Clinical study of mediastinal tumor: 110 cases report (종격동 종양의 임상적 고찰 -110례 임상 경험-)

  • Mediastinal tumors have long fascinated the thoracic surgeon because of their variety and unpredictability of diagnosis prior to exploration. We report the analysis of the 110 cases of mediastinal tumors, experienced in the dept. of the thoracic and cardiovascular surgery of the National Medical Center from December 1959 to August 1983. The age distribution was relatively even and the mean age was 37 years old. The germ cell tumors were 29 cases [31%], the neurogenic tumors were 19 cases [20%], the thymomas were 16 cases [17%], the lymphomas were 8 cases [8.5%], the primary or secondary carcinomas were 11 cases [12%], the bronchogenic and the P.W cysts were 4 cases, the mesenchymal tumors were 3 cases, the TB gangliomas were 3 cases among the 94 cases, histologically analysed. The malignant tumors were 39 cases [41%]. In classified by histological types, the tumor size, location and the clinical manifestations are presented. The successful removal was done in 53 cases [96%] among 55 cases of benign mediastinal tumors. In 39 malignant cases, the surgical intervention had been done in 21 cases [54%], and inoperable cases were 16 [41%], and the operative deaths were 2 cases [5%].

Surgical Treatment of Pulmonary Aspergillosis[II] (폐 Aspergollosis의 외과적 치료[제 2보])

  • This investigation is designed to illustrate the clinical features '||'&'||' preoperative diagnosis, surgical role in the management of pulmonary aspergilloma, '||'&'||' compare with the previous study. We reviewed 42 cases of surgically treated pulmonary aspergillosis from Jan. 1984 to July 1992. The peak age incidence laid in the 2nd '||'&'||' 3rd decade of 25 cases[59.5%]. The 41 cases[97.6%] had a history of treatment with anti-tuberculous drugs under impression of pulmonary tuberculosis. The most common complaint was hemoptysis in 28 cases[66.7%]. The 21 cases showed so called "Air-meniscus sign" on the preoperative chest X-ray. As a preoperative diagnostic modality, the positivity was 30.0%, 57.8%, 88.5% on the fungus culture of sputum for Aspergillus, Chest CT, serum immunodiffusion test for A. fumigatus, respectively. The 37 cases[88.1%] can be diagnosed as pulmonary aspergillosis pre-operatively by any diagnostic method. The anatomical location of aspergilloma was mainly upper lobe in 17 cases[40.5%] '||'&'||' the majority of cases were managed by lobectomy. The postoperative pathologic findings showed that 18 cases[41.0%] were combined with tuberculosis '||'&'||' 15 cases[34.1%] were not combined with any other disease[Pr imary Aspergillosis]. The 6 cases showed postoperative empyema including 4 cases of bronchopleural fistula, 3 cases showed postoperative bleeding. One case was died postoperatively due to respiratory failure. In conclusion, when the patient who has longstanding history fo pulmonary tuberculosis '||'&'||' hemoptysis, must be suspected fungal super infection. The resectional surgery is the treatment of choice for symptomatic localized disease. And compare with the previous study, preop. chest CT '||'&'||' immunodiffusion test were more commonly available '||'&'||' showed high positivity.

Analysis of the Content of Telephone Counseling with Breastfeeding mothers (모유수유 전화 상담 내용의 분석)

  • This study was done to analysis data on breastfeeding mothers. This stud was conducted using data from telephone counseling in one metropolitan area. The subjects who had received consultation about breastfeeding were 100 breastfeeding mothers. The period of consultation was from Mar. 9, 1994 to August 23, 1994. Consultants were referred from UNICEF, hospitals, TV, newspapers or magazines. Analysis of the problem patterns resulted in 11 classifications. These were physio -anatomical factors(11 cases) , psychological factors(15 cases), breastfeeding methods(21 cases), breastfeeding intervals and frequency(19 cases) , disease in the breastfeeding mothers(13 cases), disease in the babies (12 cases), lack of support (4 cases), food and drugs while breastfeeding(5 cases), weaning(11 cases), others(27 cases). The highest frequency was for breastfeeding methods (21 cases). When the contents of the counselling were analyzed for the 100 cases, 36 problem patterns were identified. Patterns with over 10 responses were diarrhea, insufficient milk supply, sore and cracked nipples, weaning, inverted nipples and jaundice. The age of infants when mothers were telephoned was as below : 1 week(28 cases), 2 weeks(12 cases), 3-4 weeks(18 cases), 5-8 weeks(7 cases), 9 weeks-3 menths(4 cases), 4 menths-6 months(12 cases), over 6 months(2 cases), and the number of pregnant women was 12. The nursing diagnosis were classified according to problem patterns and each diagnosis was assigned an appropriate Problem Pattern The total number of nursing diagnoses was 22. When clients are referred for counselling nurses need guidelines about problems, possible causes and nursing. In this study, the example of guidelines for sore nipples is suggested. The recommendations based on the telephone counseling results are as follows : Prenatal education about the advantages of breastfeeding and breast care, and home visits after delivery for counseling related to breastfeeding. During the hospital stay, nursing intervention such as education on breastfeeding methods using slides, audio-visual tapes, pamphlets are needed as well as an initial trial of breastfeeding. Further research is indicated on the perceived lack of breast milk and on the effectiveness of nursing interventions to pro-mote breastfeeding.

