Buckling resistance, bending stiffness, and torsional resistance of various instruments for canal exploration and glide path preparation

  • Objectives: This study compared the mechanical properties of various instruments for canal exploration and glide-path preparations. Materials and Methods: The buckling resistance, bending stiffness, ultimate torsional strength, and fracture angle under torsional load were compared for C+ file (CP, Dentsply Maillefer), M access K-file (MA, Dentsply Maillefer), Mani K-file (MN, Mani), and NiTiFlex K-file (NT, Dentsply Maillefer). The files of ISO size #15 and a shaft length of 25 mm were selected. For measuring buckling resistance (n = 10), the files were loaded in the axial direction of the shaft, and the maximum load was measured during the files' deflection. The files (n = 10) were fixed at 3 mm from the tip and then bent $45^{\circ}$ with respect to their long axis, while the bending force was recorded by a load cell. For measuring the torsional properties, the files (n = 10) were also fixed at 3 mm, and clockwise rotations (2 rpm) were applied to the files in a straight state. The torsional load and the distortion angle were recorded until the files succumbed to the torque. Results: The CP was shown to require the highest load to buckle and bend the files, and the NT showed the least. While MA and MN showed similar buckling resistances, MN showed higher bending stiffness than MA. The NT had the lowest bending stiffness and ultimate torsional strength (p < 0.05). Conclusions: The tested instruments showed different mechanical properties depending on the evaluated parameters. CP and NT files were revealed to be the stiffest and the most flexible instruments, respectively.

Comparison of postoperative pain intensity after using reciprocating and continuous rotary glide path systems: a randomized clinical trial

  • Objectives: The aim of this study was to compare postoperative pain intensity after root canal treatment with One G (OG) vs. R-Pilot (RP) files used for glide path preparation. Materials and Methods: Ninety-three single-canaled mandibular premolar teeth with asymptomatic non-vital pulp were randomly assigned into 3 groups (n = 31): OG, RP, or without glide path (WGP). After creating the glide path, the root canals were prepared using sequential Mtwo rotary files to size 30/0.05. One endodontic specialist carried out single-visit endodontic treatment. The patients were asked to rate the severity of postoperative pain on a visual analogue scale at 24, 48, and 72 hours after the visit. They were also asked to record their intake of prescribed analgesics taken. The data were analyzed using the ${\chi}^2$, Friedman, Kruskal-Wallis, and Mann-Whitney U tests. Results: In all 3 groups, postoperative pain decreased significantly at each time interval (p < 0.05). At 24 hours, the OG group had less postoperative pain than the WGP group (p < 0.05). However, no significant difference was found between the RP group and the others. No statistically significant difference was found among the WGP, OG, and RP groups in postoperative pain intensity at 48 or 72 hours or in analgesic tablet intake at the 3 assessed time intervals. Conclusions: The OG group had less postoperative pain than the WGP group in the first 24 hours. The OG and RP systems were similar regarding postoperative pain intensity and analgesic intake.

Buckling resistance, torque, and force generation during retreatment with D-RaCe, HyFlex Remover, and Mtwo retreatment files

  • Objectives: This study compared the buckling resistance of 3 nickel-titanium (NiTi) retreatment file systems and the torque/force generated during retreatment. Materials and Methods: The buckling resistance was compared among the D-RaCe (DR2), HyFlex Remover, and Mtwo R25/05 retreatment systems. J-shaped canals within resin blocks were prepared with ProTaper NEXT X3 and obturated by the single-cone technique with AH Plus. After 4 weeks, 4 mm of gutta-percha in the coronal aspect was removed with Gates-Glidden drills. Retreatment was then performed using DR1 (size 30, 10% taper) followed by DR2 (size 25, 4% taper), HyFlex Remover (size 30, 7% taper), or Mtrwo R25/05 (size 25, 5% taper) (15 specimens in each group). Further apical preparation was performed with WaveOne Gold Primary. The clockwise torque and upward force generated during retreatment were recorded. After retreatment, resin blocks were examined using stereomicroscopy, and the percentage of residual filling material in the canal area was calculated. Data were analyzed using 1-way analysis of variance with the Tukey test. Results: The HyFlex Remover files exhibited the greatest buckling resistance (p < 0.05), followed by the Mtwo R25/05. The HyFlex Remover and Mtwo R25/05 files generated the highest maximum clockwise torque and upward force, respectively (p < 0.05). The DR1 and DR2 files generated the least upward force and torque (p < 0.05). The percentage of residual filling material after retreatment was not significantly different between file systems (p > 0.05). Conclusions: NiTi retreatment instruments with higher buckling resistance generated greater clockwise torque and upward force.

