Biomimetic characteristics of mussel adhesive protein-loaded collagen membrane in guided bone regeneration of rabbit calvarial defects

  • Purpose: The aim of the present study was to evaluate the biocompatibility and barrier function of mussel adhesive protein (MAP)-loaded collagen membranes in guided bone regeneration (GBR). Methods: Eight male New Zealand white rabbits were used. Four circular defects (diameter: 8 mm) were created in the calvarium of each animal. The defects were randomly assigned to 1) a negative control group, 2) a cyanoacrylate (CA)-loaded collagen membrane group (the CA group), 3) a MAP-loaded collagen membrane group (the MAP group), and 4) a group that received a polycaprolactone block with MAP-loaded collagen membrane (the MAP-PCL group). Specimens were harvested at 2 weeks (n=4) and 8 weeks (n=4) postoperatively for observational histology and histometric analysis. Results: In the histologic analysis, MAP was completely absorbed without any byproducts. In contrast, some of the CA adhesive remained, showing an inflammatory reaction, at 8 weeks. In the MAP-PCL group, the MAP-loaded collagen membranes served as a barrier membrane despite their fast degradation in GBR. No significant difference was found in the amount of new bone between the MAP-PCL and MAP groups ($1.82{\pm}0.86mm^2$ and $2.60{\pm}0.65mm^2$, respectively). Conclusions: The MAP-loaded collagen membrane functioned efficiently in this rabbit calvarial GBR model, with excellent biocompatibility. Further research is needed to assess clinical applications in defect types that are more challenging for GBR than those used in the current model.

  • The purpose of this study is to evaluate the effects of bioresorbable membranes in guided bone regeneration of streptozotocin induced diabetic rats. 50 Sprague-Dawley rats were randomly categorized into 4 groups: Group 1 & 2 had 10 normal rats each and group 3 & 4 included 15 streptozotocin induced diabetic rats each. Defect measuring 7mm in diameter was formed on every rat calvarium. No membrane was used in groups 1 & 3 and membranes were used in groups 2 & 4. The rates were sacrificed at 2 and 4 weeks after defect formation. Routine histological specimens were prepared. Masson-trichrome and HE stain were done before light microscopy. Guided regenerative potential was evaluated by measuring the amount of new bone formation in the calvarial defect by histomorphometry. Following results were obtained. 1. New bone formation in the diabetic groups was significantly less that than in the normal groups regardless of membrane use(p<0.05). 2. In the comparison of new bone formation in the normal groups, membrane group showed significantly more bone formation(p<0.1). 3. When the amount of new bone formation was compared in the diabetic groups, more bone was formed in the membrane groups but the difference was not statistically significant.4. In the normal groups the amount of new bone formation was significantly greater at 4 weeks compared to that at2 weeks(p<0.05) but amount of bone regeneration at 4 weeks was not significantly greater than that at 2 weeks in both diabetic groups.

혈관경 전위를 이용한 선조작 골건막피판이 골 결손부 이식물에 미치는 영향과 적절한 혈관경 이식기간 (The Effects of the Prefabricated Periosteofascial Flap through the Vascular Pedicles Transfer on the Bone Defect and the Optimal Period of the Pedicles Implantation)

