Examination and Measures on Failure of Hydraulic Oil Supply Pipe of Control Valve of Steam Turbine in 200MW Thermal Power Plant (200MW급 화력발전소 고압터빈 제어밸브 압유배관의 절손 원인과 대책)

  • A case history is presented pertaining to piping fatigue by vibrations and sustain stresses in the hydraulic oil supply system for control valves in a 200MW thermal power plant that ultimately resulted in pipe rupture. The Piping was designed to supply the hydraulic oil for turbine control valves. Testing and analyses were performed on the system to develop solution to repair work on failures.

Survey on Records about Magnetism in Written Korean History (Records Regarding the Applications of Magnet for Oriental Medicine) (한국사 속의 자기(磁氣) 관련 기록에 대한 조사 연구 (자석의 한방 응용 기록을 중심으로))

  • Until the discovery of universal gravitation, a magnet had been considered to be a unique and magical stuff which exhibited an interacting force between two objects separated afar. Perhaps because of this belief, magnets were used widely as a medicinal stuff in East-Asia. In the present article, the records regarding the applications of a magnet for oriental medicine in written Korean history were surveyed. The records were categorized in terms of efficacy, prescription, and clinical case of taking-medicine. The records revealed that a magnet was used as an important component in the medicine for curing kidney filtering failure. Magnet was also prescribed to heal heart problems. Typical magnet-containing medicines included the SHINRYUKTANG, JIHWANGTANG, and INSAMBAEKHOTANG. Interestingly, it was revealed that some of the kings (Hyojong, Yungjo) in the era of Josun dynasty (1392-1910) believed the efficacy of the magnet-containing medicine and took it for curing their health problems.

Cardiovascular beriberi: rare cause of reversible pulmonary hypertension

  • Cardiovascular beriberi is caused by thiamine deficiency and usually presents as high cardiac output failure associated with predominantly right-sided heart failure and rapid recovery after treatment with thiamine. Because of its rarity in developed countries, the diagnosis can often be delayed and missed. We recently experienced a case of cardiovascular beriberi with pulmonary hypertension which successfully treated with thiamine infusion. A 50-year-old man with chronic heavy alcoholics was refered to our department for dyspnea with mental change. Echocardiography showed marked right ventricular (RV) dilatation and flattening of the interventricular septum with a D-shaped deformation of the left ventricle. Moderate tricuspid valve regurgitation was found and estimated RV systolic pressure was 52 mm Hg. Because of his confused mentality and history of chronic alcohol intake, neurological disorder due to thiamine deficiency was suspected and intravenous thiamine was administered and he continuously received a daily dose of 100 mg of thiamine. Follow up echocardiography showed marked reduction of RV dilatation and improvement of a D-shaped deformation of the left ventricle. He finally diagnosed as cardiovascular beriberi on the basis of dramatic response to intravenous thiamine. Thiamine deficiency can cause reversible pulmonary hypertension, and can still be encountered in the clinical setting. Thus high index of suspicion is critically needed for diagnosis.

A Pediatric Case of Toxic Hepatitis Induced by Hovenia Dulcis

  • Toxic hepatitis is a rare but devastating disease in children. Herbs are widely used in oriental medicine to treat various symptoms in Korea, however, several herbs have been reported to induce liver injury. We report a case of toxic hepatitis induced by Hovenia dulcis in a 3-year-old boy. He complained of nausea, abdominal discomfort, and jaundice. The patient had consumed water boiled with hovenia dulcis for about 1 year prior to presentation. A diagnosis of toxic hepatitis was made based on his history, laboratory data, viral markers, ultrasonography, and biopsied liver tissue. We administered supportive management for acute fulminant hepatitis but his symptoms and liver function progressed. He was transferred to another hospital for further evaluation and consideration for liver transplantation. Because acute liver failure due to herbs or dietary supplement taken for a long time is often fetal, it is important to make early diagnosis and stop taking the drug as soon as drug induced liver injury is suspected.

A Case of Intestinal Malrotation with Midgut Volvulus Presenting with Intermittent Vomiting and Abdominal Pain (간헐성 복통과 구토를 주소로 하는 중장염전이 동반된 장 이상회전 1례)

  • Intestinal malrotation is a developmental anomaly resulting from embryologic failure of fixation and rotation of the gut and predisposed to midgut volvulus and small bowel obstruction. Acute midgut volvulus is most often encountered in the newborn period. But older children and adults may have a history of intermittent episodes of partial volvulus presenting with recurrent colicky abdominal pain accompanying bilious vomiting. We experienced a case of intestinal malrotation complicated by midgut volvulus presented with recurrent vomiting and abdominal pain in a 6-year-old boy.

