119 구급대원에 의해 시행된 심폐소생술 및 자발순환회복 결과 (Outcomes of Out-of-hospital Cardiopulmonary Resuscitation and ROSC by Fire EMTs)

  • 이 연구는 하트세이버를 배지를 수여 받은 구급대원과 심정지 환자 중 심폐소생술로 자발순환이 회복된 후 환자를 대상으로 심정지 생존의 요인을 파악하고 분석한 결과 심정지에서 소생한 환자의 75%가 가정에서 발생되었으며, 출동에서 현장까지 도착 소요시간이 6.1분, 현장심폐소생술 소요시간 6.2분, 출동에서 병원 응급실 도착까지 소요시간 23.0분이 소요되었다. 또한 목격자에 의해 시행된 심폐소생술은 50.0%, 발견 당시 심장리듬은 심실세동이 87.5%, 무수축 12.5%였고, 심폐소생술과 동시에 자동제세동기를 사용한 환자는 87.5%, 심폐소생술만 시행한 환자 12.5%, 기관내삽관 시행이 12.5%였다. 자발순환이 회복된 환자에게 시행된 심폐소생술 소요시간은 10.9분 이었으며, 책임구급대원은 1급응급구조사가 75.0%, 출동 구급인원은 3명 출동이 87.5%였다.

  • Purpose: The purpose of this study was to provide the information of renal disease patients and to provide evidence the physical therapy recipient of renal disease patients. The reviews suggest that renal disease patients are physical inactivity situation, increase on cardiopulmonary risk factor, developed complication disease, increased psychosocial factor like depress and anxiety, decreased Quality of life because their disease effects. Thus. exercise program seems to have beneficial effect on physiologic and psychosocial functioning, Quality of life. It is expected that this study will consider rehabilitation program for internal medicine patients and should be consider as availability of therapeutic exercise program on understanding renal disease in physical therapy.

  • This paper reviews the main aspects of cardiovascular system dynamics with emphasis on modeling hemodynamic characteristics using a lumped parameter approach. Methodological and physiological aspects of the circulation dynamics are summarized with the help of existing mathematical models: The main characteristics of the hemodynamic elements, such as the heart and arterial and venous systems, are first described. Lumped models of micro-circulation and pulmonary circulation are introduced. We also discuss the feedback control of cardiovascular system. The control pathways that participate in feedback mechanisms (baroreceptors and cardiopulmonary receptors) are described to explain the interaction between hemodynamics and autonomic nerve control in the circulation. Based on a set-point model, the computational aspects of reflex control are explained. In final chapter we present the present research trend in this field and discuss the future studies of cardiovascular system modeling.

  • As mediastinal sarcomas commonly present as large tumors invading adjacent vital structures, complete resection is frequently challenging. For such tumors, aggressive surgical strategies, such as the resection and reconstruction of the invaded vital structures under cardiopulmonary bypass, may be required to achieve complete resection and to improve survival. Herein, we report a case of recurrent mediastinal sarcoma invading the aortic arch and arch vessels that was successfully removed by total arch replacement.

  • Orthostatic stress in human cardiovascular system following spaceflight remains a critical problem in the current lifercience space program. The study presented in this paper is part of an ongoing effort to use mathematical models to investigate the effects of gravitational stresses on the cardiovascular system of normals and microgravity adapted individuals. We employ a twelve compartment lumped parameter representation of the hemodynamic system coupled to set-point models of the arterial baroreflex and the cardiopulmonary reflex to investigate the transient response of heart rate to orthostatic stress. We simulate current hypotheses concerning the mechanisms underlying postspaceflight orthostatic intolerance over a range of physiologically reasonable values and compare the simulations to astronaut stand-test data pre-and postflight.

  • The aim of this article is to provide major announcements within the last decade or so about possible medical emergencies in dentistry. This would be helpful to improve the knowledge on first aid suitable for dental environment. Syncope was the most common medical emergencies in dentistry. Medical emergency situations can be divided into urgency and true emergency. Urgency situations can be solved well if proper treatment is taken. However, even in an urgency, if the correct treatment is not performed, it may soon turn into an emergency. With the joint efforts of the Korean Dental Anesthesiology Society and the Korean Cardiopulmonary Resuscitation Association, dental advanced life support (DALS) has been prepared since 2015. Through 17 training experiences until January 2020, scenarios and textbooks were developed. Dentists and dental care teams need to be prepared to strengthen their competence as professionals and to increase their team-level response capabilities..

  • As a dentist, we should be prepared for emergencies that can occur at any time in dental practice. In addition, we must be able to provide first aid to patients with the latest knowledge when an emergency actually occurs. It has been revised every five years since the American Heart Association published advanced cardiovascular life support (ACLS) guidelines. The current final guideline is the 2015 version. It is expected that the 2020 version will be released around the winter of 2020. Therefore, at this point, the latest version (2015 ver.) will be important. Many changes were made from 1995 to 2015. I would like to summarize the parts of the revised parts that remain important.

  • Trancranial Doppler(TCD) monitoring is a new application of ultrasonography which allows the nonivasive detection of blood flow velocity in the horizontal (M1) segment of the middle cerebral artery (MCA) and detects microembolic phenomena in the cerebral circulation. Recent studies emphasized the potential of using this technique in vascular surgery (carotid endarterectomy, cardiopulmonary bypass), interventional and intensive care setting. Although the disparity between CBF and blood flow velocity and number of microemboli could be used to prevent cerebral ischemic and embolism based on clinical studies. A reduction of more than 60% of MCA can reflex hemodynamic ischemic state and acoustic feedback of high intensity transient signals(HITS) from the TCD monitoring unit has a direct influence on surgical technique. TCD monitoring can immediately provide information about thromboembolism and hemodynamic changes, which may be a useful tool in the study and prevention of stroke.

  • Bilateral chylothorax as a complication of radical neck dissection is extremely rare. Early diagnosis of chylothorax is important due to the consequences on metabolic & cardiopulmonary conditions. This present report describes our recent experiences with two cases of bilateral chylothorax following left modified radical neck dissection that was treated successfully by conservative management.

  • In this study, we generated 40 somatic cell cloned (scNT) piglets. Of these, three displayed congestion in both liver and lung, and died within the first week of life. Two-dimensional gel electrophoresis experiments revealed changes in the responses of several detoxification-related proteins to stress and inflammation. As a result, congestive livers and lungs displayed extensive hepatopneumonic apoptosis.(omitted)
