Organophosphate-Pyrethroid Mixtures Poisoning (유기인계-피레스로이드계 혼합제제 농약에 의한 중독)

  • Purpose: Owing to organophosphate-pyrethroid mixtures are widely used, suicidal or unintentional poisoning is common. But there have been relatively few reports of poisoning. The purpose of this study is to evaluate the difference of the severity and toxicity between organophosphate-pyrethroid mixtures poisoning and single organophosphate poisoning. Methods: From August 2005 to July 2006, 65 patients presented with organophosphate-pyrethroid mixtures poisoning to emergency medical center. Date were gathered by report form it was drawn up. Results: 65 patients were enrolled in 28 hospitals and their mean age was $56.5{\pm}16.2$ years old. The most common cause of poisoning was suicide, in 52 cases(80%). Chlorpyrifos-cypermethrin, malathion-esfenvalerate were the most frequent chemicals involved, and the mean ingestion amount was 135.4ml. The most common symptom of the patients was nausea/vomiting, in 16 patients. The average GCS score was 13. The mean ICU stay was $4.4{\pm}5.2$ days, and mortality was 3.1%. Conclusion: The severity and toxicity of organophosphate-pyrethroid mixture poisoning were lower than that of single organophosphate poisoning.

Effects of Brain activation Intervention Programs for Mild Dementia (경증치매노인을 위한 뇌 활성화 중재프로그램의 효과)

  • This study was conducted with the purpose of identifying the effects of behavioral psychosomatic symptoms, cognitive functions, ability to perform daily life, and depression among the elderly with dementia. The sample for this study comprised 40 people in the experimental group and 20 in the contrastive group from the Dementia Supporting Center, located in NoWon-Gu of Seoul city. Data were collected from November 1, 2016 until March 31, 2017. The collected data were analyzed using SPSS 23.0, and descriptive statistics, chi-square test, independent t-test two-way repeated measures ANOVA, and ANCOVA were calculated. An 8-week brain activation intervention program including lectures was administered to elderly people with dementia. The results show improved cognitive function and reduced depressed feelings. Based on the results of this study, brain activation intervention programs can be applied to elderly people with dementia at the community level to help improve their cognitive function and relieve them from depression.

The fruit of Acanthopanax senticosus Harms improves arterial stiffness and blood pressure: a randomized, placebo-controlled trial

  • BACKGROUND/OBJECTIVES: Arterial stiffness and endothelial dysfunction are 2 of the independent predictors for cardiovascular disease, while Acanthopanax senticosus Harms (ASH) is a traditional medicinal plant that can improve cardiovascular health. This study aimed to investigate the efficacy of the fruit of ASH on vascular function in apparently healthy subjects. SUBJECTS/METHODS: A 12-week, randomized, double-blind, placebo-controlled design, consisting of healthy adults with at least 2 of the following 3 conditions: borderline high blood pressure (BP; 120 mmHg ≤ systolic BP ≤ 160 mmHg or 80 mmHg ≤ diastolic BP ≤ 100 mmHg), smoking (≥10 cigarettes/day), and borderline blood lipid levels (220 ≤ total cholesterol ≤ 240, 130 ≤ low density lipoprotein cholesterol ≤ 165, or 150 ≤ triglyceride ≤ 220 mg/dL). Randomly assigned 76 subjects who received a placebo or 2 doses of ASH fruit (low, 500 mg/day; high, 1,000 mg/day) completed the intervention. Brachial-ankle pulse wave velocity (baPWV), flow-mediated dilation, carotid intima-media thickness, and BP were measured both at baseline and following the 12-week intervention. Endothelial nitric oxide synthase (eNOS) phosphorylation was assessed by western blotting. RESULTS: Compared with the placebo group, the low-dose group showed more significant changes after the 12-week intervention period in terms of systolic BP (0.1 vs. -7.7 mmHg; P = 0.044), baPWV (31.3 vs. -98.7 cm/s; P = 0.007), and the ratio of phospho-eNOS/eNOS (0.8 vs. 1.22; P = 0.037). CONCLUSIONS: These results suggest that ASH fruit extract at 500 mg/day has the potential to improve BP and arterial stiffness via endothelial eNOS activation in healthy adults with smoking and the tendency of having elevated BP or blood lipid parameters.

