동결건조골이 재생공간 유지 및 골재생에 미치는 영향 (Space-maintaining and osteopromotive effect of freezedried bone graft in the procedure of GBR)

  • This study was performed to evaluate the effect of freeze-dried bone graft on space-making capacity and bone formation in the procedure of guided bone regeneration with titanium reinforced ePTFE membrane. After decortication in the calvaria, GBR procedure was performed on 8 rabbits with titanium reinforced ePTFE membrane filled with human FDBA(Rocky Mountain Tissue Bank,Aurora Co., USA). Decortication was performed to induce the effect of bone forming factor from bone marrow. The animals were sacrificed at 2 weeks, 4 weeks, 8 weeks and 12 weeks after the surgery. Non-decalcified specimens were processed for histologic analysis. πle results of this study were as follows: 1. Titanium reinforced-ePTFE membrane was biocompatable and capable of maintaining the space-making. 2. FDBA particle was surrounded with connective tissues but there was no evidence on new bone formation. 3. FDBA particle resorbed continuously but it remained until 12weeks after the surgery. Within the above results, TR-ePTFE membrane could be used effectively for Guided bone regeneration but It was assumed that FDBA does not appear to contribute to bone formation.

  • This study was performed to evaluate bone formation in the calvaria of rabbit by the concept of guided bone regeneration with titanium mesh membrane and demineralized freeze-dried bone. The animal was sacrificed at 2 weeks, 4 weeks, 8 weeks, and 12 weeks after the surgery. Non-decalcified specimens were processed for histologic analysis. 1. The titanium mesh but the biocompatibility was excellent the cell-occlusiveness was feeble. 2. The cell-occlusiveness was feeble and also the soft tissue growth of the upper part of the newly-formed bone after operating was excellent in early stage. 3. The maintenance ability of the space for the GBR very was excellent. 4. The titanium mesh the tissue-integration was superior the wound fixation ability excellent. 5. The demineralized freeze-dried bone did not promote the bone regeneration. 6. With the lapse of time, formation quantity of the bone some it increased, it increased quantity very it was feeble. Within the above results, the titanium mesh for the guided bone regeneration was excellent, the dεmineralized freeze-dried bone confirmed does not promote bone regeneration.

  • Intraosseous hemangioma is an extremely rare tumor that accounts for 1% or fewer of all osseous tumors. The most common sites of its occurrence are the vertebral column and calvaria. Occurrence in a facial bone is very rare. The authors aim to report a case of the surgical treatment of intraosseous hemangioma occurring in the periorbital region, which is a very rare site of occurrence and to introduce our own experiences with the diagnosis and treatment of this condition along with a literature review. A 73-year-old male patient visited our hospital with the chief complaint of a mass touching the left orbital rim. A biopsy was performed by applying a direct incision after local anesthesia. Eventually, intraosseous hemangioma was diagnosed histologically. To fully resect the mass, the orbital floor and zygoma were exposed through a subciliary incision under general anesthesia, and then the tumor was completely eliminated. Bony defect was reconstructed by performing a seventh rib bone graft. Follow-up observation has so far been conducted for 10 months after surgery without recurrence or symptoms.

  • To date, various clinical procedures have been used to restore periodontal apparatus destroyed by periodontal disease. And then, many experimental approaches have been proceeded to develop treatment methods to promote periodontal regeneration. Mechanical, chemical treatments to enhance the attachment of periodontal tissue cells as changing the physical properties of root surfaces, bone graft procedure, and treatments for guided tissue regeneration have been used for periodontal regeneration. However, recent studies have revealed that biologic factors such as growth factors promote biologic mechanism associated with periodontal regeneration. This study was done to enucleate how ELF stimulus affect the periodontal regeneration. We can have following conclusions from this experimental results. The influence of low frequency(ELF) electric stimulus (30Hz at $lO{\mu}A$) known to promote bone formation in vivo, was evaluated for its ability to affect bone cell function in vitro. After 12 hour exposure of ELF stimulus at most appropriate densities ($5{\times}10^4\;cells/cm^2$) to increase osteoblastic cells normally, rat calvarial cells were incubated for 60 hours were used in this study. We have found ELF stimulus suppress calvarial cell proliferation and the ability of protein synthesis, enhance the alkaline phosphatase activity significantly.

