial pain with Classification of International Headache Society. They were treated by acupuncture at their cervical and facial region. After treatment, their Pain intensity numerical rating scale(PI-NRS) is decreased and EuroQol five dimensions questionnaire(EQ-5D) score is increased. Treatment of acupuncture at cervical and facial region could be effective for easing the clinical symptoms of atypical facial pain. Further studies are needed to find effective treatment for atypical facial pain.

Effects of Therapeutic Exercise on Posture, Pain and Asymmetric Muscle Activity in a Patient with Forward Head Posture: case report (치료적운동이 전방두부자세 환자의 자세, 통증 및 비대칭적 근육활성에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this case report was to determine the effect of therapeutic exercise on posture, pain, and muscle activity in two patients with forward head posture (FHP). METHODS: A-31-year-old male (patient A) and a 19-year-old women (patient B) presented with FHP, neck pain, and headache. The therapeutic exercise program consisted of cervical mobilization, deep cervical flexors strengthening, and cervical extensors stretching, for 40 min/d, 2 d/week, for 8 weeks. Neck pain (VAS), neck disability (NDI), cervical range of motion (CROM), lateral view of cervical spine X-ray (indicating the FHP), and asymmetrical neck and shoulder muscular activity ratio were measured before, after 4 weeks, and after 8 weeks of corrective exercise. RESULTS: VAS and NDI decreased in patients A and B after exercise compared to before the program. CROM increased in patients A and B at flexion, extension, side bending, and rotation after exercise compared to before the program. FHP decreased in patients A and B at distance after exercise compared to before the program. In addition, asymmetrical neck and shoulder muscles activity ratio improved in patients A and B after exercise compared to before the program. CONCLUSION: We demonstrated in a case report that therapeutic exercise increases ROM, decreases pain and disability of neck, FHP, and asymmetry muscle activity ratio in patients with FHP. These finding have clinical implications for therapeutic exercise in patients with FHP.

A Comparative Review on Carthmi-Flos Pharmacopuncture Treatment - Focused on Clinical Study (홍화약침 시술에 관한 임상논문 비교연구)

  • Objectives The purpose of this review is to analyze results of case studies and controlled studies about Carthmi-Flos pharmacopuncture. Based on the review, authors desire to suggest the study model including precise information and evident the effect of Carthmi-Flos pharmacopuncture objectively in treating clinical disorders. Methods We search 44 studies about Carthmi-Flos pharmacopuncture from 6 Korean web databases, using words 'Carthmi-Flos pharmacopuncture' in Korean alphabet. This study had been conducted throughout 1 month (July, 2017). We selected case studies and controlled trials in investigated 44 thesis, excluding experimental research and thesis not using the Carthmi-Flos pharmacopuncture for major treatment. Results We analyze 13 case reports and 7 controlled trials. As a result, Carthmi-Flos Pharmacopuncture was used mostly in musculoskeletal, neurological diseases. But this can be applied to internal diseases. Disorders reported effectively were carpal tunnel syndrome, degenerative knee joint arthritis, posterior neck pain, low back pain, radial nerve palsy, shoulder pain, lumbar compression fracture, alopecia areata, chronic daily headache, duverney fracture, oligomenorrhea, cervical disc herniation, rheumatoid arthritis and cervical headache. Conclusions As we analyzed, Carthmi-Flos pharmacopuncture is specifically effective in musculoskeletal and neurologic diseases. But there are various problems in study design. To design accurately, the study design should include much more specific information. And the result can be more precise by excluding other methods.

