Anormalous Origin of Left Coronary Artery from Pulmonary Artery (좌 관상동맥-폐동맥 이상 기시증 수술치험 1례)

  • Anomalous origin of left coronary artery from pulmonary artery(ALCAPA) is a rare fatal congenital anormaly that needs early surgical intervention. Many reports say that the choice of operative procedure is reimplantation of the left coronary artery into the ascending aorta. We experienced the surgical management of a case of the ALCAPA. The patient was 44 days old and 3.45 kg weighed female baby who had a symptom of congest ve heart failure. She underwent implantation of coronary artery on the aorta with cardiopulmonary bypass and recovered without any complications.

Surgical Repair of Bochdalek Hernia in Adult; A Report of Two Cases (성인에서 발생한 Bochdalek 탈장;수술 치험 2례)

  • Congenital diaphragmatic hernia through Bochdalek foramen, posterolateral diaphragmatic hernia, is the result of a congenital malformation of the posterolateral region of the diaphragm. Bochdalek hernia is the most common type of congenital diaphragmatic hernia, but it is a extremely rare anomaly which is detected in adulthood because it has severe cardiopulmonary and gastrointestinal symptoms after birth immediately.We experienced two cases of Bochdalek hernia which were detected in adulthood. Among them, one was a 43 years old man who was treated with simple closure in left Bochdalek hernia and the other was 41 years old woman who was treated with patch closure using silastic sheet in right Bochdalek hernia. Their postoperative courses were uneventiful. So we report two cases of rare Bochdalek hernia which was detected in adulthood with review of literatures.

Isolated Congenital Left Ventricular Diverticulum in Adults

  • Isolated congenital left ventricular diverticulum is a rare cardiac malformation. Here, we report the case of a 33-year-old woman who had suffered from recurrent transient ischemic attacks for 6 years. Preoperative cardiac magnetic resonance imaging and computed tomography angiography revealed a diverticulum near the apex. The diverticulum was successfully obliterated by cardiopulmonary bypass. We suggest that isolated congenital left ventricular diverticulum can be easily corrected with a low surgical risk by patch repair and plication techniques.

  • Type II chronic dissecting thoracoabdominal aortic aneurysms are a surgically challenging disease. The conventional thoracoabdominal aortic aneurysm repair technique using cardiopulmonary bypass is a high-risk procedure. However, a recently developed endovascular technique may be an alternative treatment for the disease, but faces the obstacle of lesional restriction. This new technique uses a hybrid strategy to overcome the limits of endovascular thoracoabdominal aortic aneurysm repair. Herein, we report on a successful outcome after performing the hybrid visceral debranching procedure.

  • A coronary artery fistula was surgically ligated in a 38-year-old woman via a left anterior mini-thoracotomy without the use of cardiopulmonary bypass. In selected cases, this surgical approach can provide an excellent surgical exposure for coronary artery fistula ligation. It also offers an excellent cosmetic result and shorter hospital stay.

  • Objective & Method : By investigating physiological function of tri-energizer, symptoms, and organ coordination, we obtained following conclusion. Tri-energizer is the membrane structure that surrounds the five visceral organs and six hollow organs, and filled with fluid. Tri-energizer acts as the passage for the flow of qi energy. Result & Conclusion : Therefore, dysfunction of the tri-energizer is caused by abnormal evaporation and metabolism. Upper-energizer regulates cardiopulmonary function, middle-energizer regulates spleen and stomach functions, and lower-energizer regulates liver, kidneys, small and large intestines, and bladder functions. Such a functional specialization is possible by receiving the source of qi through the wall wrapping around the internal organs. Tri-energizer represents the exterior and interior relationship by acting as the membrane structure supporting the five visceral organs and six hollow organs and at the same time, as the pericardium surrounding the heart.

  • 80 persons who don't have past history of cardiopulmonary and neuromuscular disease. The results were as follow; 1. PCI(Physiological Cost Index) value without loading to ankle were significantly increased compared to 1kg, and 2kg (p<0.01). 2. Female Subjects showed more increased PCI value in without loading and lkg, 2kg loading compared to male subjects (p<0.01). 3. In every PCI condition the difference among height groups was observed (p<0.01). 4. The difference among weight groups in each PCI condition was observed (p<0.01). These results showed that energy consumption was increased according to loading on the ankle during gait so weight of orthosis or prosthesis must be considered when choosing them and during gait training with these ones.

  • Electronic cigarettes are novel tobacco products that are frequently used these days. The cartridge contains liquid nicotine and accidental poisoning, even with a small oral dose, endangers children. We present here a mortality case of a 15-month-old child who ingested liquid nicotine mistaking it for cold medicine. When the emergency medical technicians arrived, she was found to have pulseless electrical activity. Spontaneous circulation was restored after approximately 40 minutes of cardiopulmonary resuscitation. The cotinine level in her urine was 1,716 ng/mL. Despite intensive supportive care, severe anoxic brain injury was found on computed tomography and the child ultimately died. This fatality highlights the need for public health efforts to minimize such accidents.

  • Objectives The purpose of this article was to review the video-assisted mediastinoscopic surgery (VAM) as diagnostic or therapeutic tools. Summary The VAM was indicated in diagnosis of pleural diseases, dissection or sampling of mediastinal or paraesophageal lymph nodes, nodal staging in lung cancer, or various therapeutic approaches. The advantages of VAM were included that was less painful, cosmetic merit, no need additional drain, and no effect of cardiopulmonary function. It was provided to surgeons wider and more detail operational field rather than conventional mediastinoscopic surgery. It was useful tool as training beginner surgeon, Although the VAM had limited indications, it would be feasible and important multi-purposal operational tools in mediastinal, pleural and lung diseases.

Left Atrial Myxoma: Report of 2 Cases (좌심방내 점액종 치험 2예)

  • Left atrial myxoma, an unusual clinical entity, may cause severe and progressive cardiac disease mimicking mitral valvular disease. In recent years, increased clinical awareness and improved diagnostic techniques has led to a higher incidence of correct preoperative diagnosis. Recently we experienced 2 cases of left atrial myxoma, which were removed successfully under the cardiopulmonary by pass. The first case was 45 years old woman and the second was 23 years old female. Preoperative definite diagnosis was entertained by angiocardiography and echocardiography in both cases. In the first case, tumor was removed with left atriotomy and atrial septectomy was done with additional right atriotomy. In the second case, tumor and atrial septum were removed en bloc through the right atrium. Both patients were discharged with good results 2 weeks postoperatively.
