완전 도재관의 선택 (Selection of all ceramic crown)

  • 성공적싱 완전도재관 치료를 위한 요구조건은 통상적 치료와 크게 다르지 않다. 적절한 치아삭제, 임시치아, 및 정확한 최종인상이 중요하다. 그럼에도 불구하고 차별점은 본딩에 있다. 수복물 내면과 지대치 외면에 레진시멘트를 도포하는 것 이외에도 본딩 술식은 비생활치의 포스트 코어 치료에도 적용할 수 있다. 생활치 지대치는 비생활치보다 심미와 기능 면에서 성공을 거두기가 더 쉽다. 포스트 코어 본딩 술식은 비생활치 지대치의 장기 성공에 중요한 역할을 한다. 본 논문에서 완전 도재관의 성공 전략 중에서 포스트 코어 축성 및 완전 도재관 본딩에 관하여 임상적 경험을 나누고자 한다.

  • ;Dentistry has benefited from tremendous advances in technology with the introduction of new techniques and materials, and patients are aware that esthetic approaches in dentistry can change one's appearance. Increasingly. tooth-colored restorative materials have been used for restoration of posterior teeth. Tooth-colored restoration for posterior teeth can be divided into three categories: 1) the direct techniques that can be made in a single appointment and are an intraoral procedure utilizing composites: 2) the semidirect techniques that require both an intraoral and an extraoral procedure and are luted chairside utilizing composites: and 3) the indirect techniques that require several appointments and the expertise of a dental technician working with either composites or ceramics. But, resin restoration has inherent drawbacks of microleakage. polymerization shrinkage, thermal cycling problems. and wear in stress-bearing areas. On the other hand, Ceramic restorations have many advantages over resin restorations. Ceramic inlays are reported to have less leakage than resin restoration and to fit better. although marginal fidelity depends on technique and is laboratory dependent. Adhesion of luting resin is more reliable and durable to etched ceramic material than to treated resin composite. In view of color matching, periodontal health. resistance to abrasion, ceramic restoration is superior to resin restorationl. Materials which have been used for the fabrication of ceramic restorations are various. Conventional powder slurry ceramics are also available. Castable ceramics are produced by centrifugal casting of heat-treated glass ceramics. and machinable ceramics are feldspathic porcelains or cast glass ceramics which are milled using a CAD/CAM apparatus to produce inlays (for example, Cered. They may also be copy milled using the Celay apparatus. Pressable ceramics are produced from feldspathic porcelain which is supplied in ingot form and heated and moulded under pressure to produce a restoration. Infiltrated ceramics are another class of material which are available for use as ceramic inlays. An example is $In-Ceram^{\circledR}$(Vident. California, USA) which consists of a porous aluminum oxide or spinell core infiltrated with glass and subsequently veneered with feldspathic porcelain. In the 1980s. the development of compatible refractory materials made fabrication easier. and the development of adhesive resin cements greatly improved clinical success rates. This case report presents esthetic ceramic inlays for posterior teeth.teeth.

Collarless 도재용착주조관의 치경부 도재 변연부의 적합도 및 rounding의 비교 (A STUDY ON LABIOCERVICAL MARGINAL FITNESS AND MARGINAL ROUNDING OF COLLARLESS METAL CERAMIC RESTORATION)

  • In order to evaluate the fitness and the degree of rounding of porcelain margins in collarless metal cermic restorations, collarless metal ceramic restorations were fabricated with shoulder margins of 90 and 120 degress, each consisting of ten specimens, on master dies through the direct lift technique. And ten metal ceramic restorations with a shoulder 90 degrees were fabricated. All specimens were embedded in resin and sectioned longitudinally. The sections were observed under a stereomicroscope and photographed(${\times}$200). The labial marginal gap between the die and the porcelain margin were measured with a scale. The space between the porcelain margin and the die, that is formed from marginal rounding and cementation were calculated with a computer coordinating area curvimeter. The following results were obtained. 1. There was no statistical difference between the fitness of porcelain margin of collarless metal ceramic restorations and marginal fitness of metal ceramic restoration. 2. There was no statistical difference between a shoulder of 90 and 120 degrees in the fitness of porcelain margin of collarless metal ceramic resorations. 3. Collarless metal ceramic restorations with a shoulder of 90 degrees and 120 degrees showed significantly more labial marginal rounding than metal ceramic restorations. 4. There was no statistical difference between a shoulder of 90 and 120 degrees in rounding of porcelain margin of collarless metal ceramic restorations. According to the results, rounding of porcelain margins can be observed in collarless metal ceramic restorations. Thus, there is a need for improvement in dental materials and techniques to minimize this problem, Furthermore, care should be exercised during the clinical procedure.