백서에서 치아회분과 석고 매식 후 두개 결손부의 생물물리적 성질에 관한 실험적 연구 (BIOMECHANICAL STUDY OF THE CALVARIAL DEFECTS AFTER IMPLANTATION OF THE TOOTHASH AND PLASTER IN THE RAT)

  • The purpose of this investigation was to determine the biomechanical property of calvarial defects reconstructed using toothash, plaster and tooth-plaster mixture. Full-thickness bony defects were made on the rat calvaria with size of $10mm{\times}10mm$. Group 1 was filled with toothash only, group 2 : toothash-plaster mixture, and group 3 : plaster only. The defects were allowed to heal for 12 and 20 weeks before killing the animals. Light microscopic examinations vas taken after 12 week after operation. The hardness was evaluated for test of mechanical property. The following results were obtained : 1. In light microscopic examination 12 week after operation, there were no inflammatory and foreign body reaction. Implanted particles were resorbed gradually or united directly with newly formed bone. 2. In hardness test, the hardness of newly formed bone was lower than that of normal bone and there was significant difference(P<0.01). The site of new bone formation has enough hardness to resist the mechanical stress. These results suggest that toothash and plaster are biocompatible and osteoconductive material.

Effects of poly (ethylene glycol-propylene glycol) copolymer on hemostasis and osteogenesis in a rat calvarial defect model

  • This study aimed to evaluate the effects of a bioabsorbable bone hemostatic agent comprising poly (ethylene glycol-propylene glycol) copolymers (PEG-PPG) on hemostasis and osteogenesis. Bilateral 3 mm diameter calvarial defects were created in 99 male Sprague-Dawley rats. The defects were filled with PEG-PPG or bone wax. The defects of control group were left unfilled. Virtual autopsy was performed to evaluate bioabsorption. The calvaria were subjected to x-ray microtomography (microCT) and histological examination. Bone volume fraction (BV/TV) and bone mineral density (BMD) were measured using microCT; furthermore, white blood cell count and histological examination were performed. After application of PEG-PPG and bone wax, immediate hemostasis was achieved. Autopsy revealed that PEG-PPG disappeared within 48 h at the application site; in contrast, bone wax remained until 12 weeks. The PEG-PPG and control groups showed significantly more osteogenesis than the bone wax group with respect to BV/TV and BMD at 3, 6, and 12 weeks (p < 0.05). Histology revealed that the bone wax group exhibited little bone formation with inflammation. In contrast, PEG-PPG and control groups showed significantly more qualitative osteogenesis than the bone wax group (p < 0.01). In conclusion, PEG-PPG showed immediate hemostasis and was absorbed to allow progressive osteogenesis.

골유도재생술시 탈단백 우골이 세포증식에 미치는 영향 (Effect of deproteinized bovine bone mineral on cell proliferation in the procedure of guided bone regeneration)

  • One of the bone substitutes now in routine use, deproteinized bovine bone mineral(DBBM), is regarded as resorbable and osteoconductive, but some studies refute this. The present study was performed to evaluate the effects of DBBM on guided bone regeneration using titanium membrane on the calvaria of rabbit. At 2 weeks, 4 weeks, 8 weeks, and 12 weeks after surgery, the animal was scrificed. Non-decalcified specimens were produced for histologic analysis. The results of this study were as follows : 1. Titanium membrane was biocompatible and capable of space-maintaining, but there was ingrowth of soft tissue through the pore of titanium membrane. 2. There was no resorption or reduction of DBBM with time. 3. Some of the DBBM particles were combined with newly formed bone. But, apart from host bone, a great part of the particles were surrounded by connective tissue. 4. The bone formation was slight vertically and restricted to superficial area of host bone. Whithin the above results, DBBM dose not appear to contribute to bone formation. DBBM may disturb the migration and proliferation of mesenchymal cell derived from host bone and increase the growth of connective tissue. Therefore, careful caution is needed on selection of bone graft material and surgical protocol at guided bone regeneration for implant placement.