  • This study was designed to investigate the optimal period of pedicles implantation in the prefabricated periosteofascial flap with a vascular tissue transfer. The flap prefabrication was prepared with a transposition of left occipital pedicles on the calvarial fascia of male Sprague-Dawley rats. Thirty flaps were divided into five groups of six flaps, including control group (group I) of the conventional periosteofascial flap based on the lateral border of the rat calvarium. The prefabricated flap was elevated as an $1{\times}1cm$ sized island flap based on the implanted pedicle at 1, 2, 3, and 4 weeks after the pedicles transfer in groups II, III, IV, and V, respectively. After the completion of creating a critical-sized calvarial defect and implanting with hydroxyapatite granules, the flap was sutured back for covering the defect and kept isolated from surrounding tissues. Six weeks after flap repositioning, the osseous changes of the defect were examined with simple radiographic findings, radiodensitometric analysis, and histological studies. By simple radiographic findings, specimens of the control, groups IV and V showed homogeneous radioopacity within the defect. But in groups II and III, focal radiolucency was observed in the defect. In the radiodensitometric analysis, the control group and the group V showed significant increased radiodensites statistically. Histologically, the implanted hydroxyapatite was absorbed partly in the defect in groups II, III, and IV. In the defects of the control group and the group V, the implanted hydroxyapatite was kept in its volume and the deposition of the bone cells was observed sparsely. In conclusion, the prefabricated periosteofascial flap can be created with a vascular tissue transfer and the pedicles should be implanted at least for 4 weeks to bring out positive osseous changes in the calvarial defect.

핀 고정 천공형 티타늄막을 이용한 수직적 체조제증대술에 관한 연구 (Exophytic bone formation using porous titanium membrane combined with pins in rabbit calvarium.)

  • The purpose of this study was to evaluate exophytically vertical bone formation in rabbit calvaria by the concept of guided bone regeneration with a custom-made porous titanium membrane combined with bone graft materials. For this purpose, a total of 12 rabbits were used, and decorticated calvaria were created with round carbide bur to promote bleeding and blood clot formation in the wound area. Porous titanium membranes (0.5 mm in pore diameter, 10 mm in one side, 2 mm in inner height) were placed on the decorticated calvaria, fixed with metal pins and covered with full-thickness flap. Experimental group I was treated as titanium membrane only. Experimental group II, III, IV was treated as titanium membrane with BBM, titanium membrane with DFDB and titanium membrane with FDB. The animals were sacrificed at 8 and 12 weeks after surgery, and new bone formation was assessed by histomorphometric as well as statistical analysis. 1. Porous titanium membrane was biocompatable and capable of maintaining the regeneration space. 2. At 8 and 12 weeks, all groups demonstrated exophytic bone formation and there was a statistical significant difference among different groups only at 12 weeks. 3. The DFDB group revealed the most new bone formation compared to other groups (p<0.05). 4. At 12 weeks, DFDB and FDB groups showed the most significant resorption of graft materials (p<0.05). 5. The BBM was not resorbed at all until 12 weeks. 6. The fixation metal pin revealed excellent effect in peripheral sealing. On the basis of these findings, we conclude that a porous titanium membrane may be used as an augmentation membrane for guided bone regeneration, and DFDB as an effective bone forming graft material. The fixation of the membrane with pin will be helpful in GBR technique. However, further study is required to examine their efficacy in the intraoral experiments.

새롭게 개발된 비정질의 Calcium Phosphate가 백서두개골의 골재생에 미치는 영향 (The effects of novel biodegradable amorphous Calcium Phosphate on bone regeneration in rat calvarial defects)

  • Purpose: The purpose of this study was to evaluate the bone regeneration of novel biodegradable amorphous calcium phosphate. Materials and Method: An 8-mm, calvarial, critical-size osteotomy defect was created in each of 20 male Sprague-Dawley rats(weight $250{\sim}300g$). The animals were divided into two groups of 10 animals each and allowed to heal for 2 weeks(10 rats). The first group was the control group and the other group was the experimental group which received the novel biodegradable calcium phosphate. Results: The healing of the calvarium in the control group was uneventful. The histologic results showed little bone formation in the control group. The experimental group which received the novel biodegradable calcium phosphate showed a normal wound healing. There were a lot of new bone formation around the biomaterial in 2 weeks. The bone formation increased in 8 weeks when compared to 2 weeks and there was a significant bone increase as well(P<0.01). The nobel biodegradable calcium phosphate showed statistical significance when compared to the control group (P<0.05). The novel biodegradable calcium phosphate in 8 weeks showed a significant increase in bone formation when compared to 2 weeks $(40.4{\pm}1.6)$(%). The biodegradable calcium phosphate which is made from mixing calcium phosphate glass(CPG), NaCO and NaOH solution, is biocompatible, osteoconductive and has a high potency of bone formation. Conclusion: We can conclude that the novel biodegradable calcium phosphate can be used as an efficient bone graft material for its biodegradability and osteoconductivity.

