The morphology and growth changes of the Craniofacial Complex are important in orthodontics and cephalometrics for analysis and evaluation of facial bone growth are widely used. The author analyzed the data using lateral cephalometric roentgenogram of 75 Korean male and 50 Korean females age of 6 to 10 with normal occlusion to provide informations-relative rates of facial bone growth of Korean which is to be contributed in Korean standard. The results were as follows: 1. Means, standard deviation and coefficient of variation of Korean children were obtained. 2. The item which showed significent difference between male and female was craniofacial height in absolute dimension. 9. No difference of sex was showed in increment of craniofacial height and depth. 4. Among the craniofacial depth increments, the lower facial depth dimension increased most, midfacial depth dimension increased less, and cranial depth dimension increased the least. 5. The horizontal body of mandible showed rapid growing tendency more than did the ascending ramus.
This study was performed to aid for the case analysis and diagnosis of the maxillofacial deformities. A cephalometric analysis was made from 101 subjects over 18 years old, that consisted of 46 males and 55 females with normal occlusion, acceptable profile. The results o f this study were obtained as follows: 1. The tables of normals in adult male and female were made. 2. The angles of SNO were $65.5^{\circ}$ in male and $65.3^{\circ}$ in female and the horizontal linear measurements from 0 to NA(O-NA) were 9.0 mm in male and 8.9 mm in female. 3. The cranial base angles (SN-FH) were $8.2^{\circ}$in male and $10.3^{\circ}$ in female and the mandibular plane angles (OP-HP) were $28.1^{\circ}$ in male and $26.2^{\circ}$ in female. 4. The correlation of (O-NA) to S-O/S-A and S-O/S-N were higher than SNO, and the correlation of (N-A) to SNA and (N-B) were high.
The purpose of this study was to redefine the cephalometric landmarks in three-dimensional (3D) images, which are used in orthodontic cephalometric radiography, and to evaluate the reproducibility of each landmark for 3D cephalometric analysis. Eighteen CT scans were taken at the Department of Diagnostic Radiology at Seoul National University Dental Hospital and manipulated with V works 4.0(Cybermed Inc., Seoul, Korea). The coordinate system was established using 7 reference points, with no more than 4 points on the same plane. These 7 points were generated as a volume model, the voxel size of which was 4 by 4 by 2 (threshold value=639). The cephalometric landmarks were selected at the multiplanar reformation (MPR) window on the volume mode of V works 4.0. The selected landmarks were exported to V surgery (Cybermed Inc., Seoul, Korea) for the calculation of coordinate values. All the data were taken twice with a lapse of 2 weeks by one investigator The reproducibility of each landmark was $0.17\~1.21mm$ in the x axis, $0.30\~1.53mm$. In the y axis, and $0.27\~1.81mm$ in the z axis. In all three axes, the range of error was similar. These error ranges were acceptable with regards to the pixel space and slice thickness. The most reproducible points were 1 points which were selected on the basis of the volume model. The least reproducible points were J points that were defined by sutures.
The purpose of this study was to evaluate the reproducibility of lateral cephalometric landmarks according to radiographic image enhancement, and to contribute to the identification of cephalometric landmarks. Lateral cephalograms of ten individuals were taken and stored into computer. The images were then enhanced up to four grades by Quick Ceph Image Pro$^{TM}$ on condition that the gray-scale equalization number was 50 and the detail enhancement number was 50. After thirty two landmarks were identified on monitor images by five observers, the deviations from the mean, the distances estimated between identified points and the mean point of five identified points, were evaluated for each landmark at each enhancement grade. Through the statistical analysis, following results were obtained. 1. In case of unenhanced radiographic images, the inter-observer reproducibility of the landmarks showed a large variation. 2. The comparison of deviation from the mean according to the degree of radiographic image enhancement for each landmark showed that the inter-observer reproducibility was significantly different at 5 landmarks. 3. The landmark of pterygomaxillary fissure showed higher reproducibility at enhancement grade 1 and 2 images than at unenhanced images. So did the landmark of posterior nasal spine at enhancement grade 1 images, and the landmark of menton at enhancement grade 2, 3 and 4 images respectively. The above results suggest that the reproducibility of some landmarks can be increased by radiographic image enhancement during the identification of the lateral cephalometric landmarks on the monitor.

