The Oral Care Methodes for the Disabled (임상가를 위한 특집 3 - 장애인 구강관리법)

  • In the welfare law for the disabled in Korea, the disabled is defined as "the persons sho have significant limitations of daily or social life due to physical or mental disabilities over a long period." The number of registered domestic disabled person in 2008 survey was approximately 2.1 million. 90% of the disabled was caused by the aposteriori illness or accidents. So, in this aging society, the number of people with disabilities will increase even more. In general, people with physical and mental disabilities tend to have more oral diseases, but, are difficult to get treated. Therefore, a thorough oral care through individualized prevention is more important than ever. In this article, we review the oral care methods in dental office and home and in the hospital for the elderly and finally focus on a denture care method. According to this article, the dental professionals can be able to select and apply the oral care method in agreement with the characteristics of the disabilities. Particularly, it is important to prevent the aspiration pneumonia in the elderly in hospital.

Development of Evidence Based Clinical Practice Guideline to Improve Quality of Critical Care Nursing (중환자 간호실무 질 향상을 위한 근거중심 임상실무지침서의 개발)

  • Evidence based clinical practice guideline are designed to help healthcare practitioners evaluate and implement the increasing amount of evidence on best practice. Critical care area is one of the clinical sites where evidence based clinical practice guideline is needed most. This paper reviewed the definition of evidence based clinical practice guidelines, the development method of evidence based clinical practice guideline, and the current trends in guideline developing. Traditional method of guideline development is consensus based but it moves into evidence based development. Evidence based guideline is based on best available evidence and uses the strongest method to determine its effect on clinical outcomes. The current trends in guidelines is to develop the guideline at regional/national level and do subsequent modification to suit local circumstances. There is an urgent need of exploring the method of guideline development and adaptation which are appropriate for Korean clinical setting.

A Study of Staffing Estimation for Nursing Manpower Demand in Hospital (병원간호인력의 수요추정에 관한 연구 -환자분류체계에 의한 간호인력 수요추계의 방법을 중심으로-)

  • Changing concepts of health care, are stimulating the demand for health care, thereby orienting society to health care rights to such an extent that they are deemed as fundamental ones inalienable to man. Concomitantly, qualitative as well as quantative improvement is being sought in the nursing service field. Today, efforts are being made in various areas, especially to qualitatively improve nursing services. A second issue concerns proper staffing. It is important to study staffing, in as much as it continues to be the most persistent and critical problem facing hospital nursing administrators today. It involves quantity, quality, and utilization of nursing personnel. A great deal of attention has been focused on this problem since mid 1930's when nursing services began to be felt as an important segment of hospital operation representing the largest single item of hospital budgets. Traditionally, the determination and allocation of nursing personnel resources has relied heavily on gloval approaches which make use of fixed staff-to-patient ratios. It has long been recognized that these ratios are insensitive to variations between institutions and among individual patients. Therefore, the aim of this thesis is to point to the urgent need for the development of methodology and criteria suited to the reality of Korea. The present research selected one place, the W Christian Hospital, and was conducted over a period 10 days from January, and nurses who were them on duty in their unit. The total num-her of patients surveyed was 1,426 and that of 354. The research represents many variables affecting the direct patient care time using the result from the direct observation method, then using a calculation method to estimate the relationship between the patients care time and selected variables in the hospital setting. The amount of direct patient care time varies with many factors, such 89 the patients age. diagnosis and time in hospital. Differences are also found from hospital, clinic to clinic, ward to ward, and even shift to shift. In this research, the calculation method of estimating the required member of nursing staff is obtained by dividing the time of productive patient care activity(with the time of patient care observed), by the sum of the productive time that each the staff can supply, i.e., 360 minutes, which is obtained by deducting the time for personal activities. The results indicate a substantial difference between the time of productive patient care observed directing and the time of the productive patient care estimated using calculating method. If we know accurately the time of the direct patient care on a shift, there required number of staff members calculated if the proper method can be determinded should be able the time of the direct patient care be estimated by the patient classification system, but this research has shown this system to be in accurate in Korea. There are differences in the recommended time of productive patient care and the required number of nursing staff depending upon which method is used. The calculated result is not very accurate, so more research is needed on the patient classification system.

