Sternal Retraction and Subclavian Vein Catheter Occlusion during Cardiac Surgery

  • Background: Subclavian vein (SV) catheterization is a method for the delivery of fluids, drugs, and blood products, venous blood sampling, and central vein pressure monitoring in cardiac surgery. Catheter occlusion is a serious complication of SV catheterization during cardiac surgery, especially after sternal retractor expansion. Methods: In this observational study, 303 patients who had successful right infraclavicular SV catheterization from September 2019 to April 2020 were enrolled to determine the incidence of catheter occlusion. After catheterization, the lumens of all catheters were checked for the ability to infuse and withdraw blood from the catheter before and after sternal retractor expansion. The patients' characteristics, cannulation approach, on-pump or off-pump technique, occlusion of the catheter and its lumens, and any associated complications were recorded. The data were analyzed using IBM SPSS ver. 22.0 (IBM Corp., Armonk, NY, USA). Results: Of the 303 patients studied, 205 were male (67.7%) and 98 were female (32.3%). Catheter occlusion occurred in 11 patients with on-pump cardiopulmonary bypass (CPB) (227 patients) and 4 patients with off-pump CPB (76 patients) (p=0.863). The incidence of catheter occlusion was 4.95% (15 of 303 patients) with no cases of simultaneous 3-lumen occlusion in a catheter. The most commonly occluded lumen was the distal lumen (57.92%). Simultaneous 2-lumen occlusion occurred in 4 patients. Catheter occlusion was found in 3 of 13 malpositioned catheters (23.07%). Conclusion: The current study showed that malpositioning of the catheter tip was a risk factor for catheter occlusion and that the distal lumen of a triple-lumen catheter was the most commonly occluded lumen.

  • The purpose of this study was to investigate the single-person household's demand for a wearable healthcare system that provides the ischemic heart disease and cardiopulmonary alarm to be developed. In this study, in-depth interviews were conducted with six unmarried individuals in their thirties who were in the workplace and the results were interpreted by Giorgi's phenomenological analysis method. The study results indicated that although the wearable healthcare system was implemented with high-technology functions that serve the intended purpose, it is necessary to comprehensively consider factors such as aesthetics, comfort, and ease of washing. In this study, we found that the variables influencing the interviewees' intent to accept healthcare services are complex and cannot be attributed to one factor alone; this study has also shown a variety of interpersonal correlations. In addition, although health-care research focusing on the elderly has been conducted in the past, it is suggested that younger generations such as those in their thirties can also be studied as health care service adopters.

  • The names of medical technologist in Korea, Japan, and the United State of America vary depending on the medical system, educational system, and work areas. Conventionally, jobs equal or similar to medical technologist (commonly known as clinical laboratory technologist, medical laboratory technologist, clinical laboratory scientist, and medical laboratory scientist), histotechnologist, cytotechnologist, and genetic technologist. Jobs, such as extracorporeal technologist, medical electrophysiology technologist, and medical sonographer, allow other allied health professionals including medical technologists, to obtain their qualification. The work areas of medical technologists are classified into clinical pathologic technology, pathologic technology, clinical genetics technology, nuclear medicine in vitro technology, and clinical physiologic technology. New titles of Korean specialized medical technologists refer to certification systems, such as CLMJ, CMAJ, ASCP, IAC, ABRET, and ARMDS. Currently, there are eight types of specialized medical technologists under the cooperation of specialized academic societies. They are hematology, transfusionology, chemistry, immunology, microbiology, histology, cytology, nuclear medicine. It can be further divided into 20 categories according to the work areas such as mass spectrometry, infection control, Pathologists' Assistant, flow cytometry, HLA, cytogenetics, molecular genetics, cardiopulmonary physiology, neurophysiology (EEG/EMG/PSG/etc.), cardiac sonography, neurosonography (TCD/carotid), and medical device quality manager.

The Effects of High-Intensity Aerobic Interval Training In The Elderly With Myocardial Infarction (노인 심근경색 환자에서 고강도 간헐적 유산소운동의 효과)

  • To investigate the effect of high-intensity interval training (HIIT) on the outcomes of cardiac rehabilitation(CR) in elderly and younger patients with myocardial infarction(MI). Of the MI patients who received hospital-based CR from 2015 to 2017, we retrospectively reviewed the medical records of the patients who continued follow-up through the outpatient clinic. A total of 39 patients (20 elderly patients (${\geq}60year$($64.1{\pm}3.6$)) and 19 younger patients (< 60 year($55.6{\pm}2.3$)) was included in this study. The HIIT program was composed of 10 minutes of warm-up, 30 minutes of interval aerobic exercise, and 10 minutes of cool down. The aerobic exercise based on their initial exercise tolerance test outcome consisted of 3-minute usual intensity phase (60-70% of heart rate/VO2 reserve) and 4-minute of high-intensity phase (70-90% of heart rate/VO2 reserve). Exercise capacity was measured by exercise tests before and after hospital- based CR. Before CR, elderly group had a significantly lower exercise capacity in Exercise time, peak VO2, METs, Anaerobic threshold, VEmax, 02pulse than younger group. Both group showed similar improvement of cardiopulmonary exercise capacity after CR. And HIIT is safe and effective in elderly patients. It is considered necessary to further expansion for the participation of high intensity interval training in elderly patients.

