Objectives : There has been substantial evidence that patients with chest pain have depression and anxiety, and show impaired quality of life (QoL). This study aimed to campare the QoL according to types of chest pain and to examine the impact of depression and anxiety on QoL in patients with chest pain. Methods : Forty-seven patients with chest pain were divided into Cardiac-Typical Chest Pain (CTCP, n=22) and Non-Cardiac-Atypical Chest Pain groups (NCACP, n=25) according to the pain characteristics and cardiovascular disease. Patients were assessed for depression using the Beck Depression Inventory (BDI), for anxiety using the State-Trait Anxiety Inventory (STAI), and QoL was assessed using the Korean version of the SmithKlein Beecham 'Quality of Life' Scale (KvSBQOL). Results : Compared with the CTCP group, the NCACP group reported significantly higher anxiety, and lower QoL. There was no significant difference in QoL between the two groups after adjusting for anxiety. The QoL was associated with depression and trait-anxiety in the CTCP group, and with trait-anxiety in the NCACP group. Conclusion : The findings suggest that there are different effects of depression and anxiety on QoL in individuals with CTCP and NCACP. Understanding about these differences can be important in the treatment of patients with chest pain. A large prospective study is needed to confirm these results.
Glycyrrhetinic acid, which is a component of licorice, can cause hypermineralocorticoidism through the inhibition of $11{\beta}$-hydroxysteroid dehydrogenase. So, a high dose intake of licorice can lead to hypermineralocorticoidism with potassium loss and depression of the renin-angiotensin-aldosterone system. We report a 73-year-old man with muscle weakness of proximal lower limbs with hypokalemia (Serum $K^+$: 1.4 mEq/L) due to taking self-prescribed licorice without OMD's diagnosis. He boiled 60~100g licorice / day in water and drank it intermittently for 1 month due to arthralgia and swelling of both his knees. Patient's serum renin activity and aldosterone level were far beyond the normal range. He also had metabolic alkalosis with pH 7.552. After quitting the licorice, hypokalemia and muscle weakness of proximal lower limbs gradually improved within 1week.
이 연구에서는 절명상 프로그램이 가진 보완대체의학으로서의 가능성을 알아보기 위하여 성인 여성을 대상으로 8주 간의 절명상 프로그램을 시행하고 스트레스, 우울 및 심혈관계 위험 인자에 미치는 영향을 조사하였다. 일개 요양병원에 근무하는 성인여성 60명 중 탈락기준을 통과한 57명을 대상으로 무작위대조군 실험을 진행하였다. 연구대상자들은 실험 전후에 자기기입식 설문을 통해 스트레스, 우울을 단축형 사회심리적 건강 측정도구(PWI-SF), Beck 우울척도(BDI)로 각각 조사하였고, 체질량지수, 허리둘레, 혈압, 당화혈색소, 항상성 모델 평가(homeostasis model assessment, HOMA), 저밀도지단백 콜레스테롤, 고밀도지단백 콜레스테롤, 중성지방을 측정하였다. 실험군에게 스트레칭, 절명상, 정좌명상으로 구성된 절명상 프로그램을 8주간 시행하였으며, 대조군에는 아무런 중재를 하지 않았다. 제외기준을 통과한 57명 중, 실험군 26명, 대조군 27명이 실험을 완주하였고, 실험군은 일주일에 평균 5.16회 절명상 프로그램을 시행하였다. 8주간의 실험 후, 실험군은 스트레스(t=5.102, p<0.01), 우울(t=5.259, p<0.01), 체질량지수(t=2.942, p=007)와 허리둘레(t=2.582, p=0.016)가 유의미하게 감소하였으나, 대조군은 유의한 변화가 없었다. 그 외의 변수에서는 실험군과 대조군 사이에 유의미한 차이가 없었다. 8주 간의 절명상 프로그램 시행 후, 시행 전에 비하여 실험군에서 스트레스, 우울 및 체질량지수, 허리둘레가 유의하게 감소되었고, 변화량의 차이는 대조군과의 비교에서도 유의하였다. 이는 절명상이 심신의학의 한 방법이 될 수 있음을 시사한다.

