Expansion in the scope and technique of head and neck tumor resection during the past two decades has paralleled precise tumor localization with advanced radiographic imaging and the availability of microvascular free tissue transfer. Especially, the defect reconstruction utilizing free flap results in improvement of patient survival due to decrease of local recurrence by wide resection of cancer. The rectus abdominis free flap has been used widely in reconstruction of the breast and extremities. However, the report of cases on its applications in the head and neck, based on the deep inferior epigastric artery and vein, is rare. This flap is one of the most versatile soft-tissue flaps. The deep inferior epigastric artery and vein are long and large-diameter vessels that are ideal for microvascular anastomosis. The skin area that can be transferred is probably the largest of all flaps presently in use. The versatility of the donor site is due to the ability to transfer large areas of skin with various thickness and amounts of underlying muscle. This article is to report reconstruction of midface defects utilizing the rectus abdominis free flap in 2 patients with maxillary squamous cell carcinoma and discuss briefly considerations in flap design and orbital exenteration, and healing of irradiated recipient site by hyperbaric oxygen therapy with literature review.

By the development of medical devices, high-frequency medical devices are becoming widely used. Imaging diagnosis in hospital and clean treatment by high-frequency medical devices and cancer treatment through high-frequency heaters around us, all serve as the examples. However, it still lacks the measures to the problems of the RF(Radio Frequency) mutual interference between the high-frequency devices. In order to prevent the problematic factors for technical and industrial development, a new reasonable and suitable test method is required. In this paper, we study a standard test method for EMC evaluation to solve the RF mutual interference.

Radiation imaging systems consisting of a large number of channels greatly benefit from multiplexing methods to reduce the number of channels with minimizing the system complexity and development cost. In conventional pixelated radiation detector modules, such as anger logic, is used to reduce a large number of channels that transmit signals to a data acquisition system. However, these methods have limitations of electrical noise and distortion at the detector edge. To solve these problems, a multiplexing concept using a digital signal encoding technique based on a time delay method for signals from detectors was developed in this study. The digital encoding multiplexing (DEM) method was developed based on the time-over-threshold (ToT) method to provide more information including the activation time, position, and energy in one-bit line. This is the major advantage of the DEM method as compared with the traditional ToT method providing only energy information. The energy was measured and calibrated by the ToT method. The energy resolution and coincidence time resolution were observed as 16% and 2.4 ns, respectively, with DEM. The position was successfully distributed on each channel. This study demonstrated the feasibility that DEM was useful to reduce the number of detector channels.

Primary thymic adenocarcinoma is a very rare malignancy of the anterior mediastinum with no standardized treatment. A 36-year-old male patient presented with hoarseness over the past 3 months. A chest computed tomography (CT) scan showed an infiltrative mass to the proximal vessels and aortic arch in left upper mediastinum ($4.1{\times}3.1{\times}5.4cm$). Brain magnetic resonance imaging (MRI) showed focal lesions, suggesting metastasis in the left frontal lobe. A thoracoscopic biopsy of the mediastinal mass confirmed a primary thymic adenocarcinoma forming a glandular structure with atypia of tumor cells. The patient received four cycles of systemic chemotherapy, consisting of etoposide and cisplatin, with concurrent radiotherapy (6,000 cGy/30 fractions) to the mediastinal lesion and the metastatic brain lesion (4,200 cGy/12 fractions). A follow-up chest CT scan and brain MRI showed a decrease in the size of the left upper mediastinal mass and brain lesion. We report a rare case of the primary thymic adenocarcinoma with a literature review.

