Development and Evaluation of an e-Learning Program for Mothers of Premature Infants (e-Learning을 이용한 미숙아 어머니 교육 프로그램 개발 및 평가)

  • Purpose: It has been attempted to support mother of premature infants by providing information of premature infant care using e-Learning because premature infants need continuous care from birth to after discharge. Method: The e-Learning Program for mother of premature was developed with Xpert, Namo web editor, Adobe Photoshop, and PowerPoint and applied for 4 weeks from 4 to 30 September 2006. Result: 1) We found that the contents of information which premature infants' need when being in the hospital and after discharge were the definition of a premature infant, orientation of NICU, care of premature infants, care of premature infants' common diseases, the connection of healthcare resources, exchange of information, and the management of rearing stress. 2) The program content consisted of cause of premature birth, comparison to full-term baby, physiology character, orientation of NICU, common health problems, follow up care, infection control, feeding, normal development physically and mentally, weaning method, and vaccination. Conclusion: Considering the results, this program for mother of premature is a useful means to provide premature-care information to mothers. This information can be readily accessible and can be varied and complex enough to be able to help mothers to the information and assistance they require.

Married Female Migrants' Experiences of Health Care Services (여성결혼이민자의 의료서비스 이용경험)

  • Purpose: The purpose of this study was to describe married female migrants' experiences of health care services and to help nursing researchers, nursing educators, and clinical nurses understand married female migrants' experiences. Method: A conventional content analysis method was utilized. Individual in-depth interviews with 15 married female migrants were conducted. Subject recruitment was performed at a district in Seoul. Results: Ten categories were induced: language barrier, financial burden, insufficient time with a physician, complexity of utilization process, lack of support from peer group, health care providers' discrimination, anxiety regarding lack of information about children's health, health care providers' concerns and efforts to minimize the language barrier, family support, and advanced health care service environment. Conclusion: This study provides basic knowledge regarding married female migrants' experiences related to health care services. Future research should designate and utilize valid instruments to measure the positive and negative experiences and to explore strategies to strengthen the positive features while eliminating the negative ones. Finally, the Korean nursing education curriculum should include cultural competence and knowledge about an ethnic minority's right to health service utilization.

An Attempt of Reinterprtation on History of Nursing and Health Care Using Post-structural Method (포스트구조주의적 분석을 이용한 간호와 보건의료의 역사에 대한 재해석의 한 시도)

  • The purpose of this study is to reinterpretate the history of nursing and health care from the view-point of post-structuralism. It has been emphasized that the development of modern health care has been due to the progressive efforts of medicine and to medical discoveries. Medicine has dominated the history of health care rather than nursing or other health professions. The present study adopts the post-structural method by Foucault, which tries to unite language and knowledge. Foucault examines "the institutionalization of knowledge and the power exerted thereby, with special reference to the devices of social regulation and their function over the madness, the disease, the crime, and the sexuality. " The concept of power in Foucault's writing is that it is exerted spontaneously in verbal behaviors of individuals through knowledge of everyday life such as definition of body or mind, sexuality and relationship of family. Therefore as to the problem of power, this study tries to understand the meaning of the health care history through an analysis of the formation of medical discourse. In order to have authority in a power relation, the medical professional asserts that medical discourse is the most scientific knowledge. The authority of medical professionals can be reinforced by the fact that male medical professionals outnumber female. Devaluation of nursing care is reinforced by the medicine which has the legitimate authority through use of the political skills.

Experience of the Role Conflict of Nurses in Long-term Care Hospitals (요양병원 간호사의 역할갈등 경험)

  • Purpose: The purpose of this study was to describe the experience of the role conflict among nurses in long-term care hospitals. Method: Data were collected through in-depth interviews with nine nurses in long-term care hospitals. Colaizzi's phenomenological method was used for data analysis. Result: In this study, five categories and twelve theme clusters were generated. The five categories consisted of 'Heavy workload,' 'Nursing job delegation of unclear regulations,' 'Unprotected doctor's job performance,' 'Non-systematic nursing assistants management' and 'Limit of competency.' Conclusion: These results will contribute to the role establishment of nurse in long-term care hospitals and the qualitative improvement of work by providing the grounds for the strategy development which improve working environment and job satisfaction of nurses in long-term care hospitals apposite to the case of Korea.

The Effects of Follow-Up Care on Social Support, Self-esteem and Maternal Confidence in Low Birth-weight Infant's Mothers (추후관리를 받은 저출생체중아 어머니의 양육자신감, 사회적지지 및 자아존중감 비교)

  • Purpose: The purpose of this study was to examine the effect of follow-up care on the social support, self-esteem and maternal confidence in mothers of low birth weight infants. Method: This study applied a quasi-experiment study method to examine the pre-post intervention effects. Eighty-eight mothers whose infants were below 2.5kg of Low birth weight were studied. The period for the data collection was from July to December in 2003. Self Reported Survey and Face to Face Interview by the highly trained home Visiting Nurses were used for this study. Result: Most of the participants reported statistically significant improvements in the score of social support, self-esteem and maternal confidence after they received the intervention. Conclusion: Longitudinal study for the mothers of low birth weight infants might reveal more comprehensive findings. Moreover, follow-up care, which is linked with community health care services, is needed to continue the post hospital care services. Additionally, qualified and professional intervention programs should be provided to consider the characteristics of mothers of low birth weight infants.