백서두개골 결손부에서 천연물유래 탄산칼슘염 골대체의 골치유에 관한 조직계측학적 평가 (Histomorphometric evaluation of bone healing with natural calcium carbonate-derived bone substitutes in rat calvarial defect)

  • Purpose: This study investigated the osteoconductivity of natural calcium carbonate-derived bone substitutes, hen eggshell (ES), and compared with those of commercial bone substitutes. Materials and Methods: Osseous defects created in the rat calvaria were filled with particulated ES(ES-1), ES with calcium-deficient hydroxyapatite surface layer (ES-2), Biocoral(Inoteb, France), and Bio-Oss(Geistlich Pharma, Wolhusen, Switzerland). After 4 and 8 weeks of healing, histomorphometic analysis was performed to evaluate the amount of newly formed mineralized bone area (NB%). Results: Histologic and histomorphometric analysis showed new bone formation and direct bony contact with the grafted materials in all groups. At 4 weeks, Biocoral group showed greater NB% compared to Bio-Oss and ES-1 groups (P<0.05). At 8 weeks, Biocoral and ES-2 groups showed significantly greater NB% compared to Bio-Oss group (P<0.05). Conclusion: These results indicate that natural calcium carbonate-derived bone substitutes with microporous calcium-deficient hydroxyapatite surface layer may be an effective materials treating osseous defects.

  • This study was undertaken to determine whether the addition of calcium sulfate to toothash material (Toothash : plaster of paris=2 : 1) would improve its stabilizing property without adversely affecting its osteoconduction. The radiographic and histologic examinations of bone response of this composite material was performed after 1-, 3-, 5-, 8-, and 12-week implantation in calvaria of rats. No sign of extensive inflammatory response was detected. No movement could be observed with this composite material. Creeping substitution was observed in the surgical site. The direct union between toothash and growing bone after 12 weeks of implantation was observed in the defect margin. We could observe this composite implant material is resorbing slowly as time is over.

Promotion of Bone Nodule Formation and Inhibition of Growth and Invasion of Streptococcus mutans by Weissella kimchii PL9001

  • Lactic acid-producing bacteria (LABs) are known to have various beneficial properties for health. However, they are generally considered to have an adverse effect on teeth, since they produce acid. Nonetheless, milk and cheese containing specific LAB strains were recently found to have an inhibitory effect on dental caries in children, with an inhibitory activity towards the growth of Streptococcus mutans suggested as the responsible mechanism. Accordingly, the current study selected a probiotic candidate for oral health and studied its inhibitory mechanism against dental caries. Twenty-two LAB species belonging to eleven genuses were screened for promoting bone nodule formation using direct microscopic examination. Only one isolate, Weissella kimchii strain PL9001, increased the bone nodule formation significantly. The addition of W. kimchii strain PL9001 to bone cells prepared from mouse calvaria increased the bone nodule formation, calcium accumulation, and activity of alkaline phosphatase (the osteoblastic marker). Moreover, W. kimchii strain PL9001 inhibited the invasion of Streptococcus mutans into bone cells, and an organic extract of the culture supernatant of W. kimchii strain PL9001 inhibited the growth of Strep. mutans. Therefore, the results suggest that W. kimchii strain PL9001 can be used as a preventive measure against dental caries. This is the first time that a LAB has been shown to promote bone nodule formation and prevent the invasion of Strep. mutans into bone cells.