Distinctive bone regeneration of calvarial defects using biphasic calcium phosphate supplemented ultraviolet-crosslinked collagen membrane

  • Purpose: To overcome several drawbacks of chemically-crosslinked collagen membranes, modification processes such as ultraviolet (UV) crosslinking and the addition of biphasic calcium phosphate (BCP) to collagen membranes have been introduced. This study evaluated the efficacy and biocompatibility of BCP-supplemented UV-crosslinked collagen membrane for guided bone regeneration (GBR) in a rabbit calvarial model. Methods: Four circular bone defects (diameter, 8 mm) were created in the calvarium of 10 rabbits. Each defect was randomly allocated to one of the following groups: 1) the sham control group (spontaneous healing); 2) the M group (defect coverage with a BCP-supplemented UV-crosslinked collagen membrane and no graft material); 3) the BG (defects filled with BCP particles without membrane coverage); and 4) the BG+M group (defects filled with BCP particles and covered with a BCP-supplemented UV-crosslinked collagen membrane in a conventional GBR procedure). At 2 and 8 weeks, rabbits were sacrificed, and experimental defects were investigated histologically and by micro-computed tomography (micro-CT). Results: In both micro-CT and histometric analyses, the BG and BG+M groups at both 2 and 8 weeks showed significantly higher new bone formation than the control group. On micro-CT, the new bone volume of the BG+M group (48.39±5.47 ㎣) was larger than that of the BG group (38.71±2.24 ㎣, P=0.032) at 8 weeks. Histologically, greater new bone area was observed in the BG+M group than in the BG or M groups. BCP-supplemented UV-crosslinked collagen membrane did not cause an abnormal cellular reaction and was stable until 8 weeks. Conclusions: Enhanced new bone formation in GBR can be achieved by simultaneously using bone graft material and a BCP-supplemented UV-crosslinked collagen membrane, which showed high biocompatibility and resistance to degradation, making it a biocompatible alternative to chemically-crosslinked collagen membranes.

Acrodysostosis Associated with Symptomatic Cervical Spine Stenosis

  • 선단이골증(acrodysostosis)은 매우 드문 골격계 이형성 질환의 하나로, 말단골 이형성을 보이는 짧은 손가락과 발가락, 코뼈의 저형성 및 정신지체를 그 특징으로 한다. 본 증례에서 저자들은 전형적인 임상 양상을 보이면서 신경 증상 및 징후와 경추관 협착을 동반한 선단이골증 16세 남아를 경험하여 보고 하고자 한다. 환아는 양쪽 상지의 간헐적인 통증과 위약감을 호소하였으며 양팔을 높이 들어올리는 것이 불가능함을 주소로 내원하였다. 신체 검진상 저신장을 보였으며 넓으면서도 낮은 콧등, 작고 위로 들려 올라간 코끝, 양쪽 눈의 안쪽 눈구석 주름 및 경한 양안격리증 등 특징적 안면 소견을 보였다. 신경심리검사상 중등도의 정신지체가 확인되었으며 청력검사상 양측의 신경성 난청 소견이 동반되어 있었다. 방사선학적 검사에서는, 원뿔 모양의 골단을 보이는 넓으면서도 짧은 중수골 및 지골, 양측 전와부의 마델룽 변형(Madelung deformity), 과형성된 첫 번째 중족골과 두꺼워진 두개골이 확인되었다. 뇌 및 척추 자기공명영상검사에서는 경추관 협착, 경추-연수 접합부의 압박을 동반한 편평두개저 및 소뇌 편도의 하방 이동 소견을 보였다. 선단이골증에서 동반되는 경추관 협착증은 시간이 지남에 따라 진행하는 경향을 보이므로, 6개월 후 추적 영상검사에서 진행되는 소견을 보이거나 증상이나 징후가 악화될 경우에는 수술적 갑압술을 실시할 계획이다. 본 증례는 선단이골증의 국내 보고로는 두 번째이나, 경추관 협착증의 증상 및 징후를 동반한 선단이골증의 첫 번째 보고이다. 선단이골증에서의 척추관 협착증은 시간이 지남에 따라 진행하는 증상이며 심각한 합병증이 초래될 수 있기 때문에, 특징적 임상 양상을 보이는 환자를 조기에 진단할 수 있다면 이러한 합병증에 대한 적절한 치료로서 치명적인 신경학적 후유증의 발생을 피할 수 있다. 따라서, 말단골의 이형성을 보이는 환자의 중요한 감별 질환 중 하나로 선단이골증은 반드시 고려되어야 할 것이다.