Simultaneous Heat-Massage Therapy for Migraine Without Aura : A Case Report (무전조성 편두통 환자에서 온열과 마사지 동시 치료의 효과: 증례보고)

  • A migraine was a headache disorder characterized by recurrent moderate to severe headaches. The diagnosis was based on clinical signs and symptoms. Medication, physical therapy, nerve block, and nerve stimulation could be applied for treatment. This report described a case of severe migraine without aura that lasted several weeks periodically in a 59-year-old woman. Periodic headache had lasted for more than 14 years, and although she took medicines and nerve blocks, severe pain (VAS 7) was persisted. We recommended her to use the thermo-spinal massage device (CGM MB-1401, CERAGEM Inc., Cheonan, South Korea) continuously three times a week applying in semi-automatic mode around the neck for 40 minutes. There was no change in the pain scale in the automatic mode for the first 4 weeks. Subsequently, the semi-automatic mode of the cervical area was treated for 2 weeks to relieve the pain scale, and it was confirmed that the relieved state maintained for 2 months. This case highlighted the importance in considering thermo-spinal massage devices for managing migraine without aura.

Compairison of Effect of Manual Myofascial Release and Self Myofascial Release Technique Using a Foam Roller on Pain Thresholds and Body Schema in Subjects with Chronic Tension-type Headache (도수근막이완기법과 폼롤러를 이용한 자가근막이완기법이 만성 긴장성 두통 환자의 통증문턱값 및 신체 도식에 미치는 효과 비교)

  • Purpose: Headache is a very common disease experienced at least once in daily life, and tension-type headaches have a high and increasing prevalence. Chronic headaches can cause functional damage and huge socioeconomic impacts. This study aimed to compare the effects of myofascial release technique with manual therapy and self-myofascial release technique using a foam roller on the pain threshold and body schema in patients with chronic tension-type headaches. Methods: The study was conducted on 20 patients living in Busan with chronic tension headaches. Myofascial release technique with manual therapy was performed on the suboccipital, sternocleidomastoid, scalene, and upper trapezius muscles. The self -myofascial release technique using a foam roller was applied to the cervical and thoracic muscles. A laterality test was performed using a recognized neck application developed to evaluate body schema ability. A pressure-pain threshold test was performed using an electronic pressure algometer to compare the results before and after the myofascial release technique. Results: After applying myofascial release with manual therapy and a foam roller, the pressure-pain threshold values showed significant changes in both groups (p<.05). As a result of the laterality test, myofascial release with manual therapy and a foam roller were applied to the painful area. The values showed significant changes in both groups (p<.05), but only the group using the foam roller showed a significant difference (p<.05) in painless areas. Conclusion: The myofascial release technique with manual therapy can be the primary treatment technique for pain control in painful areas. The self-muscle release technique using a foam roller can be an effective method when there is no pain or when maintenance is needed after pain control.

Biomechanical Properties of the Cervical Muscles Depending on Using of a Smartphone (스마트 폰 사용에 따른 경추부 근육의 생체역학성)

  • The study was aimed at investigating the relationship between biomechanical properties of cervical muscles and smartphone addiction score in fifty healthy males using smartphone more than 60 minutes each day. The usage of smartphone was evaluated by smartphone addiction survey developed from Korean International Society Agency. Biomechanical properties of three major cervical muscles; splenius capitis, sternocleidomastoid and upper trapezius, were measured by Myoton, and conducted five parameters; frequency, decrement, stiffness, creep, and relaxation time. As results, all parameters had less than 2% of coefficient of variation(CV) between measurement intervals. Also, intra-class correlation coefficient(ICC) indicated a high reliability(ICC>.9, p<.01). Moreover, the smartphone addiction score was significantly different in frequency(r=.353, p<.05) and stiffness(r=.346, p<.05) on upper trapezius; in addition, in decrement(r=-.284, p<.05) and creep(r=.288, p<.05) on sternocleidomastoid. Especially, splenius capitis was closely related with the overuse of smartphone(frequency, r=-.368, p<.01; decrement, r=-.405, p<.01; stiffness, r=-.424, p<.01). In conclusion, this study implied that the overuse of smartphone is significantly related with the damage of cervical muscles, cervical pain, and headache; furthermore, Myoton can be used as an effective device to assess mechanical properties of cervical muscles.
