Legg-Calve-Perthes disease의 방사선학적 진단 및 대퇴골두절단술 증례 (A Case of Radiological Kiagnosis and Femoral Head and Neck Oatectomy for Legg-Calve-Perthes Disease)

  • One nine-minth-old female crossbred dog of Maltese and Shih-tzu with chief complaints of left hindlimb lameness and pain was diagnosed as Legg-Calve-Perthes disease(LCPD) in radiographic examination. Femoral head and neck ostectomy for the treatment of LCPD obtained effective result.

  • Legg-Calve-Perthes diseases were diagnosed in 2 dogs showing signs of lameness. In radiographs, the abnormal size and shape was due to the collapse of the left capital bone and enlarged femoral neck was seen at case I. The completely deformed left femoral head resulted from the earlier collapse of the osteolytic bone of the caput femoris at case II. In treatment, femoral head ezcision was performed in one dog.

대퇴골두의 연소성 골연골증의 중·서양 협진치료에 대한 임상 연구 동향: 2000년 이후 발표된 연구를 중심으로 (A Review of Clinical Studies of Integrated Traditional Chinese and Western Medicine Therapy for Legg-Calve-Perthes Disease Using China National Knowledge Infrastructure Database: Focused on Clinical Studies after 2000)

  • Objectives This study was conducted to investigate clinical studies about Integrated Traditional Chinese and Western Medicine therapy for Legg-Calve-Perthes disease. Methods We searched clinical studies about Integrated Traditional Chinese and Western Medicine therapy for Legg-Calve-Perthes disease through China National Knowledge Infrastructure. 17 articles published from 2000-2021 were finally chosen and analyzed by published year, study design, number of samples, diagnosis criteria, evaluation criteria, treatment period, follow up period, treatment method. Results Herbal medicine, external treatment, Chuna massage therapy were performed for traditional Chinese medical treatment. For Western medicine treatment, conservative treatment and surgical treatment were performed. Above them, herbal medicine and conservative treatment were mostly used for treating Legg-Calve-Perthes disease. Conclusions By analyzing clinical studies, We found that Integrated Traditional Chinese and Western Medicine therapy can be helpful for treating Legg-Calve-Perthes disease. In Korea, more clinical research about Legg-Calve-Perthes disease is still needed. This study will be helpful for future research on Korean medicine for Legg-Calve-Perthes disease.

  • Legg-Calve-Perthes (LCP) disease is characterized by idiopathic avascular osteonecrosis of the epiphysis of the femur head. The main factor that plays a role in the etiology of the disease is decreased blood flow to the epiphysis. Many predisposing factors have been suggested in the etiology of LCP disease, and most have varying degrees of effects. Here we present the case of a boy aged 4 years and 10 months with complaints of short stature and a diagnosis of multiple hypophyseal hormone deficiency, in whom LCP disease and difficult birth-related pituitary stalk interruption syndrome were identified by anamnesis. The present case revealed that LCP disease and hypophyseal hormone deficiency could be secondary to difficult birth and that LCP disease could be secondary to insulin-like growth factor 1 deficiency. Additionally, to the best of our knowledge there is no published case on the relation between LCP disease and insulin-like growth factor 1 deficiency. Therefore, we believe that this case is worthy of presentation.

피동형 원자로의 Hydraulic Valve 특성 실험 (The Characteristics of Hydraulic Valve for a Passive Reactor)

  • A kind of three-way check valve, so called hydraulic calve was proposed for the substitute of the density lock of passive reactor such as SPWR (System-Integrated Pressurized Water Reactor). The function of the valve are the separation of the borated water from main coolant loop for normal operation and the insurge of the water into the loop for shutdown and the removal of the decay power when the main coolant flow rate is not enough. To verify the operability and the characteristics of the valve, experimental works were executed with 1/3 scale model calve. Also a diffuser model was proposed for the theoretical analysis of the valve.

