Brown Sequard Syndrome Resulting from Cervical Disc Herniation Treated by Anterior Foraminotomy

  • The authors describe two cases of Brown-Sequard syndrome associated with cervical disc herniation. In both cases, magnetic resonance images of the cervical spine showed a large paramedian disc herniation at C5-C6 with ipsilateral severe spinal cord compression. Microsurgical removal of the herniated disc via anterior foraminotomy was performed and complete decompression of the spinal cord was achieved. Postoperatively, the neurological symptoms recovered rapidly and both patients experienced a complete remission of their symptoms. Although Brown-Sequard syndrome is rarely associated with degenerative cervical spine disease, cervical disc herniation should be kept in mind and prompt evaluations are mandatory. To the best of our knowledge, these are the first reported case of Brown-Sequard syndrome produced by cervical disc herniation which was treated by anterior foraminotomy.

  • Objectives : The aim of this study is to investigate the state of cervical disc herniation according to the degree of cervical lordosis and age. Methods : 67 records of inpatients who were diagnosed with herniated intervertebral disc(HIVD) of the cervical spine were analyzed. Cobb's method, Jochumsen method and the Ishihara index were used to measure the degree of cervical lordosis. The state of the cervical disc was identified using magnetic resonance imaging(MRI) of the cervical spine. Then correlations among cervical lordosis, age and cervical disc herniation were analyzed. Results : Disc bulging was associated with hypolordosis and disc protrusion was associated with hyperlordosis and age. Disc extrusion was not associated with either cervical lordosis or age. The number of disc herniations in the cervical spine was correlated with age significantly, but not with cervical lordosis. Conclusions : Cervical disc herniation had a tendency to correlate with age and cervical lordosis, although this is not definite. Future studies that analyze more radiographic images of patients with HIVD of the cervical spine might be necessary to identify the influence of cervical lordosis on cervical disc herniation.

  • Cervical disc herniations can press on the spinal cord and cause a problem called cervical myelopathy. The most common symptom of cervical disc herniation is neck pain that spreads down to the upper limb in various locations. There can also be associated with weakness and movement disorders of upper limb. In Oriental medicine, cervical disc herniation is treated with herb-med, physical therapy, acupuncture, Chuna, etc, but the Bee Venom therapy is the most common and effective. In this case, we used the Bee Venom therapy at cervical hyeopcheokhyeol for about 2 months to a patient who was diagnosed with disc herniation at Cervical 5-6, 6-7 and appealed weakness, limited elevation and abduction of the left upper limb. As a result, left upper limb disability was improved. Using the Bee Venom therapy at cervical hyeopcheokhyeol that are effective on movement disorders and neurological diseases of upper limb is an effective treatment to upper limb disability diagnosed with cervical disc herniation, and suggests the direction of the treatment to upper limb weakness and movement disorders diagnosed with cervical disc herniation.

  • The possible causes of Brown-S$\acute{e}$quard Syndrome (BSS) have been frequently observed with spinal trauma and extramedullary spinal tumors, but the cervical disc herniation to cause BSS is rare. The authors present five cases of patients who were diagnosed with BSS resulting from cervical disc herniation, and the results of the literature in view of their distinctive symptoms and clinical outcomes. Postoperatively, the patients showed complete or almost complete recovery from their motor and sensory deficits. On the basis of our cases, it is important to diagnose it early by cervical magnetic resonance imaging, especially in the absence of the typical symptoms of cervical disc herniation or other obvious etiology of extremity numbness. Immediate surgical treatment is also essential for a favorable functional neurological recovery.

  • Acute paraplegia attributable to disc herniation is known to occur most frequently at the thoracic level. A 50-year-old male presented with progressive limb weakness and hypoactive deep tendon reflexes. On the basis of clinical features and neurological findings, the diagnosis of Guillain-Barre syndrome was suspected. Spinal MRI showed cervical disc herniation. He underwent emergency surgery consisting of removal of herniated disc and anterior fusion. We emphasize that there is a possibility of acute progression of paralysis secondary to nontraumatic enlargement of cervical disc herniation.