The Effect of Group Treatment Based on Occupational Therapy for Children : A Systematic Review (아동 그룹 작업치료의 효과 : 체계적 고찰)

  • Objective : The purpose of this article is to study the effectiveness of a group occupational therapy for children via systematic review, and to provide clinical evidences. Methods : The systematic review initially included articles in Pubmed, Ovid, RISS, KISS, and DBpia, published until 2014. Total 14 articles have been chosen, using different keyword of each domestic/foreign studies. Selected articles were divided depending on the level of evidence, and eventually been organized into group PICO (Patient, Intervention, Comparison, Outcome). Results : The results indicated that there are more diagnosed children than normal in participants and especially children with sensory integration disorder (28.6%) appeared to be the most common. In intervention, group sensory integrative intervention had the largest frequency, and instruments measuring social skills were used most for analyzing the intervention results. The effectiveness of interventions showing statistically significant results were 40.7% of Total. Conclusion : Group occupational therapy for children should be provided to various diagnosis with widely better quality of interventions providing the enough support for the evidence level.

Clinical Efficacy of Individual Cognitive Behavioral Therapy for Patients with Primary or Secondary Insomnia (일차성 및 이차성 불면증 환자에서의 개별 인지행동치료의 임상적 효능)

  • Objectives: This study aimed to evaluate the clinical efficacy of individual cognitive behavioral therapy (CBT) for Patients with Primary or Secondary Insomnia. Methods: Participants were recruited from a primary care sleep clinic from January 2008 to June 2009. The study sample included 64 outpatients with primary insomnia (n=30) and secondary insomnia (n=34) according to the criteria of DSM-IV. Participants completed sleep diaries, Dysfunctional Beliefs and Attitudes about Sleep Scale-16 (DBAS), State-Trait Anxiety Inventory (STAI) before CBT and shortly after completion of CBT. CBT was provided in 7 weekly, 40-50-minute individual therapy sessions. Results: Both groups of patients with primary and secondary insomnia showed significant improvement in the DBAS and sleep parameters including sleep onset latency, total sleep time, and sleep efficiency. Repeated-measures ANOVA of the DBAS and sleep parameters showed no significant group-by-time interactions between patients with primary and secondary insomnia, suggesting the efficacy of CBT for patients with secondary insomnia was equivalent to that of CBT for patients with primary insomnia. Conclusion: This study suggests that CBT is effective for the management of primary and secondary insomnia in a primary care setting.

10-year Analysis of Inherited Metabolic Diseases Diagnosed with Tandem Mass Spectrometry (탠덤 매스 검사(Tandem Mass Spectrometry)를 이용한 선천성 대사이상 선별검사 10년간의 분석)

  • Purpose: From the early 1990's, use of Tandem mass spectrometry in neonatal screening test, made early stage detection of disorders that was not detectable by the previous methods of inspection. This research aims to evaluate the frequency of positive results in national neonatal screening test by Tandem mass spectrometry and its usefulness. Methods: A designated organization for inherited metabolic disorder executed neonatal screening test on newborns using Tandem mass spectrometry from January 2006 to December 2015, followed by the investigation of these data by the Planned Population Federation of Korea (PPFK), and this research analyzed those inspected data from the PPFK. Results: Among total childbirth of 4,590,606, from January 2006 to December 2015, 3,445,238 were selected for MS/MS and conduction rate was 75.1%. 261 out of the selected 3,445,238 were confirmed patients and for last decade, detection rate of total metabolic disorder was 1/13,205. In 261 confirmed patients, 120 had an amino acid metabolic disorder and its detection rate was 1/28,710 and 110 had an organic acid metabolic disorder and detection rate was 1/31,320. Also, 31 had a fatty acid metabolic disorder and detection rate was 1/13,205. Conclusion: Inherited metabolic disorder is very rare. Until now, it was difficult to precisely grasp an understanding on the national incidence of inherited metabolic disorder, due to lack of overall data and inconsistent and incomplete long-term result analysis. However, this research attempted to comprehensively approach the domestic incidence, by analyzing previous 10 years of data.

The Influence of Mother's Child-rearing Attitude, Temperament and Goodness of Fit of Infant's on Adjustment to Childcare Center (어머니의 양육태도와 영아의 기질 및 조화적합성이 보육시설 적응에 미치는 영향)