초기 허혈성 대퇴골두 괴사증을 보이는 개에서 임상학적, 수술적, 방사선학적, 컴퓨터단층촬영, 조직학적 소견 (Early Stage Legg-Calve-Perthes Disease in a Dog: Clinical, Surgical, Radiological, Computed Tomography and Histological Findings)

  • 7개월령 암컷 포메라니안견이 오른쪽 후지 파행 증상으로 내원하였다. 내원시 신체 검사에서 오른쪽 대퇴 관절을 신장하였을 때 통증을 호소하였고 일반 방사선 사진에서 오른쪽 대퇴골두에 미약한 방사선 투과성 병변과 오른쪽 대퇴관절 공간의 확장을 확인하였다. 14일 후 일반 방사선 사진에서 오른쪽 대퇴골두 골단의 변위를 확인하였고 컴퓨터단층촬영에서 오른쪽 대퇴골두에 저음영의 병변과 골절선을 확인하였다. 오른쪽 대퇴골두 절제술을 실시하였고 조직학적 검사에서 골두 골간단과 골단의 괴사를 확인하였다. 본 증례는 초기 허혈성 대퇴골두 괴사증이 있는 개의 방사선학적, 컴퓨터단층촬영, 수술적, 조직학적 소견을 기술하였다.

허혈성 대퇴골두 괴사 유발견에서 전산화 단층 영상 평가 (Computed Tomographic Assessment of Experimentally Induced Legg-Calve-Prethes Disease in Dogs)

  • This purpose of this study was to describe and compare the radiographic and computed tomographic(CT) appearance of the experimentally induced Legg-Calve-Perthes Disease(LCPD) in dogs. LCPD was experimentally induced at the left femoral head in 7 litters of a dog. The lesions were evaluated with radiography, CT, and contrast CT every week from day 7 to day 70 after operation. Histopathologic examination was performed on 92th day after operation. Every images were evaluated according to the staging system of the osteonecrosis published by Association Research of Circulation Osseous in 1997 and the scoring(0-4). On CT scan, diffuse porotic lesion and focal sclerotic lesion were detected on day 14 and on day 21 after operation, respectively. Subchondral fracture, articular collapse and crescent sign with decreased attenuation were shown on day 28 after operation for the first time and no change of the sign were detected from operation day 9 to day 70 after operation. Focal porotic lesion and irregular radiopacity of femoral head were detected on day $41{\pm}7.48$ and on day $51{\pm}5.29$ after operation respectively on radiographs. In scoring evaluation, 2.00 was scored on day 14 after operation on CT scan, on day 56 after operation on radiographs, respectively This study has shown that CT is more suitable for early diagnosis of LCPB and has superior sensitivity than radiography. Also, CT has been expected to be important for staging and treatment of LCPD.

초기의 단순 X선 촬영술 대 자기공명영상: 소아성 대퇴골두 무혈성 괴사증에서 예후 예측성 비교 (Initial Plain Radiographs versus MR Imaging: Comparison of Prognostic Efficacy in Legg-Calve-Perthes Disease)

  • 목적: 소아성 대퇴골두 무혈성 괴사증의 추적 단순 X선 촬영술에서 대퇴 골단의 구형 모양을 유지하는 것을 기준으로 좋은 예후를 기대할 수 있는 환자를 찾는데에 자기공명영상이 진단당시 단순 X선 촬영술 보다 우월한지 여부를 알아보기 위함이 목적이다. 대상 및 방법: 소아성 대퇴골두 무혈성 괴사증을 가진 41명의 환자에서 48예의 대퇴 골두의 진단 당시 단순 X선 촬영술과 자기공명영상을 후향적으로 분석하였다. 대퇴 골두 침범은 초기 단순 X선 촬영술과 자기공명영상을 Catterall 분류를 이용하여 평가하였다. 최대 대퇴 골두 변형으로 결정되는 예후는 추적 단순 X선 촬영술에서 Stulberg 분류에 의해 평가되었다. 결과: 자기공명영상에서 Catterall 분류가 1보다 큰 경우에는 추적 단순 X선 촬영술에서 대퇴 골두 기형의 나쁜 예후(Stulberg class>2)를 보이는 환자를 발견하는 데에 100% 민감도 (14/14) 을 보였다. 단순 X선 촬영술에서는 57% (8/14)의 낮은 민감도를 보였다. 자기공명영상의 특이도는 21% (7/34)였고, 단순 X선 촬영술의 특이도인 32%(11/34)와 유의한 차이를 보이지 않았다. 결론: 자기공명영상에서 Catterall 분류1은 소아성 대퇴골두 무혈성 괴사증의 좋은 예후를 시사한다. 대퇴 골두의 더욱 광범위한 침범을 가진 환자들도 좋은 결과를 보일 수 있으나, 그들은 대퇴 골두의 구형 모양을 소실하고 수반되는 골관절염을 성인에서 보일 위험성이 있다. 자기공명영상은 좋은 예후를 가진 환자 집단을 파악하는데 단순 X선 촬영술보다 우월하다.