Flail chest is a critical medical condition in which multiple segmentally fractured adjacent ribs cause paradoxical movement of the thoracic cage in patients with severe blunt trauma injury. Surgical stabilization is considered essential in patients who require mechanical ventilation. However, there is no consensus on which surgical procedure to choose among the various available techniques or when to perform surgery. We report the case of a patient with traumatic anterior flail chest due to bilateral multiple fractures of the ribs requiring surgical stabilization in whom weaning from mechanical ventilation had failed. The Nuss procedure using double bars with the bridge technique was performed for chest wall stabilization. The patient was weaned from mechanical ventilation on postoperative day 44 and she underwent bar removal on postoperative day 71. After extensive rehabilitation for multiple trauma, she was discharged successfully. The patient currently shows no recurrence of chest wall depression in outpatient follow-up.
Of all pesticides, carbamates are known to be most common, since alternatives such as organophosphates have long lifetime and are extremely toxic to produce a delayed neurotoxic effect. Although a number of studies about toxicity of carbofuran, a most widely used carbamate, have been reported, its cardiovascular toxicity has not yet been studied. In the present study, we investigated its cardiovascular toxic effect in anesthetized rat in vivo and in isolated Langendorff rat heart, In anesthetized rat model, carbofuran (10 mg/kg) significantly reduced heart rate, and transiently increased blood pressure. In isolated rat heart, carbofuran (10${\mu}{\textrm}{m}$) caused a significant depression in the left ventricular developed pressure (LVDP), indicating contractile dysfunction by carbofuran. Carbofuran (10${\mu}{\textrm}{m}$) also decreased coronary flow rate (CFR) in isolated heart, indicating carbofuran-induced coronary dysfunction. These results suggest that carbofuran can cause cardiac dysfunction in rat in vivo and vitro.
From Jan. 1981 to Dec. 1989 eleven cases of Funnel Chest, ten were males and one was female, were underwent an operation at the Department of Thoracic and Cardiovascular Surgery. Cheonbuk National University Hospital. The age of patients ranged from 3 to 29 years old. They all had symptoms of feeling inferiority about chest deformity. The degree of concavity on the funnel chest varied in extent, and the severity which was measured by water volume filled into it varied from 20 ml to 140 ml. Nine patients were corrected by Ravitch method and two patients were corrected by the Modified sternal turnover method. The Ravitch method was more effective in asymmetrical, severe depression deformity and in children. On the other hand the Modified sternal turnover method with preservation of vascular supply of repair was more simple and more effective in case of relatively less severe, wide symmetrical depression deformity of the chest and in case of associated with congenital or acquired heart disease and disease of the ascending aorta. This method has the advantage of maintaining chest wall stability in postoperative period.
Purpose: This study was to determine the effects of laughter therapy on stress response and pain of military patients with low back pain. Method: The subjects in the experimental group received 3-sessions of laughter therapy on 3 consecutive days. The primary outcome measures were state anxiety, depression, blood pressure, pulse rate and pain. Result: After 3 sessions of laughter therapy, the scores of state anxiety(p=.046), depression(p=.028) and pulse rate(p=.003) were significantly lower and diastolic blood pressure(p=.038) was significantly higher in the experimental group than those in the control group. The level of pain(p=.711) was not different significantly between two groups. Conclusion: Laughter therapy could be an effective strategic intervention for military patients with low back pain to reduce the level of anxiety and depression. Further studies are needed to determine long-term effects of laughter therapy and its effects on cardiovascular system and pain.