The purpose of this study is to develop soft tissue cephalometric standards in children with normal occlusion and to determine the differences between males and females and the differences according to age ranges of 9, 10 and 11 years. The lateral cephalometric radiographs of 169 children(82 boys, 87 girls) among the contestants in 2000-2004 Healthy Dentition Contest in Seoul were studied with several soft tissue profile analyses and cephalometric means and following results were obtained. 1. The sex differences were not statistically significant between males and females soft tissue parameters except for two soft tissue parameters(p>0.05). 2. Females had relatively more protrusive lower lip relative to the H line than males and middle third face height to lower third face height of females was larger than males(p<0.05). 3. The age differences in soft tissue parameters were not statistically significant according to age ranges of 9, 10 and 11 years(p>0.05).
This study was done to recognize the importance of errors in measurements of cephalometric radiograph and to find the anatomical structures those need special care to select as a reference points through the detection of the systematic errors and estimation of random errors. For this purose, 100 cephalometric radiographs were prepared by usual manner and 61 reference points, and 130 measurement variables were established. Measurement errors were detected and estimated by the comparison of the 25 randomly-selected samples for repeated measurements with the main sample. The following results were obtained : 1. In comparison of the repeated measurements, there were statistical significant differences in 24 variables which were 18.4% of 130 total variables. 2. The frequency of the difference in identification of the reference points between the repeated measurements was very high in the root apex of upper incisor(as), the most posterior wall of maxilla(tu), soft tissue nasion(n'), soft tissue frontal eminence(ft), and ad3 in airway. 3. After correction of reference points marking until the level of below 5% significance, the range of random errors were from 0.67 to 1.71 degree or mm. 4. The variable shown the largest random error was the interincisal angle(ILs-ILi). 5. Measurement errors were mainly caused by the lack of precision in anatomic definitions and obscure radiographic image. From the above results, the author could find the high possibility of errors in cephalometric measurements and from this point, we should include error analysis in all the studies concerning measurments. In is essential to have a concept of error analysis not only for the investigator but also for a reader of other articles.

Purpose : This study was intended to perform cephalometric comparison between the patients with and without obstructive sleep apnea (OSA). The factors influencing the OSA in the lateral cephalogram was also investigated. Patient and Method : Twenty four patients who visited Sleep Disorder Clinic in Dongsan Medical Center, Keimyung University and evaluated with polysomnograph(PSG) and cephalogram were included in the study. The patients had apnea-hypopnea episode(AHI) over 10 times per hour was diagnosed as OSA after overnight PSG. To evaluate hard and soft tissue profile, cephalometric radiogram were taken at maximal intercuspation(P1) and mandibular protruding position(P2). The diffefence between the OSA and normal group were evaluated statistically and the stepwise regression analysis was applied to analyse the cephalometric influencing factors to OSA. Result : The OSA Group(n=14) had significantly higher Body Mass Index(BMI) than control group(n=10). Lower facial height(ANSGn) was longer in OSA group. However statistically significant difference was not detected in other anteroposterior craniofacial measurements. The soft palate lenth (PNS-P), hyoid position (MP-Hyoid) had positive correlation between AHI (r=0.496, r=0.413, respectively, p<0.05). However, the measurements of oropharyngeal airway was not different between the two groups. The hypothesis, the antero-posteriorly narrow oropharyngeal airway might aggravate the airway resistance and can give rise to higher AHI, was not accepted in the study. This can be attributed by inclusion of the patients performed uvulopalatopharyngoplasty because of the tonsilar or soft palate hypertrophy in the present study. The results of regression analysis revealed that PNS-P, upper airway width(Nph1), upper facial heght(N-ANS), and lower facial height(ANS-Gn) could influence the degree of AHI (F value < 0.0001, $R^2$ = 0.829). Conclusion : We suggest lateral cephalogram may utilized as a useful method to evaluate OSA. The patient with long soft palate, narrow upper airway width, long upper & lower facial height can be expected to have high risk of OSA. However, it should be emphasized the comphrehensive intraoral inspection including soft palate and tonsilar hypertrophy because lateral cepahlogram cannot visualize oropharyngeal status completely.