Clinical Experience of Nurses in a Consultative Hospice Palliative Care Service

  • Purpose: The purpose of this qualitative study was to employ Colaizzi's phenomenological research method to elucidate and understand the essence of practical experiences among consultative hospice palliative care nurses working in hospice institutions. Methods: The participants in the study were 15 consultative hospice palliative care nurses with over 1 year of work experience in institutions located in S City, I City, and K Province in South Korea. Data were collected from 23 in-depth interviews and analyzed using Colaizzi's phenomenological qualitative method. Results: The practical experiences of consultative hospice palliative nurses were categorized into five categories, 10 theme clusters, and 25 themes. The five categories included "being aware of patients' situations at the time of transition to hospice palliative care," "empathizing with patients and their families by putting oneself in the other's shoes," "providing patient and family-centered end-of-life care," "experiencing difficulties in practical tasks," and "striving to improve hospice service quality." Conclusion: This study is significant in that it provides practical data for understanding the experiences of consultative hospice palliative care nurses caring for terminally ill patients. This could enhance our understanding of care solutions that effectively tackle the challenges consultative hospice palliative care nurses encounter while fulfilling their roles.

A Study of Understanding and Use State of Health Care Methods in Some Rural Area (일부 농촌지역의 의료방법에 관한 인식 및 이용실태)

  • A survey was made with regard to understanding attitude and practical use of modern and traditional methods of health care in a rural village with 742 inhabitants which comes under one of the whole three types of Korean rural community, to find out the following facts. 1. Among the five types of health-care given, the most understood, chosen and used one was the doctor followed next by pharmacy and continually herb doctor, folkways method, shamanistic treatment; the last shown method was principally chosen to use by only one among 139 housewives as respondants available, 2. Between herb medicine and folkways method of care, the former was more relief upon than the latter while the latter was more used than the former, seemingly because of relative low cast of the latter. 3. A considerable part of those who mainly used the doctor or pharmacy also used herb doctor or folkways care; modern and traditional care methods coexisted in the pertaining community. 4. Nearly all of the respondants ware, reasonably, aware of effectiveness of modern types of health-care; but, at the same time, more than a half of them had a positive attitude towards traditional method as well. 5, On the whole, the respondants made a distinction between traditional health-care methods and shamanism, regarding the latter as ineffective. 6. The result hints that it would be desirable for health-care professionals to inquire into traditional methods of health-care with a view to being more acquainted with rural residents and so raising their use of services by experts. 7. It should be added that the results of this piece of study may have a general significance only within those communities which shares the same socio-cultural background and characteristic of the surveyed one.

Consumers' Attitude toward Care Label Instructions and Care of Winter Outerwear (겨울 아우터웨어의 품질표시에 대한 소비자 태도와 관리)

  • Winter outerwear includes various clothes such as classical jackets/coats, padded jackets/coats, wind breakers, leather and fur jackets/coats. This research surveyed care-label instructions attached to 100 jackets/coats dropped off at five drycleaners in the Gyungin area. University students' perception and attitudes towards care labels and management of winter outerwear were examined using a questionnaire. Data collected from 230 respondents were analyzed by frequency analyses, t-tests, ANOVAs and Duncan tests with PASW 20.0. Half of the padding jackets/coats were labelled to be dry cleaned and dry cleaning was more excessively labelled than necessary. University students confirmed more care symbols than fiber compositions such as more at time of cleaning than at purchasing. Clothing-related major students understood care label symbols better than non-clothing major students. Consumers laundered some winter outerwear at home, even though they thought dry cleaning was a proper cleaning method. The appropriate cleaning method for padded jackets/coats is laundering; however, consumers who answered dry cleaning as an appropriate method were three times as those who answered washing, and those performed dry cleaning were two times as those who performed washing. Winter outerwear manufacturers should provide adequate and more specified care-information to consumers in order to maintain initial appearance and performance over longer periods.

The Effect of Payment Method of Community Medical Provider on Medical Care Use of Community Residents (지역사회 의료공급자의 지불보상체계상의 특징이 지역사회 주민의 의료이용에 미치는 영향: 미국사례분석)

  • Due to the existence of asymmetry of information between doctor and patient, it has been believed that doctor might affect patient's decision making process of purchasing medical care. Based on this notion, doctor's reimbursement method has been suggested as an effective policy device of improving efficiency of patient's medical care use by way of its affecting doctor's practice pattern. By using the Community Tracking Study (CTS) household and physician data set, which includes not only various information on patient's medical care use, but doctor's practice arrangements and sources of practice revenue, this paper investigates the effect of community doctor's characteristics of reimbursement method on community patient's medical care use under the control of patient's socio-demographic characteristics and community doctor's practice type. In the process of estimating econometric model, the endogeneity problem of individual health insurance purchase was corrected by using 2818. And due to the existence of sample selection problem, Heckman's two-step estimation method was used for strengthen the robustness of estimation which was adversely affected by sample selection problem The empirical results show that as the average value of community doctor's portion of practice revenue determined by prospective method out of total revenue increases, the community patient's total out-of-pocket medical cost decreases. This results suggest, as doctor's practice revenues are mainly determined by prospective method, such as capitation, doctors would be more conscious about practice cost, which might affect doctor's practice pattern and by which his/her patient's use of medical care would decrease.