  • 이 연구의 목적은 Mtwo 전동 파일의 사용에 있어서 제조사가 제안한 single length technique과 crown-down technique의 근관성형효율과 안전성을 비교하고 Mtwo 전동 파일의 변형된 사용방법을 제안하고자 하는 것이다. 기구의 종류와 사용방법에 따라 60개의 레진 블록 근관 모형 (Endo Training Bloc; Benstply Maillefer, Ballaigues, Switzerland)을 세 군으로 나누어 20개씩 표본을 성형하였다. MT군은 Mtwo 파일 (VDW, Munich, Germany)을 제조사의 추천방법인 single length technique으로, MC군은 Mtwo 파일을 crown-down technique으로 형성하였고, 대조군인 PT군은 ProTaper (Denstply Maillefer, Ballaigues, Switzerland)를 사용하였다. 모든 기구는 250 r.p.m., torque 2.2 N로 고정한 전동 모터 (X-smart; Denstply Maillefer, Ballaigues, Switzerland)로 사용하였다. 근관 성형 후, 스캐너로 근관 형성 전후의 이미지를 채득하여 중첩 비교함으로써 근단부로부터 1, 2, 3, 4, 5, 6, 7, 8 mm 높이에서의 중심변위율을 산출하였다. 순수 기구조작 시간 및 기구조작 동안의 binding빈도를 기록하였다. 근관 성형 후, 레진 블록의 무게 감소를 측정하였고 근관 성형에 사용된 파일의 파절이나 변형 및 근관 성형 후 형태의 이상을 조사하였다. 세 실험군의 측정 및 산출된 자료를 일원배치 분산분석 및 Duncan's Multiple Range Test로 사후 검정을 시행하여 비교하였으며, Fisher's exact test으로 근관의 이형성, 기구의 변형 그리고 기구 binding 등 발생 빈도를 유의 확률 95%에서 통계 검정하였다. 그 결과는 다음과 같다. 1. 근관의 만곡도 감소, 중심변위율, 레진 블록의 무게 감소, 기구 변형 등은 세 군간에 유의한 차이가 없었다 (p > 0.05). 2. 기구 조작 시간은 보면 MT군과 MC군이 PT군보다 짧았다 (p < 0.05). 3. Elbow는 MT군이 MC군에서 PT군보다 더 많이 형성되었다 (p < 0.05). Ledge는 세 군 모두에서 나타나지 않았다. 4. 파일 binding 횟수는 MC군이 가장 적고 PT군이 가장 많았다 (p < 0.05). 이상의 결과를 볼 때, Mtwo 전동 파일을 crown-down technique으로 사용하는 것이 single length technique과 유사한 성형 효율을 보이면서도 더 안전할 것으로 추정된다.

Root canal treatment of dens invaginatus and fused tooth

  • ;A dental developmental anomaly is defined as an isolated aberration in tooth form, caused by a disturbance or abnormality which occurred during tooth development. There are numerous types of dental anomalies, and a considerable variation in the extent of the defects occurs with each type. Teeth with these anomalies pose unique challenges. Since the defects are not always apparent clinically, they can confuse diagnosticians investigating the etiology of pulpal pathosis. When endodontic treatment is required, the defects often hinder access cavity preparation and canal instrumentation. Treatment planning also becomes more challenging, since the defects can create complicated periodontal problems, and the malformed teeth can be difficult to restore, particularly those weakened by endodontic therapy. Fusion is defined as the joining of two developing tooth germs resulting in a single large tooth structure. The incidence of fusion is < 1% in the Caucasian population, and it is believed that physical force or pressure produces contact of the developing teeth. Clinically and radiographically, a fused tooth usually appears as one large crown with at least partially separated roots and root canals. There may be a vertical groove in the tooth crown delineating the originally separate crowns. Dens invaginatus is a deep surface invagination of the crown or root that is lined by enamel. Teeth in both maxillary and mandibular arches may be affected, but the permanent maxillary lateral incisor is the tooth most commonly involved. Studies have revealed an incidence ranging from 0.25% to as high as 10%. The invagination ranges from a slight pitting to an anomaly occupying most of the crown and root. The invagination frequently communicates with the oral cavity, allowing the entry of irritants and microorganism either directly into pulpal tissues or into an area that is deparated from pulpal tissues by only a thin layer of enamel and dentin. This continuous ingress of irritants and the subsequent inflammation usually lead to necrosis of the adjacent pulp tissue and then to periapical or periodontal abscesses. If the invagination extends from the crown to the periradicular tissue and has no communication with the root canal system, the pulp may remain vital. Recommended treatment of fused tooth and dens invaginatus has been reported in the endodontic literature. This case report describes the endodontic treatment of a maxillary laterl incisors having fused crown and dens invaginatus.natus.