The Effect of Activity of Daily Living Exercise for Self-care Ability and Family Burden in Stroke Patients (일상생활동작 훈련이 뇌졸중 환자의 자가간호 수행능력과 가족의 부담감에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effect of ADL exercise in stroke patients and to define to strategy to promote their self-care ability, decrease to their family burden. Method: The experimental design was designed nonequivalent control group non-synchronized design. The study method had been done by investigating the experimental group and control group through the questionaire on 33 patients. ADL exercise was conducted by the researcher and was carried out experimental group once per day for 20 minutes for daily 28 days. Results: For the ADL exercise, self-care ability score was increased and family burden score was decreased significantly. Conclusion: The exercise increased the self-care ability and effect of family burden of stroke patients. Based upon these results, it is recommended that the nurses who take care of stroke patient carry out them the ADL exercise continuously.

Delirium Experience of the Intensive Care Unit Patients (중환자실 환자의 섬망 경험)

  • Purpose: The study aimed to understand the delirium experience of intensive care unit (ICU) patients. Methods: We performed a qualitative study using Colaizzi's phenomenological method. Eleven patients, who experienced delirium according to the Confusion Assessment Method for ICU, participated after transferring to general wards from the ICU. Individual in-depth semi-structured interviews ranging from 30 minutes to 2 hours in length were conducted between November 2018 and August 2019. Results: Nine themes and four theme clusters emerged. The four theme clusters were: 1) "Overwhelmed by fear," which describes the experience of a patient close to death and the feeling of difficulty in understanding disorganized thinking; 2) "Anxious about not understanding the situation," which means that patients' sense of time and space were disordered in the ICU; 3) "Being deserted," which indicates the feeling of being separated from others and yourself; and 4) "Resistance to protect my dignity," which indicates that the dignity and autonomy of an individual in the patient's position at the ICU, are ignored. Conclusion: Nursing interventions are needed that would enable patients to maintain orientation and self-esteem in the ICU. In addition, healthcare providers need to provide information about the unfamiliar environment in the ICU in advance.

Factors Influencing the Preventive Behaviors of COVID-19 by Health Care Workers: A Mixed Methods Study (보건의료인의 코로나19에 대한 예방행위 영향 요인: 혼합연구방법)

  • Purpose: The purpose of this study was to identify the risk perception, safety climate and preventive behaviors of COVID-19 infections, and to verify the factors influencing the adoption of preventive behaviors by health care workers. Methods: A mixed-method approach was used based on a survey carried out in a general hospital. Quantitative data (N=181) were collected through a questionnaire and analyzed using multiple regression. Qualitative data (N=8) were collected through individual interviews and analyzed through Colaizzi's phenomological research method. Results: The factors influencing adoption of preventive behaviors by health care workers were safety climate (β=.41, p<.001), education (β=-.20, p=.025), work unit (β=-.16, p=.032), and risk perception (β=.15, p=.020). The explanatory power was 28.3%, and they have shown that these health care workers felt 'the threat of a pandemic like a battlefield', experienced 'struggle with quarantine rules' and realized 'the reality of infection control and the reorganization for change'. Conclusion: The findings of this study indicate that to increase the adoption of infection control preventive behaviors by health care workers when faced with new infectious diseases in the future, it is necessary to establish organizational support and a safe climate. This study confirms the need for preemptive support and education.

  • Basel committee suggests using Value-at-Risk (VaR) and expected shortfall (ES) as a measurement for market risk. Various estimation methods of VaR and ES have been studied in the literature. This paper compares semi-parametric methods, such as conditional autoregressive value at risk (CAViaR) and conditional autoregressive expectile (CARE) methods, and a Gaussian quasi-maximum likelihood estimator (QMLE)-based method through back-testing methods. We use unconditional coverage (UC) and conditional coverage (CC) tests for VaR, and a bootstrap test for ES to check the adequacy. A real data analysis is conducted for S&P 500 index and Hyundai Motor Co. stock price index data sets.

A proposal of an assessment method of physiologic oral malodor care prognosis by a case report (증례 보고를 통한 생리적 구취증 관리의 예후 평가 방법 제안)

  • Authors have selected a physiological bad breath patient( 62 years old, male ) among the bad breath outpatients who have visited the halitosis control clinic in Korea University Medical Center(KUMC). The patient visited the halitosis control clinic for his oral malodor control, 3 times from April to June in 2018, and in August the patient visited to KUMC malodor control clinic again for his assessment of his two months efforts. Getting the data about the patient's endeavor to get over his physiologic oral malodor and the estimation of the patient's satisfaction level at his oral malodor improvement by a questionnaire method, and the organoleptic level assessment by the dentist, then we could propose an estimation method of the physiologic oral malodor patient care prognosis.
