  • Life-threatening cardiac arrhythmia is a frequent complication of open heart surgery. There are many causes of postoperative cardiac arrhythmias. Electrolyte imbalance such as hypokalemia and acidemia are major causes of ventricular arrhythmias. Infrequently, however, antiarrhythmic agents and /or hypomagnesemia induce[s] a ventricular arrhythmia such as "torsade de pointes" by increasing the repolarization time of myocardium, Recently, we have experienced two cases of "Torsade de pointes" associated with hyp-omagnesemia after replacement of mitral valve and one of whom after use of procainamide. Intravenous infusion of magnesium immediately and successfully abolished the torsade de pointes in both cases.intes in both cases.

  • Because of recent advances in cardiac surgery and diagnostic techniques, left ventricular-right atrial communication has been reported with increasing frequency. Recently we experienced 2 cases of left ventricular-right atrial communication, which were corrected surgically. Preoperative diagnosis was incorrect In both cases as ventricular septal defect. The type of defect was supravalvular in case 1 and case 2. Both cases were successfully closed with the aid of extracorporeal circulation and discharged with good results.

  • Mediastinal tumors have long fascinated the thoracic surgeon of their variety and unpredictability of iagnosis prior to exploration.We report the analysis of the 34 cases of mediastinal tumors,experenced in the dept.of the Thoracic and Cardiovascular Surgery of the National Medical Center.Between the 1993.9-1992.12. The age distribution was relatively even and the mean age was 35 years old.The thymomas were 16 cases [ 47% ].the lipoma were 4 cases [12%].the germ cell tumors were 7 cases [20%].the neurogenic tumors were 3 cases [8%].Histologically analysised .The malignant tumors were 16 cases [17.6%] in classified by hisotlogical types.the tumor size,location,and the clinical manifestation are presented.The successful removal was done in 28 cases [ 100%] among 28 cases of benign mediastinal tumors. Among the 6 cases of malignancy mediastinal tumors, the surgical intervention had done in 5 cases [ 83%] and inoperatable cases were 1 case [16.6%].There was no operative death.

  • Mucoepidermoid tumors of lung are extremely uncommon, with fewer than 50 such cases having been reported. The degree of tumor malignancy has been discussed by some authors without any definite conclusion. This report describes a case of this, occurred in a 53-year-old female, having a highly malignant process and finally confirmed by postoperative histopathological evaluation.

  • Two hundred and six cases of open heart surgery were done with over all mortality of 8.3% during 1978 in this Department. There were 130 congenital and 76 acquired cases. In congenital anomaly 58 cases were acyanotic and 72 cyanotic, which showed 3.4% and 12.5% of operative mortality respectively. Thirty-six cases of ventricular septal defect and 10 atrial septal defect were repaired without mortality but in 60 cases of tetralogy of Fallot it was 10%. In 76 cases of acquired lesions 74 were valvular cases. There were 75 cases of valve replacement with over all operative mortality of 8%. In 75-valve replacement one case of Ebstein anomaly was found. There were 62 cases of single valve and 13 double valve replacement. In every case tissue valve was utilized and coumadin and Persantin 75 were administered as anticoagulant treatment for one year.

Clinical report of 131 cases of open heart surgery in 1985 (1985년도 연간개심술 131예 보고)

  • 131 cases of open heart surgery were performed in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital in 1985. There were 116 congenital cardiac anomaly and 15 acquired heart diseases. Out of 116 congenital cardiac anomaly, 73 cases of acyanotic group and 43 cases of cyanotic group were noted. In 73 cases of acyanotic group, 17 ASD, 52 VSD and 4 other acyanotic anomaly were noticed. In 43 cases of cyanotic group, 4 Trilogy of Fallot, 34 TOF, 1 Pentalogy, 3 DORV and 1 DCRV were included. Of the 15 acquired valvular heart disease cases, individual incidence was in mitral valve 10, double valve 3, and simple aortic valve 2 cases. Total number of valve replaced was 16, and 13 for mitral, 2 for aortic, and 1 for tricuspid in position, including 1 cases of double valve replacement. Overall operative mortality for 131 cases of open heart surgery was 4.5%, and the operative mortality was 5.5% in congenital acyanotic group, 2.3% in congenital cyanotic group, 0% in TOF group and 6.7% in acquired group.