Development of TaqMan Quantitative PCR Assays for Duplex Detection of Dirofilaria immitis COI and Dog GAPDH from Infected Dog Blood (심장사상충에 감염된 개 혈액에서 Dirofilaria immitis의 COI와 개의 GAPDH를 이중 검출하기 위한 정량적 TaqMan PCR 분석법의 개발)

  • Dirofilaria immitis (D. immitis) is a filarial nematode that causes cardiopulmonary dirofilariasis in dogs. In the late stages of infection, infected dogs show one or more symptoms and advanced heart disorder with perivascular inflammation. To detect D. immitis specifically and efficiently in the early stages of infection, a duplex TaqMan qPCR assay was developed based on previous studies using primers and probes specialized to detect D. immitis cytochrome c oxidase subunit I (COI) and dog glyceraldehyde-3-phosphate dehydrogenase (GAPDH). As positive controls, plasmid DNAs were constructed from D. immitis COI or dog GAPDH and a TA-cloning vector. Simplex and duplex TaqMan qPCR assays were performed using the specific primers, probes, and genomic or plasmid DNA. The duplex reaction developed could detect D. immitis COI and dog GAPDH in the same sample simultaneously after optimization of the primer concentrations. The limit of detection was 25 copies for the simplex and duplex assays, and both showed good linearity, high sensitivity, and excellent PCR efficiency. The duplex assays for pathogen detection reduce the costs, labor, and time compared to simplex reactions. Therefore, the duplex TaqMan qPCR assay developed herein will allow efficient D. immitis detection and quantification from a large number of samples simultaneously.

Analysis of the association of serum potassium and lactic acid with neurologic outcome in out-of hospital post-cardiac arrest adult patients (자발순환 회복된 병원 외 성인 심정지 환자의 신경학적 예후와 혈청 칼륨 및 젖산 농도와의 관련성 분석)

  • Objective: This study aimed to identify the effects of serum potassium and lactate on neurologic outcomes in out-of-hospital post-cardiac arrest adult patients. Methods: This study was a single center, retrospective observational study. We recruited out-of-hospital post-cardiac arrest adult patients admitted to an intensive care unit from 2011 to 2017. Primary outcome was good neurologic outcome at discharge. To evaluate the prognostic impact of serum potassium and lactate, univariate and multivariate logistic regression analyses were performed. Results: A total of 57 patients were included in this study. The number of patients with good neurologic outcome was 19 (33.3%). In the univariate analysis, good neurologic outcome patients showed a higher smoking rate, shorter pre-hospital transportation time, higher rate of percutaneous coronary intervention, and lower severity score (all P<0.05). The good neurologic outcome patients also presented higher pH, lower partial pressure of carbon dioxide, and lower potassium regarding laboratory findings on the first hospital day (all P<0.05). In the multivariate analysis, the independent factors favoring good neurologic outcome were pre-hospital transportation time (adjusted odds ratio [aOR], 0.82; 95% confidence interval [CI], 0.69-0.97; P=0.019) and lower partial pressure of carbon dioxide on the first hospital day (aOR, 0.95; 95% CI, 0.91-0.99; P=0.034). Conclusion: Serum potassium and lactate were not significantly associated with good neurologic outcome in out-of-hospital post-cardiac arrest adult patients. The prognostic factors for good neurologic outcome were pre-hospital transportation time and initial partial pressure of carbon dioxide.

Usability of CPR Training System based on Extended Reality (확장현실 기반의 심폐소생술 교육 시스템의 사용성 평가)

  • Recently, the importance of CPR training for the layperson has been emphasized to improve the survival rate of out-of-hospital cardiac arrest patients. An accurate and realistic training strategy is required for the CPR training effect for laypersons. In this study, we develop an extended reality (XR) based CPR training system and evaluate its usability. The XR based CPR training system consisted of three applications. First, a 3D heart anatomy image registered to the manikin is transmitted to the smart glasses to guide the chest compression point. The second application provides visual and auditory information about the CPR process through smart glasses. At the same time, the smartwatch sends a vibration notification to guide the compression rate. The 'Add-on-kit' is a device that detects the depth and speed of chest compression via sensors installed on the manikin and sends immediate feedback to the smartphone. One hundred laypersons who participated in this study agreed that the XR based CPR training system has realism and effectiveness. XR based registration technology will contribute to improving the efficiency of CPR training by enhancing realism, immersion, and self-directed learning.