The most common type of refractory hypertension found in children is secondary hypertension, which is a potentially curable disease. Reninoma, a renin-secreting juxtaglomerular cell tumor, is a rare cause of severe hypertension that is usually diagnosed in adolescents and young adults. Surgical resection of the tumor completely cures the hypertension of patients with reninoma. The typical clinical presentation of reninoma includes hypokalemia, metabolic alkalosis, and features secondary to the increased activation of the renin-angiotensin system without renal artery stenosis. We report a case of reninoma in a female adolescent with a typical clinical presentation, in which surgical removal of the tumor completely cured hypertension. We discuss here the clinical features, imaging studies, and immunohistochemical examination of the tumor used to establish the diagnosis of reninoma and for the management of the condition.
Deep learning is emerging as one of the best tool in processing data related to medical imaging. In our research work, we have proposed a deep learning based framework CNN (Convolutional Neural Network) for the classification of dysplastic tissue images. The CNN has classified the given images into 4 different classes namely normal tissue, mild dysplastic tissue, moderate dysplastic tissue and severe dysplastic tissue. The dataset under taken for the study consists of 672 tissue images of epithelial squamous layer of oral cavity captured out of the biopsy samples of 52 patients. After applying the data pre-processing and augmentation on the given dataset, 2688 images were created. Further, these 2688 images were classified into 4 categories with the help of expert Oral Pathologist. The classified data was supplied to the convolutional neural network for training and testing of the proposed framework. It has been observed that training data shows 91.65% accuracy whereas the testing data achieves 89.3% accuracy. The results produced by our proposed framework are also tested and validated by comparing the manual results produced by the medical experts working in this area.
Purpose: We aim to evaluate the accuracy and effectiveness of an automatically converted radiation therapy plan between Radixact machines by comparing the original plan with the transferred plan. Methods: The study involved a total of 20 patients for each randomly selected treatment site who received radiation treatment with Radixact. We set up the cheese phantom (Gammex RMI, Middleton, WI, USA) with an Exradin A1SL ion chamber (Standard Imaging, Madison, WI, USA) and GAFCHROMIC EBT3 film (International Specialty Products, Wayne, NJ, USA) inserted. We used three methods to evaluate an automatically converted radiation therapy plan using the features of the Plan transfer. First, we evaluated and compared Planning target volume (PTV) coverage (homogeneity index, HI; conformity index, CI) and organs at risk (OAR) dose statistics. Second, we compared the absolute dose using an ion chamber. Lastly, we analyzed gamma passing rates using film. Results: Our results showed that the difference in PTV coverage was 1.72% in HI and 0.17% in CI, and majority of the difference in OAR was within 1% across all sites. The difference (%) in absolute dose values was averaging 0.74%. In addition, the gamma passing rate was 99.64% for 3%/3 mm and 97.08% for 2%/2 mm. Conclusions: The Plan transfer function can be reliably used in appropriate situations.

In 2010, the World Health Organization classified mucin-producing bile duct tumors of the liver into two distinct entities; mucinous cystic neoplasm of the liver (MCN-L) and intraductal papillary mucinous neoplasm of the bile duct. We present the case of a patient with MCN-L having a uniquely pedunculated shape. A 32-year-old woman was referred to our institution with a diagnosis of biliary cystic neoplasm. She had undergone left salpingo-oophorectomy for ovarian cancer 15 years ago. Imaging studies showed an 8 cm-sized well defined, multiloculated cystic lesion suggesting a mucinous cystic neoplasm. The cystic mass was pedunculated at the liver capsule and pathologically diagnosed as MCN-L. The mass was resected with partial hepatectomy. The patient recovered uneventfully. She was discharged 7 days postoperatively. The patient has been doing well for 6 months after the operation. The patient will be followed up annually because of the favorable postresection prognosis of MCN-L.

Purpose: To accelerate magnetic resonance fingerprinting (MRF) by developing a flexible deep learning reconstruction method. Materials and Methods: Synthetic data were used to train a deep learning model. The trained model was then applied to MRF for different organs and diseases. Iterative reconstruction was performed outside the deep learning model, allowing a changeable encoding matrix, i.e., with flexibility of choice for image resolution, radiofrequency coil, k-space trajectory, and undersampling mask. In vivo experiments were performed on normal brain and prostate cancer volunteers to demonstrate the model performance and generalizability. Results: In 400-dynamics brain MRF, direct nonuniform Fourier transform caused a slight increase of random fluctuations on the T2 map. These fluctuations were reduced with the proposed method. In prostate MRF, the proposed method suppressed fluctuations on both T1 and T2 maps. Conclusion: The deep learning and iterative MRF reconstruction method described in this study was flexible with different acquisition settings such as radiofrequency coils. It is generalizable for different in vivo applications.

As the risk of lung cancer has increased, early-stage detection and treatment of cancers have received a lot of attention. Among various medical imaging approaches, computer tomography (CT) has been widely utilized to examine the size and growth rate of lung nodules. However, the process of manual examination is a time-consuming task, and it causes physical and mental fatigue for medical professionals. Recently, many computer-aided diagnostic methods have been proposed to reduce the workload of medical professionals. In recent studies, encoder-decoder architectures have shown reliable performances in medical image segmentation, and it is adopted to predict lesion candidates. However, localizing nodules in lung CT images is a challenging problem due to the extremely small sizes and unstructured shapes of nodules. To solve these problems, we utilize atrous spatial pyramid pooling (ASPP) to minimize the loss of information for a general U-Net baseline model to extract rich representations from various receptive fields. Moreover, we propose mixed-up attention mechanism of reverse, boundary and convolutional block attention module (CBAM) to improve the accuracy of segmentation small scale of various shapes. The performance of the proposed model is compared with several previous attention mechanisms on the LIDC-IDRI dataset, and experimental results demonstrate that reverse, boundary, and CBAM (RB-CBAM) are effective in the segmentation of small nodules.