Purpose: A descriptive correlational study was designed to examine the relationship of trait anger and anger expression to blood pressure, cholesterol, and depression in middle-aged Korean men. In addition, this study investigated the mediating effect of social support in relation to anger and other variables. Methods: Two hundred and ninety nine men aged 40 to 64 years were recruited from a health center at K University Hospital located in Ansan City, Kyungki province, Korea. The instruments used were Spielberger's state trait anger expression inventory-the Korean version for trait anger and anger expression, Beck's depression inventory for depression, and a Personal resource questionnaire for perceived social support. Results: Men with high trait anger showed significantly higher systolic blood pressure(BP) and diastolic BP The level of cholesterol did not have a significant relationship with trait anger and anger expression. The severity of depression was significantly higher in men with high trait anger or more frequent uses of anger-in or anger-out. The perceived social support had a significant mediating effect in relation to trait anger and depression. Conclusions: Various nursing interventions for managing anger or improving social support need to be developed in a future study.
Patients with chronic renal failure, many of which treated with hemodialysis, present a high prevalence of impaired muscle strength which suggest that muscle mass parameters may be used as markers for changes in muscle in these patients. Measurement of handgrip strength (HGS) is a common, simple, and quick measure of muscle function an indicator of overall muscle strength which has been associated with physical activity and several anthropometric traits. Intercellular adhesion molecule-1 (ICAM-1) and insulin-like growth factor-1 (IGF-1) are biochemical markers associated with inflammatory processes which are a common consequence of dialysis. Additionally, hemodialysis patients frequently present signs of malnutrition and depression. This cross-sectional study aimed to evaluate if muscle and biochemical markers could be used to predict the risk of depression in hemodialysis patients. Several anthropometric parameters, nutrient intake, depression state and the serum levels of ICAM-1 and IGF-1 were determined and Pearson's correlation coefficient and/or Spearman's correlation coefficient were used to test the correlation between them. Our results do not show a correlation between HGF, IGF-1 and ICAM-1 with the depression status of the patients, but mid-arm muscle circumference (MAMC) was statistically and positively correlated with depression. Additionally, ICAM-1 levels were negatively correlated with HGS, MAMC, and IGF-1. Overall, the results of the present study suggest that HGS may be used as an indicator of cardiovascular diseases and MAMC may be a good predictor of the level of depression in hemodialysis patients, although further studies are required.
Long-chain (LC) n-3 polyunsaturated fatty acids (n-3 PUFAs), in particular docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), are nutrients involved in many metabolic and physiological processes, and are referred to as n-3 LCPUFA. They have been extensively studied for their effects in human nutrition and health. This paper provides an overview on metabolism, sources, dietary intake, and status of n-3 LCPUFA. A summary of the dietary recommendations for n-3 LCPUFAs for different age groups as well as specific physiological conditions is provided. Evidence for n-3 LCPUFA in cardiovascular diseases, including new studies, is reviewed. Expert recommendations generally support a beneficial effect of n-3 LCPUFA on cardiovascular health and recommend a daily intake of 500 mg as DHA and EPA, or 1-2 servings of fish per week. The role of n-3 LCPUFA on brain health, in particular neurodegenerative disorders and depression, is reviewed. The evidence for beneficial effects of n-3 LCPUFA on neurodegenerative disorders is non-conclusive despite mechanistic support and observational data. Hence, no definite n-3 LCPUFA expert recommendations are made. Data for the beneficial effect of n-3 LCPUFA on depression are generally compelling. Expert recommendations have been established: 200-300 mg/day for depression; up to 1-2 g/day for major depressive disorder. Recent studies support a beneficial role of n-3 LCPUFAs in reducing the risk for premature birth, with a daily intake of 600-800 mg of DHA during pregnancy. Finally, international experts recently reviewed the scientific evidence on DHA and arachidonic acid (ARA) in infant nutrition and concluded that the totality of data support that infant and follow-on formulas should provide both DHA and ARA at levels similar to those in breast milk. In conclusion, the available scientific data support that dietary recommendations for n-3 LCPUFA should be established for the general population and for subjects with specific physiological conditions.