Balanced Force와 Step-Back 근관성형후 치근단 폐쇄효과의 비교연구 (A COMPARISON OF APICAL SEAL FOLLOWING "STEP-DOWN/BALANCED FORCE" AND "STEP-DOWN/STEP-BACK" ROOT CANAL SHAPING)

  • The purpose of this study was to compare the apical seal following root canal shaping by different methods. From fourty extracted mandibular 1st and 2nd molars, fourty mesial roots whose canals have some degree of curvature were selected. The mesiobuccal root portion including mesiobuccal portion of a crown was sectioned in each molar. After access cavity preparation for the mesiobuccal canal, working length was determined with # 10 K-file. The sectioned roots were implanted in acrylic resin block and randomly divided into four groups. The canals of group I were shaped by step-down/balanced force, group II by stepdown/step-back, group III by step-back and group IV by conventional method. All of the shaped canals were obturated by Thermafil method and access cavities were filled with IRM. The roots were removed from acrylic resin block and placed in 100 % humidity for 7days. Except the root surfaces of apical 2mm, the root surfaces were nail-varnished 3 times. After the roots were placed in 700 torr vacuum pressure for 15 minutes, they were immersed in 2% methylene blue solution for 4 days. Nail varnishes were removed with acetone. After that, the roots were decalcified in 5 % nitric acid and dehydrated with alcohol series. Transparent specimens were made by methyl salicylate and the quality of apical seal was assessed by measuring the leakage linearly. The results were as follows. 1. The leakage in canals shaped by step-down/balanced force method was significantly less than that in canals shaped by step-back method(P<0.05) and was less but not statistically than that in canals shaped by step-down/step-back method (P>0.05). 2. The leakage in canals shaped by step-down/step-back method was less than that in canals shaped by step-back method, but there was no statistical significance(P>0.05). 3. The leakage in canals shaped by conventional method was significantly more than that in canals shaped by step-down/balanced force, step-down/step-back and step-back method (P<0.05).

모형 레진근관에서 니켈-티타늄 전동 파일의 경사도가 screw-in effect에 미치는 영향 (Influence of taper on the screw-in effect of nickel-titanium rotary files in simulated resin root canal)

  • 연구목적: 니켈-티타늄 금속을 근관치료기구에 도입함으로써 근관형성과정이 매우 단순화되었다. 그러나 이러한 새로운 기구는 단점도 함께 가지고 있는데 그 중 하나가 근관내로 기구가 빨려 들어가는 경향이다. 본 연구에서는 니켈-티타늄 전동파일의 경사도가 screw-in effect에 미치는 영향을 평가하고자 하였다. 연구 재료 및 방법: 총 20개의 S-자형 투명레진블록 (Dentsply-Maillefer) 근관에서 두 군으로 나누어 실험하였는데 Profile군에서는 0.02, 0.04 그리고 0.06 경사도의 Profile (Dentsply-Maillefer)과 0.08, 0.10 그리고 0.12의 경사도의 GT Rotary file (Dentsply-Maillefer)을 사용하였고 K3군에서는 같은 경사도의 K3 file SybronEndo)을 사용하였으며, 분당 300회전의 일정한 속도에서 단일 pecking 동작으로 기구조작을 하였다. 특수한 장치를 고안하였으며 장치내 dynamometer를 이용하여 screw-in effect를 측정하여 one-way ANOVA로 통계처리하고 Tuckey's multiple range test로 사후검정하였다. 결과: Profile 군에서는 큰 경사도의 기구가 더 큰 screw-in force를 나타내었고 (p < 0.05), K3군에서는 0.08, 0.10. 및 0.12 경사도의 기구가 0.04 경사도의 기구에 비해, 0.08 및 0.12 경사도의 기구가 0.06 경사도의 기구에 비해 더 큰 screw-in force를 나타내었다 (p < 0.05). 결론: 기구의 경사도가 클수록 screw-in force가 많이 발생하는 것으로 생각되며, 경사도가 큰 전동화일을 사용시에는 screw-in force를 억제하기 위해 더 많은 주의가 요구될 것으로 생각된다.

학생들이 사용한 세 종류 Ni-Ti file systems의 근관성형 효율 비교 (Relative efficacy of three Ni-Ti file systems used by undergraduates)