Congenital Cardiovascular Anomalies in Adults : A report 266 surgical cases (성인 심장기형 266예 수술 예)

  • During the period from 1958 to July 31, 1980, there were 1764 Cardiovascular Surgical cases consisting of 1166 congenital and 598 acquired lesions in the Department of Cardiothoracic Surgery, Seoul National University Hospital. For 1764 cases 1162 open heart surgery and 602 conventional operations were performed. Among 1166, congenital lesions 266 adult cases were found. Adult cases occupy 15.1% of the total cases consisting of 24.5% of acyanotic and 19.9% of cyanotic group. Over all operative mortality of adult cases was 6.8% compared with 12.4% of total cases. Left over congenital cardiovascular anomalies in Korea can be safely operated at the present time. The oldest operated patient among adult congenital cardiovascular anomalies was 57 years old female atrial septal defect case. For 1162 cases open heart surgery bubble oxygenators were utilized except 2 membrane oxygenators. Various kinds of bubble oxygenators have been used but recently Shiley**** oxygenators are being used exclusively with satisfactory results.

  • 103 cases of open heart surgery were performed in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital in 1984. There were 90 congenital cardiac anomaly and 13 acquired heart diseases. Out of 90 congenital cardiac anomaly, 63 cases of acyanotic group and 27 cases of cyanotic group were noted. In 63 cases of acyanotic group, 11 ASD, 45 VSD and 7 other acyanotic anomaly were included. In 27 cases of cyanotic group, 4 Trilogy of Fallot, 15 TOF, 3 Pentalogy of Gasul and 5 other cyanotic anomaly were found. Among 13 cases of acquired heart diseases, 12 valvular lesions and 1 atrial myxoma were noted. Two open mitral commissurotomy and ten valve replacements were performed for 12 valve lesions. The frequent complications were acute respiratory insufficiency and low cardiac output syndrome occurred in 5 cases. The perioperative mortality was 4.8% in acyanotic congenital cardiac anomaly, 7.4% in cyanotic congenital cardiac anomaly, and 0% in acquired heart diseases. Overall mortality for 103 cases of open heart surgery was 4.9%.

Cardiac Surgery : A report of 1640 cases (심장질환의 외과적 치료 -1640 수술예 보고-)

  • From 1958 up to the end of April 1980, during the period of 12 years 1640 cardiac surgery cases including 1069 open heart surgery and 304 valve replacement cases, were operated in this Department. There were 1070 congenital anomaly and 570 acquired disease cases. In 1070 congenital anomaly cases 673 acyanotic and 397 cyanotic anomaly patients were noted. In acquired diseases 94 pericardial and 456 valvular cases were found. Among 456 valve cases 189 mitral stenosis, 133 mitral insufficiency, 30 aortic valve lesion, 97 double valve, and 7 triple valve lesion patients were noted. Among 304 valve replacement cases 209 mitral, 34 aortic, 5 tricuspid, 34 aortic with mitral, 20 mitral with tricuspid, and 2 triple valves were replaced. Annual increase `of open heart surgery cases and decrease of operative deaths were remarkable in recent years. In recent years Shiley** oxygenator in pump-oxygenator set up and Ionescu** bovine pericardial xenograft bioprosthesis were used for valve replacement mainly.

Valve Replacement: Report of 100 Cases (심장 판막 이식 100례 보고)

  • One hundred cases of cardiac valve replacement were done at this Department in the period from June 1968 to May 15, 1978. Seventy-one cases of mitral, 12 aortic, and one tricuspd valve were replaced. There were 16 cases of double valve replacement, 10 aortic with mitral and 6 mitral with tricuspid valve replacement. Prosthetic valves-Beall, Bjoerk-Shiley, Starr-Edwards, Wada-Cutter, Magovern-Cromie, and Smeloff-Cutter valves-were used. But in recent years bioprosthetic valves-Hancock, Carpentier-Edwards, and Angell-Shiley valves-were used mainly due to the difficulties of postoperative anticoagulation, especially for the rural Korean patients. Over all operative mortality was 2896, 26.2% for single and 37.5% for double valve replacement cases. There were 4 postoperative thrombo-embolism cases with 2 deaths. Four postoperative subacute bacterial endocardities cases with 2 deaths were noted. Three cases of postoperative congestive heart failure succumbed. Two cases of peri valvular leakage, one of which needs reopration, were found. There were 28 operative and 9 late deaths, leaving 63 long-tel m survivors, who showed marked improvements.