A Qualitative Content Analysis of the Person-Centered Care Experienced by Critical Care Patients (중환자실 환자가 경험한 인간중심 간호에 관한 질적 내용분석)

  • Purpose : To explore the person-centered care (PCC) experienced by critical patients. Method : This qualitative study deductively examined the attributes of the PCC model proposed by Jakimowicz and Perry. The participants were 16 patients who were recently discharged from intensive care units at a university hospital. Data were collected through in-depth interviews and were analyzed using the deductive method of content analysis. Results : A total of 4 categories, 16 subcategories, and 33 codes were generated from 171 meaningful statements. The final 4 categories were "compassionate presence," "professional interaction," "outstanding competency," and "patient identity." These were consistent with the main attributes of Jakimowicz and Perry's model. However, most of the codes belonged to the "compassionate presence" and "patient identity" categories. Among the attributes of the model, "continuity of nursing," "therapeutic relationship," "expert knowledge," "clinical knowledge," "evidence-based intervention," and "patient's rights" were not derived as codes. Conclusion : These findings deepen the understanding of the PCC model from the patient's point of view. The main attributes of PCC identified in the current study can be applied to the development of practical guidelines for intensive care nursing. In addition, we recommend the development of a PCC measurement tool for critical care patients.

Estimation of Willingness to Pay for Long-Term Care Insurance Using the Contingent Valuation Method (조건부가치측정법을 이용한 노인장기요양보험에 대한 지불의사금액 추정)

  • According to rapid increase of the population of senior citizens, there has been growing concern of Long-Tenn Care(LTC) services recently. Long-Tenn Care services, however, haven't been established systematically in Korea and the supply of LTC services is not sufficient despite the increase in the current social demand. This study aims to estimate the 'Willingness to Pay(WTP)' for LTC insurance which the government plans to introduce by means of social insurance, using Contingent Valuation Method(CYM). In addition, this study analyzes the factors affecting WTP for LTC insurance. An interview survey was carried out to derive WTP for LTC from 450 people who lived in Seoul aged 20 and above during the period from 16th to 21st of June 2003. Double-Bounded Dichotomous Choice Method was applied among several CVMs available to estimate both use value and no-use value of goods. There was pilot survey carried out prior to the main survey. The results show that the average monthly. WTP for LTC provided in home and residential setting is 18,192Won and 19,293Won, respectively. In the case of home care, WTP goes higher depending on reliability of LTC insurance policy and need for LTC insurance, as well as marital status, education and average monthly income. On the contrary, WTP is conversely affected by higher age and higher bids. In the case of institutional care, the factors affecting WTP are similar to those of home care, except age. This study followed NOAA's suggestions generally and the value derived through survey could be reliable. However, there can be the least bias in the process of survey because the CVM should be used under the supposed circumstances. Despite those limitations, it can be concluded that the amount the citizens are willing to pay for LTC is high enough to meet the costs needed to provide LTC.

Effect of the tooth pick of the disabled person oral treatment (Toothpick method(와타나베법)을 이용한 장애인 구강건강관리 효과)

  • This study conducted a 'specialist tooth brushing' method against the severely disabled once every other week. The purpose of this study is to analyze and validate the effectiveness of maintaining healthy periodontal management when consistent oral hygiene lasts with minimum stimuli for 24 - months. The conclusions were as follows. Table 2 compares the difference between group 1 and 2 from the 1st to 11th management and verifies the actual difference in measurement of each index. Group 1 used a professional tooth brush while Group 2 used a general brush. There were found many significant differences in dental index. As for PHP, Oral Malodor index and gingival bleeding, it was found that one or two times of dental care can make substantial differences in dental health condition. These results show that as the number of toothbrush method has been increased, gingival bleeding and periodontal index are significantly improved, not to mention the improvement of gingivitis and periodontal disease. This is because periodontal tissue is affected by brushing method, brushing time and consistency of dental care. During the 6-month period, 20 minutes of active periodontal care was intensively conducted, and for 18-months the effect of consistent care was verified by 10-minute periodontal care once every other week. As a result of the test, it was found that there was no complete regression in the basic periodontal treatment and the periodontal health condition had been maintained for 24 months. Also this test shows that, despite of inconsistent dental hygienic care, regular plague control can prevent dental diseases and maintain the dental health. This study proved that periodontal condition can be maintained by periodontal care once every other week as the tooth brushing properly stimulates the gums with positive effect. Therefore, professional dental healthcareworkers should be designated for each facility for the disabled, and dental health of the disabled should be professionally cared on a regular basis, and consistent and repetitive management by the dental care specialist are required.