  • This study is a random allocation similar experimental study to compare and analyze the difference in BVM (Bag-Valve-Mask) ventilation volume according to the characteristics of the rescuer's hand and the type of mask using a standardized mannequin. To this end, the Basic Life Resuscitation Education Center of D University in gwangju. Recruiting 39 students who have completed the basic resuscitation course for emergency medical personnel and the Korean-style specialized cardiac rescue course, In addition to measuring the physical characteristics of the hand, the average amount of ventilation per minute using a bag-mask was measured and analyzed. As a result, the type of mask that was not most affected by the characteristics of the hand and provided adequate Minute Ventilation was the soft type (tube, silicone) mask. On the other hard (tube, silicone) masks were found to be unsuitable for general use as they were greatly affected by the characteristics of workers' hands. COVID-19 is currently increasing the risk of transmission to paramedics and patients. Considering this situation, the universal use of a semi-permanent hard-type mask, which is disadvantageous not only for preventing infection but also for proper ventilation, should be avoided. In addition to the ease of use, it should be actively utilized in the field by supplying a soft type mask that can provide stable ventilation even with 'predominance recognition' and proper ventilation.

The Effects of Resistance Exercise on Body Composition Physical Strength, Blood Lipids and Insulin in Elderly Women (저항성 운동이 여성 노인의 신체조성, 체력, 혈중지질 및 인슐린에 미치는 영향)

  • Purpose : This study aimed to investigate the effects of 12 weeks of resistance exercise on body composition, physical strength, blood lipids, and insulin. Methods : The study was conducted on 24 elderly women divided into two groups: 12 subjects in an exercise group and 12 subjects in a control group. Resistance exercise was performed for 50 minutes a day, three times a week, for the duration of 12 weeks, and body composition, physical strength, blood lipids, and insulin were measured before and after the subjects completed the program. For the statistical analysis, the mean and standard deviation (M±SD) of each variable were calculated using SPSS version 20, and a paired t-test and two-way repeated ANOVA were conducted to test for the differences before and after the resistance exercise. All significant levels were set to α=.05 as a result of the experiment. Results : Changes in body composition after the 12-week resistance exercise program did not show any significant difference based on the comparison between the groups, but when noting the values for body fat percentage and body in the control group before and after, a significant difference was shown in fat mass (p<.05). As for changes in physical fitness, significant differences appeared in flexibility, muscle strength, and stenotic force (p<.01) when the groups were compared. Regarding pre- and post-values within each group concerning flexibility within the exercise group, significant differences were shown in gender (p<.001), muscle strength (p<.05), (p<.01), muscle earth strength, equilibrium (p<.01), stenosis force, and cardiopulmonary earth force (p<.001). Also, comparisons between populations in changes in blood lipids the values before and after in each group, significant differences in glucose (p<.05) and insulin (p<.05) were shown in the exercise group. When comparing the values before and after in each population, a significant difference was shown in the control group (p<.05). Conclusion : When all the results were integrated, the 12-week resistance exercise program was found to enhance physical strength (flexibility, muscle strength, and coordination) and improve the blood sugar levels of elderly women. In particular, resistance exercise is believed to lower the prevalence of obesity, type 2 diabetes mellitus, and metabolic diseases by having a positive effect on insulin. Further studies are suggested to verify the effect on body composition and blood lipids by setting up a variety of exercise treatment methods (including subjects, exercise periods, exercise plans, and exercise intensity focuses).

Factors influencing success and safety of AED retrieval in out of hospital cardiac arrests in Singapore

  • Purpose: Automated External Defibrillator (AED) usage in out-of-hospital cardiac arrests (OHCAs) improves the survival of patients. In Singapore, public AEDs are protected by locked boxes with a 'break glass' mechanism to deter theft. Community responders have sustained injuries while breaking glass to retrieve AEDs. This unprecedented study aimed to elucidate the factors influencing successful retrieval of an AED and to document the prevalence of injuries. Methods: A survey was created and distributed. Participants were required to have responded to an OHCA in the past 12 months. Comparison tests were performed with the Fischer-Freeman-Halton Exact test or Pearson chi square test at 5% significance levels, and with multiple logistic regression with a logit link function. Results: Eighty-eight participants were eligible. The success of retrieving an AED was found not to be impacted by occupation, age, gender or time. Participants who responded to an OHCA because of activation by the myResponder App were more likely to retrieve an AED successfully. (AOR 11.111, 95% CI: 2.141-58.824) Conclusion: Use of the myResponder mobile application is associated with the greater success of retrieving an AED. Successful retrieval of an AED is not impacted by time, gender, age, or the occupation of the responder. Community responders in Singapore remain motivated to respond to Cardiac Arrests despite risk of injury.