  • 이 연구의 목적은 Ni-Ti file을 사용한 경험이 없는 학생들이 세 종류 Ni-Ti file systems을 사용하였을 때 근관 형성 효율을 비교하는 것이다. Ni-Ti file의 사용 경험이 없는 부산대학교 치과대학 4학년 학생 50명이 세 종류의 Ni-Ti file system - $ProFile^{(R)}$ (PF : Dentsply Maillefer, Ballaigues, Switzerland), manual $ProTaper^{(R)}$ (MPT: Dentsply Maillefer, Ballaigues, Switzerland), rotary $ProTaper^{(R)}$ (RPT: Dentsply Maillefer, Ballaigues, Switzerland) -을 사용하여 각 system으로 하나의 근관씩, 모두 150개의 레진 블락 근관모형(Endo Training Bloc; Dentsply Maillefer, Ballaigues, Switzerland)을 형성하였다. 근관형성에 사용된 file의 파절이나 변형 및 근관형성 후 형태의 이상을 조사하고, 스캐너로 근관 형성 전후의 이미지를 채득하여 중첩 비교함으로써 근단부로부터 1mm, 3mm, 그리고 5mm 높이에서의 삭제된 근관의 폭경, 근관의 변위량, 그리고 중심변위율 등을 산출하고 통계학적 비교 분석을 하였다. 그리고 세 가지 systems에 관한 학생들의 주관적인 선호도를 조사하였다. 그 결과 졸업을 앞둔 치과대학 재학생들도 특별한 문제점 없이 세 가지 Ni-Ti file systems을 사용한 근관형성이 가능함을 확인하였다. 세 실험군을 비교하였을 때, 양적인 삭제 능력은 두 $ProTaper^{(R)}$ systems (manual and rotary)이 $ProFile^{(R)}$에 비해 우세하였지만, 근관의 변위는 두 $ProTaper^{(R)}$ systems에서 더 많이 유발되었다. 근관 중심 이동률 등의 질적 평가를 포함하여 학생들의 선호도를 종합적으로 고려할 때, 삭제 능력에 있어서는 rotary $ProTaper^{(R)}$가 더 효율적이지만 안전성을 고려하면 $ProFile^{(R)}$도 추천된다. 정규 교육과정에서 Ni-Ti file systems의 도입을 위해서 초심자와 관련한 더 많은 연구가 진행되어야 하겠다.

A comparative evaluation of fracture resistance of endodontically treated teeth restored with different post core systems - an in-vitro study

  • PURPOSE. To compare the fracture resistance and the mode of failure of endodontically treated teeth restored with different post-core systems. MATERIALS AND METHODS. Root canal treatment was performed on 40 maxillary incisors and the samples were divided into four groups of 10 each. For three experimental groups post space preparation was done and teeth were restored with cast post-core (Group B), stainless steel post with composite core (Group C) and glass fiber post with composite core using adhesive resin cement (Group D). Control group (A) samples were selected with intact coronal structure. All the samples were prepared for ideal abutment preparation. All the samples were subjected to a load of 0.5 mm/min at $130^{circ}$.until fracture occurred using the universal testing machine. The fracture resistance was measured and the data were analyzed statistically. The fracture above the embedded resin was considered to be favorable and the fracture below the level was considered as unfavorable. The statistical analysis of fracture resistance between different groups was carried out with t-test. For the mode of failure the statistical analysis was carried out by Kruskal-Wallis test and Chi-Square test. RESULTS. For experimental group Vs control group the fracture resistance values showed significant differences (P<.05). For the mode of failure the chi-square value is 16.1610, which means highly significant (P=.0009) statistically. CONCLUSION. Endodontically treated teeth without post core system showed the least fracture resistance demonstrating the need to reinforce the tooth. Stainless steel post with composite core showed the highest fracture resistance among all the experimental groups. Teeth restored with the Glass fiber post showed the most favorable fractures making them more amenable to the re-treatment.

Smear layer removal by passive ultrasonic irrigation and 2 new mechanical methods for activation of the chelating solution

  • Objectives: The aim of this study was to compare smear layer removal by conventional application (CA), passive ultrasonic irrigation (PUI), EasyClean (EC), and XP-Endo Finisher (XPF), using 17% ethylenediaminetetraacetic acid (EDTA) after chemomechanical preparation, as evaluated with scanning electron microscopy (SEM). Materials and Methods: Forty-five single-rooted human mandibular premolars were selected for this study. After chemomechanical preparation, the teeth were randomly divided into 5 groups according to the protocol for smear layer removal, as follows: G1 (control): CA of distilled water; G2 (CA): CA of 17% EDTA; G3 (PUI): 17% EDTA activated by PUI; G4 (EC): 17% EDTA activated by EC; and G5 (XPF): 17% EDTA activated by XPF. SEM images (×1,000) were obtained from each root third and scored by 3 examiners. Data were evaluated using the Kruskal-Wallis and Dunn tests (p < 0.05). Results: In the apical third, there were no statistically significant differences among the groups (p > 0.05). In the cervical and middle thirds, the experimental groups performed better than the control group (p < 0.05); however, G2 presented better results than G3, G4, and G5 (p < 0.05), which showed no differences among one another (p > 0.05). Conclusions: No irrigation method was able to completely remove the smear layer, especially in the apical third. Using CA for the chelating solution performed better than any form of activation.