치과용 CAD/CAM 전용 세라믹기반 하이브리드 레진 수복물의 변연 적합 연구 (A study on marginal fit of the ceramic-based hybrid resin restoration for dental CAD/CAM systems)

  • Purpose: This study aimed to assess and compare the marginal fit of ceramic-based hybrid resin restoration (HYB) and zirconia restoration (ZIR) for dental computer-aided design/computer-aided manufacturing systems. Methods: A stainless steel master model was produced. The impression was first made with silicone, and then stone working models were produced. A total of twenty restorations were fabricated with two different materials: ZIR and HYB. The silicone film thickness of the marginal gap was measured using a digital microscope; digital photos were taken at a magnification of ×160, and then analyzed using a measurement software. The values of the result were evaluated with the independent-sample t-test (α=0.05). All statistical analyses were performed with a statistical software. Results: The mean values for the marginal gap was 37.14±2.96 ㎛ for HYB, compared with 40.37±5.26 ㎛ for ZIR. No significant difference was found between ZIR and HYB (p=0.107). Conclusion: As a result, the marginal fit of the restoration fabricated using the hybrid resin was better than that of the restoration fabricated using zirconia. Also, the marginal fit of all groups was below the clinical acceptable range of 120 ㎛. Thus, HYB for dental CAD/CAM system in this study is expected to be suitable for clinical use in dentistry.

전치부 ALL-CERAMIC PARTIAL CROWN 의 임상 (- Aesthetic Restoration of the Gingival Recession with All-ceramic Partial Crown -)

  • The black triangle is a common clinical finding in aged people with gingival recessions. Among other prosthetic treatment molalities such as composite resin filling and laminate venners, the all ceramic restoration procedure can lead to most successful result. With improved bonding strength and ceramic properties, the tooth preparation design for all-ceramic crown can be modified to minimize the reduction of sound tooth structure without loosing properties of conventional preparation design. Case selection is an important factor in acheiving succesful prosthesis. In this case report, the leucite reinforced pressable ceramic, $Authentic^{TM}$ [Ceramay, Germany] was used to fabricate the prosthesis.

A new manufacturing method of all ceramic restoration using alumina tape and presentation of clinical cases

  • In an effort to facilitate fabrication procedure of all ceramic crowns, a novel preparation method for all ceramic crown using alumina tape was developed. The alumina tape having a uniform thickness was cast by using Doctor blade method. The physical properties of newly introduced alumina tape has biaxial flexure strength of $500\sim600MPa$. The value of toughness is $3.18\sim3,28MPa.m^{1/2}$ which corelates with fracture and the linear shrinkage rate of the alumina tape is 0.44% during core production. The marginal fitness of the alumina tape all-ceramic restoration with $90{!`}$shoulder margin had average marginal discrepancy at $78.3{\S}$ > and average marginal gap at $44.4{\S}$ >. At the marginal preparation of $135{!`}$deep chamfer, the average marginal discrepancy at $82.1{\S}$ > and the average marginal gap at $40.2{\S}$ > had been reported. This fabrication procedure of all ceramic crowns with alumina tapes is easier and less technical sensitive for dental technicians. After restoration with new all ceramic crowns we followed the patients 2 years later, there were no complications as porcelain fractures or periodontal disease. We had good esthetic clinical results with new all ceramic crowns.

  • Application of ceramic materials for fabrication of dental restoration materials has been a focus of interest in the field of esthetic dentistry. The ceramic materials of choice are glass ceramics, spinel, alumina, and zirconia. The development of yttrium tetragonal zirconia polycrystal (YTZP)-based systems is a recent addition to all-ceramic systems that have high strength and are used for crowns and fixed partial dentures. Computer-aided design/computer-aided manufacturing (CAD/CAM)-produced, YTZP-based systems are popular with respect to their esthetic appeal for use in stress-bearing regions. The highly esthetic nature of zirconia and its superior physical properties and biocompatibility have enabled the development of restorative systems that meet the demands of today's patients. Many in vitro trials have been performed on the use of zirconia; however, relatively fewer long-term clinical studies have been published on this subject. The use of zirconia frameworks for long-span fixed partial dentures is currently being evaluated; in the future, more in vivo research and long-term clinical studies are required to provide scientific evidence for drawing solid guidelines. Further clinical and in vitro studies are required to obtain data regarding the long-term clinical use of zirconia-based restorations.