A Clinical Study of Branchial Cleft Cyst (새성 낭종의 임상적 고찰)

  • Branchial cleft cyst is rarely encountered congenital neck disease. It is commonly believed that the branchial anomaly is persistance of remnant of the embryologic branchial apparatus. Among the patients visited Kyung Hee Medical Center with neck mass and inflammatory sign from January, 1980 to Aprial, 1994, we reviewed 26 cases of branchial cleft cysts confirmed by histopathologic findings with retrospective study on clinical aspects. The results are as follows: 1) There was no sex difference(14 male and 12 female), and most common between 2nd and 4th decade(21 cases, 80%). 2) Palpable mass was most common complaint(21 cases). In physical exam, the mass was non-tender, mobile and soft in most cases(over 80%). 3) The most common lesion site was anterior triangle in 15 cases. Along the SCM level, 21 cases were in upper 1/3, 2 cases in middle 1/3 and 3 cases in lower 1/3. 4) According to Bailey's classification, type II were 20 cases(76.9%), type I 5 cases(19.2%) and type III 1 case. 5) Among 28 cases, fistulous tract was found in 6 cases: one was complete type and 5 were incomplete type. 6) Among 26 cases before operation, 10 cases were diagnosed as branchial cleft cyst, 6 cases tuberculosis, 3 cases parotid tumor and 2 cases thyroglossal duct cyst. 7) Type of lining epithelium in histopathologic finding was stratified squamous epithelium in 22 cases(84.6%), mixed type in 4 cases(15.4%).

Clinical Study of Inguinal Hernia in Pediatric Patients (소아 서혜부 탈장에 대한 임상적 고찰; 단일병원 10년간 1244예 후향적 고찰)

  • This study is a retrospective analysis of 1244 cases of the inguinal hernia in children under the age of fifteen years who were operated at the department of pediatric surgery, Inje University Busan Paik Hospital from March, 1997 to February, 2007. The ratio of male to female was 3.6:1. The type of hernia was indirect in all of the cases. The hernia was on the right side in 656 cases (53.9 %), left side in 467 cases (37.5 %), and bilateral in 121 cases (9.7 %). The hernia presented most frequently in infants under age 12 months; 364 cases (29.2 %). Fifty-nine cases (21.7 %) were in female and 305 cases (31.3 %) in male. There were 428 cases (33.6 %) in 1-3 years age group, 295 cases (23.7 %) in 4-6 years, 112 cases (9.0 %) in 7-9 years, 39 cases (3.1 %) in 10-12 years and 16 (1.2 %) in 13-15 years. The content of hernia sac was small bowel (59 %), omentum (31 %) in males and the ovary and tube (54 %) and small bowel (26 %) in female. The incidence of combined operation at the time was 3.2 %, and consisting of orchiopexy (67.5 %), frenulotomy (12.5 %), appendectomy (10 %), circumcision (5 %), and fistulotomy (5%). The incidence of combined disease was 2.8 % and consisting of undescended testis, Hirschsprung's disease, idiopathic hypertrophic pyloric stenosis, imperforate anus, and congenital heart disease. After unilateral inguinal hernia repairs, contralateral hernias developed in 34 patients. The laterality of the primary site of hernias were left in 19 cases (55.8 %), and right 15 cases (44.1 %). The 936 cases (75.2 %) were operated under general anesthesia; Mask bagging 663 cases (53.2 %), endotrachea intubation 257 cases (20.6 %), and laryngeal mask 16 cases (1.2 %). The remainder 308 cases (24.7 %) were operated under regional caudal anesthesia.

A Clinical Study of Congenital Muscular Torticollis (선천성 사경 환자에 대한 임상적 고찰)

  • The purposes of this study is to survey the clinical findings of congenital muscular torticollis. The subjects were 44 congenital muscular torticollis patients who were treated at physical therapy department of rehabilitation medicine, presbyterian medical center during the period Jan. Dec. 1991 The results were as follows : 1. Patients were evenly divided by gender(22 bodys, 22 girls). Left-sided involvement was found in 27 cases$(61.4\%)$, and right side in 17 cases$(38.6\%)$. 2. Abnormal delivery was 29 cases$(65.9\%)$. Those included resarean section, breech presentation with vaginal delivery and difficulty delivery. 3. Most of torticollis were primipara infant 33 cases$(75\%)$ 4. Associated congenital anomalies were noted in 12 cases, the most common cases were central co-ordination disturbance. 5. Patients was most commonly referred form the department of pediatrics. 6. Sternocleidomastoid fibrotic nodules were noted in 32 cases$(72.7\%)$. 7. Four cases had excision of the sternocleidomastoid fibrotic nodules in infancy and average at which operation was twelve months of life(range from nine months to fourteen months). 8. Facial asymmetry was noted in 13 cases at the begining of treatment, and the asymmetry was corrected after treatment in 10 cases. 9. The average duration of treatment was 3 months when the treatment was started in 3 weeks old. 10. The result was good in 22 cases$(50\%)$, fair in 12 Cases$(27.3\%)$.