Background We conducted this study to analyze the values of the key cephalometric angular measurements of the mandible using 3-dimensional (3D) computed tomography scans. Methods In the 106 enrolled patients, a 3D cephalometric analysis was performed to measure the angular variables of the mandible. These values were compared between the two sides and between the two sexes. Results The frontal measurements revealed that the mandibular body curve angle was larger on the left (Lt) side (right [Rt], $141.24{\pm}7.54$; Lt, $142.68{\pm}6.94$; P=0.002) and the gonial angle was larger on the right side (Rt, $134.37{\pm}8.44$; Lt, $131.54{\pm}7.14$; P<0.001). The sagittal measurements showed that the gonial angle was larger on the right side (Rt, $134.37{\pm}8.44$; Lt, $131.54{\pm}7.14$; P>0.05). Further, the transverse measurements revealed that the mandibular body curve angle was larger on the right side (Rt, $140.28{\pm}7.05$; Lt, $137.56{\pm}6.23$; P<0.001). Conclusions These results provide an average of the mandibular angular measurements for the Korean population, establishing a standard for determining surgical patient groups and outcome evaluations in the field of mandible contour surgery.

Purpose: Obstructive sleep apnea (OSA) is a common medical disorder with serious complications if untreated. Dentists play a vital role in the early diagnosis of this condition, thereby improving patients' prognoses. The purpose of this study was to identify patients with a high risk of OSA using simple cephalometric measurements in patients receiving routine dental care. Materials and Methods: The present study was conducted on 206 patients divided into a high-risk group and a control group after answering the Berlin questionnaire. Cephalometric analysis of a digital cephalogram was performed to measure the upper airway diameter (UAD) and mandibular-to-hyoid bone distance (MP-H) by 2 observers at 2 different times. Results: Among 206 patients, 93 (45%) were included in the high-risk group and 113 (55%) were in the control group. No significant difference was present between the groups with regard to gender, and the patients ranged in age from 18 to 65 years. The UAD measurements in the high-risk group were significantly lower than in the control group, and the MP-H measurements were significantly higher in the high-risk group than in the control group. The UAD was lower in middle-aged patients in both groups. Conclusion: Our study found that the UAD was lower in individuals with a high risk of OSA. Also, we found that middle-aged individuals of both genders were more likely to develop OSA. Dentists play a vital role in diagnosing patients at a high risk for OSA via thorough clinical examinations, risk factor analyses, and simple cephalometric analyses.

Objectives: The aim of this study was to verify the concordance of the measurement values when the same cephalometric analysis method was used for two-dimensional (2D) cephalometric radiography and three-dimensional computed tomography (3D CT), and to identify which 3D Frankfort horizontal (FH) plane was the most concordant with FH plane used for cephalometric radiography. Materials and Methods: Reference horizontal plane was FH plane. Palatal angle and occlusal plane angle was evaluated with FH plane. Gonial angle (GA), palatal angle, upper occlusal plane angle (UOPA), mandibular plane angle (MPA), U1 to occlusal plane angle, U1 to FH plane angle, SNA and SNB were obtained on 2D cephalmetries and reconstructed 3D CT. The values measured eight angles in 2D lateral cephalometry and reconstructed 3D CT were evaluated by intraclass correlation coefficiency (ICC). It also was evaluated to identify 3D FH plane with high degree of concordance to 2D one by studying which one in four FH planes shows the highest degree of concordance with 2D FH plane. Results: ICCs of MPA (0.752), UOPA (0.745), SNA (0.798) and SNB (0.869) were high. On the other hand, ICCs of gonial angle (0.583), palatal angle (0.287), U1 to occlusal plane (0.404), U1 to FH plane (0.617) were low respectively. Additionally GA and MPA acquired from 2D were bigger than those on 3D in all 20 patients included in this study. Concordance between one UOPA from 2D and four UOPAs from 3D CT were evaluated by ICC values. Results showed no significant difference among four FH planes defined on 3D CT. Conclusion: FH plane that can be set on 3D CT does not have difference in concordance from FH plane on lateral cephalometry. However, it is desirable to define FH plane on 3D CT with two orbitales and one porion considering the reproduction of orbitale itself.