후측방으로 탈출된 경추 디스크의 후방 미세간공천개절제술의 치료결과 (The Result of Posterior Microforaminotomy for Posterolateral Herniation of Cervical Discs)

  • Objective : To evaluate the effectiveness of posterior microforaminotomy in treatment of posterolateral cervical disc herniation, the authors retrospectively analyzed the result of posterior microforaminotomy in our institute. Patients and Methods : Ten patients with radiculopathy due to posterolateral cervical disc herniation have been treated with posterior microforaminotomy from August 1996 to July 2000. We analyzed clinical results in all patients who were followed up for an average of 10 months. Results : The mean age was 47.2 years and all patients were treated with posterior microforaminotomy as primary treatment. one patient was received anterior cervical interbody fusion with iliac bone 12 years before. Clinical improvement in the last follow-up were seen in all patients and there were no complications. Conclusion : Microcervical foraminotomy is considered useful operative method for posterolateral soft disc herniation. We conclude that the posterior microforaminotomy for radiculopathy due to soft posterolateral cervical disc herniation seems to be safe and effective in selective patients.

  • Objects : This study was to report a clinical effect of Chuna Treatment for Cervical Myelopathy due to disc herniation. Methods : The patient was diagnosed as Cervical Myelopathy due to disc herniation through Cervical spine MRI and treated by cervical traction technique of Chuna Treatment with conservative management. JOA score(Japanese Orthopedic Association score), Recovery rate of Hirabayashi, VAS(Visual Analogue Scale), Dynamometer score, ROM(Range of Motion) and Motor weakness Grade were used. Results and Conclusion : JOA score, VAS, Dynamometer score, ROM and Motor weakness Grade were improved and Recovery rate was 100%.

경추추간판탈출증과 경부통증에 대한 추나치료에 관한 해외연구동향분석 -Pubmed 검색중심으로- (The Trend review of Chuna Therapy on Neck disorders and Cervical disc Herniation in Pubmed)

  • Objectives : The purpose of this study is to explore the trends of Chuna(manipulation) treatment on neck pain and cervical disc displacement. Methods : We set up the two searching strategies and investigated the latest clinical trials on Chuna(manipulation) therapy for Neck disorders and cervical disc herniation through PubMed search. The searched papers were analyzed and the year, journals, article types of literatures were examined. Of the articles, Randomized controlled trials(RCTs) are assessed by Jadad scale. Results : Studies on Chuna(manipulation) treatment to cervical disc herniation were very few. Articles on Chuna(manipulation) treatment to neck pain were relatively more than those of cervical disc herniation. The most frequently published journal was J Manipulative Physiol Ther. The main design of study was RCT. The limited quality assessment of RCT was conducted by Jadad scale and the average Jadad score was 1.73, which indicates low quality in terms of Jadad evaluation criteria. Conclusions : International trends of studies on Chuna therapy of cervical disease is increasingly reviewed. Although this review has several limitations, analyzing the latest trends of studies on Chuna(manipulation) treatment on neck pain and cervical disc displacement is necessary for further studies.

연성 경추 추간판 탈출증에 대한 임상적 고찰 (Clinical Study on Soft Cervical Disc Herniation)

  • Objective : We studied the effect and possibility of the oriental medical conservative treatment on soft cervical disc herniation. Methods : From October.2001 to July. 2003, we observed 16 patients hospitalized because of soft cervical disc herniation diagnosed by Cervical M.R.I, symptoms and physical test. After treatment a standard of valuation was according to criteria of Robinson et al. Results : The clinical results were satisfactory as excellent in 4 case, good in 10 case, fair in 2 case. 87.5% of all were improved above good state. Conclusions : We considered that oriental medical conservative treatment has on useful effect on soft soft cervical disc herniation patient's treatment and recovery.

Myelopathy Caused by Soft Cervical Disc Herniation : Surgical Results and Prognostic Factors

  • Objective : The purpose of this study was to investigate the surgical results and prognostic factors for patients with soft cervical disc herniation with myelopathy. Methods : During the last 7 years, 26 patients with cervical discogenic myelopathy were undertaken anterior discectomy and fusion. Clinical and radiographic features were reviewed to evaluate the surgical results and prognostic factors. The clinical outcome was judged using two grading systems (Herkowitz's scale and Nurick's grade). Results : Male were predominant (4:1), and C5-6 was the most frequently involved level. Gait disturbance, variable degree of spasticity, discomfort in chest and abdomen, hand numbness were the most obvious signs. Magnetic resonance(MR) images showed that central disc herniation was revealed in 16 cases, and accompanying cord signal changes in 4. Postoperatively, 23 patients showed favorable results (excellent, good and fair) according to Herkowitz's scale. Conclusion : Anterior cervical discectomy and fusion effectively reduced myelopathic symptoms due to soft cervical disc herniation. The authors assured that the shorter duration of clinical attention, the lesser the degree of myelopathy and better outcome in discogenic myelopathy.