  • The current study aims to examine differences of mother's child-rearing attitude by background variables of infant's and mothers, the influence of mother's child-rearing attitude, temperament and goodness of fit of infant's on adjustment to childcare center, and relative influence among elements. Subjects for the study were 165 infant's of two year old attending 8 childcare centers in Seoul, Gyeonggi-do and Jeju-do, and their mothers and homeroom teachers at childcare centers. As a result of the study, it was found that there was no difference in background variables such as sex and order among siblings of infant's, childcare experience and average hours at childcare centers a day, and mother's age and job, while there were differences in mother's child-rearing attitude by mother's education, household's monthly income and mother's working hours per week. Also it was observed that mother's restrictive attitude, positive attitude and infant's regular temperament influenced their adjustment to childcare centers, and relatively influential elements on general adjustment to childcare centers were mother's restrictive attitude and positive attitude. That IS, it was found that as mother's restrictive attitude and positive attitude were high, infant's general adjustment to childcare centers became high. These study results show that if infant's learn basic rules and habits from parents at home by having positive relationship with mothers, getting appropriate educative instruction, having proper autonomy and obtaining positive assessment from their mothers, rather than strict restriction or bluff, can adapt themselves to childcare centers with no difficulty.

DYNAMICAL CHARACTERISTICS OF THE QUIET TRANSITION REGION: SPATIAL CORRELATION STUDIES OF H I 931 AND S VI 933 UV LINES

  • To understand the basic physics underlying large spatial fluctuations of intensity and Doppler shift, we have investigated the dynamical charctersitics of the transition region of the quiet sun by analyzing a raster scan of high resolution UV spectral band containing H Lyman lines and a S VI line. The spectra were taken from a quiet area of $100'\times100'$ located near the disk center by SUMER on board SOHO. The spectral band ranges from 906 A to 950 A with spatial and spectral resolution of 1v and $0.044 {\AA}$, respectively. The parameters of individual spectral lines were determined from a single Gaussian fit to each spectral line. Then, spatial correlation analyses have been made among the line parameters. Important findings emerged from the present analysis are as follows. (1) The integrated intensity maps of the observed area of H I 931 line $(1\times10^4 K)$ and S VI 933 line $(2\times10^5 K)$ look very smilar to each other with the same characterstic size of 5". An important difference, however, is that the intensity ratio of brighter network regions to darker cell regions is much larger in S VI 933 line than that in H I 931 line. (2) Dynamical features represented by Doppler shifts and line widths are smaller than those features seen in intensity maps. The features are found to be changing rapidly with time within a time scale shorter than the integration time, 110 seconds, while the intensity structure remains nearly unchanged during the same time interval. (3) The line intensity of S VI is quite strongly correlated with that of H I lines, but the Doppler shift correlation between the two lines is not as strong as the intensity correlation. The correlation length of the intensity structure is found to be about 5.7' (4100 km), which is at least 3 times larger than that of the velocity structure. These findings support the notion that the basic unit of the transition region of the quiet sun is a loop-like structure with a size of a few $10^3 km$, within which a number of unresolved smaller velocity structures are present.

  • There are a number of policies that have been established to reduce greenhouse gases emissions and reduce energy consumption. These polices, however, only reflect the building energy consumption saving rate without considering building energy consumption characteristic. If building that already use less energy apply the same saving rate to reduce energy consumption, it is less effective. Therefore, this study classified building energy consumption characteristics and investigated approaches to adopt individual reduction rates according to energy consumption. Analysis results showed that there are differences in energy consumption depending on building use, and therefore, we classified buildings by use. We also proposed a formula to set a reduction goal for rate of energy consumption according to energy consumption. We verified our approach in a top-end neighborhood facility in Seoul. The energy consumption savings goal of this study can be applied individually, but determining whether a building can reduce energy consumption towards a saving goal requires a closer look at each building.

Efficacy of Grapefruit Seed Extract in the Preservation of Satsuma mandarin (Grapefruit 종자추출물을 이용한 밀감의 저장효과)

  • To investigate the efficacy of grapefruit seed extract (GFSE) in the preservation of Satsuma mandarin, the citrus fruits were treated with 0 (control, only wash), 100 ppm and 250 ppm, dried and stored for 8 weeks at $15{\sim}20^{\circ}C$ and 60% RH. While 80% of the control fruits were contaminated and decayed by Penicillium sp., forming the the greenish blue spores, 27% of 100 ppm GFSE-treated fruits were contaminated and decayed and only 13% of 250 ppm GFSE-treated fruits were contaminated and decayed and only 13% of 250 ppm GFSE-treated samples were contaminated and not decayed by the fungi. GFSE showed marded inhibitory action against Penicillium sp. which was related to the decay of the citrus fruits in vitro experiments. Fungal growth was completely controlled through use of 500 ppm and the recommended range of GFSE to preserve the citrus fruits was $250{\sim}500ppm$. Transmission electron microscopic examination showed the fungal conidiospores the function of which was destroyed by dipping into GFSE.