Retroperitoneal Hematoma as a Serious Complication of Endovascular Aneurysmal Coiling

  • Retroperitoneal hematoma (RH) due to radiologic intervention for an intracranial lesion is relatively rare, difficult to diagnose, and can be lifethreatening. We report a case of RH that developed in a patient on anticoagulant therapy following endovascular coiling of a ruptured anterior communicating artery (AcoA) aneurysm. An 82-year-old man presented with a 12-day history of headache. Computed tomography (CT) on admission demonstrated slight subarachnoid hemorrhage, and left carotid angiography revealed an AcoA aneurysm. The next day, the aneurysm was occluded with coils via the femoral approach under general anesthesia. The patient received a bolus of 5,000 units of heparin immediately following the procedure, and an infusion rate of 10,000 units/day was initiated. The patient gradually became hypotensive 25 hours after coiling. Abdominal CT showed a huge, high-density soft-tissue mass filling the right side of the retroperitoneum space. The patient eventually died of multiple organ failure five days after coiling. RH after interventional radiology for neurological disease is relatively rare and can be difficult to diagnose if consciousness is disturbed. This case demonstrates the importance of performing routine physical examinations, sequentially measuring the hematocrit and closely monitoring systemic blood pressures following interventional radiologic procedures in patients with abnormal mental status.

  • Fanconi syndrome is a generalized functional disorder of the proximal tubule of the kidney and is characterized by aminoaciduria, glycosuria, hyperphosphaturia, dehydration, rickets, and growth failure. Nephrocalcinosis and hypercalciuria are rare manifestations of Fanconi syndrome. There is no case report of Fanconi syndrome complicated with nephrocalcinosis and hypercalciuria in Korea. A 6-year-old boy presented with genu valgum and waddling gaits for about 3 years. There was no family history of renal disease and his physical examination was normal except for genu valgum and corrected cleft lip and palate. Laboratory investigations showed generalized aminoaciduria, glycosuria, hyperphosphaturia, hypercalciuria, and low-molecular weight proteinuria including ${\beta}$-microglobulin. Serum 25-OH vitamin $D_3$ was within the normal range, and $1,25-(OH)_2$ vitamin $D_3$ was elevated. Bilateral renal medullary hyperechogenicity was demonstrated by ultrasonography. Analysis of the CLCN5 gene revealed no mutation. Here we describe a boy with Fanconi syndrome complicated with nophrocalcinosis and discuss the differential diagnosis.

Calcaneal Insufficiency Avulsion Fracture in a Well-Controlled Type 2 Diabetic Patient: A Case Report

  • The calcaneal insufficiency avulsion fracture usually occurs in an area of fused apophysis in adults without significant history of trauma or overuse activities. It is an uncommon injury which has been described in patients with complicated diabetes, Charcot neuroarthropathy, amyloidosis with neuropathy, severe osteoporosis, and other conditions. Discussion of the issue of fracture location is still not sufficient. We report on a case of a 50-year-old male who experienced a non-traumatic diabetic calcaneal insufficiency fracture. Intraoperatively, a biopsy specimen was obtained from the exposed fracture site for histological study. We assume that the calcaneal fused apophyseal line is the weak point of failure due to various incomplete mixtures of trabecular bone, woven bone, and cartilaginous tissues, and may fail when repeated tensile stress is imposed.

  • This paper presents a case history of a geosynthetics-reinforced segmental retaining wall, which collapsed during a sever rainfall immediately after the completion of the wall construction. In an attempt to identify possible causes for the collapse, a comprehensive investigation was carried out including physical and strength tests on the backfill, stability analyses on the as-built design based on the current design approaches, and slope stability analyses with pore pressure consideration. The investigation revealed that the inappropriate as-built design and the bad-quality backfill were mainly responsible for the collapse. This paper describes the site condition including wall design, details of the results of investigation and finally, lessons learned. Practical significance of the findings from this study is also discussed.

  • The history of mankind's lunar exploration began in 1958 with the United States of America "Pioneer 0" mission. In 1950s~1970s, the United States of America and Union of Soviet Socialist Republics carried out missions and experienced numerous failures to explore the moon. Since the 1990s, Japan, Europe, China and other Advanced country in Space technology have started to explore the moon and in 2016, Korea began to develop the lunar orbiter for lunar exploration. This paper analyzed the failure cases and causes of the lunar exploration in the USA and the USSR in the 1950s~1970s according to the mission purpose. Examples of mission delays, cancel, and failures that occurred during lunar exploration in post-1990s were presented. Through the investigation and analysis, this paper is intended to serve as a reference of the lunar exploration mission that Korea is working on or will be performing in the future.