Nose was called "Cheun-bin(天牝)", was outer orifice of the lung, was the gate of vital energe(氣), The main function of nose is divided three pacts. One is the respiration, another is the sense of smell, the other is the sounding consonance. 2. The method on the external treatment of nose diseases is classified with obstructive method, pouring method, blowing method, attaching method, plaster method, rubbing method, cleansing method, poulticing method. In frequency of practical use, Obstructive method is used 276 cases, Blowing method is used 123 cases, Attaching method is used 103 cases, Plaster method is used 76 cases. 4. In frequency of the medicinal works on the external treatment of nose diseases, Herba Asari(細辛) is used 134 cases, Pedicellus Melo(瓜 ) is used 85 cases, Radix Aconiti Praeparata(附子) is used 52 cases, Herba Agastachis(麝香) is used 51 cases, Semen Armeniacae Amarurn(杏仁) is used 44 cases etc. In frequency of a prescription on the external treatment of nose diseases, Tongcho-san(通草散) is used 9 cases, $Sesin-go{\cdot}Sinyi-go{\cdot}Changpo-san$ are used 8 cases, $Kwache-san{\cdot}Wungwhang-san$ are used 7 cases etc.

Family Migration Characteristics and Types of North Korean Defectors (북한이탈주민의 가족이주 특성과 유형)

  • Objectives: This study explores the characteristics of family migration for North Korean defectors and classifies family migration by examining who initiated the migration and who followed. Method: We analyzed the family migration using detailed stories from fifty-five North Korean defectors who were interviewed between 2005 and 2011. Results: We found that 43 out of 55 cases were family migration and the remaining 12 cases were single person migration. We also found several characteristics typical of migration. First, family migration followed the process of step migration, which indicated a serial migration in numbers. Second, migration relied heavily on informal social networks. Finally, the process of earlier migration by North Koreans was incidental and unexpected; however, unexpectedness has diminished in recent migration. Looking at who initiated the migration, the most common type was 'mother-initiated' cases (14 cases) followed by 'child-initiated' cases (10 cases). The third most common type was 'mother-child accompanied' cases (7 cases). The migration process was various; however the most common type was when a married woman initiated the family migration process. This is most likely because married women have the responsibility to support families in the informal economy of North Korea. According to the range of family migrated, the most common type was 'nuclear-family only' cases (22 cases) followed by 'maternal extended family migration' cases (12 cases). Conclusions: The findings of this study provide information on the family dynamics of North Korean defectors.

A STUDY ON POROSITY FORMATION IN COMPOSITE RESIN FILLING MATERIALS (복합(復合)레진충전물내(充塡物內)의 기포형성(氣泡形成)에 관(關)한 연구(硏究))

  • Five different labelled composite resin, Hypol (Korea), Glass-Ionomer (Japan), Simulate (U.S.A.), Durafil (U.S.A.) and Microfil (U.S.A.) were collected to employ for this experimental study. The purpose of the study is to determine the effect of finger pressure technic and traditional filling by means of plastic instruments on porosity formation in composite materials. Each material with pressure and non-pressure cases were carefully examined under electron microscope. Following were the results obtained through the study. 1. Pressured cases showed smaller number of porosity and the size of porosity was significantly smaller than non-pressured cases in each mateial. 2. Glass-Ionomer with pressure cases were seen as tube-like appearance because of pressured small porosities communicate in a certain direction. 3. Durafil cases both pressured and non pressured, appeared the least porosity formation. among the materiats. 4. Border line between the outmost periphery of the porosity and matrix of the resin in both pressured or non-pressured eases were found characteristically clear in Mierofil cases.

The Connectedness between COVID-19 and Trading Value in Stock Market: Evidence from Thailand

  • This study examines the connectedness between the number of COVID-19 cases in Thailand and trading value among investors in the Stock Exchange of Thailand. Daily data of COVID-19 cases and trading value were sourced from the Thailand ministry of public health and the Stock Exchange of Thailand, from January 12, 2020 to May 11, 2021. This study applies a multiple linear regression analysis to explain the relationship between variables. Empirical evidence clearly shows that the volatility of trading value was affected by COVID-19's new, confirmed, and deaths cases within the first pandemic period more than during the second pandemic period. Nevertheless, during the third pandemic period there is no evidence that the new, confirmed, and deaths cases significantly influenced trading value. Furthermore, the results show that COVID-19's new and deaths cases have a negative coefficient that indicated the trading value-buy/sell decreased in response to COVID-19's new and deaths cases, whereas the confirmed COVID-19 cases have a positive coefficient that indicated the trading value-buy/sell increased in response to COVID's confirmed cases. In summary, this study suggests that the number of COVID-19 cases have a significant impact on the trading value in the short term more than in the intermediate and long term.