토끼 뇌감염 모델의 CT 소견과 조영제 주입 후 동맥혈의 Hounsfield Unit의 변화 (CT Scan Findings of Rabbit Brain Infection Model and Changes in Hounsfield Unit of Arterial Blood after Injecting Contrast Medium)

  • 토끼 뇌에 대장균을 주입하여 CT 소견을 알아보고, 시간에 따른 동맥혈의 하운스필드 값의 변화를 알아보고자 하였다. 토끼 두개관에 천두공(burr hole)을 뚫고 2~3 mm 깊이에 대장균 $1{\times}10^7$ CFU/ml, 0.1 ml을 주입하여 뇌염증 모델을 제작하고, 조영 증강 CT와 동적 CT, 그리고 동맥혈의 CT영상을 얻었다.조영 증강 CT에서 뇌농양, 뇌실염 그리고 뇌막염등 다양한 뇌염증 소견이 보였다. 뇌농양은 중앙부가 거의 조영되지 않고 주변부가 강하게 조영되는 전형적인 양상을 보였고, 뇌실염은 측뇌실 벽을 따라 강하게 조영되는 소견을 보였으며, 뇌막염은 종뇌와 간뇌의 접히는 부위 뇌막이 강하게 조영되었다. 동적 CT영상에서 염증 중앙부의 조영제 주입 전 HU 값은 $31.01{\pm}3.55$였고, 주입 후 10분까지 $40.36{\pm}3.76$으로 서서히 증가하였다. 그리고 염증 가장자리구역에서 HU 값은 조영제 주입 전에 $47.23{\pm}3.12$였고, 조영제 주입 후 약 45초에 $63.59{\pm}3.31$로 가장 많이 증가 하였으나 이후 20분까지 약간 떨어졌다. 또한 균 주입 반대쪽 정상 뇌조직에서 측정한 HU 값은 조영제 주입 전에 $39.01{\pm}3.24$이었고, 조영제 주입 후 약 30초에 $49.01{\pm}4.29$로 가장 많이 조영되었고, 이후 서서히 낮아졌다. 동맥 혈액 CT에서 조영제 주입 전 HU 값은 $87.78{\pm}6.88$이었고, 조영제 주입 후 10초부터 30초까지 급격히 증가하여 $749.13{\pm}98.48$로 최대값을 보이고, 30초부터 45초까지 $467.85{\pm}62.98$로 급격히 감소하며, 45초에서 60초까지는 정체기(plateau)를 보였으며, 이후 20분까지 $188.28{\pm}25.03$으로 감소되었다. 결과적으로 대장균으로 뇌염증 모델을 만들 수 있고, 조영 증강 CT를 통하여 뇌염증의 특징적인 소견을 잘 알 수 있었으며, 동적 CT를 통해 염증 중앙부와 가장자리구역의 조영 양상을 알 수 있고, 동맥혈은 조영제 주입 후 10초부터 30초 까지 급격히 증가하다 정체기를 거쳐 서서히 감소하는 것으로 나타났다.
